Cytochrome f encoded by the chloroplast genome is imported into thylakoids via the SecA-dependent pathway.

In vitro import of the precursor of tobacco cytochrome f, which is is encoded by the chloroplast genome, into isolated pea thylakoids was analyzed. Upon incubation with thylakoids and a stromal fraction, the precursor of cytochrome f was efficiently imported into thylakoids. The imported cytochrome f was tightly integrated into the thylakoid membrane. Insertion of cytochrome f into the thylakoid membrane was blocked by nigericin, sodium azide, and antibodies against pea chloroplast SecA. These results suggest that cytochrome f utilizes the bacterial-type SecA-dependent pathway to be imported into thylakoids.

Suppressive effect of bronchoalveolar lavage fluid from pigs infected with Mycoplasma hyopneumoniae on chemiluminescence of porcine peripheral neutrophils.

Bronchoalveolar lavage fluid (BALF) from pigs experimentally infected with Mycoplasma hyopneumoniae suppressed the chemiluminescence (CL) response of porcine polymorphonuclear neutrophils (PMN). The suppressive effect was significantly correlated with the concentration of prostaglandin E2 (PGE2) in the BALF. Furthermore, purified human PGE2 suppressed the CL response of porcine PMN. The increased level of PGE2 following infection with M. hyopneumoniae may be responsible for the suppression of PMN function in the airway of infected pigs. The decrease of PMN function may be responsible for exacerbation of mycoplasmal pneumonia by secondary infection with pulmonary bacterial pathogens.

Repression by a differentiation-specific factor of the human cytomegalovirus enhancer.

We detected a novel nuclear protein, MRF, that binds to multiple sites on the modulator which is located upstream of the human cytomegalovirus major immediate early gene enhancer. The expression of MRF is differentiation specific; the DNA binding activity is present in nuclear extracts from undifferentiated Tera-2 and THP-1 cells, but significantly reduced after these cells are induced to differentiate. In undifferentiated cells the enhancer activity is repressed by the modulator and upon differentiation the enhancer becomes active. Competitive binding assays demonstrate that MRF requires the presence of multiple A+T stretches for binding to DNA, rather than binding to a specific DNA sequence. Mutations of these stretches in the modulator reduce the binding activity of MRF, as well as the repressing activity on the enhancer. These results suggest that MRF may act as a repressor of enhancer function. We propose that MRF binds over the entire modulator and exerts repressor activity.

Traumatic laceration of the intracranial vertebral artery causing fatal subarachnoid hemorrhage: case report.

A 36-year-old man who had been drinking alcohol had a fatal subarachnoid hemorrhage immediately after suffering a moderate craniofacial injury. Autopsy revealed a 3-mm longitudinal laceration of the left intracranial vertebral artery proximal to the posterior inferior cerebellar artery. There was no finding of arterial dissection. We discuss the mechanisms of the traumatic laceration of the vertebral artery in relation to traumatic dissection of the vertebral artery.

Cricoid pressure applied after placement of the laryngeal mask prevents gastric insufflation but inhibits ventilation.

We studied 50 patients, in a blind, crossover study, to assess if cricoid pressure applied after placement of the laryngeal mask prevented gastric insufflation without affecting ventilation. After induction of anaesthesia and neuromuscular block, a laryngeal mask was inserted and confirmed to be placed correctly. The lungs were ventilated with a maximum inflation pressure of 15 cm H2O. In the first 25 patients, expiratory volumes were measured with and without cricoid pressure (30 N). On both occasions, a free hand was placed under the patient's neck. In the next 25 patients, the effect of cricoid pressure on ventilation without support of the neck was also studied. The effect of cricoid pressure with support of the neck on gastric insufflation was then assessed using a stethoscope in all 50 patients, while the lungs were ventilated with a maximum inflation pressure of 30 cm H2O. At the end of the study, the position of the mask was re-assessed. Cricoid pressure significantly decreased mean expiratory volume (P << 0.001). This inhibitory effect was significantly greater when the pressure was applied without support of the neck (P << 0.001). Cricoid pressure significantly reduced the incidence of gastric insufflation (12 patients vs one patient; P << 0.001; 95% CI for difference 10.5-33.5%). In no patient was the mask dislodged after these procedures. Thus, although cricoid pressure applied after insertion of the laryngeal mask prevented gastric insufflation, it also decreased ventilation. The inhibitory effect of cricoid pressure on ventilation without support of the neck was greater than cricoid pressure with support of the neck.

Insertion methods of the laryngeal mask airway. A survey of current practice in Wales.

The efficacy of the laryngeal mask is widely accepted, but there is a lack of consensus on the best insertion method and on the use of the mask for certain surgical procedures. We sent a questionnaire to all anaesthetists in Wales to discover the frequency of use of the laryngeal mask and the preferred insertion method. The questionnaire also enquired about the use of the laryngeal mask during anaesthesia for laparoscopic clip sterilisation. Replies were received from 125 consultants (89% of those circulated) and 122 non-consultants (69%). The insertion method described in the manufacturer's instruction manual was preferred by 30% of consultants and 34% of the others. The next most popular option was insertion of the mask with the cuff partially inflated. Twenty-three per cent of consultants and 34% of non-consultants were prepared to use the laryngeal mask during anaesthesia for laparoscopic clip sterilisation. Although the insertion technique described in the instruction manual is the most widely employed, a large number of alternative methods are frequently used.

The view of the glottis at laryngoscopy after unexpectedly difficult placement of the laryngeal mask.

In 12 patients to whom a non-depolarising neuromuscular relaxant had been given and in whom placement of the laryngeal mask had failed unexpectedly, the view of the larynx at laryngoscopy and the ease of tracheal intubation were examined. The glottis was only partially seen at laryngoscopy in three patients and was not seen at all in another three patients. Tracheal intubation was difficult in three of them. It would appear that in some patients both placement of the laryngeal mask and tracheal intubation are difficult. It is thus inadvisable to paralyse patients electively and rely on the laryngeal mask to secure a clear airway when tracheal intubation is predicted to be difficult.

Jaw thrusting as a clinical test to assess the adequate depth of anaesthesia for insertion of the laryngeal mask.

We have studied the efficacy of the loss of response to jaw thrust as a clinical test to assess adequate depth of anaesthesia for insertion of the laryngeal mask in 60 patients. After induction of anaesthesia with propofol (infused using a syringe driver), the patients were randomly allocated to one of two groups. In one group, insertion of the laryngeal mask was attempted immediately after the loss of verbal contact and in the other group, after the loss of motor response to a jaw thrust. Conditions for insertion of the laryngeal mask were assessed. The mean dose of propofol required to obtain loss of verbal contact was 1.94 mg.kg-1 (SD 0.39, 95% confidence intervals (CI) 1.79-2.08 mg.kg-1) and that for the loss of response to jaw thrust was 2.55 mg.kg-1 (SD 0.46, 95% CI 2.38-2.72 mg.kg-1). When depth of anaesthesia was assessed using jaw thrusting, it was always possible to insert the mask and the conditions were optimal in 87% (95% CI 72-95%) of patients. Neither coughing nor gagging occurred. In contrast, conditions were almost always less than optimal when insertion was attempted after the loss of verbal contact. Conditions were significantly better when jaw thrust was used as a clinical test compared with loss of verbal contact (p < < 0.001). No marked haemodynamic depression occurred in any patient. Thus, jaw thrust is a reliable clinical test to assess the adequate depth of anaesthesia for uncomplicated insertion of the laryngeal mask after induction of anaesthesia with propofol.

Signalling defect in FMLP-induced neutrophil respiratory burst in myelodysplastic syndromes.

Myelodysplastic syndromes (MDS) are clonal haematological disorders and MDS neutrophils have various abnormal functions which cause an increased risk of infective mortality. We examined luminol-dependent chemiluminescence and cytoplasmic Ca2+ increase in order to characterize the mechanisms of a signalling defect in MDS neutrophil respiratory burst. In MDS patients, chemiluminescence stimulated with N-formyl-L-methionyl-L-leucil-L-phenylalanine (FMLP) and calcium ionophore A23187 was defective (17.2 +/- 13.7 v 44.3 +/- 16.6, P = 0.001; 42.2 +/- 21.3 v 82.0 +/- 23.6, P < 0.05, respectively), but phorbol 12-myristate 13-acetate (PMA) chemiluminescence was normal (73.4 +/- 26.9 v 79.5 +/- 23.8, P = 0.52). There were no statistical significances in cytoplasmic Ca2+ increase stimulated with FMLP and recombinant human interleukin-8 (rhIL-8) compared with controls (251.1 +/- 104.3 v 272.7 +/- 41.2, P = 0.295; 238.6 +/- 65.0 v 253.9 +/- 38.3, P = 0.567, respectively). Flow cytometric analysis of MDS neutrophils disclosed that most MDS patients showed normal neutrophil cytoplasmic Ca2+ response to FMLP and rhIL-8. However, two patients with refractory anaemia with excess of blasts displayed a significant decrease of both chemiluminescence and cytoplasmic Ca2+ response to FMLP, and they also displayed low expression of FMLP receptor. These data suggest that most MDS patients have low FMLP chemiluminescence which is not due to a defect in the FMLP receptor. It is proposed that defective FMLP chemiluminescence in MDS results from a putative defect in protein kinase C- and Ca(2+)-independent cell-signalling mechanisms. Only a small group of patients have numerical or structural defects in the FMLP receptor, causing significant decrease of neutrophil respiratory burst.

Phorbol ester activation of chloride current in guinea-pig ventricular myocytes.

1. Although earlier studies with phorbol esters indicate that protein kinase C (PKC) may be an important regulator of Cl- current (Icl) in cardiac cells, there is a need for additional quantitative data and investigation of conflicting findings. Our objectives were to measure the magnitude, time course, and concentration-dependence of Icl activated in guinea-pig ventricular myocytes by phorbol 12-myristate 13-acetate (PMA), evaluate its PKC dependence, and examine its modification by external and internal ions. 2. The whole-cell patch clamp technique was used to apply short depolarizing and hyperpolarizing pulses to myocytes superfused with Na(+)-, K(+)-, Ca(2+)-free solution (36 degrees C) and dialysed with Cs+ solution. Stimulation of membrane currents by PMA (threshold < or = 1nM, EC50 approximately equal to 14 nM, maximal 40% increase with > or = 100 nM) plateaued within 6-10 min. 3. PMA-activated current was time-independent, and suppressed by l mM 9-anthracenecarboxylic acid (9-AC). Its reversal potential (Erev) was sensitive to changes in the Cl- gradient, and outward rectification of the current-voltage (I-V) relationship was more pronounced with 30 mM than 140 mM Cl- dialysate. 4. The relative permeability of PMA-activated channels estimated from Erev measurements was I- > Cl- > > aspartate. Channel activation was independent of external Na+. 5. PMA failed to activate Icl in myocytes pretreated with 1-(5-isoquinolinesulfonyl)-2-methylpiperazine (H-7) or dialysed with pCa 10.5 solution. Lack of response to 4 alpha-phorbol 12, 13-didecanoate (alpha PDD) was a further indication of mediation by PKC. 6. Icl induced by 2 microM forskolin was far larger than that induced by PMA, suggesting that endogenous protein kinase A is a much stronger Cl- channel activator than endogenous PKC in these myocytes. 7. The macroscopic properties of PMA-induced Icl appear to be indistinguishable from those of PKA-activated Icl. We discount stimulation of PKA by PMA as an explanation, and conclude that endogenous PKC may activate PKA-regulated Cl- channels in these myocytes.

Activation of cardiac chloride conductance by the tyrosine kinase inhibitor, genistein.

1. Genistein (GST), an inhibitor of protein tyrosine kinase (PTK), Na3VO4 (VO4), an inhibitor of phosphotyrosine phosphatase (PTPase), and forskolin (FSK), an activator of the cyclic AMP-dependent, cystic fibrosis transmembrane conductance regulator (CFTR) Cl- channel, were applied to guinea-pig ventricular myocytes to probe for a possible role of tyrosine phosphorylation in the regulation of cardiac Cl- channels. 2. Myocytes in the standard whole-cell configuration were pulsed to various potentials and Cl- current (ICl) measured as the difference from control background current. GST (1-500 microM) activated a current that had similar biophysical properties (time- and voltage-independent; Cl(-)-dependent reversal potential and outward rectification) as ICl activated by 5 microM FSK. The EC50 for activation of Cl- conductance (gCl) by GST was approximately 100 microM, and gCl activated by GST (500 microM) was as large as gCl activated by maximally-effective FSK (5 microM). Daidzein, a GST analogue with little effect on PTK, was at least one order less effective than GST. 3. GST responses were rapidly and reversibly inhibited by 0.1-1 mM VO4 treatments that had little effect on FSK-activated ICl. 4. Niflumic acid (100-200 microM) reversibly depressed GST (100 microM)-activated gCl by 55%. 5. GST (50 microM) strongly incremented current in myocytes with cyclic AMP-dependent CFTR ICl already activated by maximally-effective FSK 5 microM. 6. Based on these results, and on evidence of a synergistic interaction between GST and FSK, we conclude that inhibition of tyrosine phosphorylation by GST causes an activation of cardiac CFTR that is not mediated by an elevation of cyclic AMP.

Surgical results of tympanoplasty with hokudai-shaped partial and total ossicular replacement prostheses.

The surgical results of tympanoplasty using a ceramic partial ossicular replacement prosthesis (PORP) or a ceramic total ossicular replacement prosthesis (TORP) were analyzed. The present study included 25 ears in 22 patients who underwent tympanoplasty between December 1986 and September 1994 and were followed up for more than 6 months postoperatively. Because the ceramic prosthesis was extruded from 2 of the 25 ears, the surgical results in the other 23 ears were analyzed, Surgical results were assessed according to a criterion that permits comparison with data from around the world. The criterion considers a postoperative airbone gap within 20 dB as a success. According to this criterion, the success rate was 90.9% for PORP, 50.0% for TORP, and 69.6% in total. These results are comparable with those found at other centers.
