We report a case of Mediterranean lymphoma treated with antibiotics. A 74-year-old woman visited the hospital due to abdominal pain. Endoscopic examination showed erosions and ulcerations on duodenal mucosa. Biopsy specimens histologically revealed massive infiltration of small-sized lymphocytes and plasma cells in subepithelial mucosa. Immunoperoxidase staining showed that the infiltrating cells were positively stained with anti-alpha heavy chain. Serum IgA concentration was elevated and immunoelectrophoresis of the serum demonstrated monoclonal protein composed of alpha heavy chain. During the antibiotic treatment her symptoms disappeared and serum IgA concentration was normalized. Endoscopic examination also showed healing of the duodenal ulceration. The similarities between Mediterranean lymphoma and gastric mucosa-associated lymphoid tissue (MALT) type lymphoma, both of which may be related to bacterial infection and can be treated with antibiotics, are discussed in this report.
The pathogenesis of hearing loss in acoustic neuromas remains to be elucidated. To obtain more detailed information about inner ear damage in this disease, especially that dealing with the possibility of ischemia of the inner ear due to compression of the blood supply in the internal auditory canal by acoustic neuromas, measurement of endocochlear DC potential (EP) was attempted in three ears with acoustic neuromas during surgical removal of the tumors by the translabyrinthine approach. The size of each tumor was 3.0, 0.8, and 1.8 cm, respectively. An EP with a positive value was recorded in each of the three ears, i.e., 54, 57, and 38 mV, respectively, although the first and third patients were profoundly deaf on the side with a tumor. These findings were interpreted as being evidence of retention of function by the stria vascularis and the existence of blood supply to the inner ear in spite of the presence of tumors. Measurement of the EP seems to provide a new tool with which to analyze the causes of hearing loss in acoustic neuromas. Relatively low EP values may suggest mild dysfunction of the stria vascularis with acoustic neuromas.
A 65-year-old Japanese female was admitted to our hospital because of right lower abdominal pain. Laboratory and roentgenological tests revealed no abnormalities. Repeated stool examinations revealed no ovum or parasite. However, colonoscopic examination demonstrated the presence of a parasite on the edematous mucosa of cecum. The adult male whipworm (Trichuris trichiura) was then removed endoscopically. Her symptoms disappeared quickly, and no additional parasite was found in the feces during or after the mebendazole treatment. Repeated stool examinations failed to show any ova, because this patient was infected by a single male parasite. Colonoscopic study should be considered a useful tool for the diagnosis of T. trichiura infection, even if no ovum is found in stool examination.

OBJECTIVE: Worldwide epidemiological studies have demonstrated that hepatitis C virus (HCV) probably is a causative agent of hepatocellular carcinoma (HCC). However, there are no available reports that clearly identify the risk factors for the development of HCC in HCV-related chronic liver disease (CLD). The aim of the present study is to explore the risk factors for hepatocarcinogenesis in HCV-related CLD. METHODS: We prospectively observed 412 patients with anti-HCV-positive CLD but without co-infection of hepatitis B virus (232 patients with chronic hepatitis and 180 with liver cirrhosis) for between 0.5 and 15.8 yr (median: 4.9 yr). Risk factors for hepatocarcinogenesis were identified with a Cox proportional-hazard model. RESULTS: Sixty-three patients (15.3%) developed HCC during the observation period; the cumulative occurrence rates at the end of the 5th, 10th, and 15th yr was 3.7%, 12.1%, and 12.1%, respectively, for chronic hepatitis patients and 23.3%, 49.4%, and 90.7%, respectively, for 180 cirrhotic patients. The Cox proportional-hazard model showed that the risk of hepatocarcinogenesis increased almost 5-fold in cirrhotic patients (risk ratio, 5.14; 95% confidence interval, 2.52-10.46, p = 0.0001), 2-fold in patients with positive antibodies against hepatitis B surface antigen and/or antibodies against hepatitis B core antigen (risk ratio, 2.14; 95% confidence interval, 1.13-4.07, p = 0.0201), and 2.5-fold in heavy smokers (risk ratio, 2.46; 95% confidence interval, 1.11-5.49, p = 0.0276). CONCLUSION: These epidemiological results indicate that previous infection with hepatitis B virus and heavy smoking (in addition to liver cirrhosis, a known risk factor) play important roles as risk factors for carcinogenesis in HCV-related CLD.
A 79-year-old woman was admitted to our hospital with complaints of dysphagia and multiple verrucous papules that had developed over the previous year. The diagnosis of esophageal carcinoma was based on upper gastrointestinal radiography and endoscopic examination with biopsy. The clinical syndrome was consistent with the sign of Leser-Trelat associated with esophageal carcinoma. Although radiation therapy and chemotherapy were undertaken, the patient died 8 months later because of the sign of Leser-Trelat in association with squamous cell esophageal carcinoma.

Changes of median hepatic venous blood flow (MHVBF) and left portal venous blood flow (Lt. PVBF) were measured continuously in 31 patients undergoing elective laparoscopic cholecystectomy (LC) by using transesophageal echocardiography (TEE). The patients were anesthetized with inhalation and epidural anesthesia. MHVBF and Lt. PVBF decreased from preoperative level of 317 +/- 61 ml.min-1, 522 +/- 86 ml.min-1 to 73 +/- 22 ml.min-1, 98 +/- 28 ml.min-1 after inflation with high pneumoperitoneum pressure and they recovered by deflation. These findings suggest that hepatic blood flow decreases by increased intraperitoneal pressure.
OBJECTIVES: The aim of the present study was to compare the safety of endoscopic variceal ligation (EVL) and endoscopic injection sclerotherapy (EIS) in terms of liver and kidney functions in patients with liver cirrhosis. METHODS: Forty-four patients admitted to Takatsuki General Hospital between February 1991 and March 1993 with esophageal varices due to liver cirrhosis were randomly assigned to receive either EVL or EIS. Serum levels of AST, ALT, total bilirubin (T-bil), direct bilirubin (D-bil), prothrombin time (PT), hepaplastin test (HPT), antithrombin III (ATIII), creatinine (Cr), and blood urea nitrogen (BUN) were measured before and 24 h, 3 days, 7 days, and 14 days after both forms of therapy. RESULTS: Significant elevations of serum T-bil, serum D-bil, and serum ALT and AST levels were observed in the EIS group but not in the EVL group. No significant increases of serum PT, HPT, ATIII, BUN, or Cr levels were observed after treatment in either group. CONCLUSION: EVL should be considered a first choice therapy for eradicating esophageal varices.
Gastrin is a well known endogenous stimulator of gastric acid. In addition, recent studies have revealed that gastrin has a growth promoting effect on gastric ECL cells. Indeed, development of ECL carcinoid tumor occurs almost exclusively in patients with hypergastrinemia such as autoimmune gastritis and Zollinger-Ellison syndrome with MEN type I. We have recently cloned human gastrin receptor gene, and by using it, we found that both gastric carcinoid tumor and endocrine cell carcinoma of the stomach express significant amount of gastrin receptor gene whereas none of gastric cancer tissue shows gastrin receptor gene expression. Thus, it is clear that gastrin plays important roles in the development of gastric carcinoid tumor as well as endocrine cell carcinoma of the stomach.
A case with multiple liver abscess accompanied by massive portal venous gas is reported. A 61-yr-old male was admitted because of left lower abdominal pain, fever, and diarrhea. Abdominal x-ray examination demonstrated multiple branching lucencies in the liver. Computed tomography revealed multiple liver abscesses and massive gas in the portal system as well as a thickened wall of the sigmoid colon. Enema study using contrast medium revealed a perforation of the sigmoid colon with diverticulitis. The outcome was favorable after sigmoid colectomy in addition to intensive treatment with antibiotics. Bacteroides fragilis, which produces gas (H2 and NH3) by fermentation, was isolated not only from the resected specimen but also from blood samples. Although the presence of portal venous gas is a sign of poor prognosis in patients with intestinal infectious diseases, the sensitive detection of hepatic portal venous gas by computed tomography and the appropriate treatment may improve the patient's prognosis.
The afferent connections of the infralimbic area (area 25) of the medial prefrontal cortex in rats were studied using WGA-HRP as the tracer. The present results confirm previously reported afferent connections of the infralimbic area and further reveal additional inputs from the anterior amygdaloid area, anterior cortical amygdaloid nucleus, posterolateral cortical amygdaloid nucleus, ventral reunions, lateral preoptic area, dorsal hypothalamic area, posterior hypothalamic nucleus, supramammillary nucleus and medial mammillary nucleus. Only afferent projections were found from CA1 of the hippocampus and amygdalo-piriform transition area. The results also confirm extensive afferent connections of the infralimbic cortex from the central autonomic nuclei and, together with previous results on efferent connections of the infralimbic cortex, provide further support for an essential role of this nucleus as a cortical center of the autonomic nervous system.

A series of deltorphin (DLT: Tyr-D-Met-Phe-His-Leu-Met-Asp-NH2) analogs in which Leu5 and/or Met6 were mainly replaced by t-Leu(Tle) and/or N alpha-alkylated glycine were synthesized and examined for their receptor binding properties and in vitro bioactivities by guinea pig ileum (GPI) and mouse vas deferens (MVD) assays. [Tle5]DLT(2) showed a dramatic decrease in the MVD potency when compared to the parent peptide and was found to have a potent delta receptor antagonist activity against various delta agonists with Ke values of 16-311 nM. Interestingly, the antagonist potency of 2 against DPDPE as agonist was 20-fold weaker than that against deltorphins or Leu-enkephalin. Among the analogs in which Met6 was replaced by an N alpha-alkylated Gly residue, [N alpha-isobutyl-Gly6]DLT(5) behaved as a mixed mu antagonist/delta agonist while its isomeric analogs in which the N alpha-alkyl is n-butyl (4) or (R or S) sec-butyl (6a,b) were very potent delta receptor agonists. Analogs 2, 4, 6a, and 6b were highly stable against rat brain and rat plasma enzymes and thus may represent a starting point for the development of novel receptor-specific compounds useful as ligands for studies of opioid receptors.
n-Acetylglycine, n-propionylglycine, n-butyrylglycine, isobutyrylglycine, n-valerylglycine, isovalerylglycine, heptanoylglycine, phenylacetylglycine and isovalerylglucuronide were identified based on their liquid chromatographic-atmospheric pressure chemical ionization mass spectra (LC-APCI-MS). We were able to detect the presence of urinary isovalerylglycine in two cases of isovaleric acidemia using LC-APCI-MS. Membrane-filtered urine samples were injected into the LC-APCI-MS system in the negative-ion mode without any further pretreatment, and large amounts of isovalerylglycine were detected as the [M-H]- ion. The urinary excretion of isovalerylglycine appeared to increase after L-carnitine therapy. This analytical method is quick and easy and it may be a useful tool in understanding dysfunctional conditions in isovaleric acidemia.
Parasympathetic preganglionic neurons in the superior salivatory nucleus (SSNNs) projecting to the pterygopalatine ganglion were labeled by retrograde transport of cholera toxin B subunit (CTB) in the rat. Morphological interactions between SSNNs and afferent fibers immunoreactive (IR) for neuropeptide and amine were examined with light and electron microscopes by double-immunostaining techniques. SSNNs were found in the ipsilateral ventrolateral part of the rostral medulla oblongata. Around SSNNs, substance P-, enkephalin-, neuropeptide Y-and somatostatin-IR nerve fibers were very rich and tyrosine hydroxylase (TH)-, serotonin (5-HT)-, vasoactive intestinal polypeptide- and calcitonin gene-related peptide (CGRP)-IR axons showed moderate density. Thyrotropin-releasing hormone-containing axons were scarce in this region. The electron microscopic examinations revealed that CTB-IR structures directly received synaptic input from axon varicosities IR for TH, 5-HT and all neuropeptides except for CGRP. These findings suggest that catecholamine, 5-HT and the neuropeptides directly influence the activity of SSNNs and are concerned with the autonomic regulation of nasal and palatal mucosa, lacrimal glands and cerebral blood vessels of the rat.
