A computerized search was made of all publications on jejunoileal bypass for obesity, recorded in Index Medicus during 1963 to 1977. A total of 171 references, dealing with 6319 patients were usable for the purpose. Fifty-three variables were registered. The great majority of publications is American and published since 1972. Only 20% are joint publications from collaborating surgical and medical departments. The study draws attention to a number of quantitative and qualitative deficiencies in the data and to the less expedient way they are sometimes presented. Principally, this applies to patient number and description, length of observation time, and frequency of complications. Lack of specified criteria for surgical eligibility is common, as is ambiguity in conclusion. It is suggested that recommendations, positive or negative, reflect the authors' subjective attitude, as they are apparently made from the same objective data.
The main purpose of this study was to compare sensitivity of several self-rating scales to drug effects in anxious and depressed patients. Twenty-two distressed nonpsychotic outpatients completed a double-blind crossover trial of chlordiazepoxide and placebo. All self-rating scales of depression, anxiety, somatic symptoms, and feelings of inadequacy discriminated significantly between chlordiazepoxide and placebo. The decrease in self-rated depression was about equal to the decrease in self-rated anxiety. Subscales describing well-being were more sensitive to drug effects than subscales describing symptoms. A questionnaire (The Symptom Questionnaire) and one of the self-rating scales (Symptom Rating Test) were more sensitive than global self-rating scales, global observer-rating scales, and the Hamilton Anxiety Rating Scale. Some of the self-rating scales discriminated significantly between drug and placebo even when the sample size was reduced to two subsamples of eight patients each. High and significant positive correlations between two scales did not indicate equal sensitivity in discriminating between drug and placebo.

An epidemiological study in two culturally contrasting north-norwegian societies indicates that persons with a psychiatric diagnosis are more prone to non-psychiatric illnesses than those without such diagnosis. The onset of the non-psychiatric ailments of these persons come mostly before the onset of their psychiatric conditions. These are cross-cultural findings.

About 17 per cent of the whole period of the medical study at Tromsø is devoted to training in community medicine, which is combined with the other medical courses and spread out over the whole period. Courses include medical sociology, general and social medicine, psychiatry-psychology, epidemiology-statistics and preventive medicine. The students meet patients undergoing care at an early stage in the training, and work in groups with individual patients.

The mental health center movement represents a new mode of care for the mentally ill. People are treated on an out-patient basis rather than in institutions. The therapeutic work is mainly carried out by so-called paraprofessionals who seem to be well qualified for the job. However, when organizing such a treatment system there appear to be several important factors which need to be taken into account: (1) the therapist's need to work individually and independently, (2) the therapist's need for personal training, (3) the therapist's need to be heard when they channel information from the community to the service system, (4) the need for open and direct communication in the system to ensure that the therapists work within the frame of accepted therapeutic procedures, (5) the importance of the therapists' being represented at the highest decision-making level in the system in order to ensure that their needs are met. The author stresses this as an inevitable necessitiy in order to give the therapists the appropriate working-background.
