Biallelic inactivating mutations and an occult germline mutation of PTEN in primary cervical carcinomas.

A tumor suppressor gene on chromosome sub-band 10q23.3, PTEN, is frequently mutated or deleted in a variety of human cancers. Germline mutations in PTEN, that encodes a dual-specificity phosphatase, have been implicated in two hamartoma-tumor syndromes that exhibit some clinical overlap, Cowden syndrome and Bannayan-Zonana syndrome. Although cervical cancer is not a known component of these two syndromes, loss of heterozygosity (LOH) of markers on chromosome arm 10q is frequently observed in cervical cancers. To determine the potential role that PTEN mutation may play in cervical tumorigenesis, we screened 20 primary cervical cancers for LOH of polymorphic markers within and flanking the PTEN gene, and for intragenic mutations in the entire coding region and exon-intron boundaries of the PTEN gene. LOH was observed in 7 of 19 (36.8%) cases. Further, one sample may have homozygous deletion. Three (15%) intragenic mutations were found: two were somatic missense mutations in exon 5, that encodes the phosphatase motif, and an occult germline intronic sequence variant in intron 7, that we show to be associated with aberrant splicing. All three samples with the mutations also had LOH of the wild-type allele. These data indicate that disruption of PTEN by allelic loss or mutation may contribute to tumorigenesis in cervical cancers. In cervical cancer, unlike some other human primary carcinomas, e.g., those of the breast and thyroid, biallelic structural PTEN defects seem necessary for carcinogenesis. Further, one in 20 unselected cervical carcinomas was found to have a germline PTEN mutation; it is unclear whether the patient with this mutation had Cowden disease or a related syndrome.

[Endoscopic surgery in obstetrics and gynecology].

There are three types of endoscopic surgery in obstetrics and gynecology: laparoscopy; hysteroscopy; and salpingoscopy. The main indications for laparoscopic surgery are myoma uteri, ovarian tumors, ectopic pregnancy, infertility, and endometriosis. Currently the indications are limited to benign disease. The main indications for hysteroscopic surgery are submucous myoma uteri, uterine anormalies, and Asherman syndrome. During hysteroscopy the uterine cavity is dilated with liquid and then a resectoscope or hysterofiberscope is used. Salpingoscopy is useful in examining the tubal cavity and a potent tool for tubal recanalization. Endoscopic surgical techniques will develop rapidly in the near future to serve the various needs of female patients.

[Advanced gastric cancer: the findings of delayed phase dynamic CT and radiologic-histopathologic correlation].

PURPOSE: The aim of this study was to describe delayed phase dynamic CT findings of advanced (T2-T4) gastric cancer and to correlate with histopathologic findings. METHOD/MATERIALS: Quadruple phase dynamic CT including delayed imaging taken five minutes after the start of injection of contrast material was performed in 43 patients with 45 advanced gastric cancer and 20 control subjects with no gastric lesions. On delayed phase CT scans, the attenuation of the gastric wall was equal to or lower than that of the liver parenchyma in the control subjects, therefore, the presence of higher attenuation in the gastric wall was considered to be abnormal and defined as delayed enhancement. Histopathologic findings in the tumors showing delayed enhancement were compared with those in the tumors without this feature. RESULTS: Delayed enhancement was seen in 26 (57%) of the 45 tumors. Eleven of 25 differentiated-type tumors and 15 of 20 undifferentiated-type tumors showed delayed enhancement (P < .05). Delayed enhancement was seen in one of five medullary type tumors, in 11 of 25 intermediate-type tumors, and in 14 of 15 scirrhous-type tumors (P < .005). CONCLUSIONS: Delayed enhancement was frequently seen in the tumors with abundant fibrous tissue stroma. Delayed phase dynamic CT may be useful for the characterization of advanced gastric cancer.

Laparoscopy with ultrasonographic guidance of intraamniotic methotrexate injection for ectopic pregnancy. A report of two cases.

BACKGROUND: The efficacy of laparoscopic intratubal injection of methotrexate is reportedly controversial. CASES: A 29-year-old woman with an interstitial-isthmic pregnancy and a 30-year-old woman with an ampullary pregnancy surrounded by dense adhesions were treated with intraamniotic injection of methotrexate with ultrasonographically guided laparoscopy (LUS). The ectopic pregnancies resolved quickly, without further intervention. The human chorionic gonadotropin concentrations were negligible by 18 and 25 days. CONCLUSION: Intraamniotic methotrexate injection with LUS is an effective treatment for unruptured interstitial and tubal pregnancies in patients with dense adhesions.

[Immunoadsorption therapy with TR-350 (tryptophan column) for Guillain-Barré syndrome: investigation including serum antiganglioside antibody assay].

Immunoadsorption therapy (IAT) using TR-350 was performed for 14 patients with Guillain-Barré syndrome (GBS). Presence of serum antiganglioside antibodies (AGA) was investigated in all the patients in the acute phase. In 14 patients studied, 6 men and 8 women, ages from 24 to 74 years(mean, 42.5 years), 7 patients had suffered from common cold and 3 from diarrhea before neurological onset. Ophthalmoplegia was seen in 6 patients, facial palsy in 6, bulbar palsy in 3 and cerebellar sign in 2. Functional grade scores (FGS) by Hughes et al. of the patients were from 5 to 1 (mean, 3.3). In 4 patients, whose FGS were 1 or 2, IAT were performed, because of worsting of bulbar palsy, bilateral facial palsy and limb weakness. Relapse occurred in one patient. IAT was started from 2 to 18 days (mean, 8.4) after neurological onset and performed 3 to 14 times (mean, 7.5) for each patient. Mean FGS improved from 3.3 to 2.1 after IAT. The mean time to improve 1 grade was 10.3 days and mean time to improve 2 grades was 39.0 days. The mean FGS after 1 month was 1.4 and that after 3 months was 0.4. Some of 14 patients had elevated titers of AGA in sera in the acute phase. Four patients had anti-GQ1b IgG antibody and showed external ophthalmoplegia. One patient with anti-GD1b IgG antibody had cerebellar signs as well as peripheral neuropathy. Those AGAs decreased after IAT in parallel with improvement. IAT is an effective treatment in acute phase of GBS.

A pair of related genes with antagonistic roles in mediating flowering signals.

Flowering in Arabidopsis is promoted via several interacting pathways. A photoperiod-dependent pathway relays signals from photoreceptors to a transcription factor gene, CONSTANS (CO), which activates downstream meristem identity genes such as LEAFY (LFY). FT, together with LFY, promotes flowering and is positively regulated by CO. Loss of FT causes delay in flowering, whereas overexpression of FT results in precocious flowering independent of CO or photoperiod. FT acts in part downstream of CO and mediates signals for flowering in an antagonistic manner with its homologous gene, TERMINAL FLOWER1 (TFL1).

A graphical method of analyzing pH dependence of enzyme activity.

A method for graphically analyzing the pH dependence of enzyme activity is presented. Linear plots can be made to test the applicability of the assumed models. For three simple cases, if the pH dependence curves are measured at two substrate concentrations, it is possible to evaluate ion dissociation constants from the linear plots. The method would be useful for studies of pH effects on enzyme activities if applied complementarily to other methods.

Time-resolved high-performance liquid chromatography fluorescence detector using a nanosecond pulsed light source for detecting lanthanide-chelated compounds.

We have constructed a time-resolved fluorescence detection (TRFD) system for the analysis of amino compounds with high-performance liquid chromatography (HPLC) using lanthanide ion chelates. In order to carry out time-resolved measurements, we have employed a nanosecond pulsed xenon-arc lamp as an excitation light source. Amino compounds derivatized by isothiocyanobenzyl-EDTA (IEDTA) with the lanthanide chelate are mixed with an enhancer solution in a post-column manner and detected by TRFD. Taking advantage of a property of the long fluorescence lifetime of the lanthanide chelates, high selectivity against background fluorescence was achieved. In order to demonstrate the usefulness of TRFD, fundamental performance tests were carried out. Details of the system are also described.

Caspase activity is required for nephrogenesis in the developing mouse metanephros.

Programmed cell death is a mechanism through which organisms get rid of unwanted cells and is thought to be an important process in organogenesis. Although large-scale cell death is observed in the developing kidney, the precise roles of cell death in kidney organogenesis remain to be elucidated. To address this question, we prevented cell death in metanephric explants by applying caspase inhibitors. Administration of caspase inhibitors (Z-D-CH2DCB and Ac-DEVD-CHO) effectively prevented the cell death that is normally observed in nondifferentiating mesenchymal cells. Both ureteric bud branching and nephrogenesis were prevented by caspase inhibition. Our results suggest that caspases are crucial in kidney organogenesis and cell death in the nondifferentiating mesenchyme.

Differential vulnerability of dopamine receptors in the mouse brain treated with MPTP.

We investigated the chronological changes of dopamine D1 and D2 receptors and dopamine uptake sites in the striatum and substantia nigra of mouse brain treated with 1-methyl-4-phenyl-1,2,3, 6-tetrahydropyridine (MPTP) by quantitative autoradiography using [3H]SCH23390, [3H]raclopride and [3H]mazindol, respectively. The mice received i.p. injections of MPTP (10 mg/kg) four times at intervals of 60 min, the brains were analyzed at 6 h and 1, 3, 7 and 21 days after the last the injection. Dopamine D2 receptor binding activity was significantly decreased in the substantia nigra from 7 to 21 days after MPTP administration, whereas such binding activity was significantly increased in the medial part of the striatum at 21 days. There was no alteration of dopamine D1 receptor binding activity in either the striatum or the substantia nigra for the 21 days. The number of dopamine uptake sites gradually decreased in the striatum and the substantia nigra, starting at 6 h after MPTP administration, and the lowest levels of binding activity were observed at 3 and 7 days in the striatum (18% of the control values in the medial part and 30% in the lateral part) and at 1 day in the substantia nigra (20% of the control values). These results indicate that severe functional damage to the dopamine uptake sites occurs in the striatum and the substantia nigra, starting at an early stage after MPTP treatment. Our findings also demonstrate the compensatory up-regulation in dopamine D2 receptors, but not dopamine D1 receptors, in the striatum after MPTP treatment. Furthermore, our results support the existence of dopamine D2 receptors, but not dopamine D1 receptors, on the nigral neurons. The present findings suggest that there are differential vulnerabilities to MPTP toxicity in the nigrostriatal dopaminergic receptor systems of mouse brain.

Prediction of photosynthetic production rate of ethylene using a recombinant cyanobacterium

A model of substrate inhibition for enzyme kinetics is applied to photosynthetic production of ethylene by a recombinant cyanobacterium, which exhibits light inhibition behavior. Kinetic parameters of the model were evaluated from a linear plot of the data. Parameter values obtained in this way described well the light-intensity dependence of ethylene production rate. The inhibition model was then extended to predict the production rate at higher cell concentrations, taking account of the light absorption effect by the bacteria. An explicit expression for the production rate was derived and the predicted results agreed with experimental data. An expression is also derived for the optimum light intensity which will result in a maximum production rate. In addition, predicted features of light-intensity dependence of total production conformed to reported results of another photosynthetic product. The approach of the present work may be applicable to a number of other photosynthetic processes. Copyright 1999 Academic Press

A method of graphically analyzing substrate-inhibition kinetics.

A model of substrate inhibition for enzyme catalysis was extended to describe the kinetics of photosynthetic production of ethylene by a recombinant cyanobacterium, which exhibits light-inhibition behavior similar to the substrate-inhibition behavior in enzyme reactions. To check the validity of the model against the experimental data, the model equation, which contains three kinetic parameters, was transformed so that a linear plot of the data could be made. The plot yielded reasonable linearity, and the parameter values could be estimated from the plot. The linear-plot approach was then applied to other inhibition kinetics including substrate inhibition of enzyme reactions and inhibitory growth of bacteria, whose analyses would otherwise require nonlinear least-squares fits or data measured in constrained ranges. Plots for three totally different systems all showed reasonable linearity, which enabled visual validation of the assumed kinetics. Parameter values evaluated from the plots were compared with results of nonlinear least-squares fits. A normalized linear plot for all the results discussed in this work is also presented, where dimensionless rates as a function of dimensionless concentration lie in a straight line. The linear-plot approach is expected to be complementary to nonlinear least-squares fits and other currently used methods in analyses of substrate-inhibition kinetics. Dynamic MR imaging of the carotid wall.

The purpose of this study was to clarify temporal changes of carotid wall enhancement using dynamic magnetic resonance (MR) and to correlate its findings with pathological conditions. Cervical carotid arteries of 84 consecutive patients were studied with a 1.5 T MR imager using phased array coils. Axial spoiled gradient-echo (SPGR) images (30-58 second scan time) with fat saturation were repeated 4-8 times after the injection of contrast material. We evaluated the presence and thickness of hypointense inner rims and hyperintense outer rims of the carotid wall, temporal changes of outer rim enhancement, and their changes in relation to pathological conditions. Hypointense inner rims and enhanced outer rims were clearly visualized in 87% (73/84) of our subjects. Enhancement of the outer rim was minimal in early phases and gradually increased. Patient age had a significant correlation with outer rim thickness. In the portions with large atheromatous plaques, inner rims were disrupted or thickened. A marked thickening of the outer rim was observed in one patient with arteritis. The outer rims adjacent to malignant tumors were often obscured. Our study suggests that dynamic MR images of the cervical carotid artery can uniquely demonstrate angiogenesis of the wall itself. The outer rim of the artery shows relatively rapid enhancement, and its thickness correlates with age.

Effects of ion channel modulators in the influx and efflux of Tc-99m-MIBI.

Possible involvement of cell membrane ion transport systems in the uptake and extrusion of Tc-99m-MIBI was investigated by using various buffers with or without Na+ and Ca++, and ion transport inhibitors in a tumor cell line. The ion transport modulators dimethyl amiloride (DMA), verapamil, flunarizine and monensin were used. The uptake of Tc-99m-MIBI was significantly increased in all buffers containing either Na+ or Ca++ alone or none of them. There was significantly increased uptake of Tc-99m-MIBI especially in buffers without Na+. Verapamil, a L-type Ca++ channel blocker, increased Tc-99m-MIBI uptake in all buffers. Flunarizine, which inhibits Na+/ Ca++ channels, caused significantly increased accumulation of Tc-99m-MIBI only in buffer containing both Na+ and Ca++. Monensin, a sodium ionophore, significantly increased uptake of Tc-99m-MIBI. DMA, a potent Na+/H+ antiport inhibitor, significantly inhibited the uptake of Tc-99m-MIBI in all buffers. In conclusion, Tc-99m-MIBI behaves like Na+ during its uptake and extrusion. Extrusion of Tc-99m-MIBI may involve both verapamil- and flunarizine-sensitive pathways.

Two single nucleotide polymorphisms of the hSNF5/INI1 gene.

We found two single nucleotide polymorphisms at the hSNF5/INI1 gene located on 22q11.2, encoding a member of the chromatin-remodelling SWI/SNF multiprotein complexes. A guanine/adenine polymorphism at codon 299 in exon 7, and another guanine/adenine polymorphism at 39 bp upstream of exon 9 were identified. As the gene was recently identified as a tumor suppressor gene for malignant rhabdoid tumor, this polymorphism may be useful for the genetic study of susceptibility for human malignancies of various tissue origins.

Gasless laparoscopic ovarian cystectomy during pregnancy: comparison with laparotomy.

OBJECTIVE: The efficacy and safety of gasless laparoscopic ovarian cystectomy during pregnancy were compared with those of conventional laparotomy. STUDY DESIGN: Subjects for the 2 study groups were selected from consecutive patients who satisfied all entrance criteria. Seventeen women who underwent gasless laparoscopic cystectomy were compared with 18 women who underwent cystectomy by laparotomy. The intraoperative and postoperative courses, pregnancy outcomes, and complications were compared. RESULTS: The patients' ages, body mass indexes, mean cyst diameters, fetal gestational ages, and types of cysts did not differ significantly between the two groups. Blood loss, analgesic use, and need for tocolytic agents were significantly less in the laparoscopy group than in the laparotomy group. No abortions or preterm deliveries occurred in the laparoscopy group, whereas 1 abortion was recorded in the laparotomy group. CONCLUSION: Gasless laparoscopic ovarian cystectomy offers significant advantages with respect to laparotomy for the pregnant patient.

Autofluorescence in human dentine in relation to age, tooth type and temperature measured by nanosecond time-resolved fluorescence microscopy.

Stationary and dynamic characteristics of autofluorescence in human dentine were examined with a nanosecond time-resolved fluorescence microphotometer. With ageing, the intensity of the fluorescence increased whereas the decay time decreased. There were no differences in fluorescence intensity, decay time or spectral profile among tooth types taken from the same individuals or from individuals of the same age. Fluorescence intensity was enhanced after the application of heat (37 and 50 degrees C), but remained unchanged when specimens were kept at 4 degrees C. These results suggest that the increase in fluorescence intensity was partially due to a temperature-dependent physicochemical mechanism.
