Maintenance of the life cycle of Echinostoma trivolvis (Trematoda) in dexamethasone-treated ICR mice and laboratory-raised Helisoma trivolvis (Gastropoda).

Echinostoma trivolvis is a ubiquitous 37-collar-spined echinostome found in aquatic birds and mammals and in the planorbid snail Helisoma trivolvis. This echinostome has not been cycled continuously in the laboratory. The present report provides details on the continuous life cycle of E. trivolvis in dexamethasone-treated ICR mice and laboratory-raised H. trivolvis snails. Previous attempts to obtain patent adult of E. trivolvis in mice hosts failed because of worm injection within 2 weeks of infection. ICR mice infected with encysted metacercariae and injected with 2 mg/kg dexamethasone daily for 28 days yielded gravid worms that produced 250-500 eggs/worms at 21 and 28 days postinfection (p.i.). Miracidia derived from these eggs or eggs containing fully developed miracidia were capable of infecting 3- to 5-mm shell-diameter, laboratory-reared H. trivolvis snails. These snails released cercariae by 35 days p.i. Cercariae encysted in the kidney-pericardium of the snails. Encysted metacercariae could be excysted in vitro in an alkaline trypsin-bile salts medium or in vivo when fed to domestic chicks.

Venous drainage of the radial forearm and anterior tibial reverse flow flaps: anatomical and radiographic perfusion studies.

The anatomy of the venous system in the pedicles of the radial forearm and anterior tibial reverse flow flaps was investigated in fresh cadavers (n = 6) and their drainage pathways were observed in a radiographic perfusion study of the veins of 2 radial forearm and 2 anterior tibial pedicles. The venous system was found to consist mainly of three types of veins: venae comitantes, communicating veins between the venae comitantes, and vasa vasorum. Valves were identified in all three types of vein. In the venous system, including the vasa vasorum, there was no route of reverse flow that did not pass directly through valves. Valve incompetence was demonstrated in the radiographic perfusion study. However, valve incompetence did not occur in all valves and a difference in valve resistance against reverse flow pressure existed. Veins with relatively weak valve resistance became the drainage pathway. A possible cause of the difference in valve resistance was differences in the anatomy of the valve sinuses.

Circadian variation of QT interval dispersion: correlation with heart rate variability.

The circadian variation of QT interval dispersion and its correlation with heart rate variability (HRV) was examined in 17 normal subjects by using 24-hour recordings of three-lead electrocardiograms. Measurements of HRV, R-R intervals, and QT intervals were made for the first 6 minutes of each hour over a 24-hour period. Spectral analysis of HRV yielded low-frequency power (LF) (0.04-0.15 Hz), high-frequency power (HF) (0.15-0.40 Hz), and the ratio of LF to HF (LF/HF). A rate-corrected QT interval (QTc) was calculated by Bazett's formula, and QT and QTc dispersion was defined as the difference between the maximum and minimum values in any two leads. High-frequency power and QT interval were greater at night than during the day: conversely, LF/HF and dispersion of QT and QTc were greater during the day. The QTc interval remained virtually unchanged throughout the 24-hour period. The dispersion of QTc showed a significant negative correlation with HF and a significant positive correlation with LF/HF. The results suggest that an increased sympathetic tone or a decreased vagal tone increases QT dispersion in healthy subjects.

Aneurysm of aberrant right subclavian [corrected] artery arising from diverticulum of Kommerell. Report of a case with tracheal compression.

A 74-year-old woman presented with severe dyspnea without dysphagia. Computed tomographic scans and Digital subtraction angiography revealed the left aortic arch with an aberrant right subclavian artery arising from the Kommerell's diverticulum and tracheal compression. The aortic arch and the Kommerell's diverticulum were aneurysmal and were responsible for this compression. Surgical relief was accomplished by replacement of the aortic arch and reconstruction of the four brachiocephalic vessels with vascular prostheses through a median sternotomy incision extending into the right supraclavicular region.

The expulsion of Echinostoma trivolvis: worm kinetics and intestinal reactions in C3H/HeN mice treated with dexamethasone.

C3H/HeN mice were each infected with 40 Echinostoma trivolvis metacercarial cysts on day 0, given intramuscular injections of dexamethasone (DEX) daily for 30 days, and necropsied on days 5, 8, 12, 15, 20 and 30 p. i. Control mice were each infected with 40 echinostome cysts on day 0, but were not treated with DEX and necropsied on the same days as the DEX-treated mice. DEX treatment caused an inhibition of worm expulsion and suppressed the increase in goblet cell numbers that peaked around day 12 p. i. in the untreated control mice. Increase in the number of mucosal mast cells and eosinophils in the control mice that peaked around day 15 and 12 p. i., respectively, was also suppressed by the DEX treatment. The mean body area of the worms from the DEX-treated mice was about the same as that of the control worms on day 5 p. i., and then significantly greater than the control worms on days 8 and 12 p. i. The worms in the treated mice continued to grow until the end of the experiment, on day 30 p. i. Enzyme-linked immunosorbent assay (ELISA) showed that serum IgM from the treated and control mice increased from day 12 p. i., peaked on day 15 p. i., and then decreased. An IgM titre of the treated mice was slightly higher than that of the controls. No marked rise in IgG and IgA titres occurred throughout the experiment in both groups.

Dissociation of Oct-1 from the nuclear peripheral structure induces the cellular aging-associated collagenase gene expression.

The cellular aging-associated transcriptional repressor that we previously named as Orpheus was identical to Oct-1, a member of the POU domain family. Oct-1 represses the collagenase gene, one of the cellular aging-associated genes, by interacting with an AT-rich cis-element in the upstream of the gene in preimmortalized cells at earlier population-doubling levels and in immortalized cells. In these stages of cells, considerable fractions of the Oct-1 protein were prominently localized in the nuclear periphery and colocalized with lamin B. During the cellular aging process, however, this subspecies of Oct-1 disappeared from the nuclear periphery. The cells lacking the nuclear peripheral Oct-1 protein exhibited strong collagenase expression and carried typical senescent morphologies. Concomitantly, the binding activity and the amount of nuclear Oct-1 protein were reduced in the aging process and resumed after immortalization. However, the whole cellular amounts of Oct-1 protein were not significantly changed during either process. Thus, the cellular aging-associated genes including the collagenase gene seemed to be derepressed by the dissociation of Oct-1 protein from the nuclear peripheral structure. Oct-1 may form a transcriptional repressive apparatus by anchoring nuclear matrix attachment regions onto the nuclear lamina in the nuclear periphery.

Alternative translation initiation generates acyl-CoA synthetase 3 isoforms with heterogeneous amino termini.

ACS3 is a recently identified acyl-CoA synthetase (ACS) isozyme that preferentially utilizes laurate, myristate, arachidonate, and eicosapentaenoate among saturated and unsaturated long chain fatty acids. The ACS3 purified from COS cells transfected with the ACS3 cDNA was separated by SDS-PAGE into two major forms of 79 and 80 kDa. We report here that alternative translation initiation from ACS3 mRNA gives rise to these two isoforms of ACS3. In vitro mutagenesis of the ACS3 cDNA revealed that the translation of the 80-kDa and 79-kDa isoforms started from the first and second in-frame AUGs, respectively. The two isoforms of ACS3 expressed in COS cells exhibited similar levels of ACS activities toward palmitate and myristate. Immunocytochemistry of intact COS cells transfected with various ACS3 expression vectors suggested that the two forms are localized in the extranuclear compartment, where they exhibit a reticular pattern. In rat cerebrum, the 80-kDa isoform of ACS3 was detected mainly in the microsomal fraction. Only a trace amount of the 79-kDa isoform was detected in rat cerebrum, whereas both forms were detected in rat glioma cell line KEG1 cells.

Three-dimensional reconstruction of cutaneous arteries of the back by computer graphic imaging.

Computer graphics using a personal computer were introduced to demonstrate the three-dimensional architecture of cutaneous arteries of the back in humans: the direct cutaneous artery of the circumflex scapular artery and the perforating cutaneous branches and musculocutaneous perforators of the superficial cervical, posterior intercostal, and thoracodorsal arteries. The entire thickness of the skin and underlying subcutaneous tissue over the deep fascia was obtained from five fresh cadavers injected systemically with lead oxide-gelatin mixture, and the integument was divided into the three layers; the skin, superficial adipofascial layer with the superficial fascia, and the deep adipofascial layer. Computer graphic reconstruction of the cutaneous arteries was performed using data based on angiograms of the three layers. The computer graphic depictions demonstrated in which layer cutaneous arteries branched and how the vessels were running in each layer by displaying each layer in different colors: red, yellow, and blue. Knowledge of the three-dimensional structure and architecture of the cutaneous arteries will aid in the development of flaps that use a portion of the skin and subcutaneous adipofascial tissue.

Immunotherapy with husband's leukocytes to primary habitual aborters and autoantibodies in their sera.

OBJECTIVE: This study was performed to determine whether primary habitual aborters have an autoimmunological tendency and whether immunotherapy with leukocytes induces autoantibodies in their sera. METHODS: We measured the levels of antiphospholipid antibodies (APAs) and antinuclear antibodies (ANAs) in the sera of 65 primary habitual aborters. Among them, 8 primary habitual aborters received immunotherapy with their respective husband's leukocytes; these 8 patients were followed up for autoantibodies in their sera before immunotherapy, at the time of pregnancy permission, during the first trimester of pregnancy, at delivery, and postpartum. RESULTS: Sixty-nine percent of the 65 primary habitual aborters were positive for IgG or IgM antibodies to at least 1 of 6 phospholipids, and 29% of them were positive for ANAs. Although the pregnancy outcomes of the 8 primary habitual aborters after immunotherapy were good, their APAs and ANAs converted to positive after the immunotherapy. CONCLUSION: Immunotherapy with leukocytes should be performed after checking and confirming the absence of autoantibodies in the sera of a habitual aborter.

Characterization of macrophages isolated from the maternal surface of human term placenta by a new method of urokinase treatment.

OBJECTIVES: This study was performed to establish a new method for isolating macrophages from the maternal surface of human term placenta by urokinase treatment and to characterize their immunological functions. METHODS: Macrophages were recovered from the maternal surface of 6 human term placentas by urokinase treatment, adherence to plastic, and density gradient centrifugation using Percoll. The cells retrieved were tested for their surface markers by immunofluorescent staining. Their antigen-presenting capacity was examined by use of a mixed lymphocytes culture (MLC) test. In 2 cases, human leucocyte antigen (HLA) typing was performed. RESULTS: An average of 4 x 10(6) macrophage-like cells were obtained per whole placenta. More than 90% of them were positive for CD14, HLA-DR, and DQ surface antigens. An MLC test revealed that they had antigen-presenting capacity. Of the 6 cases, 2 showed positive MLC test results with maternal peripheral-blood mononuclear cells. In 1 of the 2 cases, macrophages with paternal HLA phenotypes were identified. CONCLUSION: The urokinase treatment is a new useful method for isolating macrophages from the maternal surface of human term placenta. Using this method, we obtained macrophages of paternal HLA phenotypes (fetal origin) that might have migrated to the maternal surface of human term placenta.

[Aortic arch surgery in the elderly].

A consecutive series of 16 patients over 75 years old who underwent aortic arch operations with hypothermic selective cerebral perfusion from 1994 to 1997 was analyzed. These patients (group O) was compared with 18 patients aged from 70 to 74-year-old (group M) and 38 patients under the age of 69 (group Y) who underwent aortic arch operation during the same period. There were 5 males and 11 females with a mean age of 77.8 years (range 75 to 86) in group O. Twelve patients were in aortic dissection and 4 patients were in atherosclerotic aneurysms. There were 5 hospital deaths (31.2%) in group O, 2 (11.1%) in group M and 2 (5.3%) in group Y. There was significant difference in hospital mortality rate between group O and group Y. Cerebral complication occurred in 2 patients (12.5%) and postoperative respiratory failure occurred in 7 (47%) in group O. Aortic arch operations for the elderly patients can be performed with increased but operative mortality in the elderly patients remains high. In this study, we discussed that influence of risk factors on the operative result and operative indication in the elderly patients.

[An observation on tuberculosis associated with HIV infection in Japan].

OBJECTIVE: To observe the reported cases of tuberculosis (TB) with HIV infection in Japan, in terms of their main clinical features and related factors. METHODS: A voluntary reporting network has been organized by the authors who are specialists of TB or respiratory medicine in tuberculosis institutions located roughly all over the country. The members have been encouraged to report not only their own cases but cases seen by their friends or in other institutions. RESULTS: By the end of 1996, a total of 71 cases have been reported of which 59 were TB and 12 NTM cases. Nationality of the cases were; Japan 48, Other Asian countries 16, Others 7. All of the NTM cases were Japanese. 30% of the cases were aged less than 30 years, 24% were thirties, 24% forties, 17% fifties and 6% were those aged 60 years or older. The cases were clearly younger than the TB cases in the national TB registry, and older than HIV-infected persons as known from the HIV surveillance system. 97% of the TB cases were bacteriologically confirmed cases. Eight of NTM cases were positive for MAC, others for M.kansasii. 42% of the cases had extra-pulmonary disease, including disseminated infections seen among 19%. Of TB cases 25% were excreting bacilli resistant to any of the anti-TB drugs which was higher than in the case of general TB population (10-15%). 11% of TB cases had past history of TB treatment. The cases had severe immunological impairment, 79% of the cases having CD4+ cell count less than 100. The route of HIV infection were; 51% heterosexual, 13% homosexual, 13% through blood preparations, etc. DISCUSSION: Although there may be many cases not included in this observation, it is considered to well reflect the real situation of the problem of Japan. More attention should be paid to HIV infection of the patients in the clinical practice of TB in Japan.

A case of intrauterine medical treatment for cystic hygroma.

We report the first case of an intrauterine treatment for cystic hygroma. Guided by ultrasonography, we first removed intracystic fluid from two cysts and then injected OK-432 into each fetal cyst at 21 and 28 weeks of gestation. No re-enlargement of the cysts was subsequently observed. At 38 weeks of gestation, a male infant was delivered transvaginally. Only a slight skin fold was observed in the nuchal area of the neonate, indicating the effectiveness of OK-432 for the intrauterine treatment of cystic hygroma.

Biochemical studies of two rat acyl-CoA synthetases, ACS1 and ACS2.

Two types of acyl-CoA synthetase (ACS), designated ACS1 and ACS2, are structurally similar isozymes with different tissue distributions. The two enzymes are organized into the following five regions: an NH2 terminus; two luciferase-like regions; a linker connecting the luciferase-like regions; a COOH terminus. Under the control of a lac promoter, rat ACS1 and ACS2 were overproduced in Escherichia coli and purified to homogeneity. The specific activities of the purified ACS1 and ACS2 were 26.2 mumol.min-1.mg-1 and 7.4 mumol.min-1.mg-1, respectively, and the most efficiently utilized saturated fatty acids were those with 10-18 carbon atoms. Among unsaturated fatty acids with 16-22 carbon atoms, the most preferred substrates were palmitoleate, oleate and linoleate for ACS1, and, for ACS2, oleate, arachidonate, eicosapentaenoate and docosahexaenoate. To determine the functionally important regions in the ACS isozymes, we constructed five ACS1 mutants lacking each of the five regions. Introduction of these mutants into E. coli revealed that all five regions in ACS1 are required for functional expression of the enzyme in E. coli; deletion of any one of the five regions almost completely abolished the enzyme activity.

Molecular characterization and expression of rat acyl-CoA synthetase 3.

Isolation and characterization of a rat brain cDNA identified a third acyl-CoA synthetase (ACS) designated ACS3. The deduced amino acid sequence of the cDNA revealed that ACS3 consists of 720 amino acids and exhibits a structural architecture common to ACSs from various origins. ACS3 expressed in COS cells was purified to near homogeneity. The purified ACS3 resolved by SDS-polyacrylamide gel electrophoresis into two major proteins of 79 and 80 kDa. Cell-free translation of a synthetic mRNA encoding the entire region of ACS3 revealed that the two isoforms were derived from the same mRNA. The purified ACS3 utilizes laurate and myristate most efficiently among C8-C22 saturated fatty acids and arachidonate and eicosapentaenoate among C16-C20 unsaturated fatty acids. Northern blot analysis revealed that ACS3 mRNA is most abundant in brain and, to a much lesser extent, in lung, adrenal gland, kidney, and small intestine. During the development of the rat brain, expression of ACS3 mRNA reached a maximum level at 15 days after birth and then declined gradually to 10% of the maximum in the adult brain.

DNA strand cleavage by tumor-inhibiting antibiotic 6-diazo-5-oxo-L-norleucine.

A tumor-inhibiting antibiotic, 6-diazo-5-oxo-L-norleucine (DON), caused DNA single-strand breaks. Thus, supercoiled plasmid DNA was transformed into an open circular relaxed form DNA by incubation with DON at pH 7.4. DNA strand cleavage by DON was not inhibited by superoxide dismutase, but inhibited by catalase. The inhibition by catalase may not be due to the destruction of hydrogen peroxide, but to the masking DON by the interaction with the heme moiety of the enzyme. DNA strand cleavage by DON was inhibited by azide, mannitol, ethanol, cysteine and 2-mercaptoethanol, suggesting the involvement of radical species in the cleavage. The cleavage, however, was not suppressed by removal of dissolved oxygen from the reaction mixture, indicating that no oxygen-derived radicals participated in the cleavage. Electron spin resonance spin-trapping technique using 5,5-dimethyl-1-pyrroline N-oxide (DMPO) and N-tert-butyl-alpha-phenylnitrone (PBN) elucidated the generation of a carbon-centered radical from DON. Hence, the carbon-centered radical may participate in DNA strand cleavage by DON.

Molecular genetic analysis of clear cell adenocarcinomas of the vagina and cervix associated and unassociated with diethylstilbestrol exposure in utero.

BACKGROUND: Prenatal exposure to the synthetic estrogen diethylstilbestrol (DES) is associated with the subsequent development of clear cell adenocarcinoma of the lower reproductive tract in young women, and data concerning the molecular genetic alterations involved in the etiology of this tumor type have not previously been reported. Such knowledge would be of potential value by providing insight into the molecular mechanisms of hormonal carcinogenesis in general, as well as by suggesting molecular markers for risk assessment in the estrogen-exposed population. METHODS: A total of 24 samples of clear cell adenocarcinoma of the vagina or cervix, 16 associated with exposure in utero to DES and 8 with no history of DES exposure, were obtained as archival fixed and embedded tissue specimens. DNA was purified from these tissues and used to examine a number of biologically plausible molecular genetic endpoints for tumor specific alterations. RESULTS: No evidence was found for mutations in the K-ras or H-ras protooncogenes, the Wilms' tumor (WT1) tumor suppressor gene, or the estrogen receptor gene. Sporadic overexpression of the p53 tumor suppressor gene was detected in some tumor cell nuclei by immunohistochemistry, but in the absence of detectable p53 gene mutation. Genetic instability as manifested by somatic mutation of microsatellite repeats was widespread in these tumors, with evidence of microsatellite instability in all DES-associated tumors examined, and in 50% of those tumors not associated with DES exposure. CONCLUSIONS: These data are consistent with the hypothesis that the induction of genomic instability may be an important mechanism of DES-induced carcinogenesis.
