[Laboratory and clinical studies on latamoxef in the field of obstetrics and gynecology].

Latamoxef (LMOX) is a new antibiotic synthesized by Shionogi Research Laboratory. Chemically LMOX is especially unique with a sulfur atom replacing the oxygen atom in the 1 position of the conventional cephalosporin nucleus, and in addition, this antibiotic has a cephamycin-like structure. The antibacterial activity of LMOX shows high potency against Gram-negative bacteria, but tends to be weak against Gram-positive bacteria. The tissue levels of LMOX in humans after intravenous injection of 1 g were examined. The levels in uterine and adnexa uteri tissue at 1 hour after administration were 25.4 and 27.4 micrograms/g respectively. LMOX was administered to 147 cases in infections of obstetric and gynecological field. The clinical effect according to disease was 94.6% for intrauterine infections, 95.0% for adnexitis, 87.0% intrapelvic infections, and 100% for external genital organ infections, making a total of 92.5%. The rate of occurrence of side effects or abnormal laboratory findings was similar to or slightly less than that seen with other beta-lactam antibiotics.

[Studies on pre- and postoperative extravascular lung water changes in patients with esophageal cancer].

Pre- and postoperative extravascular lung water (EVLW) changes in a series of twenty patients with esophageal cancer were studied using thermal-dye double-indicator dilution method. Preoperative EVLW was 8.2 +/- 2.3 ml/kg (M +/- SD), being greater as compared to the normal range of EVLW. The pattern of postoperative EVLW changes varied between patients and changes could not be predicted by the conventional examinations including chest X-ray, determination of pulmonary hemodynamics and blood gas analysis. Analysing the relationship between the changes in COP-PWP gradient and EVLW, COP-PWP gradient decreased on the first postoperative day regardless of the EVLW changes. With regard to the changes for the 2nd to the 3rd day or for the 3rd to the 4th day, however, the patients with elevating EVLW showed falling COP-PWP gradient, whereas the patients having decreasing EVLW had an increase in COP-PWP gradient. Especially, the patients in which EVLW had increased during the 3rd to the 4th day showed a remarkable rise of PWP possibly due to depressed left ventricular function. It is considered that the EVLW increase on the first postoperative day is attributable to an alteration of the permeability characteristics of alveolar capillary but that the increase in the 3rd to 4th day is simply due to an elevated hydrostatic pressure. This suggests that there are two different kinds of mechanism involving in the production of wet lung following esophageal cancer surgery.

[Clinical evaluation of the efficacy of cefmetazole in respiratory tract infections of patients with underlying respiratory diseases].

Respiratory infections of 10 subjects with underlying respiratory diseases were treated with cefmetazole (CMZ) and its clinical effects were studied. Five subjects of them were respiratory tract infection, 3 subjects were pneumonia and 2 subjects were pneumonia followed by empyema. The underlying diseases were chronic pulmonary emphysema in 4 subjects, diffuse panbronchiolitis in 3, chronic bronchitis in 2 and bronchial asthma in 1. The doses of CMZ were 4 to 8 grams per day and the durations of administration ranged 3 to 39 days. The clinical effects were judged from the changes of fever, cough, amount of sputum, dyspnea, rale, chest X-ray, white blood cell counts, erythrocyte sedimentation rates, sputum culture and PaO2. The clinical effects of 6 subjects were evaluated as good, those of 3 were fair and that of 1 was poor. In 3 subjects H. influenzae in the sputum was eliminated and in 1 subject both H. aphrophilus and alpha-Streptococcus found in the pleural effusion were eliminated. In 1 subject Klebsiella in the sputum was eliminated and replaced by Enterobacter. No side effects were observed. We conclude that CMZ is considerably useful in the treatment of respiratory infections of the patients with underlying respiratory diseases.

Hodgkin's disease in the endemic area of adult T-cell leukemia (ATL).

Antibodies to adult T-cell leukemia associated antigen (ATLA) in the sera from 14 patients with Hodgkin's disease (HD) were examined by the indirect immunofluorescence method. All patients were born in the ATL-endemic area, Kagoshima and Miyazaki prefectures, of Japan. Seven of the 13 evaluable patients (54%) gave positive reactions in the anti-ATLA test. Most of the positive patients were over 40 years of age and the positive rate in males was higher than that in females. The most interesting finding in the anti-ATLA positive patients was the presence of circulating abnormal lymphocytes which are usually found in ATL patients. Histological subclassification of the positive patients had no tendency to converge into a particular subclass of HD. Although there was no definite evidence to show an etiological relationship between ATL virus and HD, several findings in this study seem to suggest that in anti-ATLA positive patients, especially in the ATL-endemic area, the anti-ATLA positive state might have some inevitable influence on the natural history of their HD.

Effect of heparin treatment in cases of severe toxemia.

Severe toxemia of pregnancy is associated with hypercoagulability and hyperlipemia in general. Heparin, a substance having an anticoagulant action and a lipemia-clearing action, was used clinically, and the changes induced by the heparin administration were examined. The GI value was significantly improved. The body weight showed a tendency to decrease. Fibrinogen tended to be decreased. The urine volume was significantly increased. Histologic observations on the placenta gave the impression that fibrin deposition in the placental tissue had been decreased by heparin.

Diagnosis and treatment of acute cardiac tamponade by the subxiphoid approach.

Emergency cardiac surgery has been performed on 18 cases of acute cardiac tamponade whose etiologies were as follows: 11 cases of metastatic carcinoma, four cases of idiopathic pericarditis and three with other causes. In most cases, the chief complaint was dyspnea. In many cases, the cardiac silhouette of frontal chest X-ray films showed the shape of a water-filled ice-bag placed on a table. The electrocardiogram showed a low voltage and a flat T-wave in approximately half of the patients. In cases of an echo-free space 1 cm or larger on the M-mode echocardiogram, the average amount of pericardial fluid drained was 850 ml and in those in which the space was less than 1 cm, the average drained was 557 ml. The CT values were 9-40 for patients with malignant pericardial effusion and 20-22 for cases of idiopathic pericarditis. In general, pericardiocentesis was performed in almost all the patients with acute tamponade, but if the drainage was inadequate, the subxiphoid pericardial window procedure was performed under local anesthesia. Surgical invasion in this technique was minimal and the operative results proved effective. For the operation, we resected a 2 X 2 cm pericardial segment. Since two of the patients with malignant pericardial effusion developed postoperative reaccumulation, resection of a 4 X 4 cm segment in the future has been contemplated.
