[Three resected cases of pulmonary cryptococcosis].

Case 1 was a 53-year-old female who had a small nodule in the right S3 segment on chest CT. As she was not diagnosed by transbronchial lung biopsy (TBLB), open thoracotomy was performed. Case 2 was a 65-year-old female who had a nodule with pleural indentation in the right S6 segment. As this nodule showed difficulty to differentiate from small lung carcinoma, thoracoscopic surgery was performed. Case 3 was a 63-year-old female who had multiple lesions with cavity in the left S4 and S5 segments, which was preoperatively diagnosed by TBLB. She was performed thoracoscopic partial resection of the lingular segment because of poor response to antimycotic agents. All cases received preventive antimycotic agents for one or two months after the operation. There was no recurrence or postoperative meningitis. Thoracoscopic surgery is the effective procedure for the diagnosis and treatment of the localized pulmonary cryptococcosis.

[A resected case of a bronchiolo-alveolar cell carcinoma of the lung accompanying pneumonia-like shadow on chest roentgenogram].

A 67-year-old female was admitted to our hospital because of pneumonia-like shadow on chest roentgenogram with persistent cough and sputum of 4 months duration. Diagnosis as lung cancer was delayed more than 4 months. She showed fever and inflammatory reactions. Antibiotics were effective to inflammatory reactions, but not effective to pneumonia-like shadow. Transbronchial lung biopsy was useful for the diagnosis. Right lower lobectomy was performed. In this case, tumor extents were limited within one lobe. Tumor cells did not invade blood and lymphatic vessels, and extrathoracic metastases were not detected. The prognosis of bronchiolo-alveolar cell carcinoma was determined by intra-pulmonary tumor extent. Based on a comparison with the outcome of unresected cases, bronchiolo-alveolar cell carcinoma limited within one lobe should be surgically resected.

[A case of video-assisted thoracic surgery for congenital esophago-bronchial fistula].

A 42-year-old male was admitted to our hospital because barium esophagograpm showed an esophago-bronchial fistula with an esophageal deverticulum. He has had frequent episodes of cough at drinking water since childhood. A chest CT scan showed mild inflammatory change and bronchiectasis in the right S6. Division of the fistula by video-assisted thoracoscopic surgery (VATS) was performed. There was no evidence of inflammation and adherent lymph nodes around the fistula. This case was diagnosed as a congenital esophago-bronchial fistula by operative findings and clinical course. The fistula was dissected easily and divided by an auto-suturing instrument. The affected lung could be preserved. The patient was discharged on the 10th postoperative day. VATS is an effective treatment for the patients of congenital esophago-bronchial fistula (Braimbrige type I and II), if they are not accompanied with pulmonary abscess or pleural empyema.

[A case of intrathoracic lipoma arising from the chest wall resected by video-assisted thoracoscopic surgery].

We present a case of lipoma arising from the chest wall spreading into the thoracic cavity. Although asymptomatic, a 65-year-old female was pointed out an abnormal shadow on the chest X-ray film taken at the mass survey. The tumor, measuring 3.4 x 3.0 x 2.0 cm in diameter, was surgically removed under thoracoscopic visualization through a small thoracotomy incision of lt. 1st intercostal space, and the diagnosis of lipoma was confirmed postoperatively by histopathologic examination.

Spectroscopic mimicry for the protonated retinal Schiff base in vivo with modified amphiphilic clay interlayers as a possible model of opsin environment.

We have found that clay acts as a novel model matrix for the amphiphilic protein-opsin to mimic the visible absorption spectrum of a protonated retinal Schiff base (RSB) in vivo. Without strong acids at ambient temperature, a visible broad absorption spectrum with a lambda max at 530 nm covering the range from 400 to 680 nm was achieved for the protonated RSB with cationic surfactant-modified montmorillonite clay. The interlayers of the dimethyloctadecylamine (DOA) modified clay were found to provide amphiphilic space allowing the amphiphilic RSB to be intercalated easily and sequentially and protonated by the DOA. It is proposed that the visible absorption spectrum at lambda max 530 nm was attributable to electrostatic effects, permitting the appropriate distance between the nitrogen of the protonated RSB and the negatively charged clay interlayers and also to the anisotropic orientation of the RSB molecules in the interlayers.

Cystic lymphoplasmacyte-rich meningioma--case report.

A 47-year-old female presented with an unusual cystic meningioma. Computed tomography and magnetic resonance imaging showed a cystic tumor with marked edema in the left parietal region of the brain, resembling a metastatic tumor. The tumor was totally removed. Histological examination revealed meningioma with dense lymphoplasmacytic infiltrates.

[The usefulness of radiotherapy in the management of intracranial meningioma: a review of 29 patients].

Twenty-nine patients with intracranial meningiomas were treated with a high-energy linear accelerator (4-10 MV. X rays and electron beams) between 1980 and 1992 at the Hiroshima Prefectural Hiroshima Hospital. We investigated the relationships between tumor size and tumor response, histological type and tumor response, and grade of operation using Simpson's classification and recurrence rate. Meningiomas were greatly reduced between 1 month and 46 months after radiotherapy, the average being 14 months. In the group with a tumor size of less than 5 cm2, 3 patients obtained CR, 5 patients NC and 1 patient PD. Tumor response was 89% in the group with a combination of CR with NC. In the group with a tumor size of 5 to 20 cm2, 2 patients obtained PR, 1 patient NC and 1 patient PD. Tumor response was 75% in the group with a combination of PR with NC. In the group with a tumor size of more than 20 cm2, 3 patients showed NC, 3 patients PD. Tumor response was 50% in the group with NC. In the group with a tumor size of less than 20 cm2, tumor response was 85%. In the group with a tumor size of more than 20 cm2, it was 50%. Though there were no statistically significant differences between the two groups, a tumor size of less than 20 cm2 had a tendency to be more responsive than one of more than 20 cm2. The response of meningioma of the transitional type was worse than that of the meningothelial type and fibroblastic type. Fourteen patients after Simpson grade II or III operation were free of recurrence. The results of this study support the role of radiotherapy for meningioma which is less than 20 cm2, and also for treatment after Simpson grade II or III operation.

Stringently and developmentally regulated levels of a cytoplasmic double-stranded RNA and its high-efficiency transmission via egg and pollen in rice.

A very restricted amount of high-molecular-weight double-stranded RNA (dsRNA) has been found in healthy japonica rice plants. We discriminated dsRNA-carrying rice plants from noncarriers. The endogenous dsRNA was localized in the cytoplasm (about 100 copies per cell) and was transmissible to progeny plants by mating. In crosses between carriers and noncarriers, the RNA was transmitted efficiently to F1 plants via both egg and pollen. The rice dsRNA was maintained at an almost constant level by host plant cells from generation to generation. The high-efficiency transmission of the endogenous dsRNA to progeny plants appears to depend on the autonomously controlled replication of the dsRNA localized in cytoplasmic vesicles. However, an increase in copy number (about 10-fold) of the dsRNA was observed during the suspension culture of host cells. The number of copies of dsRNA returned to the original low value in regenerated plants, suggesting that the copy number is stringently and developmentally regulated in rice cells.

The relationship between brain surface elastance and brain reexpansion after evacuation of chronic subdural hematoma.

BACKGROUND: Brain stiffness has been thought to be a factor affecting brain re-expansion after the evacuation of chronic subdural hematoma. METHODS: As an index of brain stiffness, the pressure for compressing the brain using an ophthalmodynamometer was defined as brain-surface elastance. This elastance in 14 patients was measured and analyzed in relation to brain re-expansion. RESULTS: Patients with an enlarged subdural space on computed tomography 1 month after the operation had higher elastance. There was a correlation between high elastance and high age. CONCLUSIONS: Measuring the elastance after evacuation of the hematoma may help predict the persistence of a subdural space.

Tissue-specific expression of the gene for a putative plasma membrane H(+)-ATPase in a seagrass.

A cDNA clone corresponding to the gene (ZHA1) for a putative plasma membrane H(+)-ATPase of a seagrass (Zostera marina L.) was isolated and sequenced. Comparison of the amino acid predicted sequence from the nucleotide sequence of ZHA1 with those encoded by known genes for plasma membrane H(+)-ATPases from other plants indicated that ZHA1 is most similar to the gene (PMA4) for a plasma membrane H(+)-ATPase in a tobacco (84.4%). Northern hybridization indicated that ZHA1 was strongly expressed in mature leaves, which are exposed to seawater and have the ability of tolerate salinity; ZHA1 was weakly expressed in immature leaves, which are protected from seawater by tightly enveloping sheaths and are sensitive to salinity. In mature leaves, in situ hybridization revealed that ZHA1 was expressed specifically in epidermal cells, the plasma membranes of which were highly invaginated and morphologically similar to those of typical transfer cells. Therefore, the differentiation of the transfer cell-like structures, accompanied by the high-level expression of ZHA1, in the epidermal cells of mature leaves in particular may be important for the excretion of salt by these cells.

Extensive pulmonary metastases in malignant pleural mesothelioma. A rare clinical and radiographic presentation.

We report a case of malignant pleural mesothelioma in a patient who presented with pleural effusion and reticulonodular shadow on chest radiograph. Pulmonary metastases were diagnosed by transbronchial lung biopsy specimen and the patient died of extensive pulmonary metastases. This pattern of clinical and radiographic presentation is seldom reported for malignant pleural mesothelioma.

Double-stranded RNA in rice: a novel RNA replicon in plants.

The entire sequence of 13952 nucleotides of a plasmid-like, double-stranded RNA (dsRNA) from rice was assembled from more than 50 independent cDNA clones. The 5' non-coding region of the coding (sense) strand spans over 166 nucleotides, followed by one long open reading frame (ORF) of 13716 nucleotides that encodes a large putative polyprotein of 4572 amino acid residues, and by a 70-nucleotide 3' non-coding region. This ORF is apparently the longest reported to date in the plant kingdom. Amino acid sequence comparisons revealed that the large putative polyprotein includes an RNA helicase-like domain and an RNA-dependent RNA polymerase (replicase)-like domain. Comparisons of the amino acid sequences of these two domains and of the entire genetic organization of the rice dsRNA with those found in potyviruses and the CHV1-713 dsRNA of chestnut blight fungus suggest that the rice dsRNA is located evolutionarily between potyviruses and the CHV1-713 dsRNA. This plasmid-like dsRNA in rice seems to constitute a novel RNA replicon in plants.

The unusual structure of a novel RNA replicon in rice.

A linear, plasmid-like, double-stranded RNA (dsRNA) was isolated from rice, and its entire sequence of 13,952 nucleotides (nt) was determined. The dsRNA encodes a single, unusually long, open reading frame (13,716 nt, 4,572 amino acid residues), which includes an RNA helicase-like domain and an RNA-dependent RNA polymerase-like domain. A series of Northern hybridization and primer extension experiments revealed that the coding (sense) strand of the dsRNA contains a discontinuity (nick) at a position 1,211 nt (or 1,256 nt) from the 5' end. This discontinuity divides not only the coding strand of dsRNA molecule into a 1,211-nt fragment and a 12,741-nt fragment (or a 1,256-nt fragment and a 12,696-nt fragment) but also divides the long open reading frame into a 5' part of 1,045 nt (348 amino acid residues) and a 3' part of 12,671 nt (4,224 amino acid residues) or a 5' part of 1,090 nt (363 amino acid residues) and a 3' part of 12,626 nt (4,209 amino acid residues). It seems likely that almost all dsRNA molecules in rice plants contain such a discontinuity. This rice dsRNA appears to be a novel and unique RNA replicon.

Expression of Cu,Zn-superoxide dismutase mRNA after cold and contusion injury in the rat brain.

We analyzed the expression of Cu,Zn-superoxide dismutase (SOD) mRNA in both contusion and cold injury. Twenty-three rats were divided into 3 groups: a control group, a contusion group, and a cold injury group. Six hours after the injury, the rats were decapitated and the gray matter was resected from 3 portions: the core of the injured cortex, its periphery, and a distal portion on the non-lesion side. Based on the specific gravity of each sample, almost the same degree of edema developed in both injury groups. The mRNA expression in the cold injury group, however, significantly decreased in all portions. The extremely low temperature associated with cold injury is a possible cause of the decrease in Cu,Zn-SOD mRNA.

Discrete subcellular localization of membrane-bound ATPase activity in marine angiosperms and marine algae.

The subcellular distribution of membrane-bound ATPases was compared among terrestrial plants, seagrasses and marine algae by cytochemical techniques. High ATPase activity was detected in the copiously invaginated plsma membrane that was characteristic of transfer cells but not in the tonoplast of epidermal cells in mature leaves of seagrasses. Magnesium- or Ca(2+)-dependent ATPase activity was induced together with the characteristics of transfer cells during the development of leaf tissues able to resist seawater. Northern hybridization revealed the effective induction of the synthesis of mRNA for plasma-membrane H(+)-ATPase during the development of leaves. Such high ATPase activity was not detected in the smooth plasma membranes of marine macro-algae but was found in the membranes of some cytoplasmic vesicles or microvacuoles, providing evidence of the excretion of salts by exocytosis. It appears, therefore, that two essentially different methods for excreting excess salts have developed separately in these two classes of marine plants. The evolution of mechanisms of salt tolerance in the plant kingdom is discussed in terms of the differential subcellular distribution of ATPase activity.

Prognostic determinants of hypertrophic cardiomyopathy--the results of the Shiga Cardiomyopathy Study.

Previous studies have reported that patients with hypertrophic cardiomyopathy (HCM) are prone to sudden death. In this study, we retrospectively assessed the prognosis and any influencing factors in 83 patients (66 male, 17 female) with HCM. Twenty-two patients were obstructive, 40 were non-obstructive, 18 were apical HCM and 3 were unclassified. Four of the 83 cases progressed to left ventricular dilatation and dysfunction during the follow-up period. The mean age was 51.2 years (range 16 to 73) and the mean duration of follow-up was 6.7 years (range 0.3 to 15.1). The 5- and 10-year survival rates were 98% and 89%, respectively. Five patients dies; 3 from cardiac events (two sudden deaths and one due to congestive heart failure) and 2 from malignant diseases. The lack of a family history of sudden death and the amplitude of the S wave in lead V1 (less than 2.0 mV) were associated with a favorable prognosis. None of the patients who were diagnosed before age 50 died, but this observation was not statistically significant. None of the patients with apical HCM died, but the classification of HCM was not significantly associated with the prognosis. None of the patients without medication died and medical treatment did not influence the prognosis. Sex, family history of HCM, the patient's symptoms and physical signs, NYHA classification of cardiovascular disability, thickness of the septum and the posterior wall of the left ventricle, dimension of the left atrium, and the left and right ventricles in echocardiogram, amplitude of the R wave in lead V5, the depth of the negative T wave and atrial fibrillation did not influence the prognosis.

[Effectiveness of the J-shaped LMA-holding forceps--F-forceps for LMA insertion].

F-forceps (Fukuhara's J Shaped LMA holding forceps) which enables correct laryngeal mask placement with ease was devised. Three methods using the forceps were tried in more than 1000 cases for administration of general anaesthesia. The results were compared with original Brain's method. The results were as follows; (1) Using F-forceps, success rate of proper placement without difficulties is much higher than with Brain's method, especially in F-III method with which 100% success was obtained. (2) After 7 trials, even beginners could shorten the time required for laryngeal mask placement by 10 seconds compared with their first trial. (3) Our method enables placement of the laryngeal mask without contacting patients. (4) One can master handling of the F-forceps with ease and shorten time necessary for placement of a laryngeal mask properly. (5) By applying F-II method, insertion of a laryngeal mask can be carried out without bending the patient's head. It is therefore suitable in patients with cervical vertebral injuries. F-forceps is considered to be highly useful for both anaesthesia and first aid.
