Psychiatric evaluation of physical rehabilitation patients.

We conducted a study to demonstrate the frequency and types of psychiatric/psychological symptoms. A Structured Interview according to the DSM-III-R was conducted which demonstrated that 46 (41.4%) of 111 rehabilitation inpatients met the criteria for some forms of psychiatric disorders: 34 patients for major depression, 10 for adjustment disorder with anxious mood, and 2 for posttraumatic stress disorder. The remaining 65 patients (58.6%) showed normal reactions to their diseases. Average length of hospital stay for patients with major depression was significantly longer than those with no or the other types of psychiatric disease. They were also tested with Zung's Self-Rating Anxiety Scale (SAS), Zung's Self-Rating Depression Scale (SDS), and Profile of Mood States (POMS). Three psychological tests were useful in detecting depression or adjustment disorder among rehabilitation patients; however, these tests are not always specific to the type of psychiatric disorders. Patients with higher scores in those three tests should be referred to a psychiatric consultant for detailed examinations and proper treatments, if necessary.

Determining the optimal dose of lansoprazole injection by 24-hour intragastric pH monitoring.

Two studies were performed on healthy adult males using 24-h intragastric pH monitoring to investigate the acid secretion inhibitory effects of the proton pump inhibitor lansoprazole in its injectable form (lansoprazole injection). In the first study (study 1), eight subjects received 30 mg once daily and 30 mg twice daily of lansoprazole injection by the crossover method. In the second study (study 2), 12 subjects received 15 and 30 mg twice daily of lansoprazole injection and 20 mg twice daily of a histamine H2-receptor antagonist using the crossover method. The results of study 1 showed that 30 mg once daily of lansoprazole injection was not able to achieve sufficient suppression of acid secretion, and the results of study 2 suggested that the effectiveness of the three treatment regimens studied in suppressing gastric acid secretion can be ranked as follows: lansoprazole 30 mg twice daily > lansoprazole 15 mg twice daily > H2-receptor antagonist 20 mg twice daily.

Inhibitory effects of intravenous lansoprazole on gastric acid hypersecretion in patients with postoperative stress.

The inhibitory effects of intravenous lansoprazole on gastric acid hypersecretion caused by stress after surgical invasion were studied in patients who underwent surgery under general anesthesia for gastrointestinal diseases. Patients in this study received either 15 mg or 30 mg of lansoprazole by slow intravenous injection twice daily for 3 days (six injections). The total number of patients studied was 82, with 41 patients in each group. Intragastric pH was measured every 8 h after the start of administration, and the inhibitory effects of intravenous lansoprazole on postoperative gastric acid secretion were evaluated using four grades: excellent, good, fair, and poor. In the 15-mg group, 65.8% (25 of 38 patients) were classified as good or excellent, and in the 30-mg group, 75.7% (28 of 37 patients) were good or excellent. No adverse effects directly attributable to the administration of lansoprazole were observed.

An optimal order for ECG inverse problems with MDL.

A numerical simulation for body surface potential mapping was investigated using Boundary Element Method (B.E.M.). After forward simulation, we investigated the accuracy and stability of ECG inverse solution from the viewpoint of Minimum Description Length Principle (MDL). It became obvious that the solution containing up to 16th expansion components is optimal for ECG inverse problem.

Intravenous administration of lansoprazole: a preliminary study of dose ranging and efficacy in upper gastrointestinal bleeding.

Using 24-h pH monitoring, we studied the optimal dose and mode of administration of lansoprazole (LAN) injection in healthy adult male volunteers in three tests of antisecretory effect. We followed this with a dose-finding study, using the drug's suppressive effect on increased gastric acid secretion due to surgical stress as the indicator. Finally, we conducted a pilot study to determine the optimal mode of administration and efficacy of the drug in patients with haemorrhage from the upper gastrointestinal tract. Twenty-four-hour intragastric pH monitoring established the order of potency of the different regimens: LAN 30 mg b.d. > LAN 15 mg b.d.; and LAN 30 mg b.d., intravenous drip infusion = LAN 30 mg b.d., bolus injection. Lansoprazole 30 mg b.d. appeared to be slightly more effective than LAN 15 mg b.d. in suppressing increased gastric acid secretion due to post-operative stress and in haemostasis in patients with upper gastrointestinal bleeding. No severe adverse effects were observed in either study.

DNA flow cytometric and immunohistochemical analysis of proliferative activity in spindle cell haemangioendothelioma.

Although spindle cell haemangioendothelioma was initially described as a low-grade angiosarcoma, recent reports have suggested that it is a reactive or benign vascular proliferation. In order to assess the proliferative activity in spindle cell haemangioendothelioma, 12 cases, one of which was associated with Maffucci's syndrome, were immunohistochemically analysed with antibodies against proliferating cell nuclear antigen (PCNA), Ki-67 and p53. DNA flow cytometry was performed on six of the 12 cases. Seven of the 12 patients had multiple nodules or papules. Although two cases recurred once and twice, respectively, after surgery, there was no evidence of metastasis. Immunohistochemically, the percentages of PCNA, Ki-67 and p53 positive tumour cells ranged from 0.1% to 6.4% (mean 3.3%), 0.1% to 14.9% (3.5%) and 0.1% to 2.8% (1.1%), respectively, indicating a low proliferative activity and a low p53 expression in this tumour. All seven lesions from the six cases examined flow cytometrically were DNA diploid with proliferative indices (S + G2/M-phase fractions) ranging from 4.9% to 19.5% (mean, 10.9%). These findings are compatible with a bland-looking histological picture and an indolent clinical course of spindle cell haemangioendothelioma.

RAPD analysis of Aeromonas salmonicida and Aeromonas hydrophila.

The randomly amplified polymorphic DNA (RAPD) technique was used to analyse the genetic differentiation of 13 strains of Aeromonas salmonicida subsp. salmonicida, and seven strains of Aer. hydrophila. Reproducible profiles of genomic DNA fingerprints were generated by polymerase chain reaction (PCR) using a single randomly designed primer. The RAPD profiles of all the non-motile aeromonads, Aer. salmonicida subsp. salmonicida were identical. However, profiles of the motile aeromonads, Aer. hydrophila differed between isolates. These findings reveal genomic homogeneity in Aer. salmonicida subsp. salmonicida and genetic variety in Aer. hydrophila strains.

Antagonizing correlation between periodic synchronous discharges and photically induced giant evoked responses in Creutzfeldt--Jakob disease (Heidenhain type): a case study.

In a patient with Creutzfeldt-Jakob disease (Heidenhain type), which was characterized by the initial onset of visual symptoms and proved by autopsy, there was an antagonizing correlation between spontaneously-induced periodic synchronous discharges and photically induced giant evoked responses. In addition, the two electroencephalogram features appeared and disappeared in parallel with each other during the clinical course of the illness.

Intramuscular myxoma with fibrous dysplasia: a report of two cases with a review of the literature.

Two cases are reported of a rare association of intramuscular myxoma with fibrous dysplasia in a 70 and 40 year old Japanese woman, respectively. One of them had a solitary intramuscular myxoma, and the other patient suffered from two intramuscular tumors that had been initially misdiagnosed as myxoid liposarcoma. Only 24 cases of this kind of association have been recorded in the literature. This association should be taken into consideration to avoid inappropriate treatment, when cases of myxoid soft tissue tumor with a bone lesion are encountered.

Angiomyofibroblastoma of the vulva: a clinicopathologic study of seven cases.

A clinicopathologic and immunohistochemical review was made of seven cases of angiomyofibroblastoma. The patients were middle-aged women who had a slowly growing mass, measuring 1.5-6 cm in maximum dimension, located subcutaneously in the vulva. The tumors were well-demarcated and characterized by well-vascularized, alternating hypercellular and hypocellular edematous areas composed of bland, plump spindle- or oval-shaped stromal cells frequently aggregated around small blood vessels. An epithelioid appearance of the stromal cells was seen in two cases. Immunohistochemically, the stromal cells were consistently positive for vimentin and desmin, but negative for muscle specific actin, alpha-smooth muscle actin, myosin, cytokeratins, S-100 protein or von Willebrand factor. Ultrastructurally, the plump stromal cells had a small amount of peripherally located rough endoplasmic reticulum, numerous pinocytotic vesicles and abundant intermediate filaments, on which immunogold probes for desmin were localized, whereas fine filaments were few and there were no electron dense plaques. Thus, while the proliferating stromal cells expressed an immunohistochemical profile of peculiar myoid differentiation, ultrastructural findings differed from those of smooth muscle cells or those seen in typical myofibroblasts. At 1-4 years after surgery, there was no evidence of recurrence.

A new cis-acting element for RNA replication within the 5' noncoding region of poliovirus type 1 RNA.

Mouse cells expressing the human poliovirus receptor (PVR-mouse cells) as well as human HeLa cells are susceptible to poliovirus type 1 Mahoney strains and produce a large amount of progeny virus at 37 degrees C. However, the virus yield is markedly reduced at 40 degrees C in PVR-mouse cells but not in HeLa cells. The reduction in virus yield at 40 degrees C appears to be due to a defective initiation process in positive-strand RNA synthesis (K. Shiroki, H. Kato, S. Koike, T. Odaka, and A. Nomoto, J. Virol. 67:3989-3996, 1993). To gain insight into the molecular mechanisms involved in this detective process, naturally occurring heat-resistant (Hr)-mutants which show normal growth ability in PVR-mouse cells even at 40 degrees C were isolated from a virus stock of the Mahoney strain and their mutation sites that affect the phenotype were identified. The key mutation was a change from adenine (A) to guanine (G) at nucleotide position (nt) 133 within the 5' noncoding region of the RNA. This mutation also gave an Hr phenotype to the viral plus-strand RNA synthesis in PVR-mouse cells. Mutant Mahoney strains with a single point mutation at nt 133 (A to G, C, or T or deletion) were investigated for their ability to grow in PVR-mouse cells at 40 degrees C. Only the mutant carrying G at nt 133 showed an Hr growth phenotype in PVR-mouse cells. These results suggest that a host cellular factor(s) interacts with an RNA segment around nt 133 of the plus-strand RNA or the corresponding region of the minus-strand RNA, contributing to efficiency of plus-strand RNA synthesis.

In vivo effects of endothelin A- and B-receptor antagonists in guinea pigs.

Endothelin (ET)-1, a novel 21-amino acid constrictor peptide, has been recently reported to have a potential pathophysiological role in asthma. We hypothesized that ET-1 might affect guinea pig lung via different ET receptor subtypes, i.e., ETA and ETB, in vivo. To test this hypothesis, we investigated the effects of ET-1 on airways in anesthetized, open-chest, mechanically ventilated [frequency (f) = 1 Hz; tidal volume (VT) = 9 ml/kg; positive end-expiratory pressure (PEEP) = 4 cmH2O] guinea pigs in the absence or the presence of ETA and ETB selective antagonists, i.e., BQ-123 and BQ-788, respectively. We affixed alveolar capsules to the lungs to measure alveolar pressure and calculated the elastance of lung (EL) and the resistance of lung (RL), tissue (Rti), and airway (R(aw)) under control conditions and after intravenous administration of ET-1 (10(-8) mol/kg). ET-1 induced a concentration-dependent increase in RL, Rti, R(aw), and EL.BQ-123 (2 mg/kg) partially blocks delta RL and delta R(aw) during ET-1 induced constriction, while delta Rti and delta EL were not significantly affected. BQ-788 (2 mg/kg) significantly inhibited delta RL, delta Rti, delta R(aw), and delta EL during ET-1-induced constriction. The combination of BQ-123 and BQ-788 completely ablated the response to ET-1. These data suggest that both ET receptor subtypes, i.e., ETA and ETB, may have physiological roles in guinea pig airways in response to ET-1, a potential mediator of asthma.

Endothelial cell differentiation into capillary structures by copolymer surfaces with phenylboronic acid groups.

A ternary copolymer composed of m-acrylamidophenylboronic acid, N,N-dimethylaminopropylmethacrylamide and N-isopropylacrylamide was synthesized. Long-term culture of bovine aortic endothelial cells on this copolymer substrate demonstrated adhesion and proliferation of the cells. After 26 days in culture, endothelial cells spontaneously developed into capillary networks. The interactions between phenylboronic acid groups in copolymer and glycoconjugates on endothelial cell plasma membranes are proposed to regulate the induction of tissue formation, since phenylboronic acid groups are known to specifically form reversible complexes with cis-diol compounds such as glucose. This copolymer is a novel material capable of mediating specific signals analogous to extracellular matrix to promote proliferation of endothelial cells, inducing capillary structures and prompt angiogenesis.

Conversion of cathepsin E to enzymatic unstable form in gastric cancer cells.

A new acid proteinase in human gastric cancer, named medium moving proteinase (Med.P), was found also in a gastric cancer transplanted into nude mouse. However, Med.P disappeared when the samples prepared from gastric cancer tissues were left for over 4 weeks at -80 degrees C, whereas the activity of cathepsin E (CE) increased. When these samples were reduced by dithiothreitol (DTT), Med.P appeared again and the CE activity decreased. These phenomena, revealed by electrophoretic analyses, indicated that Med.P is a monomeric form of CE (mono-CE). At weakly alkaline pH and after heating, mono-CE appeared to be more unstable than CE. These results indicated that CE assume an enzymatically unstable monomeric form in cancer cells.

[Relationship between chilliness of the limbs and daily-life conditions in young females].

In winter, many people have trouble with chilliness in their arms and legs. Many women are especially sensitive to cold, and for them the chilliness of the hands and the feet is difficult to ease even after entering a warm room, or taking a bath. They even feel pain owing to coldness of their limbs in daily life. This symptom has been called "hiesho" in Japanese. The problem of this chilliness is difficult to study because this symptom, in general, can be cured when spring comes. Coldness of the limbs has been considered to be one of the symptoms of the climacteric disturbance among middle-aged females. Recently, however, it was reported that many young females also feel pain owing to coldness of the hands and legs in their daily life during winter. This study investigates the problems of chilliness in the limbs of young females in their daily life. By means of a questionnaire, 642 female college students aged from 18 to 20 years answered questions concerning: (1) physical characteristics, (2) physical conditions in their daily life, (3) physical conditions in winter, and (4) physical conditions in summer. The replies to the questionnaires were tested using the chi 2 test. The major results were as follows: 1. Half of the subjects were sensitive to cold, and had difficulty sleeping owing to chilliness of their limbs in winter even in a warm environment. For the following results the subjects who were sensitive to cold had significant differences (p < 0.05) compared to non-sensitive subjects to cold.(ABSTRACT TRUNCATED AT 250 WORDS)
