Autosomal dominant early onset dementia and leukoencephalopathy in a Japanese family: clinical, neuroimaging and genetic studies.

We report here the results of clinical, neuroimaging and genetic studies of autosomal dominant dementia and leukoencephalopathy in a Japanese family. Twenty-two individuals in this family were examined clinically (17 living, 5 deceased), neuroradiologically and genetically (16 of 17 living members). Ten (5 deceased) of 22 individuals had early onset dementia (age of onset: 45.2 +/- 12.1 years on average) and four of them had multiple white matter lesions and brain atrophy on brain MRI without history of brain ischemic attack. Another four individuals had abnormal white matter lesions on brain MRI without dementia. Linkage studies for chromosome 1q31-42, 14q24.3 and 21q21 responsible for Alzheimer's disease, chromosome 19p13.1-13.2 for cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) and chromosome 3 for familial non-specific dementia suggested no specific haplotypes cosegregated with the disease. Apo E genotypes were E2/2 and E2/3 in this family. Clinical, neuroimaging and genetic studies revealed that the disease in this family was distinguished from known familial dementia. This is the first report of a large Japanese family with autosomal dominant early onset dementia and leukoencephalopathy.

Prostaglandin E1 resuscitates hepatic organic anion transport independent of its hemodynamic effect after warm ischemia.

Prostaglandin E1 (PGE1) is a promising agent against ischemic liver damage, but direct evidence of the benefit to intrinsic hepatocyte function has been lacking. We demonstrate here that organic anion transport can be supported by treatment with PGE1 even at a lower dose which does not affect hepatic microcirculation in rabbits with liver inflow occlusion. Near-infrared spectroscopy was applied to directly measure hepatic clearance of indocyanine green (ICG), an exogenous organic anion, and to estimate microcirculation as measured by oxygen saturation and the content of hemoglobin in the sinusoid. Also, morphological changes in microtubules, the cytoskeleton which is known to be associated with organic anion transport, and energy status, as measured by adenine nucleotide levels, were observed. ICG removal rate in hepatocytes decreased significantly from 0.100 +/- 0.018 to 0.027 +/- 0.019 min-1 (mean +/- SD, P < 0.01) by 60-min warm ischemia, whereas the value increased to 0.082 +/- 0.030 min-1 (P < 0.05) when PGE1 was given at a dose of 0.05 microgram/kg/min. The treated livers also showed early reorganization of microtubules, as well as amelioration of ATP resynthesis after reperfusion. However, there were no significant differences in intraoperative changes in oxygen saturation and the content of hemoglobin in the sinusoid between PGE1-treated and untreated groups, indicating that the influence of PGE1 at this dose on hemodynamic changes is not considerable. These results indicate that PGE1 resuscitates an inherent hepatocyte function of organic anion transport on reperfusion after warm ischemia and suggest that the benefit could be attributed solely to direct action on hepatocytes.

Assay of gamma-glutamyl transpeptidase activity in amniotic fluid offers a possible prenatal diagnosis of biliary atresia in the rat model.

Muller et al. analysed gamma-glutamyl transpeptidase (GGTP) activity in the amniotic fluid of more than 2000 pregnant women for a prenatal diagnosis. They reported that at 18-19 weeks' gestation, two fetuses associated with lower amniotic fluid GGTP levels were diagnosed after birth as having biliary atresia (BA). If low GGTP values correlate closely with BA, chemical assay of amniotic fluid GGTP could possibly be used in the prenatal diagnosis of BA. A fetal model of cholestasis in the rat was created by the administration of the toxic cytopharmacological agent phalloidin on day 15 of gestation, after which amniotic fluid was aspirated and analysed for GGTP. Fetal liver specimens were examined histopathologically. In the normal rats, amniotic fluid GGTP values rose rapidly after 18 days 8 h, reaching a maximum value at 19 days of gestation. Significantly lower GGTP values were observed in the cholestasis models between 18 days 16 h and 19 days 16 h of gestation (P < 0.05). Our data corroborate Muller et al.'s suggestion that fetuses with cholestasis might demonstrate lower GGTP values in their amniotic fluid at a given stage of gestation. Prenatal diagnosis of BA using the amniotic fluid GGTP assay therefore has considerable potential.

Acute popliteal artery occlusion after total knee arthroplasty.

We describe the case of a 71-year-old man with calcium pyrophosphate dihydrate crystal deposition disease who developed acute popliteal artery occlusion immediately after total knee arthroplasty. Reperfusion was achieved thanks to the early diagnosis and thrombolytic therapy with urokinase. Arterial occlusion occurring after total knee arthroplasty is a rare but serious complication. Detailed preoperative evaluation of the peripheral arterial circulation and an early diagnosis seem to be important.

A simple and reliable quick-freezing/freeze-fracturing procedure.

We describe a simple method for the quick-freezing/freeze-fracturing of cells in tissues or culture monolayers. Tissue slices or cultured cells were covered with thin copper foil (10-micron-thick), and frozen by smashing them against a liquid helium-cooled copper block. Freeze-fracturing was accomplished by mechanically separating the copper foil from the frozen specimen. The fracture faces were replicated by platinum and carbon. Replicas were processed for conventional electron microscopic observation or cytochemical labeling. This method allows the ultrastructural and cytochemical examination of large areas of fractured membrane without chemical fixation.

Development of striated rootlets during ciliogenesis in the human oviduct epithelium.

Striated rootlets in ciliated cells are conical banded structures composed of longitudinally aligned filaments. The formation of striated rootlets during ciliognesis in the human oviduct epithelium was studied by electron microscopy. Primitive rootlets appeared at the proximal side of basal bodies before or at the same time as ciliary budding. After the formation of several striations, the tip of the rootlets extended deeply toward the interior of the cell and became differentiated into two distinct parts, viz., the proximal conical part connected to the basal body and the distal fibrillar part. The periodicity of the striations in the fibrillar part was 68. 5+/-2.95 nm, about 5 nm longer than that of the conical part (63. 9+/-2.25 nm). The dark band in the striation was thicker in the fibrillar part than in the conical part. Since the fibrillar part was not observed in the mature cilium, this part was considered as being either degraded or changed into the conical part during ciliogenesis.

Use of synaptophysin polyclonal antibody for the rapid intraoperative immunohistochemical evaluation of functional bowel disorders.

Intraoperative biopsies are essential for accurately distinguishing between Hirschsprung's disease (HD) and intestinal neuronal dysplasia (IND), and vital for determining the extent of abnormal bowel for surgical correction. IND can be associated with HD and can be a cause of postoperative complications. Routine hematoxylin and eosin (H&E) staining is sometimes inadequate for identification of ganglion cells in biopsy specimens and can be the cause of confusion. The authors found synaptophysin (SY) to be more specific as an indicator of abnormal bowel innervation and invaluable for surgical planning. Twenty patients (15 with HD, 3 with IND, and 2 with IND complicating HD) received biopsies intraoperatively. There was markedly reduced immunoreactivity (ie, a decreased number of SY-positive synapses) seen in the intestinal smooth muscle layers of transitional, aganglionic, and IND bowel segments, whereas immunoreactive synapses were abundantly present in the smooth muscle layers of ganglionic colon in HD. SY immunoreactivity also showed ganglion cells and hypertrophic nerve trunks clearly. Rapid SY staining is a simple and consistently reliable method for the intraoperative evaluation of the distribution of synapses in myenteric plexuses as well as smooth muscle layers.

Treatment outcome and prognostic factors in childhood acute myeloblastic leukemia: a report from the Japanese Children's Cancer and Leukemia Study Group (CCLSG)

Treatment outcome and prognostic factors were evaluated in 152 children with acute myeloblastic leukemia (AML) treated on three consecutive protocols (ANLL 861, 8912, 9205) of the Children's Cancer Leukemia Study Group (CCLSG, Japan). In the ANLL 9205 protocol, anthracycline was used with a continuous infusion of cytosine arabinoside, followed by an intensive sequential post remission chemotherapy of short duration. Forty-two of these 46 patients (91.3%) achieved complete remission, and 58.8% of these patients projected a 3-year disease-free survival. These results were apparently superior to those obtained with the ANLL 861 and 8912 protocols, which used conventional doses of multiple drugs followed by a moderate post remission chemotherapy of long duration. This favorable response with the ANLL 9205 protocol was attributed mainly to the high induction rate of patients with the M4 and M5 FAB subtypes, as compared to those in the previous two protocols (93.3% in ANLL 9205 vs. 57.9% in ANLL 861 + 8912; P < 0.05). The ANLL 861 and 8912 protocols, an older age (> or = 8 years), higher WBC counts (> or = 10 x 10(9)/1) and all predicted an increased risk of relapse and decreased the survival following univariate analysis (P < 0.05). An older age and high WBC count continued to predict an increased risk of relapse in multivariate analyses: patients with an age > 8 years and WBC counts > 10 x 10(9)/1 had a 4.5 times higher risk of relapse than patients without these adverse features.

Differential induction of apoptosis on human lymphoblastic leukemia Nalm-6 and Molt-4 cells by various antitumor drugs.

To investigate how chemotherapy agents interact with the leukemic cell death pathway, we examined apoptosis of human lymphoblastic leukemia cells (Nalm-6 and Molt-4) treated with various anticancer drugs (etoposide (VP-16), camptothecin (CPT), adriamycin (ADR), cytosine arabinoside (Ara-C), methotrexate (MTX), 6 mercaptopurine (6MP), cyclophosphamide (CPM), vincristine (VCR) and prednisolone (PRD)) by flow cytometric procedures. The proportion of apoptotic cells was estimated from the presence of cells with a fractional DNA content in the DNA histograms after the incubation of drug-treated cells with a DNA extraction buffer. Treatment with Ara-C, CPT, VP-16 and ADR resulted in rapid apoptosis with 40-60% apoptotic cells by 8 h. Treatment with MTX, VCR, 6MP and PRD induced no apparent apoptosis until 12 h, but further treatments with these drugs resulted in apoptosis with 50% (MTX), 20-30% (6MP and VCR) and 5-10% (PRD) apoptotic cells, respectively, at 24 h. CPM induced apoptosis with 10-20% apoptotic cells at 10(-6) M, but higher doses (> 10(-5) M) caused a rapid cell death by necrosis. The cell cycle position of apoptotic cells was assessed by the terminal deoxynucleotidyl transferase (TdT) assay of DNA strand breaks combined with DNA staining. MTX, Ara-C, CPT, VP-16 and ADR preferentially induced apoptosis in the S phase. PRD and 6MP induced apoptosis in the G1 phase and G1 + S phases, respectively. CPM showed no cell cycle phase specificity. These findings suggested that the susceptibility of cells to apoptosis was not the sole determinant of cellular sensitivity of cytotoxic drugs.

Effects of antihypertensive agents on circadian blood pressure in hypertensive patients with previous brain infarction.

To evaluate the effects of antihypertensive agents on the circadian blood pressure (BP) of patients with previous brain infarction, the ambulatory BP was measured non-invasively for 24 h before and after administration of antihypertensive agents. One hundred milligrams of acebutolol twice daily (n = 15) is effective in lowering the BP during the daytime, but has little effect during the night and the morning. Twenty milligrams of slow-release nifedipine twice daily (n = 14) produced a consistent reduction in the BP over the entire 24-h period and effectively blunted the rise in BP in the morning. Captopril (12.5 mg) twice daily (n = 15) produced a mild reduction in BP with little change in the circadian pattern. The slow-release nifedipine group had the greatest decrease in mean systolic and diastolic BP. The heart rate significantly increased after administration of slow-release nifedipine and decreased after administration of acebutolol. To reduce stroke recurrence, we should consider the effects of antihypertensive agents on circadian BP in hypertensive patients with previous brain infarction.

Ultrastructural observation on 'transitional tubules' in human oviductal ciliogenic cells.

In the human oviduct epithelium during ciliogenesis, short tubular structures were found in the transitional zone between the basal body and cilium. The tubules, termed transitional tubules from their location, were 34-36 nm in diameter and 0.13 +/- 0.06 micron in length; the number around a basal body was variable, but usually 4-6. The cytoplasmic leaflets of the tubule membranes were coated by electron-dense material and appeared to be connected to alar sheets. The transitional tubules existed transiently during ciliogenesis. The exact role of transitional tubules is unknown, but considering their location, they may fix the basal body in the apical cytoplasm during ciliary elongation and/or may be related to formation of alar sheets.

Distribution of plasmalemmal Ca(2+)-pump and caveolin in the corneal epithelium during the wound healing process.

PURPOSE: Caveolae are small plasmalemmal invaginations which are assumed to play various physiological functions. In the present study, distribution of two caveolae-specific proteins, the plasmalemmal Ca(2+)-pump and caveolin, was examined in the corneal epithelium in the normal state and after artificial wounding. METHODS: A central epithelial ablation was made in the mouse cornea by a razor blade. After various intervals, the corneas were excised, fixed, and rapidly frozen. The specimens were subjected to immunofluorescence microscopy and immunoelectron microscopy, using antibodies against the plasmalemmal Ca(2+)-pump or caveolin. RESULTS: In the normal corneal epithelium, both plasmalemmal Ca(2+)-pump and caveolin were observed along the cell surface by immunofluorescence microscopy, and were localized to caveolae by immunogold electron microscopy. In the regenerating epithelium, 12-18 h after injury, plasmalemmal Ca(2+)-pump was seen as many dots in the cytoplasm by immunofluorescence microscopy; in contrast, caveolin persisted along the cell surface. Immunoelectron microscopy revealed that the labeling for the plasmalemmal Ca(2+)-pump was located around membranous structures in the cytoplasm and was scarce along the plasma membrane, while caveolin remained in caveolae. The Ca(2+)-pump regained normal distribution when the wound was closed. By quantitation in electron micrographs, the number of caveolae per unit plasma membrane length was found to be decreased in the wounded corneal epithelium. CONCLUSIONS: The present results indicate that caveolae undergo compositional modification during the wound healing process of the corneal epithelium. Considering putative caveolar functions, the phenomenon may be related to possible fluctuations of the intracellular Ca(2+)-concentration in the regenerating epithelium.

Implant-supported prostheses after maxillary cancer resection: preliminary report.

A preliminary study of 4 partial maxillectomy patients who received a total of 17 endosseous implants and implant-supported prostheses after oral cancer resection is reported. The masticatory function index (chewing efficiency) after implant placement was higher than the preoperative value. Speech with an implant-supported prosthesis improved as compared with previous prostheses or without a prosthesis. All of the patients received radiation and adjunctive hyperbaric oxygen therapy. The success rate after a 2-year follow-up period was 88.2 percent.

Study of the effect of atrial contraction on mitral prosthetic valve by high speed video camera.

To clarify the effect of atrial contraction on the dynamic behavior of mitral prosthetic valves, a mechanical mock circulatory system has been developed. It is able to simulate the inflow characteristics during diastole. The disc motions of Björk-Shiley Monostrut (BSM29) and CarboMedics (CM29) valves (both with an annulus diameter of 29 mm) were measured and compared with a high speed video camera. After contraction of the artificial atrium (100 ms), there was a delay of 75 ms before the onset of the ventricular contraction. Mitral flow similar to physiological conditions could be achieved. The BSM29 was tested in the anterior position (a) and the posterior position (p). Under the condition of active atrial contraction (AC), we confirmed that the closing motion was initiated and the period during closing motion (PDCM) was prolonged (BSM29[a]: 22.4 +/- 3.4-->63 +/- 3.2; BSM29[p]: 71.2 +/- 38-->94.2 +/- 11; and CM29: 14 +/- 0.0-->28.4 +/- 2.3 [unit: ms]), the prolongation of the PDCM of the CM29 due to the AC being smaller than that of the BSM29. We also confirmed that the closing volume (CV) increased slightly (BSM29 [a]: 7.8 +/- 0.4-->8.5 +/- 0.8; BSM29[p]: 6.9 +/- 0.6-->7.0 +/- 0.5; and CM29: 3.6 +/- 0.6-->4.1 +/- 0.6 [unit: ml]). The CM29 had a shorter PDCM, less prolongation of the PDCM due to the AC, and less CV than the BSM29. Thus, we confirmed that the CM29 produced better response at valve closure than the BSM29.

Meningoencephalocele associated with Tripterygium wilfordii treatment.

We treated a male infant with occipital meningoencephalocele associated with the taking of Tripterygium wilfordii. The infant was delivered normally at 38 weeks of gestation with a huge cystic mass protruding from the occiput. He was diagnosed with occipital meningoencephalocele and cerebellar agenesis. His mother had taken T. wilfordii for rheumatoid arthritis early in her pregnancy. T. wilfordii is a herbal medicine used for rheumatoid arthritis and male contraception. Since its toxicity is high and its use during pregnancy is restricted, it is the most likely cause of this infant's anomalies.

Metal sandwich method to quick-freeze monolayer cultured cells for freeze-fracture.

We describe a simple quick-freezing method to obtain a large fractured plane of the plasma membrane from monolayer cultured cells. Cells were grown on thin gold foil, inverted on a thin layer of gelatin on thin copper foil, and frozen by a quick press between two gold-plated copper blocks precooled in liquid nitrogen. The frozen cell sandwich was mounted on the cold stage of a freeze-fracture device with the gold side up and was fractured by separating the sandwich with a cold fracture knife. When this technique was applied to confluent monolayer cells, large replicas of the E-face of the upper plasma membrane and the P-face of the lower plasma membrane were obtained. The present metal sandwich method is simple, does not require any expensive equipment, and provides a large fracture plane of the plasma membrane for subsequent histochemical manipulation.

Crosslinked plasmalemmal cholesterol is sequestered to caveolae: analysis with a new cytochemical probe.

[symbol: see text]-Toxin (perfringolysin O), a cholesterol-binding toxin, was partially proteolyzed and biotinylated (BC theta) to eliminate hemolyzing activity and was used as a cytochemical probe. In fixed cells, binding of BC theta was intense in the plasma membrane, especially at the base of apical microvilli and in lateral processes. The labeling was abolished by pretreatment with filipin, digitonin, or tomatin. When living cultured cells were treated with BC theta and then with either fluorescein-avidin D or colloidal gold-streptavidin, the labeling in fine dots was distributed on the cell surface without local concentration as long as cells were kept on ice. When the temperature was raised to 37 C after treatment, the probe formed discrete large patches and became sequestered to caveolae. Binding of BC theta alone without the secondary reagents did not cause redistribution even at 37 C. Because the plasma membrane maintains integrity even after binding of BC theta, the probe can be used not only for cytochemical labelling of fixed cells but for pursuing the behavior of crosslinked cholesterol molecules in living cells. By use of this new probe, the present study revealed that crosslinked cholesterol in the plasma membrane is sequestered to caveolae.

Evaluation of propolis. I. Evaluation of Brazilian and Chinese propolis by enzymatic and physico-chemical methods.

To establish a method of evaluating propolis, the effects of the ethanol and water extracts from various collecting of propolis from different countries and plant sources on hyaluronidase activity were investigated along with their absorption spectra and specific absorbance (E(1%)1 cm value). The relations between the hyaluronidase inhibitory activities of these extracts and their E(1%)1 cm values were also examined, and the following was found: 1) the enzyme inhibitory activities of the ethanol extracts were more potent than those of the water extracts; 2) the enzyme inhibitory activities of the ethanol extracts from Araucaria angustifolia (BERT.) O. KTZE were low compared with those of other ethanol extracts; 3) the enzyme inhibitory activities of all the ethanol extracts correlated excellently with their E(1%)1 cm values, but in the water extracts, they decreased with increase in E(1%)1 cm values; 4) the water extracts of Chinese propolis from Hebei, Jiangsu, Sichuan and Zhejiang Provinces inhibited weakly compared with that from Brazilian and other Chinese propolis; 5) the shapes of absorption bands of the propolis extracts and the E(1%)1 cm values were approximately dependent on the place or the plant source from which propolis was collected. These experimental results indicated that, for the exact evaluation of propolis, the enzymatic method, measuring the hyaluronidase inhibitory activity, was superior to the physicochemical method.
