A method of percutaneous laryngeal electromyography is described. The technique is simple, safe and it allows a sampling of muscles within the superior laryngeal and recurrent laryngeal territories. The procedure can be performed on an out-patient basis in a conventional electromyography laboratory, and complications have not been encountered.
The techniques and observations of earlier authors on laryngeal electromyography are briefly reviewed. Our personal experience with the use of a percutaneous method in a series of 27 patients is described and discussed. The percutaneous technique is a simple, safe method and permits the examination to be performed on a routine out-patient basis. The presence of denervation, the degree of paralysis, evidence of recovery, can be readily obtained and this permits a more accurate assessment and prognosis to be made.

This is a study of 339 patients with carcinoma of the hypopharynx, seen in Toronto from 1960-72. Patients were grouped according to the U.I.C.C., T.M.N. classification but a modified staging method was employed since there is no generally accepted procedure for stage-grouping at the present time. Patients treated by primary radiation were compared to a group treated by combined therapy (radiation followed by surgery). The results show that the overall crude survival of all hypopharyngeal lesions was approximately 20 per cent. A high percentage of patients had advanced lesions at first presentation and almost 30 per cent of patients were treated palliatively. Analysis indicated that combined therapy is the treatment choice for Stage I and II hypopharyngeal lesions. Patients with more advanced lesions do poorly regardless of the treatment modality. The morbidity and mortality rates are high in the combined therapy group but the risks were doubled when salvage surgery was undertaken for failed primary radiotherapy.
Twenty-nine adult volunteers with stable bilateral sensorineural hearing loss were treated by acupuncture. Subjects were randomly assigned to one of four experimental groups: active needling, active needling followed by sham needling, sham needling followed by active needling, and sham needling. All subjects received 30 treatments. Only one ear was treated by acupuncture. The second ear was kept for an intrasubject control. Prior to treatment a baseline of hearing was established. During the course of acupuncture, audiological assessments (pure tone thresholds and speech tests) were made after sets of five treatments. Hearing was again measured during the year following acupuncture. The results in individual subjects and statistical comparisons of active and sham groups showed that acupuncture did not produce significant shifts in hearing.
The electrophysiologic approaches to the assessment of electrical activity of the various portions of the human auditory pathway are reviewed. A method of detection of click evoked eighth nerve and brain stem responses with the use of surface electrodes is described. Preliminary observations in the normal subject with different stimulus parameters and in patients with unilateral deafness are presented. Simultaneous recordings of the eighth nerve and brain stem responses with the cortical evoked response and with the extratympanic electrocochleogram are described. The technique of click evoked eighth nerve and brain stem potentials offers us an additional method of objective assessment of auditory function in a human subject.
The origin of waves two to five occurring in the first 10 milliseconds after repetitive click stimuli is not definitely established. Published studies have suggested that waves two and three are derived from the cochlear nucleus and superior olivary complex respectively and that waves four and five are generated by the inferior colliculus. Our studies of the effect of chronic brain stem lesions in the cat do not support this hypothesis of specific localization, but reveal that a more general reduction in amplitude, and at times prolongation in latency, occurs with lesions in the pons and lower midbrain and that wave one amplitude is also diminished with brain stem lesions.

Chordomas are uncommon tumors arising from embryonic remnats of the fetal notochord. The spheno-occipital and sacrococcygeal regions account for over 90 per cent of cases, the remaining 10 per cent occurring anywhere along the vertebral axis. A case of chordoma arising from the clivus is presented with a view to discussing management. The ability to obtain reasonable survival times associated with a useful and meaningful quality of life is emphasized. This may be accomplished by repeated, judicious surgical intervention with surprising ease and little morbidity to the patient in spite of its intracranial location.
