The effects of amphetamine, imipramine and ICI 58,834 (Vivalan), a potential antidepressant, on unconditioned behaviour in rats.

The effects of amphetamine, imipramine and ICI 58,834 (Vivalan), on the unconditioned behaviour of rats were compared using a novelty preference test and the open field test. Overall, the effects of ICI 58,834 (Vivalan) showed a marked resemblance to those of imipramine but differed from those of amphetamine. It is suggested that an adequate explanation of these drugs' effects can be offered in terms of changes in emotional reactivity, in general activity, and in exploratory activity. A dissociation of drug effects on general and exploratory activity in the novelty preference test was demonstrated. The use of this test and the open field test in drug screening is briefly discussed.

Late fetal pontine destruction.

A child with an absent pons is described who died 8 weeks after birth. At postmortem examination, the defect was consistent with a vascular event in the last trimester.

The effectiveness of psychiatric treatment for HIV-infected patients.

The study sought to determine the effectiveness of a model program of psychiatric care for human immunodeficiency virus (HIV)-infected patients. This was a cohort study of 126 HIV-positive outpatients referred for psychiatric evaluation and treatment (average follow up of 14 months) in a HIV-dedicated primary-care outpatient clinic in the inner city. A global outcome measure (encompassing symptom relief, functioning, and HIV-risk behaviors), and a measure of abstinence from alcohol and illicit substances were used. Fifty percent of patients improved, with 19% "nearly well" at follow-up. Abstinence was achieved 48% of the time. Good compliance with treatment and the absence of injection drug use were the primary predictors of good outcomes. Of the compliant patients, 94% improved, with 45.7% being nearly well. Psychiatric treatment of HIV-infected patients is effective when located in the HIV primary-care setting and administered by a multidisciplinary team under the direction of a psychiatrist, using evidence-based interventions.

Managing health and safety in hospitals.

This article is set against the background of recent changes in health and safety legislation which directly affect hospital staff. Research into the central concepts of hazard, risk and harm is contributing to a broader understanding of the management of health and safety. Both occupational medicine and safety practice must recognise the role of psychosocial and organisational processes in managing workplace hazards.
