Primary choriocarcinoma of the stomach presenting as gastrointestinal hemorrhage: report of a case.

A case of primary choriocarcinoma of the stomach presenting as gastrointestinal hemorrhage in a 58-year-old postmenopausal woman is presented. Arteriographic features included a large hypervascular gastric mass and multiple liver masses with vascular poolings, mimicking those of cavernous hemangioma. Postcontrast CT including delayed scan showed a large central nonenhanced area with marginal peripheral contrast enhancement. Close observation of the angiographic findings revealed peripheral vascular poolings and central avascularity, which are characteristic of uterine choriocarcinoma. Human chorionic gonadotrophin (HCG) titer was significantly elevated, and extragenital choriocarcinoma of the stomach was confirmed by autopsy.

Adrenoleukodystrophy: new CT findings.

Three cases of adrenoleukodystrophy are presented with sequential findings by computed tomography (CT). In two cases, CT confirmed not only the findings of several previous reports of a caudocranial direction of low-density white matter and contrast enhancement at the periphery of affected regions during the active demyelinating process, but also two other unusual features: (1) calcification in the affected white matter along the trigones of the lateral ventricles and (2) development of a mass effect during the active demyelinating period. Calcification alone without low-density white matter was the initial CT presentation in one case. Periventricular calcification may be specific for adrenoleukodystrophy in the appropriate clinical setting.

Ultrastructure of Trichophyton mentagrophytes stained with neutral red.

The ultrastructure of Trichophyton mentagrophytes cells stained with neutral red was investigated using electron microscopy and X-ray microanalysis. Fixatives containing molybdenum and chromium were used to prevent the outflow of neutral red. Electron-dense particles composed of metals and dye were observed exclusively in the vacuoles, which were increased in number and size, but not near the cell wall. Results indicate that neutral red passes directly through the fungal cell wall and is incorporated into the vacuoles.

Unintentional epidural administration of thiamylal. Case report.

BACKGROUND AND OBJECTIVES: A 36-year-old woman was scheduled for surgery of bicornuate uterus under epidural anesthesia. METHODS: As a test dose, 3 mL 2.5% thiamylal was injected unintentionally through the epidural catheter instead of 1.5% lidocaine with 15 micrograms epinephrine. RESULTS: Immediately the patient complained of burning pain in the low back area. Administrations of 10 mL 2% lidocaine without epinephrine and 10 mL normal saline containing hydrocortisone 100 mg were performed through the epidural catheter. CONCLUSIONS: The patient recovered without permanent neurologic sequelae.

Plasma alcohol concentrations after celiac plexus block in gastric and pancreatic cancer.

OBJECTIVE: The objective of this study was to compare the plasma alcohol concentrations after celiac plexus block in different types of cancer. The authors studied the consecutive changes of plasma alcohol concentrations after celiac plexus block in a gastric cancer group (group 1, n = 6) and a pancreatic cancer group (group 2, n = 5). METHOD: Celiac plexus block was performed with 10 ml of absolute ethyl alcohol. In all patients, operations had been performed 1.2-3.5 years before the block. Arterial blood was sampled at 0, 5, 10, 15, 30, 60, 120, 240, and 480 minutes after the block. RESULTS: The average dose of absolute ethyl alcohol was 0.186 g/kg in group 1 and 0.182 g/kg in group 2. Plasma alcohol concentrations in group 1 were about four times greater than those of group 2 (p < 0.01). The mean maximum level was reached at 15 minutes after injection in group 1 (44.0 +/- 5.8 mg/dl) and at 30 minutes in group 2 (13.1 +/- 2.4 mg/dl). CONCLUSION: Plasma alcohol concentrations after celiac plexus block showed different patterns according to the site of cancer and the type of operation performed.

Discrepancy between clinical and pathological lymph node evaluation in Epstein-Barr virus-associated gastric cancers.

We investigated the clinicopathological characteristics of EBV-associated gastric cancers among poorly differentiated gastric adenocarcinomas. A total of 412 patients with poorly differentiated gastric adenocarcinoma were studied for Epstein-Barr virus (EBV) infection by in situ hybridization for EBV-encoded small RNA. EBV genomes were detected in the tumor cell nuclei of 83 gastric cancers (20.1%). Of these EBV-positive cancers, 60 were histologically classified as gastric carcinoma with lymphoid stroma. Comparing various clinicopathological data with EBV status in gastric cancers revealed a significantly large discrepancy between the clinical and pathological lymph node evaluation of the EBV-positive cancers as compared with that of EBV-negative cancers. These results indicated that a strong host immune reaction occurs in the regional lymph nodes of EBV-associated gastric cancers, and that surgeons should be warned against clinically overestimating the stage of EBV-associated gastric cancer.

Dedifferentiated chordoma arising in irradiated sacral chordoma.

A patient who received radiation therapy for a sacral lesion 10 years ago and eventually developed dedifferentiated chordoma at the primary site is reported. Initially, the patient received 54 Gy with Co-60 to the sacral tumor, and 6 years later an additional 50 Gy with 10 MV X-rays to the local recurrence. Four years later, an evident sacral tumor was noticed. When readmitted for re-confirmation of histology, a large rapidly growing lung metastasis was observed. The patient eventually died of progressive disease with incidental aspiration pneumonia. At autopsy, 10 years after the initial treatment, the recurrent sacral tumor proved to be dedifferentiated chordoma accompanied by typical chordoma. Although this case suggests the possible malignant transformation of chordoma rather than collision, the factors inducing this change remain unknown.

Pentavalent technetium-99m dimercaptosuccinic acid uptake in primary amyloidosis: comparison with autopsy findings.

Pentavalent technetium-99m labeled dimercaptosuccinic acid (99mTc(V)-DMSA) scanning was performed on a 71-year-old woman with histologically confirmed primary amyloidosis. Definite uptake of the tracer was noted in the thyroid, liver, spleen, and kidney as well as in the heart. Three months later, she died of heart failure, and autopsy findings demonstrated diffuse deposits of AL-type amyloid in those organs that were consistent with the accumulation of 99mTc(V)-DMSA.
