[Nine cases of debrancher deficiency (glycogen storage disease type III) presenting muscle weakness--study on clinicobiochemical analysis].

In nine patients aged 3 to 45 years with glycogen storage disease type III (GSD III) presenting muscle weakness, the clinical manifestations and biochemical subtype-classifications based on organ specificity or enzymatic varieties of debrancher enzyme were analyzed. All the patients developed muscle weakness since childhood. Five patients who showed muscle weakness beginning from childhood became apparently progressive during adult life. Cardiac involvements were noticed in six patients. Eight patients were diagnosed as having type IIIa and one type IIId. Our results suggest that progressive muscle weakness and cardiac symptoms are not rare in GSD III patients, especially in the patients with enzyme deficiency in muscle tissue as well as liver. Hence we recommend to measure debrancher activity in muscle tissue in order to predict the clinical prognosis of the patients with GSD III.

[A successful staged complete repair in a neonate with coarctation of aorta, ventricular septal defect, and left ventricular outflow tract obstruction--a new palliative operation].

An eight day old neonate with coarctation of aorta (CoA), ventricular septal defect (VSD), and severe left ventricular outflow tract obstruction (LVOTO) underwent a successful staged repair following a new palliative operation, which comprised aortic arch reconstruction, PTFE bypass graft between main pulmonary artery and descending thoracic aorta, and pulmonary artery banding (PAB) distal to the graft. Insertion of the bypass graft was aimed to allow enough systemic blood flow and reduces after load of ventricles after PAB. The second staged definitive operation was performed 21 days after the first operation and the procedure comprised partial resection of posterioly deviated infundibular septum via VSD, patch closure of VSD, and division of the graft. No evidence of progression of LVOTO was noted until the second stage operation despite the tight PAB. This new palliative operation, which does not need aid of cardiopulmonary bypass, is particularly effective for those with LVOTO, and can be safely applied even to seriously ill neonate. Thus, for the staged repair in aortic arch obstructive disease with VSD and severe LVOTO, this procedure can be a good alternative for palliative operation.

Damage to respiratory epithelium by guinea-pig eosinophils stimulated with IgG-coated Sepharose beads.

BACKGROUND: There are no reports of respiratory epithelial damage induced by immunoglobulin-stimulated eosinophils. OBJECTIVE: We tried to induce damage to respiratory epithelium by guinea-pig (GP) eosinophils stimulated with guinea-pig IgG (GP-IgG)-coated Sepharose 4B beads. METHODS: GP tracheal epithelium was cultured together with GP eosinophils that had been collected from the peritoneal cavity, purified on a Percoll gradient, and stimulated with GP-IgG-coated beads (GP eosinophils:beads = 20:1). Damage to the epithelium was observed with an inverted microscope. RESULTS: After 24 h of culture with three 10(6) eosinophils, irregularity of the surface of the epithelium, desquamation, shedding of cilia, and abnormal beating of cilia were observed. This damage was first observed after 12 h of incubation, and was more severe at 24 h. No damage was found when beads coated with human serum albumin (HSA) were used. GP eosinophils stimulated with GP-IgG released significantly more EPO than those stimulated with HSA, at 6 and 24 h. Heparin and catalase partially inhibited the epithelial damage. O2-production by eosinophils was also enhanced with GP-IgG-coated beads. CONCLUSION: Both granule basic proteins and reactive oxygen radical may be responsible for epithelial damage, probably via an EPO + H2O2 + halide system. These results confirmed that stimulated eosinophils can damage respiratory epithelium.

[A case of advanced gastric cancer with multiple liver metastasis successfully treated with combination chemotherapy using UFT, CDDP and etoposide].

We reported a 66-year-old male patient with advanced gastric cancer accompanied by multiple liver metastasis, who responded to combination chemotherapy using UFT, CDDP and etoposide. The patient was administered three courses of 600 mg/body UFT po daily, 50 mg/body CDDP iv and 50 mg/body etoposide iv on days 1 and 8 every 4 weeks. As a result, both the primary and metastatic tumors decreased remarkably in size. Adverse reactions were leukocytopenia (Grade 2), thrombocytopenia (Grade 1) and nausea (Grade 1). he is alive 16 months after the beginning of therapy in a condition of partial response (PR). This combination therapy seemed to be effective for advanced gastric cancer.

[A study of the abuse of alcoholic beverage among college students by using the Adolescent Alcohol Involvement Scale (AAIS)].

I carried out the Adolescent Alcohol Involvement Scale (AAIS), which was developed for screening of alcohol abuse in youth. The subjects are 288 college students (74 boys, 213 girls and unknown 1). In AAIS, the score of 42 or above is defined as indicating alcohol misuse interfering with psychological functioning, social relation, or family living. In this study, students who marked 42 or above are 6 boys (8.1%) and 1 girls (0.5%). The scores of AAIS of boys have a tendency to distribute to a higher side than those of girls and the average scores of AAIS are 30.7 in boys and 26.8 in girls respectively (significant difference, p < 0.01). The average score of AAIS of good health condition students (27.1) was lower than those of bad health condition students (30.9) and normal condition students (28.7) respectively. The average score of AAIS of one's own house students (26.4) was lower than that of lodging house students (29.5). The average score of AAIS of the students belonging to club activity (28.3) was higher than that of the students not belonging to club activity (25.8). The average score of the students belonging to athletic club (29.0) was higher than those of the students belonging to cultural club (26.8) and the students not belonging to club activity (25.8) respectively. The orderliness of AAIS in selection limbs was generally reasonable but the nessity of replace of the selection limbs in the questionnaire seemed to be requested in some cases. The latter facts and the items in which the rates of no answer were high, requested the future investigations.

[A case of total cavopulmonary connection with preceding tricuspid valve closure in pulmonary atresia and intact ventricular septum, right ventricular-coronary artery fistulae, and coronary artery stenoses].

More than half of the patients with pulmonary atresia and intact ventricular septum (PA/IVS) are known to complicate right ventricular-coronary artery fistula (fistulae) and particularly those with coronary artery stenoses bear a high mortality rate and remain in a surgical challenge. A 4-year-old girl was first admitted to our hospital at 5 days of age and right ventriculogram revealed markedly hypoplastic tripartite ventricle and multiple fistulae through which both coronary arteries and aortic root were retrogradely opacified. Echocardiographically measured diameter of the tricuspid valve was 5 mm (Z value: 4). She underwent pulmonary valvotomy and central aorto-pulmonary (AP) shunt at 16 days of age. Repeat right ventriculogram at 1.9 years of age disclosed multiple stenoses in left anterior descending coronary artery (LAD) with proximal dilatation and tortuosity. Additional findings of left ventricular dysfunction (LVEF of 61.5%) and depressed ST segment in left precordial leads prompted us to proceed to the second palliation which comprised take-down of central AP shunt, bidirectional cavopulmonary shunt and closure of tricuspid and pulmonary valves. Catheterization at 3.4 years of age disclosed antegradely filled LAD with apparent relief of stenoses and improvement of LVEF to 68.9%. She underwent definitive repair of total cavopulmonary connection at 4.0 years of age and is leading a normal life 2 years after surgery. This experience draws us to conclude that tricuspid valve closure is a meaningful palliative procedure for PA/IVS with fistulae and coronary artery stenoses, provided that proximal segments of both coronary arteries remain intact.

[Histopathological changes and hyperresponsiveness in asthmatic airways: relationship to peak expiratory flow].

The application of bronchial biopsies in asthma has shown that even in daily asthma there is evidence of airway inflammation. These include inflammatory cells, mainly eosinophil, infiltration, epithelium damage, thickening of basement membrane and bronchial smooth muscle hypertrophy. Furthermore, there is increasing evidence that these inflammatory changes are associated with airway hyperresponsiveness, airflow limitation and respiratory symptoms. Our biopsy data also showed that the degree of eosinophil infiltration in the bronchial mucosa and the thickness of basement membrane significantly correlated with non-specific airway responsiveness. In addition, changes in PEF value over a period of 10 weeks in a patient with asthma, coincided well with those in sputum eosinophil count and sputum eosinophil cationic protein (ECP) level determined during the same period. These findings suggest that non-specific airway responsiveness and PEF may reflect the inflammatory status of the bronchi in asthmatics.

[PAF receptor antagonist in asthma therapy].

Platelet-activating factor (PAF), proposed as an important inflammatory mediator in asthma, reproduces several of the features of asthma, such as micro vascular leakage, mucus secretion, broncoconstriction, and possibly increased airway responsiveness. The recognition that PAF may be a central mediator in asthma has been a relatively recent event, so that those interested in asthma research may be unfamiliar with the chemistry, biochemistry and biology of this material. It seemed appropriate therefore to assemble a group of expert investigators to make presentations and to discuss the current status of PAF research with a specific bias toward asthma. In this way, it has been possible to focus upon PAF in asthma and to provide the necessary background for consideration of the effects of existing antiasthma drugs to suppress pulmonary responses to PAF. The capacity of existing antiasthma drugs to suppress pulmonary responses to PAF implies that this will be necessary feature of many of the newer antiasthma drugs and this knowledge allows a more balanced judgment of the prospects for PAF receptor antagonists in this disease.

Persistent c-fos expression in the brains of mice with chronic social stress.

The present study was conducted to demonstrate immunohistochemically, the sites of c-fos protein expression in the brains of mice subjected to acute and chronic social defeat stress. To induce social stress, mice were placed in situations of species-specific intermale aggression either only once or five times at 24 h intervals. Two hours after the single or fifth defeat stress, many c-fos immunoreactive neurons were observed in a variety of brain regions including the limbic system and sensory relay nuclei. The c-fos immunoreactive neurons in the brains of acute defeat mice decreased in number with time and the c-fos staining pattern of acute defeat mice became indistinguishable from that of normal control mice by 24 h after the single defeat stress. In contrast, chronic defeat stress induced persistent c-fos expression in the forebrain and brainstem even 24 h after the fifth defeat stress. In the forebrain of chronic defeat mice, the olfactory bulb, cingulate cortex, hippocampus, entire hypothalamus, septal nuclei and the amygdaloid complex, except for the central nucleus, were labeled intensely with c-fos antiserum. In the lower brainstem, nerve cells with c-fos immunoreactivity were seen mainly in ascending and descending sensory relay nuclei relevant to auditory and vestibular transmission, epicritic sensation (gracile and external cuneate nuclei), pain inhibition (central gray and raphe nuclei), and viscerosensory transmission (solitary tract nucleus). The differences in c-fos expression among the normal control, acute and chronic defeat mice were evaluated by an enumeration of the immunopositive neurons within each brain nucleus examined, and they were confirmed subsequently by statistical analysis. There was little c-fos expression in the nucleus putamen, lateral, ventral and posterior thalamic nuclei, pretectal nuclei, medial geniculate nucleus, red nucleus, substantia nigra, cerebellum, spinal cord, or cranial nerve nuclei. These findings suggest that chronic but not acute defeat stress causes persistent c-fos expression in more widespread brain regions than do any other stresses so far investigated. The present study may shed light on the central mechanisms by which behavioral abnormalities and/or chronic sociopsychological stress leads to the occurrence of abnormal behavior and/or psychosomatic disorders in experimental animals and humans.

[The expression of adhesion molecules on eosinophils from atopic and non-atopic subjects].

The accumulation of eosinophils in the airway is one of the characteristics seen in patients with chronic asthma. There is now considerable evidence that airway eosinophilia is associated with the development of late asthmatic response (LAR) and bronchial hyperresponsiveness. In this study, we observed PAF induced up-regulation of inducible adhesion molecules Mac-1 and LFA-1 beta on eosinophils of atopic patients compared with non-atopic and normal subjects. In conclusion, we found that adhesion molecule expression is different between atopic and non-atopic patients. Therefore, adhesion molecule expression is thought to be important in the prolonging and pathogenesis of allergic inflammation.

Effect of nipradilol, a beta-adrenergic blocker with vasodilating activity, on oxotremorine-induced tremor in mice.

The effect of nipradilol, a nonselective beta-adrenergic receptor blocker with nitroglycerin-like vasodilating activity, on oxotremorine-induced tremor was studied in mice. General tremor in mice was elicited by 0.5 mg/kg oxotremorine. The tremor was quantified using a capacitance transducer, then analyzed by a signal processor. The strength of the tremor was expressed in "points". The point values of the tremor (mean +/- SE) in control mice for 5 mg/kg (+/-)-propranolol, 2.5 mg/kg arotinolol, 0.5 mg/kg nipradilol, 1.0 mg/kg nipradilol and 2.5 mg/kg nipradilol were 87 +/- 16, 42 +/- 6, 38 +/- 6, 99 +/- 28, 28 +/- 6 and 31 +/- 7, respectively. The strength of the tremor was reduced by all beta-blockers. Although 1.0 mg/kg nipradilol significantly reduced the tremor, further inhibition of the tremor was not obtained with dosages up to 2.5 mg/kg of the drug. In conclusion, nipradilol was effective for suppressing oxotremorine-induced tremor, as were other beta-blockers.

An autopsy case of malignant lymphoma with a high serum CA 125 level occurring only in the brain and pericardium.

We report an autopsy case of a central nervous system (CNS) B-cell type lymphoma followed by cardiac tamponade. In this case, lymphoma cells which occurred primarily in the pericardium appear to metastasize to the brain and gradually increase in size in spite of control of the CNS lesion. The serum carbohydrate antigen 125 (CA 125) level was highly elevated and the autopsy findings showed the involvement of lymphoma only in the pericardium rather than the CNS, and the pericardium, not the tumor cells, was immunohistochemically positive for CA 125. Through this rare case, two things are suggested: first, the serum CA 125 level is elevated when B-cell type lymphoma infiltrates the coelomic epithelium; second, CA 125 is overproduced and secreted by the pericardium but not the lymphoma cells. Measurement of the serum CA 125 level may become a diagnostic marker for the infiltration of malignant lymphoma in the coelomic epithelium such as the pericardium, peritoneum or pleura.

Egg production by Strelkovimermis spiculatus (Nematoda:Mermithidae).

Egg production by the mermithid nematode Strelkovimermis spiculatus, a parasite of mosquitoes, was examined over a period of 34 days. Oviposition did not occur in the absence of males. Egg production was best when males were continuously present (6.4 x 10(3) +/- 0.9 x 10(3) eggs/female). Fewer eggs were produced when males were removed after 7 days (2.8 x 10(3) +/- 0.2 x 10(3) eggs/female) and oviposition partially recovered after males were returned 11 days later (4.4 x 10(3) +/- 0.5 x 10(3) eggs/female). The nematodes deposited substantially more eggs in sand (6.4 x 10(3) +/- 0.9 x 10(3)/female) than in water (1.9 x 10(3) +/- 0.3 x 10(3)/(female).

The murine BCL6 gene is induced in activated lymphocytes as an immediate early gene.

The chromosomal translocation involving 3q27 is often detected in human B-cell lymphomas, especially diffuse lymphomas with a large-cell component. The BCL6 gene has been isolated from the chromosomal breakpoint in these lymphomas. Here we cloned the murine BCL6 (mBCL6) cDNA from the muscle cDNA library using the human BCL6 (hBCL6) cDNA as a probe. The predicted amino acid sequence was 95% identical to that of hBCL6. It contains six repeats of the Krüppel-like zinc-finger motif that are completely identical to those of hBCL6, indicating that the BCL6 gene is well conserved between humans and mice. Expression of the mBCL6 gene was ubiquitously detected in adult mouse tissues including lymphatic organs. Furthermore, it was induced in lymphocytes activated with phorbol ester and Ca2+ ionophore within 30 min after stimulation. This induction was not inhibited by treatment of the cells with a protein synthesis inhibitor, cycloheximide. These results suggest that BCL6 plays a role in activated lymphocytes as an immediate early gene.

Osteosarcoma versus malignant fibrous histiocytoma of bone in patients older than 40 years. A clinicopathologic and immunohistochemical analysis with special reference to malignant fibrous histiocytoma-like osteosarcoma.

BACKGROUND: It is often difficult to discriminate between osteosarcoma and malignant fibrous histiocytoma (MFH) of bone, especially in older patients because of the clinical similarities, including the lytic radiologic appearance. A histologic analysis of MFH-like osteosarcoma, which closely resembles MFH of bone both clinically and radiologically, has not yet been conducted thoroughly, and therefore this issue remains controversial. METHODS: Using clinicopathologic and immunohistochemical techniques, the authors studied 24 cases of osteosarcoma arising in patients older than 40 years of age and compared them with 20 cases of MFH of bone from similarly aged patients. RESULTS: Radiography revealed that 68.2% of the osteosarcoma cases were predominantly lytic, whereas all cases of MFH of bone showed either a predominantly or purely lytic pattern. Histologically, osteosarcoma was subclassified as conventional osteoblastic (54.2%), MFH-like (29.2%) containing various amounts of tumor osteoid and/or bone in each of the cases, and conventional fibroblastic (4.2%), whereas all the cases of MFH of bone had a storiform-pleomorphic pattern. Immunohistochemically, no overexpression of p53 protein was found in MFH-like osteosarcoma, whereas it tended to occur more frequently in osteoblastic osteosarcoma (66.7%) and MFH of bone (50.0%). The Ki-67 labeling index was significantly lower in MFH-like osteosarcoma than in MFH of bone. The 5-year survival rate was 18.2% in patients with osteoblastic osteosarcoma, 66.7% in patients with MFH-like osteosarcoma, and 21.5% in patients with MFH of bone. A significant difference in the survival curve was observed between osteoblastic and MFH-like osteosarcoma. CONCLUSIONS: It is proposed that MFH-like osteosarcoma, which shows characteristically different clinical and histologic features from that of conventional osteosarcoma, thus may be considered a variant of osteosarcoma.

Platelet-activating factor and its structural analogues in the earthworm Eisenia foetida.

The earthworm Eisenia foetida was shown to contain large amounts of ether-containing phospholipids such as alkylacylglycerophosphocholine (61.3% of choline glycerophospholipids) and alkenylacylglycerophosphoethanolamine (66.0% of ethanolamine glycerophospholipids). We also found a substantial amount of ether-containing PAF-like lipid in this animal, its level being increased after the animal is injured. We showed evidence that this PAF-like lipid consists of PAF and PAF analogues containing short chain fatty acids other than acetic acid. Notably, a propionic acid-containing species but not PAF itself, is the most predominant species in this animal. We also confirmed that the earthworms contain enzyme activities involved in the synthesis of PAF and short chain fatty acid-containing PAF analogues. Interestingly, the acetyltransferase activity in earthworms is resistant to high concentrations of the substrate lysophospholipid. Thus, both the structure of the PAF-like lipid and the properties of the enzymes involved in the PAF-like lipid metabolism in the earthworms are somewhat different from those in mammalian tissues.
