Variations in humoral and cellular immunologic changes in relation to the evolutive stage of chronic active liver disease.

The humoral immune changes (of Ig, CIC, C3) and the cellular ones (T lymphocytes with the T1, T2, Ta subpopulations and B lymphocytes) have been followed up for a mean period of 36 months in 22 patients with chronic active liver disease (CALD), of whom 11 with chronic active hepatitis and 11 with active liver cirrhosis. Thirteen of these patients received long term corticotherapy and 9 only a hepatotropic treatment. A comparison of the mean values of the immune parameters investigated in the active and in the remission stages has revealed an improvement of the immune disorders in the remission stages, both at humoral and cellular levels, more evident in the patients treated by corticotherapy.

Diagnostic value of echotomography in alcoholic liver disease.

In 32 patients with alcoholic liver disease a comparison was made between the classical liver morphologic changes (laparoscopic and histologic) and the echotomographic aspects, to demonstrate the diagnostic value of the latter. Though the echographic changes were nonspecific, they supply some indications on the anatomoclinical form of disease. The only element of echotomography with certain value is the differentiation between cirrhosis and the other forms: steatosis and alcoholic hepatitis. To this, some possibilities of detecting incipient fibrosis and concomitant involvement of the pancreas can also be added.

Persistent acute hepatitis, an evolutive modality of the acute viral hepatitis, with high cirrhogenic potential.

An early detection of the progress to chronic stages of the acute diseases has particular pathogenic and therapeutic implications in hepatology. A complex clinical, biologic and morphologic study has been carried out in 86 patients with persistent acute hepatitis, recently released after 4-8 weeks of hospitalization for acute viral hepatitis, who were submitted to sequential investigations and follow up for a mean period of 16 months (range 2-24 months). All the patients have shown signs of activity of the liver injury, more than three months after the viral hepatitis onset (GOT/GPT 134 +/- 41 KU/219 +/- 59 KU, gammaglobulins 24.2 +/- 2.4%) and a characteristic immune pattern. In some cases, the morphologic investigations (endo-histologic and infrastructural) have revealed elements of acute and chronic active hepatitis. In 70% of the case the disease had a favourable course, while 30% of them showed a tendency to chronicization and even to cirrhosis, within a period of two years.

Action of calcium channel inhibitors on acid gastric secretion in ulcer patients.

The action of an inhibitor of the calcium channels (Verapamil) on stimulated gastric acid secretion was studied in 14 patients with duodenal ulcer. Two histamine stimulations (0.5 mg) of acid gastric secretion were performed at an interval of 48 hrs, the second being preceded by Verapamil administration in doses of 80 or 120 mg. The results obtained, showed a varied response to the drug: decrease of acid secretion by 40% in half of the cases no influence or even increase of acid secretion in the rest of the cases. Correlation with the prevalent secretory type showed the inhibitory action of Verapamil in the beta-adrenergic-gastrin type thus pleading for an extracellular, probably membranous action of the drug.

Influencing immune perturbations by prolonged corticotherapy in the chronic active liver disease.

This paper deals with the influencing of the imbalance of humoral and cellular immunity in chronic active liver disease (CALD) due to prolonged administration of continuous corticotherapy in moderate doses. This paper presents a controlled study carried out in 30 patients with CALD of whom 15 received corticotherapy, while the other 15 only a conventional hepatotropic treatment for a year. The results obtained show, beside a clinical and biological improvement, a positive influence on the immune parameters with a tendency of restoration of immune homeostasis, i.e., an increase of the T total and T1 (suppressing/cytotoxic) lymphocytes in the peripheral blood, with the concomitant decrease of IgG, IgM and circulating immune complexes (CIC). In the control group the changes were insignificant, but sometimes an aggravation of the immune disturbances could be observed. This study confirms the favourable effect of prolonged corticotherapy in CALD and argues for its immune-modulating action.
