[Liver transplantation for biliary atresia with special reference to the Japanese children who received liver transplantation in foreign countries].

Seven cases out of 8 Japanese children who received orthotopic liver transplantation (OLT) in foreign countries because of biliary atresia as of Oct 1988 are reported. Their clinical courses after OLT have been good. Despite being satisfied with the results, their parents and family feel anxiety such as long term postoperative care and side effects of immunosuppressive therapy. Cost and the handling of rejection or retransplantation were pointed out as the greatest problems related to receiving OLT in foreign countries.

[Three cases of dry cleaning dermatitis].

Acute irritant dermatitis caused by contact with dry cleaned clothes (slacks, jacket, and skirt) was observed in three young females. Irritant sensations were noticed within an hour after wearing the clothes in all cases, but two patients thought that stocking were the cause. All patients continued wearing the clothes for various reasons. When they took off the clothes 3 to 9 hours later, erythema, edema, and bullae were noted on the posterior aspect of the thighs, the inner side of the right upper arm or the belt portion of the waist. Dry cleaning solvents used were not perchloroethylene, but were so-called new petrolatum solvents in all three cases. They were composed of paraffins in one case and paraffins plus naphtens in the other two cases. All solvents contained practically no aromatic substances. No spontaneous flare up was noted in any case. Therefore, dry cleaning solvents remaining in the clothes were thought to be the causative agent of this acute irritant contact dermatitis. This seems to be the first published report of dry cleaning dermatitis in Japan.

[Morphological changes of the hamster gallbladder in the contracted and distended states].

The hamster gallbladders were investigated to clarify the morphological changes in the contracted and distended states. 1) The mucosal surface showed the highly convoluted folds in the contracted state, and the flat structure in distended state. 2) The height and width of mucosal epithelial cells were changed in correspondence with the contracted and distended state of the gallbladder. 3) A subepithelial blood capillary formed a well developed network, and possessed many pores opening mainly toward the mucosal epithelial cell. Three-dimensional structure of blood capillary was closely related to the morphological changes of the mucosal fold. 4) A contraction of distention of muscle layer of the gallbladder produced concomitant widening or narrowing of the lymphatic vessel lumen. It is suggested that muscular contraction worked as a "pump" for the lymphatic flow. 5) Smooth muscle cells in the muscle layer had many cell-to-cell junctions, but few neuromuscular junctions. It seems that there is a complicated interaction between nervous and humoral control mechanisms.

[Exercise radionuclide ventriculography in evaluating successful percutaneous transluminal coronary angioplasty (PTCA)].

To evaluate late success of PTCA in 29 patients with coronary artery disease, exercise radionuclide ventriculography was performed. Twenty-nine patients were classified into two groups according to the presence of restenosis (Group N: 19 patients without restenosis, Group R: 10 patients with restenosis). LVEF improved significantly in Group N, and the sensitivity, specificity and overall accuracy for detecting restenosis were 100%, 68% and 79%, respectively. Regional wall motion abnormality during exercise did not worsened in Group N, and the sensitivity, specificity and overall accuracy for detecting restenosis were 89%, 96%, and 94%, respectively. The left ventricular end-systolic volume during exercise increased in group R but decreased in Group N. The exercise response of the ratio of systolic blood pressure to end-systolic volume in Group N was significantly larger than that of Group R, and was considered to be useful parameter for detecting restenosis.

Assessments of left ventricular function during exercise in patients with dilated cardiomyopathy: comparison with ischemic cardiomyopathy.

Responses to supine bicycle ergometer exercise were assessed in a study population consisting of 26 patients with dilated cardiomyopathy (DCM) and 23 patients with ischemic cardiomyopathy (ICM). Left ventricular ejection fraction (LVEF) and regional wall motion were analyzed at rest and during supine bicycle ergometer exercise with radionuclide ventriculography. Although the same degree of LVEF between DCM (23 +/- 8%) and ICM (26 +/- 4%) occurred at rest, the left ventricular regional wall motion abnormality was more prominent in DCM. LVEF during the peak exercise stage in DCM was almost unchanged (24 +/- 8%), but in ICM it decreased significantly (22 +/- 5%). Exercise-induced regional wall motion abnormalities were detected in nine patients (35%) in DCM and 13 patients (57%) in ICM. Although patients with DCM are believed to have diffuse hypokinesis of the left ventricle, severe regional wall motion abnormalities (akinesis or dyskinesis) were frequently observed. During the follow-up period of up to six years, eight patients with DCM died of congestive heart failure. In eight patients with DCM who showed decreased LVEF during exercise, five patients died. However, only three of 18 patients without decreased LVEF during exercise died. Exercise-induced left ventricular dysfunction in DCM seems to be a poor prognostic sign.

[Surgical resection of pulmonary metastasis from genitourinary cancers].

Three patients who underwent surgical resection for pulmonary metastases were reviewed. The primary lesion was testicular tumor, bladder cancer and renal cell carcinoma. One of these patients is alive without disease at 30 months after the pulmonary resection, while the others died of recurrence at 3 and 7 months after the surgical resection, respectively. As a factor affecting prognosis, characteristics of the primary lesion, especially its chemosensitivity, was thought to be important. The surgical resection of pulmonary metastasis may be effective, if the indication is assessed carefully.

[A case of remarkable bronchial stenosis due to aspiration of delayed-release iron tablet].

This report describes a rare case of bronchial mucosal injury caused by aspiration of a delayed-iron tablet. A 44-year-old woman had been taking delayed-iron tablets (Feto-Gradumet) regularly because of iron deficiency anemia, and accidentally aspirated a tablet into her bronchus. After this accident, she had complained of wheezing and shortness of breath. On bronchoscopy, the bronchial mucosa of left lower lobe initially showed remarkable swelling and redness, and became corroded and narrowed gradually by iron toxicity over several months, although we tried to remove some fragments of tablet as completely as possible. While we had examined her bronchi repeatedly by bronchoscopy, the bronchial mucosa gradually normalized during 15 months, but moderate stenosis of the left basal bronchus remained. The pathological findings showed infiltration of inflammatory cells, edema and granuloma formation in the submucosal region. This report suggests that the bronchial injury was due to the toxicity of persistent ferrous sulfate eluted from the iron tablet which had adhered firmly to the wet bronchial wall for a long time.

[Various methods of catheter placement in hepatic arterial infusion--technique of catheter placement via a femoral artery].

A comparative analysis of the individual techniques was performed on a total of 106 cases undergoing various methods of catheter placement in hepatic infusion. The comparison of laparatomy (n = 44) with a percutaneous method (n = 62) disclosed a higher incidence of peptic ulcer in the latter. When two modes of percutaneous approach, including a trans-left subclavian arterial method (n = 36) and a trans-femoral arterial method (n = 26), were compared, a shorter average catheterization time (approximately 70 minutes) and a higher selective insertion rate (92.3%) were noted in the trans-femoral approach. For these reasons, our department adopts the trans-femoral method as the percutaneous method of choice. It is advisable for further studies to be made in order to clarify indications for the individual techniques and for anti-complication measures.

[Clinical significance of exercise induced ST segment depression after successful percutaneous transluminal coronary angioplasty--assessment by thallium-201 SPECT].

To evaluate the clinical significance of ST segment depression during repeated Treadmill exercise after successful PTCA, Thallium-201 SPECT was performed. The Thallium-201 SPECT was performed before, one week after and 3-6 months after PTCA. All thirty-five patients had one vessel disease and positive Thallium-201 exercise test. During follow-up period for 3-6 months, 11 of 35 patients had persistent ST segment depression. Restenosis of dilated coronary lesion was demonstrated in 6 of 11 patients. In another 3 of 35 patients, exercise induced ST segment depression was disappeared during follow-up Treadmill exercise. In 14 patients with persistent or transient ST segment depression after PTCA, Thallium-201 SPECT demonstrated transient ischemia in 5 of 6 patients with restenosis. In other 8 patients without restenosis, SPECT images did not demonstrate myocardial ischemia and coronary arteriographic findings could not verify side branch stenosis or intimal dissection which might cause myocardial ischemia. The etiology of ST segment depression after successful PTCA in one vessel disease is not produced by exercise induced myocardial ischemia but still unknown mechanisms may be present.

Rare case of internal hernia with a new type of hepatodiaphragmatic interposition of the stomach and colon.

We report a new type of retroperitoneal internal hernia with hepatodiaphragmatic interposition of the stomach and colon. The hernia neck was formed in the lesser omentum, and the hernia sac consisted of the lesser omentum and retroperitoneum. The herniated organs (the stomach and colon) were incarcerated in the right extraperitoneal subphrenic space from the left dorsal side of the sulcus vena cava. This is the first reported case of this type.

[Clinical significance of reverse redistribution in exercise thallium-201 SPECT after percutaneous transluminal coronary angioplasty].

Clinical significance of reverse redistribution on thallium image was evaluated in 54 patients who had undergone PTCA. Thallium SPECT imaging was performed one week and three to six months after PTCA. Reverse redistribution was detected eight of 54 patients one week after PTCA and five of 38 patients three to six months after PTCA. In the segments with reverse redistribution, reduced regional wall motion and lesser degree of coronary stenosis was common features (p less than 0.05) angiography. In conclusion, reverse redistribution had a tendency to appear in the region with mild myocardial injury and relatively high coronary blood flow after PTCA. But in cases with new occurrence and disappearance of reverse redistribution during follow up period, we can not assess the factors to explain these phenomena. In these segments, "coronary flow reserve", "stunned myocardium", "hibernating myocardium" or other factors may be related.

Anaphylactic symptoms due to chlorhexidine gluconate.

Six patients who developed urticaria, dyspnea, and anaphylactic shock due to topical application of chlorhexidine gluconate solution are described. Chlorhexidine gluconate was confirmed as the causative agent of type I hypersensitivity by intradermal, scratch, and epicutaneous tests. To prevent life-threatening adverse reactions, it seems important to use chlorhexidine gluconate on wound surfaces at a concentration of 0.05%, as recommended by the manufacturer; chlorhexidine gluconate may not be suitable for application to mucous membranes.

Thrombocytopenia: a complication of Kawasaki disease.

Thrombocytopenia was observed in 10 (2.0%) of 486 children with Kawasaki disease. In nine of the ten, the minimal platelet count of 94,000 +/- 38,000 (SD)/mm3 was seen on day 6.8 +/- 2.2 (SD) of illness and the platelet counts were elevated to the normal level in 1-2 weeks. Thrombocytopenia in the nine appeared to be caused via coagulation-mediated platelet consumption, while the remaining child was diagnosed as having idiopathic thrombocytopenic purpura. One of the two who had severe coagulation-mediated thrombocytopenia of less than 50,000/mm3 developed coronary aneurysms persisting over 1 year.
