Giant vascular leiomyoma in the hand.

Vascular leiomyoma or angioleiomyoma is a benign solitary tumour that arises from the mediae layer of the vein walls. It is usually located in the wall of the uterus. We describe a 73-year-old man who presented with a slowly growing mass in the thenar region of his right hand which, on histological examination, was shown to be a giant vascular leiomyoma. This is extremely rare and we could find only about 100 such cases reported in English.

Hypoglycemic activity of to-kai-san (Chinese medicines) in normal and KK-Ay mice.

The hypoglycemic effect of To-Kai-San (TS) was studied in normal mice, streptozotocin-induced diabetic mice, one of the animal models of insulin-dependent diabetes mellitus (IDDM) with hypoinsulinemia, and KK-Ay mice, one of the animal models of non-insulin-dependent diabetes mellitus (NIDDM) with hyperinsulinemia. TS (1,500 mg/kg) reduced the blood glucose of the KK-Ay mice from 441 +/- 36 to 285 +/- 52 mg/100 ml 7 h after a single oral administration (p < 0.001). However, no changes in blood glucose were observed in the normal and STZ-induced diabetic mice. TS also decreased the blood glucose 20 days after repeated administration to normal and KK-Ay mice. In addition, TS-treated KK-Ay mice showed decreased plasma insulin levels after both single and repeated administration. From these findings, it is believed that TS may require the presence of insulin to display its activity.

Effects of 20 days bed rest on muscle morphology.

Using ultrasound, muscle thickness and fascicle angles from aponeurosis were evaluated before, during and after 20 days bed rest (BR). Subjects were healthy adults (4 women and 4 men). Measurements were carried out before and after BR and after 10 weeks of recovery, respectively. Muscle measurements were taken at nine sites in trunk and upper and lower extremities, respectively. For the m. triceps brachii, m. vastus lateralis, and m. gastrocnemius medialis, fascicle angles from the aponeurosis as well as muscle thickness were measured. There was a high statistical significant correlation between muscle thickness and cross-sectional area for quadriceps muscles, suggesting applicability of muscle thickness for evaluation of muscle size. Muscle thickness decreased in muscles of the lower extremity by 2.1-4.4 % after bed rest. In triceps brachii and vastus lateralis muscles, there were no prominent changes in muscle thickness and fascicle angles. It was concluded that muscle morphology deteriorates with changes in muscle architecture by bed rest but the response is small and muscle-specific. It was also suggested that bed rest affects not only muscle mass but muscle tone as well.

Effects of 20 days of bed rest on physiological cross-sectional area of human thigh and leg muscles evaluated by magnetic resonance imaging.

The present study was to investigate the effects of 20 days of bed rest on morphological characteristics of lower limb skeletal muscles. Ten sedentary volunteers (5 males and 5 females) were participating in this study. Magnetic resonance imaging techniques were used to measure the physiological cross-sectional areas (PCSAs) of the major muscles and muscle groups of the lower limb. Consecutive images were taken from the right thigh and leg of subjects, and muscle volumes (MV), muscle length, and fiber length were calculated. PCSA of each muscle was determined as MV times the cosine of the angle of fiber pennation divided by fiber length. PCSA of knee extensor and flexor muscles were significant reduced during and after bed rest. MV and PCSA of individual muscles in the knee extensors decreased by -5.1 % to -8.0%. In knee flexors, MV and PCSA in biceps femoris (long head), semitendinosus, semimembranosus, and sartorius decreased during and after bed rest. MV and PCSA in medial and lateralis [correction of andateralis] gastrocnemius, and soleus were remarkably reduced by -9.4 to -10.3% after bed rest. The results suggest that there is a great variability of muscle atrophy in the lower limb muscle groups or individual muscle after bed rest and that the plantar flexors primarily affected.

Changes in force-velocity and power output of upper and lower extremity musculature in young subjects following 20 days bed rest.

Ten healthy sedentary students were exposed to 20 days bed rest (BR) to study the effect of simulated weightlessness on force(F)-velocity(V) characteristics and power(P) output of upper and lower limb movements. F, V and P were measured using a special dynamometer applicable to single joint movements [elbow flexion(EF) and extension(EE), knee flexion(KF) and extension(KE), and hip flexion] or multi-joint movements (squatting). Physiological cross-sectional areas(PCSA) of KF and KE muscles were measured by a magnetic resonance imaging technique. After BR, attenuation in P were observed in lower limb movements (decreased by 19.8-43.6% for squatting, KF and KE), in upper limb movements on the other hand, the decreases in P were not significant (approximately -5% for EF and EE). Decrease in P in lower limb were more pronounced in heavier loaded conditions which were characterized by decreases in both F and V. For KF and KE, decreases in maximal static F (-18.9 to approximately -26.8%) were more pronounced than the decreases observed in the PCSA (approximately -7%), resulting in decreases in specific tension (-12.3 to approximately -22.1%). Neural excitation potentials to generate maximal muscle tension or P might also be influenced by weightlessness.

Effects of long term bed rest on stretch reflex responses of elbow flexor muscles.

Stretch reflex responses of m. biceps brachii and m. brachioradialis of ten normal adults were studied before and after 20 days of strict bed rest. A standard torque perturbation (15 Nm, 170 ms) was applied to the forearm to induce reflex electromyographic (EMG) activities of the two muscles investigated. Totally 30 perturbations were applied during submaximal isometric elbow flexion movements at 80 deg flexed joint angle, and ensemble averaged EMG waveforms were calculated by aligning the signal to the onset of perturbations. All subjects showed that both short and long latency stretch reflex FMG activities of m. biceps brachii were reduced immediately after 20 days bed rest, and then recovered gradually to pre-bed rest levels at one- to two-months after bed rest, whereas there was no such variation in the stretch reflex induced in m. brachioradialis. It was demonstrated that the muscle stretch reflex gain might be reduced with long-term inactivity, but the effects on stretch reflex gains were different in the two tested muscles.

[Reasonable lymph node dissection for T2 or T3 midthoracic esophageal cancer with cervical lymph-node metastasis].

In order to determine the reasonable lymph node dissection for T2 or T3 midthoracic esophageal cancer with cervical lymph node metastasis, a retrospective study was carried out on 106 patients receiving resection between 1983 and 1996. Metastasis to cervical lymph node was obtained in 27.4% (29/106) of patients with T2 or T3 midthoracic esophageal cancer. Within 29 patients, metastasis in cervical node only, in two fields and in three fields occupied 17.2%, 41.4% and 41.4%, respectively. And according to the histologic examination of dissected lymph nodes, metastatic sites spreaded from neck to perigastric region. Five-year survival rate of 23 patients receiving curative operation was 33.0%, and that of 13 patients excluding 3-field metastasis was 51.9%. But the main sites of nodal recurrence were cervical or superior mediastinal nodes along the bilateral recurrent laryngeal nerves, and the rate of nodal recurrence was 47.8%. These results of actual state of lymph node metastasis and prognostic benefit of aggressive dissection suggest that 3-field lymph node dissection is mandatory for T2 or T3 midthoracic esophageal cancer with cervical lymph node metastasis. And we should endeavor to upgrade the precise dissection in order to decrease the nodal recurrence.

[Arterial infusion chemotherapy in pelvic space for advanced rectal cancer].

Three rectal cancer patients were treated with arterial infusion chemotherapy through the internal iliac artery. Two patients with pelvic recurrences unresectable after APRA were treated with intensive chemotherapy and the other patient with neoadjuvant chemotherapy. In all cases, 5-FU (500 mg/ body/day) was administered continuously for 5 days or 14 days. We attempted three methods for this procedure which were a bilateral catheterization to the internal iliac artery, a single catheterization with an embolization to the other internal iliac artery and a single catheterization without the embolization. As the result of this treatment, in the resected specimen with neoadjuvant case, histological necrosis was found in 50% in the main tumor and the metastatic lymph nodes. One patient with pelvic recurrence showed a partial response in CT imaging, but died one year later of the recurrence around the external iliac artery. The other patient with pelvic recurrence treated with the bilateral catheterization had no efficacy on CT imaging, but his CEA level has decreased at present. It was concluded that arterial infusion chemotherapy was effective for advanced rectal cancer and the pelvic recurrences. However, the efficacy of this treatment is limited to the area to which the drugs are delivered. Thus, it is important that the method and the location of the catheterization are determined adequately for each case.

[Evaluation of CTA for arterial infusion chemotherapy for liver metastasis from colorectal cancer].

To compare between the arterial blood supply of metastatic liver tumor and effects of intrahepatic arterial infusion chemotherapy (IHAC), we examined 8 patients with 27 liver metastasis from colorectal cancer. They were treated with ADM/lipiodol/5-FU/LV (19 nodules) or 5-FU/LV (8 nodules). To evaluate the arterial blood supply, CT arteriography (CTA) was performed, which classified tumors into 3 grades. Grade (Gr) 0; almost no enhancement, Gr1: less than one-third of tumor, Gr 2; less than two-thirds; Gr 3 over two-thirds (no case). As a result, 3 of NCs and 3 of PDs were Gr 0, 7 of MRs, 3 of NCs and 2 of PDs were Gr 1, and CR, PR and 7 of MRs were Gr 2. These results suggest that the arterial blood supply is necessary for a better response of IHAC and CTA is effective to forecast the response to IHAC.

Liquefied petroleum gas (LPG) poisoning: report of two cases and review of the literature.

Two autopsy cases of men who died while connecting a liquefied petroleum gas (LPG) pipe are reported. Their blood concentrations of propane (the main content of LPG) were 0.12 and 3.40 mg/100 g, respectively. The cause of death after exposure of LPG has generally been considered to be asphyxia from hypoxia. The large differences in the blood propane levels found here and reported in the literature, however, suggest that direct toxic effects of propane poisoning may be the cause of death in some cases. Propane concentrations and the cause of death are reviewed and discussed.

Force generation capacity of knee extensor muscles in speed skaters.

The force generation capacities during a single as well as repetitive maximal knee extensions were investigated in speed skaters in relation to the cross-sectional area (CSA) of quadriceps femoris muscles. The subjects were 15 male and 12 female speed skaters, and an age-matched untrained group (20 men and 21 women). An isokinetic dynamometer was used to determine force output at three constant velocities of 1.05, 3.14 and 5.24 rad.s-1 and to perform 50 repetitive maximal contractions at 3.14 rad.s-1. The CSA was measured by using a B-mode ultrasound technique at the midpoint of the thigh length. The isokinetic force produced at each test velocity was significantly correlated to CSA in all the subjects (r = 0.867-0.920, P < 0.05). There was no significant difference in force (F) per unit CSA (F.CSA-1) at the three test velocities between the speed skaters and untrained subjects within the same sex. In both the speed skaters and untrained subjects, the women showed significantly lower F.CSA-1 at 3.14 and 5.24 rad.s-1 than the men. The means of force output (F) of every five consecutive and all trials during the repetitive maximal bout were significantly correlated to CSA in all the subjects (r = 0.889-0.934, P < 0.05). Compared to the untrained subjects, the speed skaters showed significantly higher F for every five consecutive contractions even when calculated per unit of CSA (F.CSA-1), and had lower percentage of decline of F during a trial span between the 6th-10th and 41st-45th trials. For the untrained subjects, F.CSA-1 for every five consecutive contractions was significantly lower in the women than in the men. For the speed skaters, the men showed significantly higher F.CSA-1 than the women during a trial span from the 1st-5th trial to the 31st-35th trial, although there was no significant sex difference in the ratio on and after the 36th-40th trial. From these results, it is concluded that the speed skaters show a higher muscle performance in a repetitive maximal contraction task rather than in a single contraction compared to the untrained subjects. In addition, judging from the results for the speed skaters, the women might be less trainable than the men in the F.CSA-1 during a single contraction at a fast velocity as well as repetitive maximal contractions.

Sex difference in force generation capacity during repeated maximal knee extensions.

The force generation capacity, during 50 repeated maximal knee extensions with a constant velocity of 3.14 rad.s-1, and cross-sectional area (CSA) of the quadriceps femoris muscles were determined for untrained women (n = 36) and men (n = 27) aged from 18 to 25 years. As force scores in the maximal repetitions, the mean values of force (F) of every 5 consecutive and all trials and the percentage of decline of F (%D) with 50 contractions were calculated. The CSA was measured by using a B-mode ultrasound technique at the midpoint of the thigh length. The F decreased from 303 (SEM 13) N at the 1st-5th trial to 155 (SEM 9) N at the 46th-50th trial for the men, and from 202 (SEM 9) N to 94 (SEM 4) N for the women. The F of every 5 consecutive and all trials were significantly correlated to muscle CSA: for the men r = 0.552-0.872 (P < 0.01) and for the women r = 0.609-0.857 (P < 0.01). The men showed significantly higher F at every 5 consecutive trials than the women even when calculated per unit muscle CSA (F.CSA-1). There were significant correlations between %D and F.CSA-1 at the 1st-5th trial: r = 0.538 (P < 0.01) for the men and r = 0.631 (P < 0.01) for the women, respectively. The average values of %D were almost the same in both sexes: for the men 48 (SEM 3) % and for the women 52 (SEM 2) %, respectively. However, an ANCOVA calculation on %D, using F.CSA-1 at the 1st-5th trial as the covariate, indicated that the women had significantly higher %D than the men. Thus, the force output during the maximal repetitions was significantly correlated to the CSA of quadriceps femoris in both sexes. The force output was lower in the women than in the men even when the difference in the muscle CSA was allowed for. The women had higher %D than the men when force output per unit muscle CSA during the initial 5 repetitions was compared.

Ethnic differences in the expression of blood group antigens in the salivary gland secretory cells from German and Japanese non-secretor individuals.

Type 1 ABO blood group antigens (peripheral core structure: Gal beta 1-3GlcNAc beta 1-R) are expressed mainly in endodermally-derived tissues, but are not synthesized in mesodermally-derived tissues. In the former tissues, H type 1 antigen is generated largely by alpha-2-L-fucosyltransferase encoded by secretor (Se) gene and acting on the terminal galactose of the type 1 precursor chain. This theory has been generally accepted, and it seems that the expression of ABO blood group antigens is absent, or expressed at a low level, in these tissues from non-secretor individuals. In this immunohistochemical study on the secretory cells of salivary glands, we found ethnic difference between German and Japanese non-secretor individuals in the expression of blood group antigens: i.e. the expression of the type 1 blood group antigens is present in these cells from Japanese non-secretor individuals but absent from German. A possible explanation is that another alpha-2-L-fucosyltransferase, independent of the secretor gene, is present in Japanese non-secretor individuals.

The detection of picoplankton 16S rDNA in cases of drowning.

Picoplankton belonging to the Synechococcus genus in cyanobacteria (approximately 1 micron in size) are found ubiquitously in Lake Biwa, Japan. However, they could not be morphologically discriminated from other bacteria by microscopy. In this study we attempted to use picoplankton for the diagnosis of drowning by PCR analysis of the 16S ribosomal DNA (rDNA). We designed primers complementary to the variable regions of 16S rDNA of the picoplankton we had sequenced. A comparison was made of the PCR products from the three picoplanktons, five other cyanobacteria, Melosira (diatom), Staurdstrum (green alga), bacteria from Lake Baikal, and humans. The picogram order of template DNA from picoplankton was specifically amplified by the primers. When the template of picoplankton was mixed with human lung tissue, at least 10 ng of template DNA was needed to obtain a PCR product. The isolation of the picoplankton from human lung tissue increased the sensitivity of PCR more than a hundred-fold. The specific PCR products of the picoplankton were obtained from formalin-fixed drowning tissue. Molecular biological diagnosis of drowning was successful using picoplankton 16S rDNA.

Novel polymorphisms and haplotypes in the human coagulation factor XIII A-subunit gene.

Novel polymorphic sites within the coding region of the human coagulation factor XIII A-subunit (F13A) gene and their haplotypic combinations with the other polymorphic sites thus far reported are presented. Polymorphic bands were detected in exons 2, 5, 8, 12 and 14 by using single strand conformational polymorphism analysis and antithetic forms of the polymorphic exons were linked with each other, cosegregating as distinct sequence haplotypes. In Finnish, German, and Russian populations a total of 18 haplotypes were observed of possible 72 haplotypic combinations of the 5 exons. Ten of the haplotypes detected were found to have no novel mutations but to be only combinations of preexisting mutations. No tightly associated combinations in pairwise comparisons between antithetic forms of the polymorphic exons were observed, indicating that there may be recombinational hotspots within the F13A gene region.

Primary culture of chicken hepatocytes in serum-free medium (pH 7.8) secreted albumin and transferrin for a long period in free gas exchange with atmosphere.

To study liver functions of chicken, we examined the primary culture of chicken hepatocytes, and found an easy method of long-term culture with free atmosphere exchange. Chicken hepatocytes were obtained by collagenase perfusion and cultured at 37 degrees C as a monolayer without substratum in serum-free L-15 medium (pH 7.8) with free atmosphere exchange. The amounts of albumin and transferrin in medium were assayed by ELISA. The culture of chicken hepatocytes was maintained in the serum-free L15-medium )pH 7.) and 37 degrees C with free atmosphere exchange for 20 days. The amount of albumin secreted in the medium decreased to low levels early in culture; however, this was followed by marked increase from day 9 to day 17 of culture. The amount of transferrin was constant until day 6, then it too increased with further culture. We reported an easy method for the simple monolayer culture of chicken hepatocytes in serum-free L12 medium (pH 7.8) with free atmosphere exchange over an extended period. Expression of liver-specific functions, viz. albumin and transferrin synthesis, was observed after 1 week of culture.

Anthropometric variables and muscle properties of Japanese female ballet dancers.

Physiological characteristics of japanese female classical ballet dancers were investigated in comparison with sedentary women of similar age (non-dancers). Subjects were 20 professional female ballet dancers with a mean age of 26.5 +/- 4.4 years (mean +/- SD) and 66 sedentary women with a mean age of 23.7 +/- 3.6 years. Dancers had been trained in ballet classes for 20.4 +/- 2.4 years and had no experience of regular exercises except dance training. Body mass and % body fat measured by under-water weighing method were significantly smaller in the dancers than in the non-dancers. For all extremities, percentage of the muscle in total cross-sectional area calculated by ultra sound apparatus was significantly larger and percentage of the adipose tissue in total cross-sectional area was significantly smaller in the dancers. The maximal isometric and isokinetic force of knee flexors and the maximal isometric force of dorsi flexors (MVC force) were significantly larger in the dancers when the values were divided by body mass of each subject. During 50 repetitions of maximal plantar flexion, the isometric force of the dancers was significantly larger in all repetitions except first and second reps, though no significant difference was observed in MVC force during plantar flexion. It was suggested that the significant differences between the dancers and the non-dancers in some physiological variables might be attributed to the characteristics of dance movements/training.
