[Application of a lung stapler to reduce tension on the internal mammary artery].

In-situ internal mammary artery (IMA) grafts have a better patency rate than free IMA grafts. Every effort is made to leave the IMA as an in-situ graft attached to the subclavian artery. A 56-year-old patient underwent reoperation because of occlusion of the vein graft to the left anterior descending coronary artery (LAD). At the second operation, the left IMA was anastomosed to the segment of the LAD distal to the first anastomotic site. The hyperinflated lung crossed the midline and stretched the IMA graft. The herniated portion of the left lung which stretched the IMA graft was resected with a 9-cm long linear stapler along the course of the IMA graft. No harmful effects were observed. This is a simple and safe method to reduce the tension on the IMA graft due to the hyperinflated lung.

[Analysis of the retinal blood flow by means of the blue field entoptic phenomenon and simulation of entoptic leucocytes with a personal-computer system].

A method was developed for determining the velocity of macular blood flow. Leukocytes flowing in the macular capillaries can be recognized when the retina is illuminated with blue light (blue field entoptic phenomenon). The motion of the leukocytes was simulated by a personal-computer system. The velocity of the macular blood flow was determined by adjusting the motion of the simulated corpuscles to that of the entoptically recognized leukocytes. The velocity of the maximum, minimum and mean macular blood flow were measured as 1.25 +/- 0.25, 0.42 +/- 0.11 and 0.80 +/- 0.14 mm/sec respectively. The mean coefficients of variation were 6.7%, 13.4% and 5.7% for each velocity. This method may be useful for the analysis of the retinal circulation and the diagnosis of retinal diseases.

[Spontaneous coronary artery dissection with left ventricular aneurysm--a case report of successful surgical repair].

Spontaneous coronary artery dissection is a rare entity. To our knowledge, 86 cases have been reported, and there are 7 operative treatment. The patient was 56 year-old male with a history of the previous myocardial infarction. A selective coronary angiography demonstrated marked dilatation of both coronary arteries. The thin radiolucent lines were shown within the LAD and first diagonal branch as a result of partial separation of the intima. A very large left ventricular myocardial aneurysm was also demonstrated within anteroapical walls, he underwent open heart surgery with left ventricular myocardial aneurysmectomy and aorto-first diagonal branch saphenous vein graft. This present case is the second report which describe a successful surgical treatment for the spontaneous coronary artery dissection with left ventricular aneurysm.

[Evaluation of internal mammary artery graft patency by intraarterial digital subtraction angiography].

Intraarterial digital subtraction angiography (DSA) was performed to evaluate graft patency in 45 patients undergoing coronary artery bypass grafting with the use of internal mammary artery (IMA). Fourty-five IMA grafts and 59 saphenous vein grafts were evaluated using a DSA equipment with a pulse mode of 2 frames per second. A 4F catheter was introduced percutaneously into the left brachial artery. Nonionic contrast medium was nonselectively injected into the left subclavian artery for visualization of IMA grafts and also into the aortic root for visualization of vein grafts. No serious complications occurred. Forty-four IMA grafts and 53 vein grafts were patent. The patency rate was 97.8% for IMA grafts, 89.8% for vein grafts, and 93.3% for all grafts. In 11 patients (24%), intercostal or pericardial branches were visualized. Two patients showed a string sign of the IMA. Intraarterial DSA with nonselective injection of contrast medium into the subclavian artery is a new method for evaluation of IMA graft patency with a low risk.

Novel phospholipids with aliphatic dicarboxylic acid residues in a lipid extract from bovine brain.

A vasodepressor phospholipid fraction purified from a lipid extract of bovine brain was found to contain novel phospholipids with both a long-chain acyl group and an aliphatic dicarboxylic acid residue. This was shown by analyzing the fraction as tert-butyldimethylsilyl derivatives of glyceride by capillary GC-MS after hydrolysis with phospholipase C. Six molecular species with a palmitoyl group and an aliphatic dicarboxylate (chain length C4-C9), and two species with both a stearoyl group and a succinate or glutarate residue were detected.

A hydrophobic protein, chargerin II, purified from rat liver mitochondria is encoded in the unidentified reading frame A6L of mitochondrial DNA.

Previous studies showed that a hydrophobic protein called chargerin II may have a key role in energy transduction of oxidative phosphorylation, since antibody against chargerin II labeled with monoazide ethidium inhibited ATP synthesis, ATP-Pi exchange, and reversed electron flow from succinate to NAD coupled with succinate oxidation by O2. In the present work, unlabeled chargerin II was purified from intact rat liver mitochondria by high performance liquid chromatography. The purified preparation of chargerin II, which was a single protein as judged by polyacrylamide gel electrophoresis and Western blotting, was digested with lysylendopeptidase. The digest was separated on a reverse-phase column into five peptides, which all cross-reacted with the antibody against chargerin II, indicating that they were fragments of chargerin II. The sequences of two of these peptides (a total of 12 amino acids) were determined and found to be highly homologous with the sequence of the carboxyl-terminal peptide of the putative polypeptide encoded by the unidentified reading frame A6L (URFA6L) of mammalian mitochondrial DNA. The amino acid compositions of the purified preparation of chargerin II were in good accord with those of the putative product of the URFA6L. Thus, we concluded that chargerin II is encoded by the URFA6L. This is the first demonstration that the URFA6L product was identified in rat liver mitochondria and purified from the membranes.

Bilateral giant intracavernous aneurysms. Technique of unilateral operation.

We report a case of bilateral intracavernous giant aneurysms of which the right side was clipped by direct surgery. Our case was a 74-year-old woman who developed bilateral abducens palsy. Computed tomography and angiography showed bilateral intracavernous giant aneurysms. This patient was followed up in the outpatient clinic. She developed headache and right trigeminal neuralgia 2 years later. The trigeminal pain was quite severe and very disturbing to her. Repeat angiography showed an increase in the size of the aneurysms. An operation was carried out after a balloon Matus test monitoring electroencephalogram, somatosensory evoked potential, and clinical symptoms. The cavernous sinus was opened and the aneurysm was clipped. There was no postoperative complication except right ophthalmoplegia which resolved after 3 months. Headache and trigeminal neuralgia also disappeared.

Serum CA 125 levels in patients with endometriosis: changes in CA 125 levels during menstruation.

We investigated serum CA 125 levels in 120 patients with benign gynecologic disease, and examined variations in CA 125 levels during the menstrual cycle in six normally cycling women. CA 125 levels were elevated transiently during menstruation, suggesting that blood samples taken during menstruation can give a false-positive result. Whereas the CA 125 values during the nonmenstrual part of the cycle were significantly higher in the patients with adenomyosis and advanced endometriosis (stages III and IV) than in those with a normal pelvis, there was no difference between the patients with mild endometriosis (stages I and II) and the normal-pelvis group. CA 125 levels during menstruation were significantly elevated, not only in women with adenomyosis and advanced endometriosis, but also in those with mild endometriosis, compared with those with a normal pelvis. These data suggest that the measurement of serum CA 125 during menstruation may increase the rate of detection of endometriosis over that in studies done during other phases of the cycle.

[Multiple primary malignant tumors of rhabdomyosarcoma in an upper arm and breast cancer].

Multiple primary malignant tumors of rhabdomyosarcoma in a upper arm and cancer of breast are extremely rare. In September 1970, a female, aged 86 years, underwent the first resection operation performed on a pleomorphic rhabdomyosarcoma of the right upper arm. She underwent radiation therapy (8000 rads at the operative field) postoperatively. In August 1980, a simple mastectomy was performed for a cancer of the right breast. In December 1984, fourteen years after her first operation, a recurrence was detected. The tumor recurred five times during the next two years and was resected each time. The patient died of cachexia sixteen years after the first operation.
