Inhibitory effects of hyaluronan on [14C]arachidonic acid release from labeled human synovial fibroblasts.

The effects of hyaluronan (HA) on the release of arachidonic acid (AA) from phospholipids induced by bradykinin in synovial fibroblasts of osteoarthritic patients were examined. HA inhibited [14C]AA release from prelabeled synovial cells stimulated with and without bradykinin 1 hr after incubation with HA and thereafter. The inhibitory effects of HA on [14C]AA release were dependent on the concentration and molecular weight of HA. However, inhibition of [14C]AA release by HA was not merely due to the viscosity of HA. The [14C]AA release induced by calcium-ionophore A23187 was also inhibited by HA with a high molecular weight. In addition, HA did not affect [14C]AA uptake by the cells. Our results suggest that HA with a high molecular weight elicits anti-inflammatory effects, at least in part, by inhibiting AA release in inflamed joints.

[Mercury in the environs of Kagoshima Bay].

Mercury contents of samples of sea water and fish from Kagoshima Bay, sediments in rivers, and the surface soil from the area surrounding a waste incinerator in the city of Kagoshima were measured to search for the source of mercury in Kagoshima Bay. The results obtained were as follows: 1) Mercury contents of sea water samples at 26 stations in Kagoshima Bay ranged from 6.3 to 19.7 ng/l. When the 26 stations were classified into four areas, the entrance, the middle and the interior of the Bay, and the water around the Sakurajima area, mercury contents of the samples from the last area were significantly higher than either at the entrance or in the interior of the Bay. 2) Mercury contents in the cardinal fish, Apogon notatus, were significantly higher than those in either the dragonet, Callionymus lunatus, or the sillaginoid, Sillago japonica. Mercury contents of fish from the Ushine coast station, the innermost part of the Bay, were significantly higher than those from the other collecting stations. Moreover, significantly interactions between the species of fish and the sampling stations were detected, and mercury contents of cardinal fish from Ushine coast station were 6.7-fold higher than those from the sampling station at the mouth of the Shinkawa river. 3) River sediments obtained 1 km from the mouth of each river contained from 4 to 96 micrograms/kg of mercury. Specific measurement of a major mite allergen, Der f II, by an enzyme-linked immunosorbent assay system using monoclonal anti-Der f II antibodies.

An enzyme linked immunosorbent assay system using a monoclonal antibody, 15E11, specific for a major allergen Der f II in house dust mite, was developed. This system detected only Der f II in the presence of Der p II and other allergens. The Der f II contents in several house dust samples significantly correlated with the numbers of the mites in the same house dust samples (n = 14, r = 0.88, p < 0.001). These data showed that this system was useful for specific measurement of Der f II in house dusts.

Spontaneous lesions in cynomolgus monkeys used in toxicity studies.

Spontaneous lesions in wild-caught, laboratory-maintained cynomolgus monkeys used in drug-toxicity studies were examined histopathologically in an effort to better distinguish toxic changes from spontaneous lesions and assess the toxicity of drugs more exactly. In the liver and kidney, where toxic changes are observed frequently, many spontaneous lesions were observed. Infiltration of mononuclear cells, vacuolization of the hepatocytes, dilatation of the renal tubules, and vacuolization of the renal epithelia were observed at a relatively high frequency. It is considered important to examine these changes carefully, because they closely resemble the changes recognized as toxic. Deposition of brownish pigment was observed in various organs such as the liver, kidney, spleen, intestinal tract, lung and brain, however the type of pigment differed among the organs, and histochemical examination revealed anthracosis or accumulation of hemosiderin, or melanin. Since the monkeys were caught in the wild, many parasitic lesions were observed especially in the large intestine and liver. Helminthous worms were frequently observed in the granulomas in the large intestine, however, no parasites were observed in the granulomas in the liver. Such lesions in the liver may be misinterpreted as toxic changes, when only scars or inflammatory lesions are observed.

Appropriate conditions for the plaque forming cell (PFC) assay in rats and effects of cyclophosphamide on PFC response.

The appropriate conditions for the plaque forming cell (PFC) assay using rat splenocytes were determined and effects of cyclophosphamide on PFC response were investigated using these conditions. The number of PFCs produced by immunization with a suspension of sheep red blood cells (SRBCs) was higher with i.v. injection than with i.p. injection. Subcutaneous injection of the suspension did not produce PFCs. The highest PFC response was observed when the number of PFCs was determined 4 days after i.v. immunization with 0.5 ml of a 1% SRBC suspension. Cyclophosphamide (3, 10, 30, 100 and 300 mg/kg, p.o.) dose-dependently decreased PFC response under the above-mentioned optimal conditions, and decreased PFC responses were noted even at the very low dose of 3 mg/kg: a dose at which a decrease in the number of PFCs has not been reported in studies using mice. From these results, the appropriate conditions for the PFC assay in rats are considered to be i.v. immunization with 0.5 ml of a 1% SRBC suspension and determination of the number of PFCs 4 days after immunization. Furthermore, it is considered that the PFC assay using rats might be more sensitive to immunosuppressive agents than that using mice.

Lectin cytochemistry combined with silver colloid staining of argyrophilic nucleolar organizer regions in human gliomas.

Investigation of lectin cytochemical staining of inflammatory cells in human gliomas showed that Allomyrina dichotoma (Allo-A) cytochemistry can reliably distinguish inflammatory from neoplastic cells. Allo-A cytochemistry combined with silver colloid staining of argyrophilic nucleolar organizer regions (Ag-NORs) was performed in human gliomas. Inflammatory cells possessed usually one but sometimes two Ag-NORs and macrophages often possessed several Ag-NORs. The mean Ag-NOR number per nucleus of inflammatory cells ranged from 1.81 to 2.34, and that of neoplastic cells ranged from 2.57 to 3.53 and from 2.84 to 4.46 in low- and high-grade gliomas, respectively. The mean Ag-NOR number per nucleus of inflammatory cells was significantly smaller than that of neoplastic cells (p less than 0.001). Combined Allo-A cytochemical and silver colloid Ag-NOR staining can provide a reliable Ag-NOR number in human gliomas by distinguishing inflammatory cells.

The association of serum hepatitis C virus RNA with serum aminotransferase activities.

Serum samples from 71 patients with hepatitis C virus infection at various stages were studied to determine the clinical significance of the detection of hepatitis C virus RNA by the polymerase chain reaction. There was a significant correlation of serum HCV RNA with elevation of the serum aminotransferase levels. These data suggest that the detection of HCV RNA in the serum might be useful for evaluation of the extent of ongoing liver damage.

Chronopharmacokinetics and chronotoxicity of lithium in mice eating normal and low-sodium diets.

The temporal aspects of the pharmacokinetics and toxicity of lithium were studied in mice eating normal and low-sodium diets. ICR male mice, housed under a light:dark (LD; 12:12) cycle, were injected with variable doses of lithium chloride i.p. A circadian rhythm was found in lithium clearance after a single administration in mice eating the normal diet showed the maximum value in the early dark phase and the minimum in the early light phase. The repeated administration of lithium did not affect the rhythm of the pharmacokinetics of the drug under the LD cycle. Although the low-sodium diet significantly decreased the lithium clearance, it did not influence the rhythm of the clearance. Higher toxicity was demonstrated in mice injected with the drug at the time of day with lower lithium clearance in the single-dose study but not in the repeated-doses study, regardless of the diet conditions. The low-sodium diet increased the acute and chronic toxicity of lithium. The results indicate that there is a circadian rhythm of acute toxicity and clearance of lithium after a single dose or repeated administration of the drug in mice eating normal and low-sodium diets and that the low-sodium diet increases lithium toxicity by reducing the clearance of the drug without influencing the rhythm characteristics.

[Influences of motor disability on the development of scene concepts].

Two studies were conducted to investigate the influence of motor disability of the child on cognitive development. In Study I, elementary school teachers and undergraduate students were requested to rank the school subjects according to how much the teaching of each subject would facilitate scene concepts development of children. The results visually represented by the rank graphs indicated that "physical training" and "arts" which require body actions and movements were judged to be most important for scene concepts development. In Study II, both normal and physically handicapped children were asked to perform free recalls to each of the three stimuli: "my house," "my school," and "my town." The results showed the influence of the size of space implied by the stimuli on the number of the items recalled by healthy children but this influence was not the same with physically handicapped children. These results, which indicate the importance of body actions and movements for scene concepts development, were discussed in terms of education for physically handicapped children.

[Intra-arterial infusion chemotherapy for recurrent cervical lymph nodes after surgery in esophageal cancer].

We attempted intra-arterial infusion chemotherapy using a reservoir system in recurrent cervical lymph nodes after surgery for esophageal cancer, and obtained favorable results. Intra-arterial infusion chemotherapy (75 mg of cisplatin and 5 mg of peplomycin) was performed using a reservoir system connected with a catheter inserted into the left subclavian artery, because recurrent lymph nodes developed in the left supraclavicular fossa. The therapy was effective for 6 months and the quality of life was improved without side effects.

[Morphometry of the normal cadaveric cervical spinal cord].

We measured the cross-sectional area and diameters of the C7 segment in 80 cadaveric specimens obtained from patients without spinal cord lesions died at 60 to 79 years of age. We found considerable individual variation in the spinal cord size. The transverse area of the C7 segment varied from 33.3 mm2 to 67.7 mm2 (mean 48.4 +/- 6.9 mm2). The size of the cord had no correlation with body weight, and had some correlation with body height. But the individual variation in the cord size was considerably large even among the cases of the same height. This large variation in the cord size should be under consideration in morphometric analysis of the spinal cord. We showed the morphometric data based on the measurements of the cross-sectional area and diameters in each segment from C2 to Th1 in 14 cases. Although the size of the spinal cord varied markedly from case to case, the relative ratio of cross-sectional area in each segment to that of C3 was similar among the cases (C2; 1.01 +/- 0.04, C4; 1.02 +/- 0.03, C5; 1.09 +/- 0.04, C6; 1.09 +/- 0.06, C7; 1.07 +/- 0.06, C8; 0.98 +/- 0.05. Th1; 0.80 +/- 0.04). Based on our data, the proper cross-sectional area of each segment and of each individual may be calculable from measurement of a given single segment.

[Cervical root avulsion presenting proximal segmental muscular atrophy of unilateral upper extremity].

A 56-year-old woman noticed non-progressive weakness in the proximal part of the right upper extremity from her childhood. At the age of 37 years, she was diagnosed as "spinal muscular atrophy" by an orthopedic surgeon. At the age of 56, neurological examinations revealed muscular atrophy and weakness confined to the right deltoid, biceps brachii and brachioradialis together with minor sensory disturbance in the lateral side of the right shoulder and forearm without pyramidal sign in the lower extremities. The neurological features of this case differed from those of juvenile type of distal and segmental muscular atrophy of upper extremities in distribution of muscular atrophy, and simulated those of cervical spondylotic amyotrophy. Myelography demonstrated root avulsion of the right C5 and C6 roots. CT myelography revealed traumatic meningocele. Therefore a clinical diagnosis of cervical root avulsion resulting from unrecognized birth injury was made. The reason of motor dominant pictures of this case may be vulnerability of the anterior nerve roots to traction injury.

[Serial magnetic resonance imaging of spinal syrinx formation in a case of multiple sclerosis].

A 27-year-old woman had a subacute onset of back pain, dysesthesia and weakness of both arms. Neurological examination revealed bilateral pyramidal signs, paresthesia of both hands and arms, and hypalgesia below T-4. CSF showed no abnormal findings. T2-weighted MR images revealed linear high signal within the cervical and upper thoracic cord, but no syrinx. The signs and symptoms resolved substantially within three months, with the exception of paroxysmal itching localized to the right forearm. At age 30, she experienced an acute onset of back pain, and dysesthesia of both feet. She developed weakness of both legs and urinary retention two days after the onset. Neurological examination showed bilateral pyramidal signs in the lower extremities, hypalgesia below T-4, hypopallesthesia on both legs, but no abnormalities in the upper extremities. CSF contained 8 white cells/mm3, protein 17 mg/dl and glucose 44 mg/dl. Oligoclonal bands were not detected. T1-weighted, proton density, and T2-weighted MR images revealed a syrinx formation within the spinal cord, extending from the level of T-2 to T-5. There was no evidence of spinal tumor. MRI of the brain revealed multiple areas of high signal intensity on T2-weighted image, consistent with multiple sclerosis. The signs and symptoms resolved substantially within two months. The syrinx within the thoracic cord reduced in size after two months and disappeared after three months. Two months after the second episode of myelopathy, she experienced right optic neuritis, resolving substantially within three weeks. This case was diagnosed as definite multiple sclerosis based on the clinical and radiological findings.(ABSTRACT TRUNCATED AT 250 WORDS)

[Spinal cord sarcoidosis--diagnostic value of Gd-enhanced MRI].

A 50-year-old man was admitted because of slowly progressive myelopathy for the last ten months without a correct diagnosis. The CSF showed mild pleocytosis (predominantly mononuclear cells) together with elevated protein content. MRI findings after administration of Gd-DTPA were characteristic: diffuse swelling of the lower cervical and upper thoracic cord, multiple patchy enhanced areas in the cord parenchyma adjacent to meninges, and linear meningeal enhancement. Based on these observations, a scalene node biopsy was performed, yielding a pathological diagnosis of sarcoidosis. The clinical neurological signs as well as the abnormal MRI findings of myelopathy were considerably improved after prednisolone therapy. An early diagnosis of spinal cord involvement in sarcoidosis should be established, because it is treatable in the earlier stage. Mild inflammatory evidence in CSF and Gd-enhanced MRI findings may be the cardinal clues to the diagnosis.

Is a trypsin-like protease of mites a Der f III allergen?

A trypsin-like protease was purified from mite (Dermatophagoides farinae) fecal extract. In SDS-PAGE, the mite trypsin-like protease showed a single band at 34 kD. The purified trypsin-like protease possessed potent allergenic activity. Both the twenty N-terminal amino acid sequence and the amino acid composition of the purified protease were very similar to those of Der f III. These data strongly suggest that the trypsin-like protease in the mite is a Der f III allergen.

[Anesthetic management of an infant with a single ventricle (asplenia syndrome) for non-cardiac surgery].

Ketamine and fentanyl were used for surgery of esophageal hiatus hernia in a 9 month old boy with single ventricle (asplenia syndrome). The patient was orally premedicated with diazepam 2.5 mg, and intravenously with atropine 0.04 mg. General anesthesia was induced with ketamine-fentanyl-pancuronium-100% oxygen, and maintained with fentanyl-pancuronium-100% oxygen. The total dose of ketamine or fentanyl was 0.8 mg.kg-1 or 15 micrograms.kg-1, respectively. Systolic blood pressure and heart rate of the patient were stable during ketamine-fentanyl anesthesia. Arterial oxygen saturation measured by pulse oximetry was over 90% and arterial oxygen tension was above 60 mmHg during the operation. Ketamine-fentanyl anesthesia might be useful for non-cardiac surgery of a child with cyanotic congenital heart disease.

["Dynamic" MR imaging of the cervical cord in patients with cervical spondylosis and ossification of the posterior longitudinal ligament--significance of dynamic cord compression].

This investigation was designed to assess the influence of dynamic cord compression on severity and course of myelopathy. Sixty-seven patients studied comprised 54 cases of cervical spondylosis and 13 cases of ossification of the posterior longitudinal ligament. These patients underwent "dynamic" MR imaging of the cervical spine. MR images in the sagittal view were obtained in three different neck positions: flexion, neutral, and extension. MR imaging was performed with a 0.15 T resistive unit. For technical reasons, the body coil was used. The pulse sequence was 500/30 (Tr msec/echo time msec) for T1 images. The spinal cord compression was accelerated in 32 cases when extended, in 2 cases when flexed, and in 4 cases when both extended and flexed. In 21 cases, we compared myelograms with MR images in a same neck position. Findings of myelograms well corresponded with those of MR images on 83 percent of intervertebral levels. The patients with dynamic cord compression were proved to have severer long tract signs, and their disability was regressive or progressive case by case for an average of 21-month follow-up. The "dynamic" MR imaging can provide dynamic nature of spinal cord compression, and prognostic clues.
