Comparisons of tetracaine spinal anesthesia with clonidine or phenylephrine in normotensive and hypertensive humans.

To compare the hemodynamics and anesthetic effects of spinal tetracaine containing either clonidine or phenylephrine administered to normotensive or hypertensive patients, we studied 75 patients allocated to one of three groups, each of which included nine hypertensive patients: clonidine group (n = 25) received 2 mL of 0.5% tetracaine (10 mg) containing clonidine (0.15 mg); phenylephrine group (n = 25) received 2 mL of 0.5% tetracaine (10 mg) containing phenylephrine (3 mg); and control group (n = 25) received 2 mL of 0.5% tetracaine (10 mg) alone. Analgesic levels and the intensity of motor block obtained did not differ significantly among the three groups. However, the regression of sensory block (skin dermatome) was significantly slower in patients given clonidine (T9 +/- 3) or phenylephrine (T9 +/- 3) at 300 min after the injection than in those (L1 +/- 3) in the control group (P < 0.01). Between normotensive and hypertensive patients, there was no significant difference in sensory and motor block in the three groups. Mean blood pressure (MBP) in both hypertensive and normotensive patients given tetracaine-clonidine remained approximately 20% lower than that in the control group for as long as 7 h after the injection (P < 0.05). We conclude that the inclusion of clonidine can provide similar effects to that of phenylephrine with respect to prolongation of hyperbaric tetracaine spinal anesthesia, but could cause more hypotension, without bradycardia, in both normotensive and hypertensive patients for a prolonged time (i.e., 420 min).

Proportionality of protease activities in malignant cells to their metastatic potentials.

It has been suggested that the activities of type IV collagenase and/or ectopeptidases possessed by malignant cells are related to their metastatic potentials. In the present study, we examined the activities of three aminopeptidases, two serine proteases, as well as type IV collagenase, in three kinds of cell lines of malignant cells. The activities of the aminopeptidases and serine proteases, rather than of type IV collagenase, were found to be proportionate to the metastatic potentials of those cell lines. Such activities of aminopeptidases were effectively suppressed by the addition of low molecular weight inhibitors.

Case report: ciliated foregut cyst of the pancreas mimicking teratomatous tumour.

A ciliated cyst of foregut origin is often found in relation to the tracheobronchial tree and the upper gastrointestinal tract, when it is called bronchogenic cyst and enterogenous cyst, respectively. Cysts of this type rarely occur in the pancreas. We report a case of ciliated foregut cyst involving the pancreas, and mimicking teratomatous tumour on radiology.

Case report: the "aurora sign"--a new sonographic sign of pneumatosis cystoides intestinalis.

Pneumatosis cystoides intestinalis (PCI) is a rare condition characterized by multiple gas-filled cysts of varying size within the intestinal wall. Although the characteristic findings on plain abdominal radiograph, computed tomography (CT), and barium enema have been well described, the sonographic findings have not. We recently encountered a case of PCI in which abdominal sonography disclosed a unique finding. We were able to reproduce the finding experimentally and have coined it the "aurora sign".

Combination of ascorbic acid and methylprednisolone pulse therapy in the treatment of idiopathic thrombocytopenic purpura.

Ascorbic acid, reported in 1988 to be effective for idiopathic thrombocytopenic purpura (ITP), is an attractive drug because of its lack of toxicity. Further studies are necessary in order to improve its effectiveness without increasing secondary effects. We present a chronic ITP patient treated with a combination of ascorbic acid and methylprednisolone pulse (MP) therapy who was previously treated with MP therapy alone. The effect of this combination therapy seems to be better than MP therapy alone. This therapy is worth further examination as another therapeutic choice due to its fewer secondary effects than the usual regimen of corticosteroids, splenectomy, and other immunosuppressive drugs.

Pigmentary retinopathy with nephrotic syndrome, Ménétrier's disease, and diabetes mellitus.

A patient with pigmentary retinopathy, nephrotic syndrome, Ménétrier's disease, and diabetes mellitus is presented. Other complications were congestive heart failure, hypothyroidism, hypertension, and hypertriglyceridemia. Hypogenitalism was also suspected. Pigmentary retinopathy is known to associate with many systemic diseases, which are classified into several syndromes. This case superficially resembles Alström's disease due to the common characteristics of pigmentary retinopathy, diabetes mellitus, renal disease, and hypogenitalism. But clinically and histologically, there are distinct differences. To our knowledge, this association has never been reported.

Low expression of the deoxycytidine kinase (dCK) gene in a 1-beta-D-arabinofuranosylcytosine-resistant human leukemic cell line KY-Ra.

Molecular change of the deoxycytidine kinase (dCK) gene in a 1-beta-D-arabinofuranosylcytosine-resistant human leukemic cell line (KY-Ra) was investigated. KY-Ra showed the same restriction pattern of genomic DNA and the same nucleotide sequences of the dCK gene as the parental cell line. However, the amount of deoxycytidine kinase mRNA was markedly decreased in KY-Ra compared to the parental cell line. This is the first report showing that the down regulation of dCK gene expression may be affected by a different mechanism than mutation.

Suppression of splenic enzyme activities by administration of aminopeptidase N (CD13) inhibitors: relationship between actions in vivo and in vitro.

The enzymatic changes in murine spleen caused by the administration for 20 successive days of various inhibitors of aminopeptidase N (leucocyte antigen CD13) have been compared. When compared with the control (saline), most of the inhibitors significantly suppressed splenic enzyme activities including those of ectoenzymes. A multivariate study indicated that the in vivo effects of the inhibitors were closely related to their inhibitory actions in vitro.

Fibrosarcoma versus fibromatoses and cellular nodular fasciitis. A comparative study of their proliferative activity using proliferating cell nuclear antigen, DNA flow cytometry, and p53.

We analyzed the proliferative activities, immunoreactivity of the p53 protein, and aneuploidy in patients with benign and malignant fibrous lesions, including 19 with nodular fasciitis (cellular type) (6-88 years old, mean 42.9), 11 with abdominal fibromatoses (22-74 years old, mean 37.9), 13 with extraabdominal fibromatoses (2-38 years old, mean 19.5), and 23 with fibrosarcomas (adult type: 16-71 years old, mean 47.3; infantile type: 3 months to 9 years, mean 2.9) using immunohistochemistry to determine proliferating cell nuclear antigen (PC10) and p53 protein (CM1) as well as performing DNA flow cytometry. The proliferating cell nuclear antigen (PCNA) score was measured as the ratio of PCNA-positive nuclear size/total nuclear size determined by an image analysis computer system. The distribution pattern of the PCNA-positive cells was uneven in each instance of nodular fasciitis, in contrast to the distribution in abdominal fibromatosis, extraabdominal fibromatosis, and fibrosarcoma. Both fibrosarcoma (28.4 +/- 20.0) and nodular fasciitis (33.6 +/- 20.9) exhibited a larger value and a greater variation in the PCNA score than did either abdominal (13.5 +/- 14.5) or extraabdominal fibromatosis (19.9 +/- 21.5). Abdominal fibromatosis exhibited a smaller value and less variation in the score. In short, the PCNA score did not correlate with the malignant potential. The proliferative index (S + G2 + M fraction) in fibrosarcoma was significantly higher than in either nodular fasciitis or abdominal fibromatosis. Aneuploidy was detected in five cases (26%) of fibrosarcoma, while six (26%) fibrosarcomas showed p53 positivity. Furthermore, p53-positive patients had a worse survival (0.01 < p < 0.05), and p53 positivity correlated with the proliferative index (p < 0.01). In conclusion, the PCNA score simply indicates the proliferative activity independent of malignant potential. On the other hand, p53 positivity, proliferative index, and aneuploidy are all indicators of malignant potential in fibroblastic lesions, and p53 positivity may reflect a poor prognosis.

[Five cases of phosphorylase b kinase deficiency affecting muscle or liver: clinical symptoms and diagnosis].

Muscle, erythrocyte and/or leukocyte phosphorylase b kinase (PBK) activities were measured in 5 patients and in 2 families. The relation between phenotype and enzyme deficiency in clinically variable samples based on measurement of erythrocytes, leukocytes or muscle was described. Three patients showed the enzyme deficiency in liver and the disease was transmitted as an X-linked recessive trait. In these cases, our results suggested that carrier detection was more reliable with erythrocytes than leukocytes. Two patients showed enzyme deficiency in muscle, and the mode of inheritance was not clear. In these cases, muscle biopsies were necessary for diagnosis.

Seasonality, prevalence and pathogenicity of the gregarine Ascogregarina taiwanensis (Apicomplexa: Lecudinidae) in mosquitoes from Florida.

Aedes albopictus larvae collected in Gainesville, FL, were infected with the gregarine Ascogregarina taiwanensis. Natural prevalence varied from 68 to 100%. Eight mosquito species were tested in the laboratory for susceptibility to A. taiwanensis isolated from field-collected Ae. albopictus. Aedes aegypti, Aedes albopictus, and Aedes taeniorhynchus became 100% infected in the larval stage, whereas Aedes triseriatus was less susceptible; Culex quinquefasciatus, Culex nigripalpus, Culex territans, and Anopheles quadrimaculatus were not susceptible. Viable A. taiwanensis oocysts from adults were recovered from Ae. taeniorhynchus (30%) and Ae. albopictus (100%); no oocysts were produced in the other exposed hosts. Mortality induced by A. taiwanensis infection was low in all mosquitoes except Ae. taeniorhynchus. We conclude that A. taiwanensis has little short-term impact on the mortality of the 3 most common container-inhabiting mosquito species in Florida; however, the long-term impact on overall host population regulation has yet to be determined.

[The influence of the addition of epinephrine and sodium bicarbonate on the hemodynamics after epidural anesthesia].

We examined the effects of epinephrine and sodium bicarbonate in the hemodynamics after epidural anesthesia. The solution was prepared by adding 2 ml of either 7% sodium bicarbonate or saline to 1% lidocaine 20 ml with or without epinephrine (1:200,000) and we gave 3 ml of each solution in the epidural space of dogs anesthetized with halothane. Epinephrine and sodium bicarbonate addition exerted no significant influence on the hemodynamics except tendency to continuous hypotension. Although sodium bicarbonate addition tended to decrease heart rate, the decrease was not significant and the duration of hypotension was short.

[Anesthetic management in a patient with Beckwith-Wiedemann syndrome].

Beckwith-Wiedemann syndrome has three major symptoms, exomphalos, macroglossia and giantism. We report the anesthetic management of a 2-year-old male, weighing 16.1 kg and with a height of 94.6 cm, associated with bilateral retention testis. The anesthesia was maintained with oxygen, nitrous oxide, and sevoflurane. The surgery was completed safely without adverse events such as difficulty in tracheal intubation or refractory hypoglycemia. The anesthetic management of this syndrome requires: an assessment of difficulty in tracheal intubation and treatment, an understanding of the capacity of the patient's glucose metabolism, and assessment of complicated deformities. In particular, it is necessary to carry out a preoperative examination of the cardiovascular system.

Effects of host resistance and injury on the susceptibility of Aedes taeniorhynchus to mosquito iridescent virus.

An iridescent virus is found at low prevalence in population of Aedes taenirhynchus. Attempts at experimental transmission produced low levels of infection, regardless of the dosage applied. In a test for genetic resistance in colonized Ae. taeniorhynchus, the mean infection rates +/- SD for groups of randomly selected and sibling larvae were compared. The standard deviation of the sibling groups was not higher than the random groups (4.0 +/- 3.1% and 3.0 +/- 2.1%), rendering genetic resistance unlikely. Injury to the larvae by feeding silicon carbide fibers consistently caused higher infection rates (4.8 +/- 2.0% by virus alone and 17.5 +/- 5.3% by virus and fibers concurrently). Similar results were obtained for vertical transmission. These results support the hypothesis that this virus has no active means of penetration, invading only through random breaks in the cuticle or peritrophic membrane.

Comparison of bi-directional Fay, omni-directional, CDC, and duplex cone traps for sampling adult Aedes albopictus and Aedes aegypti in north Florida.

We compared the number of Aedes albopictus and Aedes aegypti females collected in CDC, duplex cone, bi-directional Fay, and new omni-directional traps in a series of trials in northern Florida during 1992 and 1993. Bi-directional Fay and omni-directional traps collected significantly more Ae. albopictus females than did the other traps tested. The bi-directional Fay trap collected significantly more Ae. aegypti females than did any other trap. The results of these studies indicate that these traps may be useful tools for sampling Ae. albopictus and Ae. aegypti adults.

The organization of the BCL6 gene.

The chromosomal translocation involving 3q27 has been recently described in B-cell malignancies, especially in diffuse large cell lymphomas. We have previously cloned the breakpoint cluster region of 3q27 designated as the BCL6 locus, previously known as BCL5, and subsequently cloned the cDNA for the BCL6 (we previously reported it as BCL5) gene encoding a novel Cys2-His2 zinc-finger protein, which locates adjacent to the breakpoints and is activated through the translocation. To elucidate whether rearrangements occur within the BCL6 gene, we characterized the genomic structure of the gene. The BCL6 gene encompasses about 26 kilobases (kb) and consists of nine exons. Translation start site is located in exon 3 and zinc-finger motif is distributed in the exons 6 to 9. We have identified at least two types of mRNA alternatively spliced, which contain or do not contain exon 2 of 134 bp coding for the 5' untranslated region. A large intron 1 of 9 kb is not efficiently spliced out, which might result in the creation of minor 10-12-kb transcripts observed in the Northern blot analysis in addition to major 3.8-kb transcripts. The breakpoints are clustered around the first exon, and the putative regulatory region of the BCL6 gene is removed through the translocation, leading to the over-expression of the gene.

[Comparative study of the drinking patterns of heavy-drinking and other college students].

In order to contribute to measures to solve the problems caused by college students who are heavy drinkers, a comparative study of their drinking patterns, as compared to the drinking patterns of other college students, was conducted. Significant differences were observed between heavy-drinking and other students in the following 14 instances, which include drinking frequency, attitude when offered a drink, coerced drinking, like or dislike of drinking, insatiable drinking, flushing or turning pale during drinking, attitude towards a doctor's advice to quit drinking, desire to quit drinking, cost of drinking, attitude towards someone who cannot drink, experience of morning drinking during the past year, experience of loss of consciousness due to drinking, lack of success in efforts to stop drinking, and opinion of those who gulp down drinks. Almost no significant differences were revealed between heavy drinkers and other college students in regard to the desire to drink after taking an examination, drinking on borrowed money, and acting surly or abusive after drinking. Relationships between the ratio of heavy-drinking students and club activities, place of residence and place of birth were also investigated. Results which implied relationships in these cases were found, but the sample was too small to detect significant differences. Further investigation is needed by increasing the sample numbers.
