Peritoneoscopy in the management of ovarian cancer.

The results obtained from the routine use of peritoneoscopy in the initial evaluation and follow-up of 99 patients with ovarian cancer treated at the National Cancer Institute between 1970 and 1978 have been analyzed. Prior to treatment, peritoneoscopy documented new sites of involvement undetected by conventional radiologic and isotopic procedures in 42 patients (48%) and provided the only evidence for follow-up of disease in 25 patients (28%). Twenty-one percent of patients referred with Stages I and II disease were upstaged to Stage III on the basis of diaphragmatic disease detected at peritoneoscopy. In the 66 restaging (posttreatment) peritoneoscopies, residual disease was found in 33 (50%) and peritoneoscopic findings provided the only evidence for disease in 24 cases (36%). These patients were spared an unnecessary second-look laparotomy. Twenty-two patients with negative peritoneoscopies underwent laparotomy. Residual ovarian cancer was found in 12 (55%), mainly in the pelvis and mesentery. Thus, a negative peritoneoscopy must be followed by a laparotomy before a patient with ovarian cancer can be considered disease free. Peritoneoscopy was found to be a safe and feasible procedure, even in patients who had prior laparotomies. It could not be performed for technical reasons in only 6% of the patients. There were few serious complications and in only 2.5% of the cases was medical therapy required to deal with a complication. There were no deaths or perforations of the viscus and no patient required surgical exploration because of a peritoneoscopy complication.

Prognostic factors in Burkitt's lymphoma: importance of total tumor burden.

In 42 patients with African Burkitt's lymphoma, we have studied biochemical and clinical correlations with prognosis. Clinical stage and anti-EA titer were the best predictive factors, but lactic dehydrogenase (LDH) and uric acid (UA) concentrations also correlated with stage and prognosis, whereas lactic acid (LA), which was significantly elevated in Stage D, did not significantly correlated with prognosis or with LDH and UA levels. All of these factors with the possible exception of LA reflect the total body burden of tumor. We conclude that the tumor burden is the single most important prognostic factor in Burkitt's lymphoma, and that this is reflected directly by LDH and UA concentrations, and probably indirectly by anti-EA titer.

Biliary colic heralding systemic relapse in non-Hodgkin lymphoma.

Though lymphocytic lymphoma often presents as a disseminated disease, involvement of the gallbladder or biliary tree in the absence of jaundice is very uncommon [1]. A patient with poorly differentiated lymphoma in complete clinical remission after combination chematherapy experienced repeated episodes of biliary colic; at laparotomy her right-upper-quadrant pain was found to be due to lymphomatous involvement at the common bile duct. Recurrence in nodes surrounding the biliary tree heralded by several months the onset of a systemic relapse which did not respond to further cytotoxic therapy.

Dynamic imaging of the stomach by real-time ultrasound--a method for the study of gastric motility.

The use of real-time ultrasonic imaging of the stomach for the study of gastric contractions in response to a liquid test meal is described. Gastric contractions in the pyloric antrum and distal body of the stomach were observed on closed circuit television, recorded on to cassette tape and also imaged on polaroid and ciné film. Gastric contractions were recorded from the pyloric antrum by longitudinal scanning in the lower epigastrium and reproducible motility tracings were obtained on a fibreoptic chart recorded. Intravenous metoclopramide enhanced the magnitude and frequency of antral movement, which was abolished by intravenous propantheline. Real-time ultrasonic imaging permits the non-invasive study of gastric contractions. It is safe, may be repeated as required, and provides a method for the study of the effect of drugs and disease states on gastric motility.

Chlorozotocin, an anti-tumour agent lacking bone marrow toxicity at therapeutic doses: effects on lymphocyte subpopulations in mice.

Chlorozotocin is a new nitrosourea anti-tumour drug that does not produce bone marrow suppression at therapeutic doses in mice. CDF1 mice which were injected i.p. with a dose lethal to 10% of animals within 60 days (LD10), 20 mg/kg, developed a 50% reduction in circulating peripheral blood lymphocytes without a decrease in circulating granulocytes by day 3. Spleen weight also decreased markedly. The percentage of spleen B and T cells, determined by immunofluorescence with goat anti-mouse IgG and rabbit anti-mouse brain antisera, did not differ in control and chlorozotocin-treated mice. However, the ability of residual spleen cells to proliferate in response to phytohaemagglutinin, concanavalin A, pokeweed mitogen, and allogeneic cells was significantly suppressed although the lipopolysaccharide response was not reduced. The ability of the mice to respond to a primary immunization with sheep red blood cells was not significantly impaired. Therefore, chlorozotocin has a cytotoxic effect on both B and T cells but selectively inhibits the proliferative capacity of T cells. B cell proliferation and B cell function as measured in a primary antibody response were not reduced. These studies suggest chlorozotocin may be useful as an immunosuppressive drug as well as an anti-tumour agent.

Central nervous system complications of non-Hodgkin's lymphoma. The potential role for prophylactic therapy.

In 38 patients with non-Hodgkin's lymphoma, involvement of the central nervous system (CNS) by malignant lymphoma developed during an eight year period. All patients had lymphomatous meningitis; clinical involvement of the spinal nerves or cranial nerves suggested the diagnosis. Spinal fluid was abnormal in 97 per cent of the patients although a positive cytology could be documented in only 67 per cent by lumbar puncture. The histology in 82 per cent of the patients was diffuse. Involvement of the CNS in nodular lymphoma was uncommon (3 per cent), and the histology in virtually all of these patients had converted to diffuse. At the time of diagnosis of CNS disease, 95 per cent of the patients had other evidence of advanced disease; 66 per cent had bone marrow involvement. In only 18 per cent of the patients did CNS disease develop while they were in clinical remission. Eighty-five per cent of the patients treated with whole brain irradiation and intrathecal chemotherapy had a good clinical response. Knowledge of these risk factors permits definition of a group of patients who may benefit from CNS prophylaxis.

Mutagenicity, cytotoxicity and DNA crosslinking in V79 Chinese hamster cells treated with cis- and trans-Pt(II) diamminedichloride.

The mutagenicity and cytotoxicity of cis- and trans-Pt(II) diamminedichloride (PDD) were examined in V79 Chinese hamster lung cells and compared with effects on DNA measured by alkaline elution. DNA--protein crosslinks and DNA interstrand crosslinks were detected following doses of cis-PDD which reduced cell survival 80--90% and which produced a mutant frequency of 3 X 10(-4) at the HGPRT locus. Equitoxic doses of trans-PDD were much less mutagenic than cis-PDD. At equitoxic doses, trans-PDD produced more DNA-protein crosslinking than did cis-PDD, but interstrand crosslinking for the two isomers was comparable. Hence, the interstrand crosslink could be the cytotoxic lesion produced by these Pt compounds whereas neither of these DNA lesions are necessarily mutagenic. The mutagenesis produced by cis-PDD could be due to crosslinks of a different type than those produced by trans-PDD or it may be due to monofunctional damage.

Computed tomography in evolution of testicular cancer during intensive chemotherapy.

The evolution of malignant testicular tumor to mature teratoma has been studied in 4 patients. Computed tomography has been helpful in early diagnosis of this biologic phenomenon in these patients receiving intensive chemotherapy for disseminated non-seminomatous testicular cancer. Although the potential significance of this conversion in terms of survival is not known its early recognition by computed tomography has been useful in selecting and monitoring the therapy of these patients.

L-Chlorozotocin.

L-Chlorozotocin (2-[[[2-chloroethyl)nitrosoamino]carbonyl]amino]-2-deoxy-L-glucose) was synthesized in seven steps from L-arabinose for comparison with chlorozotocin, which is the D enantiomorph and an antineoplastic agent with clinical potential. Purification of the intermediate 2-amino-2-deoxy-L-glucose as the Schiff's base formed with 4-methoxybenzaldehyde ensured complete separation from the manno epimer. Comparative screening against leukemia L1210 with concurrent toxicity controls revealed no significant difference between D- and L-chlorozotocin in either activity or toxicity.

A rotating transducer real-time scanner for ultrasonic examination of the heart and abdomen.

A mechanical real-time ultrasonic scanner utilizing a rotating transducer head has been successfully applied clinically to visualize the heart and abdomen. The design features of this scanner are discussed. The small dimensions and the 90 degree field of view of the sealed oil-filled transducer head optimize visualization of the heart avoiding bone and lung. The single point of entry aspect of this scanner results in a good field of view from any point on the abdominal surface. For example, in upper abdominal studies the liver and kidney can be readily viewed. Rapid and thorough searching of the abdomen is easily carried out and good quality selected sections can be captured by optimizing frame rate and angle of view. In obstetrical applications the complete mature fetus can be visualized using the alternative large 180 degree field of view.

Hypomagnesemia and renal magnesium wasting in patients receiving cisplatin.

We studied renal function and serum electrolytes in 51 patients receiving cisplatin chemotherapy by retrospectively reviewing the charts of 44 patients and prospectively following seven patients. Hypomagnesemia developed in 23 of 44 evaluable patients who were receiving cisplatin. We documented inappropriate renal magnesium wasting in four patients. Two patients required hospitalization for symptomatic hypomagnesemia. We conclude that cisplatin can induce a renal tubular defect in magnesium conservation and serious clinical syndromes of magnesium deficiency.
