Normal values of inner canthal distance, interpupillary distance and palpebral fissure length in normal Chinese children in Taiwan.

Ocular measurements, including inner canthal distance, outer canthal distance, interpupillary distance, and palpebral fissure length are important in the evaluation of congenital deformities and posttraumatic telecanthus. In this research, 4446 normal Chinese children in Taiwan were enrolled in our study. The sample of 284 full term neonates, 2742 infants and children aged from 1 month to 3 years, and 1420 preschool children were measured for inner canthal distance, outer canthal distance, interpupillary distance and palpebral fissure length. We calculated the mean value and standard deviation of the ocular measurements in normal Chinese newborns, infants and preschool children in Taiwan under 5 years. No significant sex differences were observed. Compared with previous studies, inner canthal distance, outer canthal distance and interpupillary distance in Chinese children in Taiwan were wider than those in Caucasian children, but the palpebral fissure length was not significantly different. We also found that inner canthal distance was wider than palpebral fissure length at the same age; therefore it was not correct to diagnose hypertelorism in Chinese children in Taiwan; as if an imaginary third eye could fit between the eyes. Thus, we suggest that measurements should be adjusted with normal standards specific for race. Consideration of the position of eyes is relevant for the diagnosis of a large number of syndromes.

Carrier detection of Duchenne/Becker muscular dystrophy by using fluorescent linkage analysis in Taiwan.

The mutation analysis of Duchenne/Becker muscular dystrophy (DMD/BMD) is made difficult by the size and structure of the gene. The dystrophin gene deletion is responsible for 45-58% of DMD/BMD cases in Taiwan. For the others, who have no deletions, carrier detection was performed by DNA linkage analysis. To determine frequencies of each allele and heterozygosity of each short tandem repeats (STR) marker, we analyzed 50 unrelated Taiwanese males and 50 unrelated Taiwanese females unaffected by DMD/BMD using ten fluorescently labeled intragenic markers (these ten being located in 5' terminus, intron 1, 44, 45, 48, 49, 50, 55-57 and 3' untranslated region of the human dystrophin gene). The predicted heterozygosity frequency is 46.7-88.3% in our study population and these STR markers are quite informative for linkage analysis. Using these ten intragenic STR markers, we analyzed 14 DMD/BMD families with 62 family members for carrier detection. Our retrospective study of DMD/BMD families highlights the informative power of STR haplotyping. In summary, STR analysis using (CA)n repeats within the human dystrophin gene is well suited for routine use in clinical laboratories engaged in linkage studies for carrier detection and prenatal diagnosis in DMD/BMD families.

Hypercholesterolemia and its correlates in Taiwanese elderly people.

BACKGROUND: Our study used data collected in Chung-Shing-Shin-Tseun community in Taiwan in May 1998 to evaluate the relationship between hypercholesterolemia and the cardiovascular and sociodemographic risk factors in elderly people. METHODS: Individuals aged 65 and over were recruited as study subjects. A total of 1,093 persons, out of 1,774 registered residents, were contacted in face-to-face interview. The response rate was 61.6 percent. However, only 586 respondents took blood tests and completed questionnaires. Analysis in this study was based on these 586 subjects. The t-test, chi-square analysis, and multivariate logistic regression were used to study the significant correlates of hypercholesterolemia. RESULTS: Our results showed that 66 percent were men and 34 percent were women. The mean age was 73.1 +/- 5.3 years. The mean total cholesterol value was 5.1 +/- 1 mmol/l in elderly men and 5.5 +/- 1.3 mmol/l in elderly women. The proportions of hypercholesterolemia were 43.7 percent in elderly men and 59.6 percent in elderly women. After controlling the other covariates, the multivariate logistic regression analysis showed that the significant related factors of hypercholesterolemia were age, hypertriglyceridemia, and hyperuricemia. No significant association was found between hypercholesterolemia and gender, obesity, high systolic pressure, high diastolic pressure, hyperglycemia, educational level, retirement status, or marital status. CONCLUSION: Hypercholesterolemia is significantly associated with hypertriglyceridemia and hyperuricemia in elderly people. It is important to determine other metabolic disorders if one metabolic disorder is disclosed.

Epidemiology of obesity in elderly people.

Our study used data collected at Chung-Shing-Shin-Tseun community in Taiwan in May 1998 to evaluate the relationship between obesity and the cardiovascular and sociodemographic risk factors in elderly people. Individuals aged 65 and over were recruited as study subjects. A total of 1093 persons, out of 1774 registered residents, were contacted in face-to-face interview. The response rate was 61.6 percent. However only 586 respondents took blood tests and completed questionnaires. Analysis in this study was based on these 586 subjects. The chi-square analysis and multivariate logistic regression were used to study the significant correlates of obesity. Our results showed that 66 percent were men and 34 percent were women. The mean age was 73.1 +/- 5.3 years. The overweight proportions were 24.8 percent in elderly men and 29.7 percent in elderly women. The obesity proportions were 12.7 percent in elderly men and 11.5 percent in elderly women. After controlling the other covariates, the multivariate logistic regression analysis showed that overweight and obesity were associated with hypertension, hypertriglyceridemia, and hyperglycemia. In conclusions, the prevalence of overweight and obesity is high in Taiwanese elderly people. Thus, it is necessary to evaluate other metabolic disorders if one metabolic abnormality is observed.

Hypertension and its related factors in Taiwanese elderly people.

BACKGROUND: Our study used data collected in Chung-Hsing Village in May 1998 to evaluate the prevalence of hypertension and its correlates in Taiwanese elderly people. METHODS: All of individuals aged 65 and over were recruited as study subjects. A total of 1,093 persons, out of 1,774 registered residents, were contacted by face-to-face interview The response rate was 61.6 percent. However, only 586 respondents had blood tests and completed questionnaires. Analysis in this study was based on these 586 subjects. In order to study the significant correlates of hypertension, the t-test, chi-square analysis, and multivariate logistic regression were used. RESULTS: Our results showed that 66 percent were men and 34 percent were women. The mean age was 73.1 +/- 5.3 years. The proportions of hypertension were 53.09 percent in men and 56.06 percent in women (p > 0.05). After controlling the other covariates, the multivariate logistic regression analysis showed that the significant related factors of hypertension were obesity (OR = 1.88, 95 percent CI = 1.06-3.34, p < 0.05) and renalfunction impairment (OR = 1.69, 95 percent CI = 1.02-2.80, p < 0.05). CONCLUSIONS: The prevalence of hypertension was high in elderly people. Hypertension is significantly associated with obesity and renalfunction impairment in elderly people.

Skill and selection bias has least influence on motor unit action potential firing rate/frequency.

INTRODUCTION: Motor unit action potential (MUAP) rise time in quantitative electromyography (QEMG) using multi-MUAP analysis may influence neuromuscular disease diagnosis. OBJECTIVE: To demonstrate in QEMG that MUAP firing rate/frequency (FR) is the parameter least dependent on electrodiagnostician skill (EDXc) skill and selection bias. METHODS AND MATERIALS: During minimal contraction (< 6 MUAPs/4.8 s analysis period) of tibialis anterior, 20 MUAPs were selected for QEMG at each of the 3 insertion sites. There were three EDXcs (professor & two fellows), each an independent MUAP selector + EMG machine itself (EMGM), for a total of four independent MUAP selectors. EDXc and selector order variation at each site involved the following. At insertion site1, during EMG by EDXc 1, the MUAP selector order was EDXc 1, EMGM, EDXc 2 and EDXc 3. At the following two sites, the previous 1st selector was sequentially rotated to become the last selector during EMG by the subsequent EDXc. The protocol was repeated in two subjects. MUAP inclusion criteria by EDXcs was MUAP rise time < 1 ms by visual inspection and, by the EMGM, any and all MUAPs during the 4.8 s analysis period. RESULTS: The reproducibility of measurements between MUAP selectors for mean frequency was satisfactory (correlation coefficient for inter-EDXc was 0.57, and 0.66 between the most experienced EDXc and EMGM. Inter- EDXc correlation coefficient was poor for amplitude (0.10), duration (0.34), size index (0.35), phases (-0.13) and turns (-0.22). Of the MUAPs collected by EMGM, the mean amplitude was smaller and mean duration was longer than those collected by the other 3 selectors with no significant differences for the other parameters. CONCLUSIONS: FR was the only satisfactorily reproduced parameter from QEMG of MUAPs between selectors, and least dependent on skill and/or selection bias. From this, FR appears a relatively objective parameter, while other parameters appear unduly influenced by EDXc skill and/or MUAP selection bias.
