Dissecting aneurysm of aorta complicating aortic valvular stenosis.

Six cases of classical dissecting aneurysm of the aorta complicating aortic valvular disease in which aortic stenosis was dominant are described. In three of the cases the aortic complication occurred after the natural aortic valve had been replaced by a prosthetic valve. The longest postoperative period was four months. The factor common to all reported cases in our series was cystic medial necrosis of the aorta. The authors agree with the concept of McKusick and associates that aortic dissecting aneurysm complicating aortic valvular disease is an association other than chance and is related to cystic medial necrosis of the aorta. The latter, in turn, is a consequence of the altered hemodynamics of aortic valvular disease, particularly stenosis.

Anatomic causes of pulmonary stenosis in complete transposition.

Among 166 specimens with classical complete transposition, pulmonary or subpulmonary stenosis was present in 22 cases. The bases for obstruction were pulmonary valvular stenosis (one case), membranous subpulmonary stenosis (nine cases), anomalous attachment of the mitral valve to the ventricular septum (five cases) and combinations of causes (seven cases). Conditions contributing to combinations of causes included the above, in addition to accessory mitral or pulmonary valvular tissue and herniation of a tricuspid valvular pouch through a related ventricular septal defect. In addition to the 166 cases mentioned, our collection contained six cases of atypical complete transposition characterized by a conus being present in the each ventricel. In three of these six, subpulmonary stenosis was present and caused by a narrow inlet to the left ventricular conus.

Lesions of conduction tissue complicating aortic valvular replacement.

A pathologic study of the cardiac conduction system was performed in 57 patients who died within 30 days after aortic valvular replacement. Histologically, there were old and recent lesions present in 34 (60 percent) and 43 (75 percent) of the patients, respectively. Old lesions did not show meaningful correlation with preoperative electrocardiograms. Although nontraumatic recent lesions were rarely the cause of postoperative abnormal cardiac conduction, traumatic lesions were frequently associated with loss of sinus mechanism. The main cause of trauma to the major cardiac conduction tissue was injury by sutures. The short membranous septum predisposed to injury, while the configuration of the left ventricular outflow tract was not a significant factor in injury. Traumatic lesions of the cardiac conduction tissue were more common in subjects with bicuspid than tricuspid aortic valves.

[Blood concentration and urinary excretion in human of OE-7, a gastric hard capsule containing enteric-coated granules. I (author's transl)].

The investigation of physico-chemical properties and measurement of blood concentration and amount of excretion into urine in human of OE-7 (main constituent is erythromycin stearate) were performed as the fundamental studies. The results obtained were as follows; i) OE-7 is a gastric hard capsule containing enteric coated granules. ii) The contents of OE-7 capsule (granules) were resistant to acid and was favarable in the enteric solubility. iii) In comparison with the conventional erythromycin stearate capsule in healthy adults, OE-7 showed a relatively small individual difference in blood concentration after administration. iv) OE-7 was higher in the mean blood concentration and was superior in the persistence of effective blood concentration compared with the conventional erythromycin stearate capsules.

Preparation of anti-etiocholenic acid antiserum and anti-cholesterol succinate antiserum and their cross reactivities.

Anti-3-hydroxy 5-androstene carbonic acid (etiocholenic acid) antiserum was obtained by immunizing rabbits with anetiocholenic acid-polylysine complex. Anticholesterol succinate antiserum was also prepared by immunizing rabbits with a cholesterol succinate-polylysine complex. The cross reactivities of these antisera were examined by means of radioimmunoassay system, immunolysis of liposome and passive hemagglutination reaction. Anti-etiocholenic acid antiserum was specific against the structure of ring A of cholesterol and the nature of OH group at C3 position and anti-cholesterol succinate antiserum was specific against the side chain similar to that of cholesterol.

Ultrastructure of developing germ cells in the fetal human testis.

Electron microscopic studies of the testis were performed on 12 human embryos and fetuses between 9 and 30 weeks post conceptionem. According to their ultrastructural features, the fetal germ cells could be divided into the following three stages of differentiation: 1) gonocytes, 2) intermediate cells, and 3) fetal spermatogonia. Sertoli cells were present among the germ cells in all the testes studied. The gonocytes showed the highest nucleo-cytoplasmic ratio. Their round nucleus contained a centrally located, prominent nucleolus. The cytoplasm displayed a well developed Golgi apparatus, lipid droplets and parallel arrays of short cisternae of the rough surfaced endoplasmic reticulum (rER). Microfilaments were numerous, particularly just beneath the cell membrane. The intermediate cells were found to extend several cytoplasmic processes and to contain a moderate number of long, branched and/or widened rER cisterna which were frequently connected to the perinuclear cisterna. Intermediate cells were often connected to one another by intercellular cytoplasmic bridges. The fetal spermatogonia also displayed cytoplasmic bridges. These cells showed the lowest nucleo-cytoplasmic ratio and more condensed nuclear chromatin. The mitochondria were situated close to the nucleus. Many of them were connected by a cementing substance. Lipid droplets and rER cisternae were rare in these cells. Infoldings of the inner nuclear membrane were often present in the gonocytes and in the intermediate cells, but were rarely observed in the fetal spermatogonia. Glycogen particles, polyribosomes, and chromatoid bodies ("nuage") were present in all the three germ cell types. With the maturation of the fetus, the number of gonocytes was found to decrease, whereas the number of fetal spermatogonia increased. The Sertoli cells also changed their ultrastructure, showing an increase in the number of rER cisternae, as well as of microfilaments, lipid droplets, and secondary lysosomes.
