Interrelation between plasma kinin and the sympathetic nervous system in normal subjects.

To investigate the relationship between plasma kinin and the sympathetic nervous system, we measured plasma kinin, norepinephrine, and angiotensin-I-converting enzyme (ACE:kininase II) activity in normal subjects. In six normal subjects, subpressor and pressor doses of norepinephrine (10 ng/kg/min and 50 ng/kg/min, respectively) were infused for 1 hour. In five normal subjects, 60-degree head-up tilting was performed for 20 minutes. After norepinephrine infusion, plasma kinin levels were decreased significantly (p less than 0.01) with the subpressor dose of norepinephrine and were further decreased (p less than 0.01) with the pressor dose. Plasma ACE activity, however, showed no significant changes following these infusions. During the tilting, plasma kinin levels decreased significantly (p less than 0.01), whereas plasma norepinephrine levels increased significantly (p less than 0.01). From these results, the assumption of elevated levels in plasma endogenous or exogenous norepinephrine leads to a decrease in plasma kinin levels; this suggests that these two systems may be closely interrelated.

The role of plasma angiotensin I converting enzyme in regulation of renin-angiotensin system activity in patients with essential hypertension.

In order to investigate the role of plasma angiotensin I (pAI) converting enzyme on the activity of the renin-angiotensin system, we measured plasma renin activity (PRA), plasma angiotensin II (pAII), and plasma angiotensin I converting enzyme (pACE) activity in fourteen patients with essential hypertension before and after two hours of ambulation combined with intravenous furosemide administration. Significant increases were observed in the values of PRA (p less than 0.005), pAII (p less than 0.005) and ACE activity (p less than 0.05) after ambulation. The ratios of log pAII/log PRA were increased significantly after ambulation (p less than 0.005), and a significantly positive correlation was observed between log pAII/log PRA and ACE activity (p less than 0.01). These results suggested that not only PRA but also ACE activity contributed to the activity of renin-angiotensin system through the generation of AII.

[Successful treatment of advanced gastric cancer by combined cisplatinum, mitomycin C and 5-FU administration. A case report].

A 55-year-old woman with gastric cancer underwent laparotomy and was found to have an unresectable tumor at the MC region, characterized by S3 (invasion into the pancreas), N3, P0, H0, Borrmann-II and moderately differentiated tubular adenocarcinoma. She was then treated by combined administration of cis-dichlorodiammine platinum (CDDP), mitomycin C and 5-fluorouracil (5-FU). A complete response was confirmed by upper gastrointestinal roentgenography and endoscopy. Upon reoperation 4 months after the initial laparotomy, a successful total radical gastrectomy with R2 (partially R3) lymph node dissection was performed. Histological examination of the specimens, including the stomach and lymph nodes, revealed no cancer cells in any region.

[A case of congenital unilateral multicystic kidney with renal matrix calculi].

A forty-two-year-old man was seen with right lumbar pain. Physical examination revealed a right flank mass. Conventional excretory urography showed lack of right renal function, whereas left kidney was visualized. Right nephrectomy was performed. A cluster of several different sized cysts was disclosed in the right renal region. The renal surface was smooth. The removed kidney weighed 1,150 g. The ureter was completely obstructed at the ureteropelvic junction. Cysts were filled with matrix calculi. Pathological examination showed dysplastic glomeruli and primitive tubules within loose embryonic mesenchyme between two cysts whose walls consisted of smooth muscle strands and connective tissue. The final diagnosis was a congenital unilateral multicystic kidney with renal matrix calculi. The multicystic kidney is the most common form of renal cystic disease in infancy. However, few cases in adults have been reported. The diagnostic approach, treatment and outcome of a congenital unilateral multicystic kidney are discussed.

A case of primary carcinoma of the epididymis.

We present a rare case of primary carcinoma of the left epididymis in a 32-year-old man. Pathological diagnosis was anaplastic carcinoma of epididymis. The patient is alive and free of metastases 2 years after orchidectomy. Clinical and pathological aspects of epididymal carcinomas are discussed with reference to previously reported cases.

[Measurement of physical strength and rheumatoid arthritis patients' kinesitherapy].

It is difficult to examine the physical strength of patients with rheumatoid arthritis (RA). The graded exercise testing by bicycle ergometer was used for RA patients. The following factors were measured: 1) lactic acid in blood, 2) heart rate, 3) blood pressure and 4) expired gas analysis. Generally speaking, the index of physical strength of healthy human beings is attributed to the maximum oxygen consumption, aerobic threshold or anaerobic threshold. Although none of the nine patients' maximum oxygen consumption was able to be measured, three patients' anaerobic threshold and seven patients' aerobic threshold were measured. In this series the results suggested that aerobic threshold was useful as an index for physical strength. Three of the nine patients were treated by kinesitherapy at the aerobic threshold work load using a bicycle ergometer. The effect of kinesitherapy is discussed.

Interrelation between the renin-angiotensin system and kallikrein-kinin system in patients with essential hypertension.

In order to investigate the relationship between the kallikrein-kinin (K-K) system and renin-angiotensin (R-A) system, plasma kinin (pKIN), plasma angiotensin II (pAII), plasma renin activity (PRA) and angiotensin converting enzyme activity (ACEA) were determined in 19 essential hypertensives (EHT). pKIN and pATII measurements were performed by highly sensitive radioimmunoassay (RIA), both of which were established in our laboratory. The assay sensitivity of pKIN and pAII were 0.5 pg/tube and 0.1 pg/tube, respectively. In pKIN RIA, pKIN was extracted by ethanol from 0.8 ml of plasma obtained with a syringe containing kinin generating and destroying enzyme inhibitors. In pAII RIA, pAII was measured directly in small amounts of 50-100 microliter, unextracted plasma samples. The level in pAII was significantly higher (p less than 0.05) in the normal renin group (NRH: n = 13) as compared with low renin group of EHT (LRH: n = 6). However, no significant difference was found in pKIN and ACEA between these two groups. Although a significantly positive correlation was observed between pAII and PRA (p less than 0.001) in EHT, the ratio of pAII/PRA tended to be higher in LHR than in NRH. ACEA correlated positively with pAII (p less than 0.01) or PRA (p less than 0.02), respectively. On the other hand, a significant negative correlation was also found between pKIN and pAII (p less than 0.05). From these findings, it was assumed that there was a closed relationship between R-A and K-K systems, and that angiotensin converting enzyme (kininase II) might play some role in the interrelation between both systems.

Comprehensive studies on the renal kallikrein-kinin system in essential hypertension.

In order to investigate the role of the renal kallikrein-kinin (K-K) system in normal (NRH) and low renin (LRH) subgroups of essential hypertension (EHT), daily urinary excretions of renal K-K system components including kallikrein (KAL), total KAL, pre-KAL, kinin (KIN) and kininase (total, I and II), were measured in 21 normotensives (NT) and 45 patients with EHT (NRH: 29, LRH: 16). Urinary KAL and KIN quantities, KAL activity, total and pre-KAL, and kininase (total, I and II) were measured by direct RIA, kininogenase assay, direct RIA of KAL after trypsin treatment, and KIN destroying capacity, respectively. The daily excretions of KAL quantity and activity, total and pre-KAL, and KIN were significantly lower in EHT than in NT. That of total kininase and kininase I were significantly higher in EHT than in NT while no significant difference was found in kininase I between EHT and NT. In comparing NRH and LRH, the urinary KAL activity and KIN were lower in LRH than in NRH, and kininase I was higher in LRH than in NRH. No significant difference, however, was found in total and pre-KAL, KAL quantity and kininase II between NRH and LRH. The ratio of KAL quantity/total KAL which reflects the conversion rate from pre-KAL in the kidney, did not show any significant difference among NT, NRH and LRH.(ABSTRACT TRUNCATED AT 250 WORDS)

An improved method for the measurement of rat tissue kallikrein using a monoclonal antibody which recognizes only active enzyme.

We have used a monoclonal antibody which recognizes active kallikrein to develop a method for measurement of inactive and active kallikrein in rat urine. The inactive kallikrein levels were calculated from the difference between values before and after trypsin pretreatment in the kallikrein direct radioimmunoassay. In this assay, the final dilution of ascitic fluid containing monoclonal antibody was 1:1.6 X 10(7) which gave 35% specific binding to 125I-labelled kallikrein, and the minimal detectable amount was 0.08 ng/tube. When inactive kallikrein was assessed by this method and a kininogenase assay in 23 randomly collected urines, a significant correlation was observed between the values obtained from the two assays (p less than 0.01). The regression line for this relation was similar to that observed between purified active kallikrein concentration and kininogenase activity. The active and inactive kallikrein excretion rates and the inactive/total kallikrein ratio in rats on normal sodium diet were 74.0 +/- 17.3 micrograms/day (m +/- SD), 82.8 +/- 14.4 micrograms/day and 53.1 +/- 7.1%, respectively. This method can now be applied to studies of prokallikrein activation.

Postnatal development and enlargement of primary middle cranial fossa arachnoid cyst recognized on repeat CT scans.

The etiology and mechanism of expansion of primary intracranial arachnoid cysts have been much debated. A rare case of an 8-month-old boy is reported, in which postnatal development and enlargement of a middle cranial fossa arachnoid cyst was detected on follow-up CT scans. Based on intraoperative and histological findings, the cyst was found to be intra-arachnoid. The wall was excised completely, and the lobe adjacent to the cyst appeared normal apart from signs of atrophy. Histological study of the excised cyst revealed a common arachnoid membrane with neither ependymal nor inflammatory cells; the cyst fluid was similar to CSF. The etiology of the lesion remains unclear, but it was considered that the expansion of the cyst might have occurred through a ball-valve mechanism of the membrane in communication with the general subarachnoid space.
