[Surgical management in synchronous bilateral renal cell carcinoma].

Six patients with synchronous bilateral renal cell carcinoma were treated surgically over a 7-year period in our department. They were all males and the mean age was 60.3 years. They comprised 7.1% of all patients with renal cell carcinoma encountered during the same period. In four out of the six cases, radical nephrectomy for the larger tumor plus partial nephrectomy for contralateral kidney was performed simultaneously. In the remaining two cases, bilateral partial nephrectomy was performed simultaneously or as separate procedures. Two patients required chronic hemodialysis and died of cardio-pulmonary insufficiency on the 70th and 75th day. One patient, who underwent bilateral partial nephrectomy with incomplete tumor removal, subsequently died of metastatic disease at 27 months. Although one of them required transient hemodialysis, the remaining three patients were alive and disease-free 84, 42, and 17 months after operation, without evidence of tumor. This series suggests that partial nephrectomy is an appropriate option in the management of selected cases of bilateral renal cell carcinoma.

[Clinical efficacy of recombinant interleukin-2 for renal cell carcinoma and the effect of blood transfusion upon the immune response of these patients].

We studied the efficacy and safety of recombinant interleukin-2 (rIL-2: S-6820) for the treatment of renal cell carcinoma as well as the effect of blood transfusion upon the immune response of these patients. Among 14 cases of renal cell carcinoma treated by i.v. infusion of rIL-2, a partial response (PR) was achieved in one patient, 10 patients had no change, and in 3 had the disease progressed. The overall efficacy rate was 7.1%. However, the rate increased to 12.5% in cases with pulmonary metastases and to 14.3% in cases without any blood transfusion within a year before treatment with rIL-2. No severe side effects were observed, except for central nervous system disturbance in one case. During the rIL-2 therapy, LAK activity was suppressed in the transfused patients. On the other hand, NK activity was augmented in transfused patients to the same degree as in non-transfused cases. No significant changes of lymphocyte count and the subsets of peripheral blood lymphocytes were observed in either group treated with rIL-2. Anemia and radical nephrectomy did not affect the immune response in these patient. Thus, it appeared that blood transfusion altered the immune response in patients treated with rIL-2. However, it could not be concluded that transfusion definitely had an adverse effect on the clinical efficacy of rIL-2 for renal cell carcinoma.

Utilization of anti-acinar cell monoclonal antibodies in the purification of rat and canine islets.

We have developed two immunological methods of islet purification using anti-acinar cell monoclonal antibodies (McAb). Pancreatic digestion with collagenase followed by depletion of acinar cells with McAb and complement or magnetic immunomicrospheres (MIMS) yields large numbers of intact islets. We compared the islets thus obtained with either handpicked (HP) islets, or islets separated on density gradients alone, for yield, purity, in vitro in sulin secretory capacities, and in vivo functional viability. Exposure of canine acinar tissue to the cytotoxic McAb resulted in a seven-fold enrichment in islet concentration, with preparations of 65% to 87% purity. Islet yield, however, was impaired by the procedure of McAb treatment, reducing the average yield from the control digestate of 40,000 to 60,000 islets/pancreas by 40% to 45%. Using the MIMS method in the rat model, the islet yield was 73% that obtained by the HP method (378 +/- 8 vs 519 +/- 31 per pancreas). The purity of the MIMS isolated islets was 84 +/- 1.9%, ranging from 75% to 95%. Analysis of the complement and MIMS treatments by in vitro static glucose tests and the capacity to restore normoglycemia after isografting in streptozotocin-induced diabetic rats indicates that the functional integrity of the islets was not affected by the purification process.

[Antitumor activity of UFT against murine renal cell carcinoma: a study on the suppression tumor metastases].

Experimental chemotherapy with UFT was performed against murine renal cell carcinoma (Renca) of spontaneous origin in BALB/c mice and the antitumor activity of UFT was compared with that of 5-FU. By oral administration started from the day after inoculation of Renca cells under the capsule of a kidney, the growth of tumor and formation of the spontaneous metastases to the lymph nodes, lung, spleen, and abdominal wall were inhibited significantly. The UFT or 5-FU treatment started on the 8th day after tumor inoculation also extended the survival time of the tumor-bearing mice and the anti-tumor effect of UFT was more marked than 5-FU. However, UFT treatment started on the 15th day did not prolong the survival time of Renca-bearing mice. From these results, UFT therapy seems to be beneficial for prevention of metastases after nephrectomy in the patients with renal cell carcinoma.

[A case of renal cell carcinoma with polymyositis].

We report a case of renal cell carcinoma with polymyositis. A 62-year-old female with a diagnosis of polymyositis was referred to our department because of the right renal mass discovered by computed tomography. An ultrasonography and renal angiography showed a solid and hypovascular area at the middle pole of the right kidney. A fine needle aspiration biopsy of the mass was performed and the cytology showed a papillary adenocarcinoma. Therefore, the right radical nephrectomy was carried out and her myopathic symptoms were slightly improved postoperatively. The patient has remained well without recurrence or metastasis for two years.

[Evaluation of antihypertensive therapy by ambulatory blood pressure monitoring and establishment of the level of antihypertensive goal on the circadian rhythm of blood pressure].

We have developed a new method for the evaluation of antihypertensive therapy on the circadian rhythm of blood pressure and attempted to determine the indications for antihypertensive therapy and the level of antihypertensive goal. Blood pressures were measured for 24 hours by the use of ambulatory blood pressure monitoring using 630 (ABPM-630) in 50 normotensives, 50 untreated hypertensives and 50 hypertensives undertreatment with various antihypertensive drugs (110 males and 40 females, with a mean age of 53.4 +/- 13.3 yrs). Blood pressure profiles were prepared for determination of the hyperbaric and hypobaric indexes. According to the WHO's definitions for blood pressure, the hyperbaric index was defined as the area above 140 mmHg in systolic blood pressure or 90 mmHg in diastolic blood pressure, and the hypobaric index, as the area below 100 mmHg or 60 mmHg, respectively. The criteria of the hypobaric index was obtained from the mean basal blood pressure (the lowest blood pressure during sleep) of the 50 normotensives. The mean hyperbaric index of the 50 normotensives was 20.4 +/- 40.2/5.5 +/- 15.3 (systole/diastole) mmHg.hour/day and the mean hypobaric index, 12.2 +/- 22.5/9.0 +/- 24.0 mmHg.hour/day. The 50 untreated hypertensives showed a mean hyperbaric index of 281.8 +/- 197.0/156.0 +/- 126.1 mmHg.hour/day and a mean hypobaric index of 0.1 +/- 0.6/0.3 +/- 1.5 mmHg.hour/day. Comparison of the indexes before and after treatment with various antihypertensives showed that a decrease in the hyperbaric index without an increase in the hypobaric index was the most optimal reduction of blood pressure.(ABSTRACT TRUNCATED AT 250 WORDS)

Use of lectins for detection of glycoconjugate changes in mucous epithelium of the chicken proventriculus.

The lectin binding pattern in the mucous epithelium of the chicken proventriculus was studied by using a peroxidase labeling method and correlated procedure. The nature of glycoconjugates in the mucous epithelium was found to change during the upward cell migration from deep to superficial parts. The superficial mucous epithelium (plica) was found to contain predominantly neutral glycoconjugates with N-acetyl-D-glucosamine residues and terminal galactose-N-acetylgalactosamine disaccharides whereas the deep epithelium (sulcus) contained acidic sulfated glycoconjugates with terminal sialic acid-galactose dimer. A continuous intraluminal mucous layer over the surface epithelium was found to contain a mixture of glycoconjugates secreted from both the plica and sulcus mucous cells. The data suggests that the changes in glycoconjugates of epithelial cells were closely correlated with cellular maturation.

[A histological evaluation of the therapeutic effects and the clinical findings in prostatic carcinomas].

A histological evaluation of the effects of therapy has been conducted on 22 prostatic carcinoma cases, according to the criterion proposed by the committee to establish the general rule for clinical and pathological studies judging the effects of therapy in cases of prostatic cancer. Studied in particular was the relation between the histological effects of therapy and the clinical course. Marked therapeutic effects were observed in cases of low grade tumors. Further, the patients' prognosis tended to be affected by the histological grade rather than by the histological effect of the treatment. The course of the serum total acid phosphatase levels tended to correlate with the histological effect of the treatment.

[Combined effect of intraperitoneally administered OK-432 and recombinant interleukin (rIL-2) against mouse tumors].

Combined effect of the streptococcal preparation, OK-432 and rIL-2, both administered intraperitoneally, was examined against Meth-A tumor, a syngeneic tumor of inbred BALB/c mice and an analysis of the effector cells also was performed. The combined treatment resulted in significant in vitro inhibition of tumor growth and increase in the length of survival of the Meth-A bearing mice. The spleen cells obtained from Meth-A bearing mice treated with both OK-432 and rIL-2 on the 12th day after the tumor transplantation were capable of lysing syngeneic Meth-A tumor cells in addition to NK-sensitive allogenic YAC-1 cells and LAK-sensitive EL-4 cells in vitro in a 4 hour 51Cr-release assay. Neither spleen cells obtained from the tumor bearing mice treated with OK-432 or rIL-2 alone, nor those obtained from normal mice treated with both agents, showed cytotoxic activity against Meth-A cells. The in vitro growth of NK-resistant allogenic P-388 was not inhibited by the spleen cells obtained from any treated mouse, with or without Meth-A tumor. In order to determine the effector subpopulation against Meth-A cells, the spleen cells obtained from the tumor-bearing mice treated with OK-432 and rIL-2 on the 12th day after tumor transplantation, were treated with either anti-Thy-1, 2 antibody plus complement, anti-asialo GM1 serum plus complement, or by adherence to a plastic plate or nylon wool column.(ABSTRACT TRUNCATED AT 250 WORDS)

Quantitative analysis of outflow pathway of corpora cavernosa by pressure flow technique.

Using a pressure flow technique, quantitative analysis of the physiological characteristics of the outflow pathway of the corpus cavernosum was carried out in 19 male dogs weighing 7.5 to 23.0 kg. Pressure flow curves were made on dogs whose pelvic nerve was stimulated electrically and on dogs left unstimulated. When a cyclical change in saline perfusion rate was applied without nerve stimulation, the variable of the intracorporeal pressure showed a large hysteretic loop, indicating that the resistance of the outflow canals to flow was altered by the distension of the sinusoidal space. In dogs whose pelvic nerve was stimulated, the pressure flow curves shifted to the left side in comparison with the outward phase of the pressure flow curve of animals without pelvic nerve stimulation, and this curve piled on the returning phase. No hysteretic relation was observed between the outward and returning phase of the pressure flow curve with pelvic nerve stimulation, but in the detailed analysis, in which the % flow rate was used instead of actual flow rate of saline perfusion, a small hysteretic loop based on the difference of the elasticity of the outlet canals was found. The distension of the corpora cavernosa and the pelvic nerve electrostimulation probably act as the triggers of the same occlusive mechanism in the outflow pathway. The percentage decrease in the blood flow in the outflow canal of the corpus cavernosum induced by the distension of the sinusoidal space or by the pelvic nerve electrostimulation was 69.6 +/- 14.4% (mean +/- SD).

[The clinical effectiveness of OFLX in the treatment of chlamydial pneumonia].

We treated three patients of chlamydial pneumonia with OFLX, two patients were diagnosed as psittacosis and one as pneumonia associated with TWAR strain by serologic test. Three hundred mg of OFLX was orally administered three times per day and the duration of treatment was from seven to fourteen days, and we compared the clinical effectiveness of OFLX in three cases (mild: 1, moderate: 2) with that of MINO in eleven cases (mild: 3, moderate: 8), who were administered two hundred mg of MINO two times per day orally or intravenously. We isolated C. psittaci from three pet birds including case 2 and the in vitro activity of OFLX and MINO against three strains of C. psittaci was determined. Clinical effectiveness were observed and obtained results were as follows. 1) In OFLX group, three patients judged as "Good", and in MINO, one patient as "Excellent", ten as "Good". 2) The duration of pneumonic shadow was 9.33 +/- 3.21 in OFLX group, 10.3 +/- 3.50 in MINO group, and there was no significant difference between both groups. 3) The in vitro activity of OFLX and MINO against 3 strains was 0.78-1.56 micrograms/ml and 0.025 microgram/ml, respectively. From these results, it was concluded that OFLX was considered to be a useful antichlamydial agent in the treatment of mild or moderate cases of chlamydial pneumonia.

Histochemistry of glycoconjugates in ovarian follicles of the adult house musk shrew, Suncus murinus.

In ovarian follicles of the adult house musk shrew, Suncus murinus, glycoconjugates have been studied by means of light- and electron-microscopic histochemistry. The results obtained are that: (1) glycoconjugates of the zona pellucida of oocytes are provided with vicinal diol and acidic groupings, and sialic acid-galactose dimer, alpha-D-mannose, alpha-D-glucose, beta-D-galactose, N-acetyl-D-glucosamine and alpha-L-fucose residues; (2) glycoconjugates of the intercellular matrix of the granulosa and theca folliculi are comparable in histochemical properties to those of the zona pellucida, except for the relatively smaller amount of vicinal diol and acidic groupings, and (3) the zona pellucida can be divided into outer and inner layers by the binding degrees of the lectins used. The possible histophysiological significances of all these glycoconjugates are discussed with special reference to the particular ovarian follicular structures.

Antigenic relationship between streptococcal preparation OK-432 and tumors and its effect on immunotherapy with OK-432 in patients with superficial bladder tumor.

The cross-antigenicity between streptococcus, Su strain (OK-432), and rat bladder cancer BC-47 was studied by the immunocytochemistry. Both anti-Su (OK-432) and anti-BC-47 reacted with both homo- and heterogenous antigens. The specificity of the PAP reactions using these sera was confirmed by absorption study of the first antibodies. But, OK-432 did not share with normal rat tissues. Furthermore, the presence of common antigens between OK-432 and animal tumors where OK-432 was effective was indicated by the PAP studies, but OK-432 did not share with tumors where OK-432 was ineffective. By the PAP studies using antigroup A streptococcus-specific C-polysaccharide and antigroup-specific sera, it was suggested that the determinant of the cross-antigenicity was group A-specific C-polysaccharide. The common antigens between OK-432 and human urogenital cancers were also identified by the PAP study. Then, local immunotherapy with OK-432 was carried out in 38 patients with superficial bladder tumor and the tumors were eliminated in 23.7%. In 77.8% of the complete response and in 10.3% of the no change, the tumor tissues demonstrated antigens common between OK-432 and tissues. From these results, it was concluded that common antigens which OK-432 shared with tumors could favorably augment host defense against human bladder tumors, the same as animal tumors.

Affinity resins for the purification of opioid-binding materials.

Two new affinity resins for the purification of opioid-binding materials were prepared. One was AH-Sepharose coupled with [D-Ala2, D-Leu5]enkephalin and the other was AF-Amino Toyopearl with [D-Ala2, Leu5]enkephalin. Solubilized-opioid receptors from rat brain were treated with these affinity resins and the materials with opioid-binding activities were purified. On sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis, the purified materials showed one major band with a molecular weight of 62000-64000. The results suggested that the prepared resins are useful tools for the purification of opioid receptors.

Synthetic studies on diuretics. 5-(3,3-N,S-substituted-2-propenoyl)-2,3-dihydro-2- benzo[b]furancarboxylic acids.

6,7-Dichloro-2,3-dihydro-2-benzo[b]furancarboxylic acid derivatives having a 3,3-N,S-disubstituted-2-propenoyl group at the 5-position were prepared by alkylation of 5-(thiocarbamoyl)acetyl derivatives of the 2,3-dihydro-2-benzo[b]furancarboxylic acid ester or by acetal exchange reaction of 5-[3,3-bis(alkylthio)-2-propenoyl] derivatives. Synthesis of 5-[4 and/or 5-(di)substituted-4-thiazolin-2-ylidene]acetyl-2,3- dihydro-2-benzo[b]furancarboxylic acids was also achieved by the reaction of 2-halo-1-methoxyethyl isothiocyanate with the 5-acetyl derivative in the presence of base or through sulfide contraction of 2-[[6,7-dichloro-2-methoxycarbonyl-2,3-dihydrobenzo[b]furan-5-yl) carbonyl)-methylthio]thiazolium bromide. Some of the compounds which were synthesized showed potent natriuretic activities in rats and mice. The structure-activity relationship is also discussed.

[Two cases of metastatic renal tumor].

Two cases of metastatic renal tumor are reported, one in a 78-year-old male who had undergone total gastrectomy for gastric carcinoma, and the other in a 45-year-old female who had undergone hysterectomy for cervical carcinoma of the uterus. The chief complaint was flank pain and nephrectomy was performed in both cases. Histopathological examination of the removed specimen revealed metastatic renal tumor the primary site of which was probably the stomach in the first case and cervix uteri in the second case. The literature on metastatic renal tumors in Japan is reviewed.

[A clinical evaluation of AMS 700 penile prosthesis in the treatment of impotence].

The AMS 700TM & 700 CXTM penile prosthesis was implanted in 34 organic and mixed-type impotent patients between November, 1984 and July, 1988. The patients' ages ranged between 32 and 70, with an average age of 57.2. Twelve weeks after implantation, the prosthesis was evaluated in terms of patient satisfaction with sexual intercourse and postoperative complications. Twenty three patients (67.7%) were highly satisfied, 7 (20.3%) satisfied, 3 (8.8%) dissatisfied and 1 patient worsened (2.9%). Severe complication was found in two cases, both of them being complicated by infection; as a result the prosthesis was removed. Utility of the prosthesis, e.g. effectiveness and safety was; high utility in 26 cases (76.5%), moderate utility in 7 cases (20.6%), and no favorable in one case (2.9%). Thus, overall effectiveness and safety of the AMS 700 penile prosthesis, with its excellent cosmetic appearance upon implantation, were demonstrated by the above findings.
