Eosinophilia in children. Cytochemical and immunological analysis of increased eosinophils.

Cytochemical and immunological studies of increased eosinophils were performed in three children with marked eosinophilia. These children were diagnosed as having non-Hodgkin's lymphoma, transient eosinophilia, and hyper-eosinophilic syndrome. All showed marked eosinophilia, with eosinophil counts of more than 5,000/mm3 in the peripheral blood and with eosinophils in all maturational stages in the bone marrow. In contrast to normal eosinophils, the increased eosinophils in three patients on cytochemical analysis showed moderately to strongly positive reactions for staining with alpha-naphthyl-butyrate esterase. Also, the mature eosinophils in the patient with lymphoma showed a strongly positive reaction for periodic acid-Schiff staining. The positive reaction for alpha-naphthyl-butyrate esterase and periodic acid-Schiff stain indicates the presence of eosinophilic proliferative disorders in these cases. Surface marker analysis of peripheral eosinophils demonstrated that an increased proportion of eosinophils expressed My7, a marker of granulocytes, in two patients with transient eosinophilia or hyper-eosinophilic syndrome, but not in a patient with lymphoma. In addition, 30-40% of eosinophils expressed Ia antigen, a differentiation marker of eosinophils, in two patients with lymphoma or hypereosinophilic syndrome. Thus, eosinophils in transient eosinophilia showed mature features, and those in hypereosinophilic syndrome showed immature features immunologically. Whereas, eosinophils in lymphoma may have a dysmaturity on the expression of Ia and My7. Cytochemical and immunological analyses of increased eosinophils may play an important role in clarifying the etiology of eosinophilia, especially its association with malignancy or other proliferative disorders.

[Preliminary results of a phase II study of 5-FU, adriamycin, and mitomycin C (FAM) in combined hepatic infusion in patients with non-resectable metastatic liver cancer].

Sixty-eight patients in which non-resectable liver metastases were the predicted limiting factor of their survival were treated with 5-FU, ADM, MMC combined hepatic infusion chemotherapy using the following regimen: 5-FU 334 mg/m2/w shot, ADM 20 mg/m2/4 shot, MMC 2.7 mg/m2/2w shot. In the case of colorectal cancer, 5-FU 167 mg/m2/day continuously for over 3 months, then 334 mg/m2/w shot was used. In every case, the catheter was placed into the hepatic artery via the left subclavian artery and an implantable portal system was used for shot infusion and a portable pump for continuous infusion. The results were as follows: Except for one drop-out case, 67 patients were able to receive this therapy. Bone marrow suppression occurred in 32.8% of patients, gastroduodenal trouble in 31.3%, and hepatic arterial occlusion in 19.4%. Response rates and 50% survival times were 65.6%, 324 days for colorectal cancer, 80.0%, 393 days for breast cancer, and 87.5%, 342 days for gastric cancer. These are preliminary results. However, we conclude that it is appropriate to go on with this study, although discussion of this regimen involving more cases is required.

[Follow-up study on triple-cup arthroplasty of osteoarthritis of the hip].

Although various operative methods have been applied to relatively young patients with osteoarthritis of the hip, none of them has provided fully satisfactory results. Since 1974 we have been performing triple-cup arthroplasty on these patients. In this paper, the design, operative procedures and postoperative management are described, and the clinical results evaluated by the rating score system set by the Japanese Orthopaedic Association are reported. There were 67 patients consisting of 64 women and 3 men. Their ages ranged from 24 to 72 years (mean 43.5 y). The period of postoperative follow-up ranged from one year to 8.5 years (mean 4.7 y) and revealed high scores in 71% but gradual declination in 29% during five years postoperatively. The main cause of the poor results was thought to be migration of the outer cup leading to recurrence of the hip joint pain.

[A study of hepatic arterial occlusion by infusion chemotherapy via the left subclavian artery].

Hepatic arterial infusion chemotherapy via the left subclavian artery was performed in 112 patients with primary or metastatic liver tumors, and those cases showing hepatic arterial occlusion were discussed. Hepatic arterial occlusion occurred in 22.3% of cases, and the rates of occurrence in males and females showed no significant difference. Therefore, when the catheter was placed and replaced in the RHA or LHA, the occurrences of arterial occlusion seemed to be relatively high, i.e., 45.5% and 50.0%. The diameter of the hepatic artery, which was measured in 25 patients with hepatic arterial occlusion (occlusion group) and 17 patients in a control group (catheter placed in position for over 150 days) on angiographs was 5.2 +/- 1.42 mm (mean +/- S.D) in the occlusion group and 6.1 +/- 1.77 mm in the control group, but the difference was not significant (p less than 0.05). Clinical symptoms and flow scintigraphy were useful for the diagnosis of hepatic arterial occlusion. However, recently, a frequent check of drug delivery has been necessary.

[The effect and CT imaging of FAM hepatic infusion chemotherapy in patients with liver metastasis from colorectal carcinoma].

A study of FAM (5-FU, ADM, MMC) hepatic infusion chemotherapy via the left subclavian artery was performed in 40 patients with liver metastasis from colorectal carcinoma. The response rate was 61.1% (1 CR and 21 PR among 36 evaluable cases), and the 50% survival period was 11.6 months. CT imaging in the 22 responders was discussed. The CT scan images showed no remarkable changes except for tumor size in 10 cases (45.5%), a decrease of marginal density in 4 cases (18.2%), an increase of calcification in 2 cases (9.1%), dimpling sign in 3 cases (13.6%) and fatty liver degeneration and abscess formation in the hepatic duct in 1 case (4.5%). These findings on CT scan were important for evaluating the effects and side effects in patients receiving hepatic infusion chemotherapy, and provided useful information for follow-up.

[Blood access puncture point pseudoaneurysms in two hemodialysis patients].

This is a report of blood access puncture point pseudoaneurysms which occurred in two hemodialysis patients. Case 1: A 58-year-old male had been undergoing hemodialysis treatment since June, 1975. In January, 1981 a subcutaneous mass had developed at the blood access puncture point above the previously superficialized left femoral artery. An operation was performed in February, 1981 and the mass was dissected. The same artery has been used since the operation for blood access without any problems. The dimensions of the egg-shaped dissected mass were 3.5 X 4 X 2.5 cm. A histological diagnosis of the wall of the mass showed that it was a pseudoaneurysm. Case 2: A 48-year-old female had been undergoing hemodialysis treatment since September, 1983. The left basilic vein, connected to the brachial artery, has been used for blood access. In April, 1984, a subcutaneous mass had developed at the blood access puncture point and an operation was performed within a few days. Operative findings revealed that the mass was a capsulized infected hematoma with a smooth but extremely thin and easily ruptured surface, and the section of the basilic vein surrounded by the mass showed evidence of necrotic change due to compression. The brachial artery was resutured at the region where it was previously connected to the basilic vein without disturbance of arterial blood flow. In July, 1985, an operation was performed in which a new blood access was constructed in the left thigh by superficializing the femoral artery and connecting its side to the end of the saphenous vein.(ABSTRACT TRUNCATED AT 250 WORDS)

Solar urticaria. The relationship of photoallergen and action spectrum.

We studied six cases of solar urticaria immunologically and photobiologically. Serum in five patients and plasma in one patient yielded an immediate reaction when injected into the patients' own skin after light irradiation. These photoallergens were gel filtrated, and the molecular weights were determined. The result was compared with the action spectrum obtained by a monochrometer. In three patients, with the action spectrum in short visible light ranges (400 to 500 nm), the molecular weight of the photoallergen was 25,000 to 45,000 daltons. In a patient with an action spectrum from ultraviolet-A to short visible light ranges (330 to 520 nm), there was another photoallergen molecular weight that was 300,000 to 1,000,000 daltons. In a patient with a wide action spectrum from ultraviolet-B to long visible light ranges (290 to 630 nm), photoallergens, which were believed to be multiple, were generated only from plasma.

The characterization of a conjugative R-plasmid isolated from Aeromonas salmonicida.

We have analyzed two related R-plasmids isolated from Aeromonas salmonicida: pAr32 and pJA8102-1, which encode resistance to chloramphenicol, streptomycin, and sulfonamides (Su). The genetic map of pJA8102-1 indicated that this plasmid consists of at least three functionally different regions. One is the region conferring ability to transfer (tra), one is the region coding for drug resistance genes (r-det), and one is the region encoding the replication functions (rep). Both plasmids contain repeated sequences (RS); pJA8102-1 has four copies of this RS, while pAr-32 has three copies. The RS sequence of pJA8102-1 are located adjacent to each of the drug-resistance genes. These sequences might have been responsible for the duplication of the Su-resistance gene on this plasmid.

Postmortem changes in uric acid and ascorbic acid in human cerebral cortex tissues excised after cardiac death.

There has been no report on the determination of uric acid (UA) in human brain and heart tissues. UA and ascorbic acid (AA) in human cerebral cortex and heart tissues excised after cardiac death have been studied by reversed-phase high-performance liquid chromatography (HPLC) with electrochemical detection (ECD). It has been found that the levels of AA and UA in the human cerebral cortex tissues tend to decrease and increase, respectively, after cardiac death as a function of time between death and forensic operation. In addition, it has been found that there is no special relationship between UA levels in human heart tissues and time after cardiac death, also that the UA levels in the heart are high as compared with those in human cerebral cortex tissues. We have emphasized that the HPLC-ECD method is useful in determining UA and AA in mammalian tissues by one-time chromatography to gain a better understanding of the relationship between disease and serum urate level.

Increased cadmium resistance of skin fibroblasts from Menkes disease patients.

Cultured skin fibroblasts from patients with Menkes disease and Wilson disease were analyzed as to their sensitivities to copper and cadmium by means of a colony-forming ability and cell growth study. All the Menkes strains exhibited about 3-fold higher levels of resistance to cadmium, whereas the cytotoxicity of copper did not differ among the Menkes, Wilson and normal fibroblast strains. The resistance to cadmium of Menkes skin fibroblasts may provide a diagnostic marker of Menkes disease and useful or valuable model for the understanding of detoxification system against heavy metals.

Ring chromosome 21 and SOD activity of blood cells.

A female infant was found to have an abnormal karyotype with mosaicism: 45,XX,-21/46,XX,r(21) (p11.2 q22.3). She showed a slightly decreased level of superoxide dismutase (SOD) activity in erythrocytes, polymorphonuclear and mononuclear leukocytes. The cuprozinc SOD (SOD-1) activity of leukocytes was 70-80% of normal level, while their manganese SOD (SOD-2) activity was within the normal range. The relationship between the chromosomal aberration and the SOD activity is discussed in relation to the gene dosage effect.
