Use of nucleolar organizer regions in the histopathological diagnosis of colorectal epithelial neoplasia.

The AgNOR technique, specific staining for nucleolar organizer regions, was applied to twenty-seven lesions (sixteen tubular adenomas with low grade atypia and eleven well-differentiated adenocarcinomas invading the submucosa) of colorectal epithelial neoplasia, to examine the possibility of its application to histopathological diagnosis. The mean AgNOR number per adenoma nucleus (2.62 +/- 0.16) was significantly higher (P less than 0.01) than that per carcinoma nucleus (2.28 +/- 0.25). The mean cross-sectional area and the maximum diameter of carcinoma AgNORs (1.91 +/- 0.20 micron2 and 1.89 +/- 0.09 micron, respectively) were significantly larger than those of adenoma (1.15 +/- 0.17 and 1.45 +/- 0.11, respectively). The result of the discriminant analysis using these two variants corresponded to the pathological diagnosis with an accuracy of 96.3% (26/27). We thus concluded the AgNOR technique to be one of use in the histopathological diagnosis of colorectal epithelial neoplasia.

[Effect of DQ-2556, a new cephalosporin derivative, on fecal microflora].

A new cephalosporin derivative antibacterial agent, DQ-2556, was administered intravenously to 4 healthy adult male volunteers at 2 grams per dosage 2 times a day for 5 days, and degrees of effects of drug concentrations on the fecal microflora were investigated. 1. The total viable count remained unchanged during the study period due to the minimal change in the number of members of the family Bacteroidaceae, which were the most dominant organisms. In one of the volunteers, however, the total count was remarkably reduced immediately after the end of administration of the drug. 2. Among the obligate anaerobes, most of the organisms decreased except members of the family Bacteroidaceae. A marked increase of lecithinase-positive clostridia was noted in 2 volunteers. 3. The number of aerobes, including facultative anaerobes, members of the family Enterobacteriaceae were suppressed but bacilli and yeasts showed slight increases. In many cases, increases in enterococci were also observed. 4. DQ-2556 was not detected in the feces from any of the volunteers during the study period. 5. Neither Clostridium difficile nor its toxin D-1 was detected in any of the volunteers. 6. Diarrhoea or other side-effects were not noted in any of the volunteers.

The effect of levofloxacin, an optically-active isomer of ofloxacin, on fecal microflora in human volunteers.

Following oral administration of levofloxacin (LVFX, (S)-(-)-Ofloxacin; formerly designated as DR-3355) at 200 mg per dose 3 times a day for 7 days to 6 healthy male volunteers, degrees of disturbance of the fecal microflora and fecal drug concentrations were examined. The total viable count remained unchanged during the study period due to the minimal change in the number of members of the family Bacteroidaceae, the most predominant organisms. Most of the aerobes including facultative anaerobes were suppressed by LVFX with only a slight increase in yeasts. In particular, the members of the family Enterobacteriaceae were reduced to below the detection limit on and after day 3 through the time of discontinuation of the drug in all subjects but one. Among the obligate anaerobes, peptostreptococci and bifidobacteria decreased or disappeared in some volunteers, but no significant changes were observed in other anaerobes. Neither Clostridium difficile nor its toxin D-1 was detected in any of the volunteers. No side effects attributable to the drug were observed. During administration, LVFX was detected in the feces at high concentrations which correlated well with the decrease of susceptible members of flora as well as to their detection rate.

[In vitro antimicrobial activity of rokitamycin, a macrolide antibacterial agent, against clinically isolated strains of Campylobacter and other enteritis-causing bacteria].

We determined the minimum inhibitory concentrations (MICs) of rokitamycin (TMS-19-Q, RKM), a macrolide antimicrobial agent, against strains of various bacterial species isolated from enteritis patients, and compared them with those of josamycin (JM), erythromycin (EM) and ofloxacin (OFLX). MIC90 of RKM against 147 strains of Campylobacter jejuni, and each 25 strains of Shigella spp., Salmonella spp. and diarrheagenic Escherichia coli were 1.56, 200, 800 and 200 micrograms/ml, respectively. There was only one RKM resistant (MIC greater than 100 micrograms/ml) C. jejuni strain, while most of the strains of the other species were resistant to RKM. MIC values of the other drugs were all similar to those of RKM. MIC90 of OFLX against 147 strains of C. jejuni was 0.78 micrograms/ml, lower than other drugs.

[The usefulness of nucleolar organizer regions in diagnosis of colonic epithelial neoplasia].

The Ag-NORs technique, staining nucleolar organizer regions, was performed on 23 areas of colonic normal mucosa, 17 adenomas with low grade atypical and 8 adenocarcinoma invading submucosa, to examine its possibility of application to the pathological diagnosis of colonic epithelial neoplasia. In normal colonic gland, the average number of Ag-NORs per 1 nucleus in cells of lower two-thirds showed significantly higher value (p less than 0.01) than higher one-third, and the mean cross-section area per 1 Ag-NOR in lower two-thirds showed a tendency to be larger than higher one-third. This results suggested that there was a positive correlation between the proliferative activity of cells and the number and size of Ag-NORs. A comparison of normal gland, adenoma and adenocarcinoma showed that the mean cross-section area of Ag-NORs correlated positively to their histological grade of atypia, but the average number did not. However using these two parameters, these three lesions had a tendency to be discriminated respectively at the average Ag-NORs number of 2.8 dots and the area of 2 microns. These results suggested that the Ag-NORs technique was expected to be a useful method for histopathological diagnosis of colonic epithelial neoplasia.

Cytotoxic component(s) of Klebsiella oxytoca on HEp-2 cells.

A cytotoxic component(s) was detected in culture filtrates of Klebsiella oxytoca isolated from patients with antibiotic-associated hemorrhagic colitis. Eleven of 12 isolates exhibited cytotoxicity on HEp-2 cells. The cytotoxic activity of K. oxytoca strain MH43-1 was stable for the treatment of 60 C for 30 min, but inactivated by the treatment of 100 C for 15 min. This cytotoxicity was not destroyed by the treatment with trypsin or pronase, and the component was filtrable through a membrane filter which cut off molecular weight 5,000.

[A case of retroperitoneal mucinous cystadenocarcinoma].

We reported is the histogenesis of a case of a primary retroperitoneal cystadenocarcinoma that was resected surgically in a 42-year-old woman who was admitted to hospital for SLE. On echographic examination, she was found to have a right abdominal tumor and underwent an operation for the complete removal of an abdominal tumor located in the right retroperitoneal cavity. A histological examination of the tumor revealed it to be compatible with a mucinous cystadenocarcinoma of the ovary. Only four cases of a primary retroperitoneal mucinous cystadenocarcinoma have been reported in the literature. This rare lesion was studied histopathologically and its histogenesis is discussed.

[Prevalences of allergic diseases among synthetic chemical workers].

We studied the prevalence of allergic diseases among synthetic chemical workers, using a cross-sectional questionnaire, to clarify the epidemiological evidence of occupational allergy due to chemicals which workers were handling and manufacturing. A file, registered in 1981, of 6,819 person, was used to calculate the prevalence during the preceding 12 months of "allergy," "asthma," and "skin disease," and to analyze the relationship between the prevalence and the exposure risks such as the work or the chemicals being manufactured. The main results were: 1) In workers who were exposed to the various chemicals, the prevalence of "allergy," "asthma," and "skin disease" were 3.2, 1.9, and 9.0%, respectively. 2) Among the workers who were engaged in the manufacturing of synthetic resins, paints, films, titania or pharmaceuticals, the prevalence of "allergy" was significantly higher than among others. The rates of "asthma" were significantly higher than others among workers in cosmetics, synthetic resins, or fertilizers. The rates of "skin disease" were significantly higher among those working with cosmetics, synthetic resins, paints, dyes, or organic solvents. The results indicate that allergy research should be concentrated on workers engaged in manufacturing the above chemicals.

[Some experiments on the allergic reaction among workers in a pharmaceutical factory (author's transl)].

A survey was made on workers handling powdered drugs in a pharmaceutical factory. In this factory, two kinds of anti-inflammatory enzyme (bromelain and trypsin), one anti-inflammatory agent (flufenamic acid), one antispasmodic (flopropion) and two kinds of antibiotics (ampicillin and cephalexin) are mainly produced. Twenty four workers were examined by interviews and checked by Cornell Medical Index, and 18 of them complained of respiratory symptoms. These 18 workers were physically examined by skin scratch tests, pulmonary function tests and serum immunological tests. Among 24 workers, 9 handled powdered drugs (A group), 5 handled the same in the past and had already been transferred to other sections for their symptoms (B group), 3 engaged in the process of capsul-filling (C group) and 7 handled several times occasionally during one year (D group). Their average months spent in handling powdered drugs were, in the case of anti-inflammatory enzyme, A group 53.2, B group 66.2, and in the case of antibiotics, 5 workers in A group 24.0, 2 workers in B group 7.0, 3 workers in C group 25.7. Twenty workers complained of symptoms which were mainly irritation of mucosa including the respiratory system and itching of the skin while they were working, and accelerated nasal discharge, urticaria and asthma after working. Group A and group B were higher than group D in the rate of respiratory complaints in C.M.I. (p less than 0.001). Fourteen workers pointed out anti-inflammatory enzyme as a cause of main symptoms, 7 workers flufenamic acid, 3 workers flopropion, 4 workers antibiotics. Three workers who had past history of asthma or articular rheumatism had been transferred to other sections. Of 18 workers who were physically examined, 11 workers showed positive reactions to skin scratch tests with handling drugs. On 8 workers of them, some kinds of drugs which were pointed out as drugs causing main symptoms reacted positively. Numbers of workers with increased immunoglobin values were, IgE 3, IgM 2, IgA 4, IgM 2. Two workers showed decreased FVC and FEV (1.0 sec.) values in pulmonary function tests. The causes of the occupational allergic reaction in this factory are guessed as follows: 1) control of powdered materials was incomplete in the process of production, 2) various kinds of sensitizing drugs were handled by the same workers.

Primary retroperitoneal mucinous cystadenocarcinoma: report of a case.

A case of primary retroperitoneal mucinous cystadenocarcinoma is presented. Ultrasonography (US), computed tomography (CT), and magnetic resonance imaging (MRI) examinations demonstrated a large cystic tumor with solid excrescences. Angiographic examination demonstrated an avascular area with small areas of faint blush. It must be borne in mind that mucinous cystadenocarcinoma originating from the retroperitoneum can occur, although it is very rare.
