[A case of malignant lymphoma of the remnant stomach].

A 46-year-old woman was transferred to our facility with the complaint of epigastric discomfort of more than four months duration. She had undergone partial gastrectomy with Billroth I anastomosis 19 years prior to the admission. An upper GI series revealed multiple, variously sized elevated lesions scattered over the remnant stomach. Fiberscopic examination showed the gastric mucosa replaced by several elevated lesions and giant fold embellished by erosion, injection, and necrotic debris. Pathological examination revealed malignant lymphoma of the large cell type whose main growth was in the mucosa and submucosa. This case was significant in that malignant lymphoma occurred in the remnant stomach and that the tumor converted M-protein in the serum, which was confirmed as IgM-k in the cytoplasma immunohistochemically.

[Photodynamic therapy of superficial bladder tumors].

Photodynamic therapy (PDT), using hematoporphyrin derivative (HPD) and the red light (wavelength 630 nm) of an argon-dye laser as the source of excitation energy was performed on 46 patients with superficial bladder tumors. Two methods of laser irradiation, (1) focal PDT using a 400 micron quartz fiber through a cystourethroscope in 22 patients with superficial bladder tumors and (2) whole bladder wall total PDT using a motor-driven laser light scattering device in 24 patients with multifocal carcinoma in situ and/or dysplasia of bladder mucosa associated with multicentric concurrent superficial tumors, were used. The patients in (2) had been referred for total cystectomy, and 19 of these 24 patients had a history of several transurethral resections, hyperthermia and/or instillation therapy. HPD 2-4 mg/kg was i.v. injected 48 to 72 hours before PDT. Judging from the results of 60 protrusions treated by focal PDT, the light power should be 200 mW/cm2 for 5-10 minutes or more and the total light energy should be 100 J/cm2 or more in tumors up to 2 cm in size. With focal PDT, 4 of the 22 patients had no recurrence with the mean tumor free time of 20.8 months. In 6 of the 24 patients treated with total PDT using 10, 20 or 30 J/cm2 of light energy, there was no recurrence with a mean tumor-free time of 7.5 months and there was no significant relationship between the recurrence rate and total light energy used.

[An in vitro chemosensitivity study using human tumor clonogenic assay in urologic malignancies].

A soft agar colony formation assay, so called human tumor clonogenic assay (HTC assay) similar to that originally described by Salmon and colleagues, was utilized to measure the sensitivity of a total of 85 urologic malignancies including 36 urothelial cell carcinomas, 41 renal cell carcinomas, 5 testicular tumors, and 3 Wilms' tumors to anticancer drugs. In addition, the results obtained were compared with those of a novel dye exclusion method (NDE assay) described by Weisenthal and colleagues. The NDE assay was utilized to measure the sensitivity of a total of 63 urologic malignancies including 28 urothelial cell carcinomas, 25 renal cell carcinomas, 6 testicular tumors, and 4 Wilms' tumors to anticancer agents. In both assay series, the concentration of anticancer drugs tested was approximately one tenth of the maximum serum level achievable after single bolus injection. The colony forming rate inhibition of 70% or more in the HTC assay and the cell survival rate of 30% or less in the NDE assay were defined as "sensitive." Sixteen of the 36 urothelial cell carcinomas, 11 of the 41 renal cell carcinomas, and 1 of the 5 testicular tumors had both more than 30 colonies grown in control plates and enough cells in the specimens to provide at least one drug sensitivity testing. In urothelial cell carcinomas, 3 out of 13 tumors were "sensitive" to adriamycin, 3 out of 16 to cis-platinum, and 4 out of 15 to carboquone. In renal cell carcinomas, 2 out of 9 tumors were "sensitive" to adriamycin, 4 out of 11 to vinblastine, and none of 4 to Interferon alpha.(ABSTRACT TRUNCATED AT 250 WORDS)

Preparation and biologic characterization of fragments containing dimeric and monomeric C gamma 2 domain of rabbit IgG.

A number of fragments derived from acid-treated rabbit IgG by digestion with plasmin have been separated by high-resolution gel filtration. Fragments isolated included a dimer and monomer Facb, named F(acb)2 and Facb, respectively and a heterodimer composed of Facb and Fab subunits, named F(acb)(ab). A C gamma 2 fragment was obtained by papain digestion of Facb. A heterodimer composed of Facb and Fab', named F(acb)(ab'), was also prepared by oxidizing a reduced mixture of these fragments. Fragments thus obtained are classified into two groups--those carrying paired C gamma 2 domains, i.e. F(acb)2, and the disulfide-linked dimeric C gamma 2 fragment; and those having a single C gamma 2 domain, i.e. reduced, alkylated Facb and C gamma 2 fragment, F(acb)(ab) and F(acb)(ab'). These fragments exhibited marked differences in their capacity to activate complement in assay systems of hemolysis and complement consumption by immune complexes or aggregates on polystyrene latex. Fragments of the former group could activate complement but with a definitely reduced efficiency (50%) compared to intact IgG, whereas fragments of the latter group were practically inactive. Although it was not determined whether the C1-binding capacity itself is changed by monomerization of the C gamma 2 domain, the results suggested that the intact paired C gamma 2 module is required at least for the activation process of complement.

Effects of (D-Met2,Pro5)-enkephalinamide and naloxone on pial vessels in cats.

To elucidate the fundamental actions of endogenous opioids and naloxone on the cerebral circulation, the effects of (D-Met2,Pro5)-enkephalinamide and naloxone on pial vessels were investigated in cats. Pial arteries (165.7 +/- 24.9 microns) were found to dilate after the intravenous administration of 1 mg/kg of (D-Met2,Pro5)-enkephalinamide, and a definite dilatation of 7.1-7.6% persisted for 15 min. Pial veins (100.6 +/- 20.2 microns) also dilated but to a lesser degree. The MABP (118.7 +/- 10.5 mm Hg) decreased by 20 mm Hg immediately after the injection, but gradually returned to the initial value 15 min later. The observed cerebral vasodilatation may be attributable to sympathetic inhibition mediated either by the presynaptic opiate receptors of the cerebral vessels or by the opiate receptors in the brainstem. After the intravenous administration of 1 mg/kg of naloxone, pial arteries (122.0 +/- 17.2 microns) showed a slight but significant dilatation of 2.3-5.3%. There were no significant changes in pial veins (87.0 +/- 12.4 microns). MABP (130.4 +/- 12.3 mm Hg) was slightly increased after the injection. Although the mechanism involved was unclear, the cerebral vasodilatation occurring after the administration of naloxone may contribute to its ameliorating effect on the neurological symptoms following cerebral ischemia.

Inhibition of prostaglandin biosynthesis by a new anti-inflammatory drug, TA-60.

Effect of a new anti-inflammatory drug which has a little ulcer inducing property on gastrointestinal tract, TA-60 (2-[4-(3-methyl-2-butenyl) phenyl] propionic acid), on the prostaglandin (PG) metabolism was investigated. TA-60 inhibited the PGE2 biosynthesis of the bovine seminal vesicle microsome dose-dependently. The inhibition constant (Ki) of TA-60 was approximately 8 microM. The potency of TA-60 was approximately the same and two times that of ibuprofen (IP) and phenylbutazone (PBZ), respectively. TA-60 did not show the time dependent inhibition of the PGE2 biosynthesis unlikely to indomethacin (IM). The decrease in the PGE2 contents in the stomach of the rats by TA-60 reached a plateau and the content was not decreased to less than a certain level. The PGE2 content of the intestine was not changed by TA-60. TA-60 did not inhibit the activity of the PG degradating enzyme, 15-hydroxy PG dehydrogenase (15-OH-PG-DH) of the gastric mucosa like the other non-steroidal anti-inflammatory drugs (NSAIDs): IP, PBZ and IM. These results suggest that the slight ulcerating effect of TA-60 on the gastrointestinal tract might be resulted from the small decreasing effect of TA-60 on the gastrointestinal level of PGE2.

[PPD, PHA and Su-PS skin tests in genitourinary malignancies].

PPA, PHA and Su-PS skin tests were performed on 152 urogenital malignancy patients and the results obtained were analysed in terms of clinical course. The survival rate of 86 patients showing a positive response to the PPD skin test was significantly higher (P less than 0.005) than that of 65 patients showing a negative response to the PPD skin test. Among the 33 renal tumor patients, the positive rate of the 10 patients with stage I tumors to PPD skin test was significantly higher (P less than 0.05) than that of the 13 patients with stage IV tumors. In 68 bladder tumor patients, the response rate to PPD skin test in 47 patients with a low stage tumor was significantly high (P less than 0.05) compared with that in 21 patients with a high stage tumor. Out of 47 patients who died during the 84-month observation period, 10 (21.7%) and 3 (13.3%) showed a response change from negative to positive, in the PPD test and Su-PS test, respectively. It was assumed that 32 of the 46 patients (69.6%), 4 of the 6 patients (66.7%) and 17 of the 23 patients (73.9%) submitted to the PPD, PHA and Su-PS tests, respectively, showed a negative response at the time of their death.

Intracellular Ca2+ elevation induced by a neurotransmitter in a glial cell clone.

By fluorometry using a Ca2+-indicator Quin 2, we found an elevation of intracellular Ca2+ concentration ([Ca2+]i) in response to an application of serotonin in a rat clonal glial cell (C6BU-1). The [Ca2+]i rise depended on the dose of applied serotonin and the level of environmental Ca2+. The possibility was suggested that neuron-glia interactions might be controlled by a receptor-coupled [Ca2+]i-regulation system.

Serotonin-receptor coupled with membrane electrogenesis in a rat glioma clone.

The electrophysiological consequences of serotonin application to glial cells in culture are described. Membrane hyperpolarization due to K+ permeability-increment was induced by serotonin in rat clonal glioma (C6-BU-1) cells. Similar response was recognized in some of rat cerebral cortical glial cells. Pharmacological examinations in the C6-BU-1 glioma cells suggested a participation of serotonin-receptors sharing properties known in neuronal receptors.
