Separation and determination of lipophilic corticosteroids and benzothiazepin analogues by micellar electrokinetic chromatography using bile salts.

The separation of corticosteroids and benzothiazepin analogues by micellar electrokinetic chromatography (micellar EKC) was studied in comparison with capillary zone electrophoresis. The separation of these substances was not successful under neutral and alkaline conditions because they migrated with the same velocity as that of the electroosmotic flow. Micellar EKC with sodium dodecyl sulphate (SDS) solutions was also not successful because these substances migrated with almost the same velocity as that of the SDS micelle, owing to their high lipophilicity. The use of bile salts, which have a similar skeleton to corticosteroids, as the micellar phase permitted the separation of these substances with high theoretical plate numbers (150,000-350,000) within a short time (ca. 15 min). Sodium cholate was particularly useful. The effects of bile salt concentration, pH and the addition of methanol were investigated. Micellar EKC was also applied to the determination of the drug substances in tablets and cream using the internal standard method and to purity testing of drug substances and tablets.

Separation and determination of the ingredients of a cold medicine by micellar electrokinetic chromatography with bile salts.

The separation of fourteen active ingredients used in a cold medicine was investigated by micellar electrokinetic chromatography (EKC) employing bile salts. Basic drugs were also successfully separated by micellar EKC using bile salts with high theoretical plate numbers (2.0 x 10(5)-3.5 x 10(5)) within a relatively short time (ca. 20 min). The separation of these solutes by micellar EKC was not successful using sodium dodecyl sulphate. The effects of micellar concentration, pH and organic modifier content on migration times and selectivity were investigated. This technique was also applied to the determination of several active ingredients combined in commercial preparations by an internal standard method.

Effect of surfactant structures on the separation of cold medicine ingredients by micellar electrokinetic chromatography.

Using micellar electrokinetic chromatography (micellar EKC or MEKC) the retention behavior of twelve active ingredients used in cold medicines was investigated. The role of five different anionic surfactants was investigated by MEKC and the results were compared with those obtained by conventional capillary zone electrophoresis (CZE). The relative retention order of the 12 ingredients was significantly different among the five surfactants; the different elution orders were ascribed to the differences in the hydrophilic groups of the surfactants. The effects of surfactant concentration and buffer pH were also investigated. The ingredients were successfully separated within 30 min by micellar EKC with high theoretical plate numbers (approximately 200,000), and selectivity was much improved in comparison with CZE. Micellar EKC was applied by the internal standard method to the quantitation of some active ingredients combined in commercial preparations.

[Observations on the effects of Nd-YAG laser irradiation in the canine tongue].

Recently, the Nd-YAG laser has been widely applied for head and neck surgery. This medical instrument provides several advantages, including hemostatic efficacy and cutting capability which are dependent on manipulation. The purpose of this study is to discuss the clinical application of Nd-YAG laser in the field of oropharyngeal surgery. For this reason, the canine tongue is used in this study. After irradiation by Nd-YAG laser, observations were performed on histological findings, measurement of tissue temperature and changes in blood vessels. The results obtained are as follows: 1) Irreversible tissue changes were found in the center of the irradiated area, with other tissue degeneration occurring in the surrounding areas. 2) Within the coagulated degenerative area, blood vessels were completely embolized. 3) Temperature measurements revealed that there were minimal effects on the surrounding tissues. 4) The coagulated degenerative area was replaced by granulation tissue subsequently occurring after wound healing. 5) The Nd-YAG laser was useful for oral and paranasal surgery, because these diseases are localized in the cavity. 6) Non-contact type Nd-YAG laser was indicated for vaporization of the lesions and was particularly useful for vaporizing superficial benign lesions. In this case, close range, low power vaporization is recommended. Conversely, vaporization for hemangioma and/or malignant lesions were most effective with high power irradiation. 7) Contact type Nd-YAG laser might be suitable for cutting and excision of lesions, but careful attention must be given determining the safety margin for application in malignant lesions, because tissue degeneration when using contact type Nd-YAG laser is less reactive than in non-contact type.

[A case of stunned myocardium determined by nuclear cardiac imaging].

A 75 year old woman with acute chest pain was diagnosed as unstable angina. Anterior akinesis of left ventricule and small anterior perfusion defect were found by 99mTc blood pool imaging and 201Tl myocardial imaging. Coronary arteriography showed no organic stenosis, though anterior akinesis was still continued. Furthermore this akinesis and perfusion defect by 201Tl were all disappeared two weeks later. Thus, we determined this case as a stunned myocardium followed by severe angina with transient coronary obstruction.

[Coronary artery disease and the results of coronary bypass surgery in diabetics].

To evaluate the influence of diabetes mellitus on coronary artery disease and the results of coronary bypass surgery, a review was made of 63 consecutive patients undergoing isolated saphenous vein aortocoronary bypass, of whom 38 patients (G-1) were nondiabetic, 9 patients (G-2) had impaired glucose tolerance, and 16 patients (G-3) were diabetic. The severity of coronary artery disease was assessed using an angiographic grading system. Among three patient groups, there was no difference in total coronary score per patient reflecting total extent of disease (G-1; 15.7 +/- 5.5, G-2; 14.1 +/- 5.9, G-3; 17.1 +/- 6.3, mean +/- S.D.) and the incidence of diffusely diseased vessels. The mean number of diseased vessels (greater than 50% stenosis) per patient was 2.5 +/- 0.6 in G-1, 2.6 +/- 0.7 in G-2, and 2.5 +/- 0.7 in G-3, and the mean number of bypass grafts per patient was 2.3 +/- 0.9, 2.2 +/- 0.9, and 2.1 +/- 0.7, respectively. Coronary luminal diameters and the severity of atherosclerotic changes of coronary arteries at the site of graft anastomosis, studied intraoperatively, were similar in all groups. Vein graft blood flows and early graft patency in diabetics (112 +/- 55 ml/min, 96.9%, respectively) were also similarly good as those in the other groups (98 +/- 49 ml/min, 91.4% in G-1, and 92 +/- 39 ml/min, 94.1% in G-2). Overall hospital mortality was 1.6% (one of 63). The only death from meningoencephalitis occurred in the diabetic group.(ABSTRACT TRUNCATED AT 250 WORDS)

Subcutaneous transposition of the spleen enhances the survival rate following 90% hepatectomy in rats.

The effects of subcutaneous transposition of the spleen (STS) on the survival rate following 90% hepatectomy were investigated in rats. The survival rate was significantly higher in the STS group than in the non-STS group. Light microscopy enabled us to note that congestion in the terminal portal veins and sinusoids occurred either slightly earlier or to a higher degree in the non-STS group.

Quadricuspid aortic valve--report of a case associated with severe aortic regurgitation and review of the literature.

A case of a 59-year-old woman who had a quadricuspid aortic valve associated with severe aortic regurgitation is reported herein. The anomaly was revealed by aortography, after which the incompetent valve was excised and replaced by a St. Jude Medical prosthesis. A quadricuspid aortic valve must be considered as a malformation which leads to severe valve failure in later life.

Ultrastructural changes in rat liver sinusoids during storage in cold Euro-Collins solution.

Changes in the sinusoids of rat livers stored in cold (2 degrees C) Euro-Collins solution for various periods were observed using combined scanning (SEM) and transmission electron microscopy (TEM). The sinusoidal endothelial cells were vulnerable to cold ischemia. Fenestrations of the endothelial cells were enlarged and became mesh-like after a 4-h preservation period. Following 8 h storage the sieve plates and cytoplasmic processes of the endothelial cells were destroyed and there was a tendency for the perikaryon to desquamate. Blebs derived from hepatocytes were seen after 4 h and these increased in number and size with prolonged preservation. Although the sinusoids were filled with blebs after 24 h preservation. no irreversible ultrastructural damage in the parenchymal cells was observed. Within 12 h storage, the liver had a mosaic pattern after perfusion fixation indicating uneven fixation and profound circulatory disturbance. These results suggest that endothelial cell destruction and/or numerous blebs may have unfavorable effects on the microcirculation of the transplanted liver after prolonged preservation.

[Quantification of myocardial infarct size by technetium-99m pyrophosphate single photon emission computed tomography].

Myocardial infarct size in 41 patients with the first attack of acute transmural myocardial infarction (MI) was assessed by technetium-99m pyrophosphate single photon emission computed tomography (99mTcPYP-SPECT). A ratio of the number of voxels of 99mTcPYP uptake into the infarct area to that into the thorax was calculated as a parameter of MI size. The ratio was positively correlated with both peak CPK activity (r = 0.53, p less than 0.005, n = 24) and extent score in 201TI-SPECT (r = 0.70, p less than 0.005, n = 14) significantly in patients with anterior MI but not in patients with inferior MI. There was also significant negative correlation between the ratio and the left ventricular ejection fraction (LVEF) measured by RI angiography in both acute (r = -0.67, p less than 0.005, n = 18) and chronic (r = -0.75, p less than 0.005, n = 25) phases in patients with anterior MI. Recovery in LVEF at chronic phase was noted in patients with small anterior MI but not with large anterior MI. 8 of 14 patients with inferior MI had right ventricular MI, that might have affected evaluation of MI size and resulted in no correlation between variables. It was suggested that 99mTcPYP-SPECT was a useful method to evaluate MI size and to predict prognosis of cardiac function in patients with anterior MI but not in patients with inferior MI.

[A case of mucinous carcinoma of the skin].

We were consulted by an 81-year-old man who had been complaining of a slow-growing tumor on his abdomen for 20 years. Histologically, small islets of the tumor cells were floating in mucinous lake separated by fibrous septa, so we diagnosed this tumor as mucinous carcinoma of the skin (Mendoza). Electron-microscopically, nuclei of the tumor cells were slightly folded. There were a few secretory granules and many secretory vacuoles in cytoplasm, but no findings of decapitation secretion. Like some other authors' reports, our data suggest that this tumor has an eccrine gland origin. To our knowledge, this is the 12th case of mucinous carcinoma of the skin in the Japanese literature.

[Coronary artery bypass grafting in a patient with Sjögren's syndrome and other autoimmune diseases].

Coronary arterial involvement in collagen disease is a very rare complication. Particularly in Sjögren's syndrome is there no report of coronary artery bypass grafting (CABG). We report a 68-year-old woman with various autoimmune diseases who underwent successful CABG for unstable angina pectoris. She had a history of Sjögren's syndrome, hypothyroidism, thrombocytopenia and lupoid hepatitis, and had been on the regimens of prednisolone and levothyroxine sodium during the last 4 years. Two months prior to admission when the doses of these drugs were increased, she began to complain of angina on mild exertion and occasionally even at rest. Selective coronary angiograms revealed 75% stenosis at Seg. 2 and 99% stenosis at Seg. 6. Based on these findings and severity of the angina, saphenous vein CABG to the posterior descending and the left anterior descending branches was performed. Despite the associated autoimmune diseases, the patient's postoperative course was uneventful and she is now doing well with no attack of angina, 12 months following the operation.

[PTCA and coronary artery bypass operation prior to subsequent surgery of gastric cancer in patients with coronary artery disease].

Two patients with coronary artery disease required operation for gastric cancer. In one patient with single vessel disease, 37 days later after successful PTCA, gastric resection for early gastric cancer was performed without any cardiac complication. The other patient with severe three vessel disease initially underwent coronary artery bypass operation, and 13 days later gastric resection for advanced cancer was performed. This patient developed supraventricular arrhythmia as a sole complication which was easily controlled by medical treatment. In both cases, fentanyl was used as an anesthetic agent and nitroglycerin was continuously administered intravenously during the perioperative period. ECG, central venous pressure, and direct arterial pressure were also carefully monitored. We feel that noncardiac operation such as gastric resection can be performed without major cardiac complications even shortly after coronary artery bypass operation.

[Restorative effect of muroctasin; MDP-Lys (L18) [DJ-7041] on leukopenia in urogenital cancer patients treated with chemotherapy].

Muroctasin, a derivative of muramyl dipeptide (MDP), increases WBC count through induction of the colony stimulating factor (CSF). This action of muroctasin is useful to treat cancer patients with leukopenia on chemotherapy. Urogenital cancer patients were randomized to either a 50 micrograms or 100 micrograms dose group of muroctasin when the WBC count dropped below 3,000/cmm in the first cycle of chemotherapy. The patients were then subcutaneously treated with muroctasin once daily for 6 consecutive days, and restoration of their WBC count determined. Out of the 32 patients who were enrolled in the study, 25, 24 and 23 patients were assessed for safety, usefulness and efficacy, respectively. The 100 micrograms dose was more effective in restoring the WBC count than the 50 micrograms dose based on the evaluation by the physicians. This result was also confirmed when we used the criteria for evaluation of adverse reactions as recommended by the Japanese guidelines for evaluation of efficacy of chemotherapy on solid tumors. Mild fever and reaction at the injection site were noted in 16% (4/25). In conclusion, muroctasin is useful for leukopenia after chemotherapy.

Diagnosis and operative indications for polypoid lesions of the gallbladder.

In 411 patients undergoing cholecystectomy, benign polyps were present in 32 gallbladders and malignant polyps were present in eight. Histologically, cholesterol polyps accounted for most of the benign lesions, and all the malignant lesions were adenocarcinomas. Gallstones coexisted in 50% of the malignant lesions and endoscopic retrograde cholangiography revealed anomalous pancreaticobiliary junctions in three of five patients with malignant lesions. Sixty-nine percent of patients with benign lesions were under 60 years of age, whereas 75% of those with malignant lesions were over 60. Ninety-four percent of the benign lesions were under 1.0 cm in diameter, while 88% of the malignant lesions exceeded this size. Spread and size of the tumor showed a close correlation. Therefore, size of the tumor is a vital indicator in the treatment of polypoid lesions of the gallbladder, and a malignancy should be considered when the tumor exceeds 1.0 cm in diameter.
