A case of congenital cystic adenomatoid malformation (CCAM) of the lung is reported. The patient had recurrent pneumonia after the second day of delivery, and was admitted to our hospital, at the age of 18. He received right pneumonectomy under the diagnosis of CCAM. The resected lung was composed of multiple cysts with thin wall measuring 20-30 mm in diameter. Microscopic examination showed that the cyst wall was lined by ciliated columnar epithelium, and infiltration of chronic inflammatory cells was found in the submucosal layer. Bronchial cartilage was absent through the specimen. These microscopic findings are identical with those of CCAM, which is very rare in this age.
A retrospective study was conducted on 32 patients who had had bronchial carcinoid tumors between 1965 and 1989. The average age of the patients was 48.5 years, with a male to female ratio of 3.6:1. Of the 32 patients, 28 were diagnosed pathologically to have typical bronchial carcinoid tumors and the other four, atypical bronchial carcinoid tumors. Twenty-two of the 28 typical bronchial carcinoid tumors were classified as stage I, but only one of the four atypical bronchial carcinoid tumors was at stage I. Two typical carcinoid tumor patients and two atypical carcinoid tumor patients were found, pathologically, to have lymph node metastasis. The typical carcinoid tumors showed a more significant endobronchial polypoid growth than the atypical carcinoid tumors (P = 0.0138). The five-year-survival rate was 100% in patients with typical carcinoid tumors and 25% in those with atypical carcinoid tumors. The difference between the five-year-survival rate for the typical carcinoid and atypical carcinoid patients was statistically significant (P = 0.001).
The L-P ring complex is thought to be a molecular bushing that supports flagellar motor rotation at about 10,000 revolutions per minute with presumably very little friction. Structural studies of this complex have been limited because only very small amount of samples are available. Therefore devising an efficient method of crystallization was essential. The addition of a phospholipid and its subsequent slow depletion by phospholipase A2 have been used to successfully grow well-ordered monolayer crystals that extend up to about 10 micrometers. The interaction of the L-P ring complex with lipid membranes was also visualized during this process.

The Bacillus sp. no. AH-101 alkaline protease showed higher hydrolysing activity against insoluble fibrous natural proteins such as elastin and keratin in comparison with subtilisins and Proteinase K. The optimum pH of the enzyme toward elastin and keratin was pH 10.5 and pH 11.0-12.0 respectively. The specific activity toward elastin and keratin was 10,600 units/mg protein and 3970 units/mg protein, respectively. The enzymatic activity was not inhibited by p-chloromercuribenzoic acid and iodoacetic acid. Carbobenzoxy-glycyl-glycyl-L-phenylalanyl chloromethyl ketone completely inhibited the caseinolytic activity, but 36% elastolytic activity remained. No inhibitory effect on caseinolytic and elastolytic activity was shown by tosyl-L-phenylalanyl-chloromethyl ketone, tosyl-L-lysine chloromethyl ketone, carbobenzoxy-L-phenylalanyl chloromethyl ketone, and elastatinal. The amino acid composition and amino terminal sequence of the enzyme were determined. The no. AH-101 alkaline protease was compared with subtilisin BPN', subtilisin Carlsberg, no. 221, and Ya-B alkaline proteases. Extensive sequence homology existed among these enzymes.
The nucleotide sequence of the G6-amylase gene from alkalophilic Bacillus sp. H-167 was determined. The open reading frame of the gene consisted of 2865 base pairs, encoding 955 amino acids. The NH2-terminal amino acid sequence analysis of the G6-amylase indicated that the enzyme had a single peptide of 33 amino acid residues and the mature enzyme was composed of 922 amino acids, giving a molecular mass of 102,598. Identity of the NH2-terminal amino acid sequences among each component of the multiform G6-amylase suggested the proteolytic processing of the COOH-terminal side of the enzyme. The DNA sequence and the deduced amino acid sequence of the G6-amylase gene showed no homology with those of other bacterial alpha-amylases although the consensus amino acid sequences of the active center were well conserved.
Unilateral renal agenesis with impaired renal function was found in all 3 siblings of a single family, in association with various metabolic disorders such as diabetes mellitus, hyperuricemia and hyperbilirubinemia with gallstones. The present report suggests that unilateral renal agenesis could occur as a manifestation of a genetic disorder.
Ureteral obstruction has been known to cause a renal acidification defect. To determine the underlying mechanisms of this defect, vacuolar-type H(+)-pump activity of the membrane vesicles taken from the rabbit renal outermedulla was measured in 24 hr obstructed and contralateral kidneys. H(+)-pump activity measured by acridine orange quenching method was resistant to oligomycin (10 micrograms/ml), vanadate (0.1 mM), and the absence of ambient K+, and it was highly sensitive to N-ethylmaleimide (0.5 mM) and dicyclohexylcarbodiimide (0.2 mM). H(+)-pump activity was 6.69 +/- 1.40 in the obstructed kidney, which was significantly lower than 10.32 +/- 1.88% AFU/mg.min in the contralateral kidney. In sham-operated rabbits, H(+)-pump activities were not different in both kidneys. Passive H+/OH- permeability measured by the rate of dissipation of the imposed pH gradient was not different in obstructed and contralateral kidneys. These results suggest that ureteral obstruction decreases vacuolar-type H(+)-pump activity of the kidney outermedulla, and this decrease may contribute to the distal acidification defect of obstructed uropathy.

A 52-year-old woman admitted with difficulty of breathing and had an anterior chest wall tumor. Primary lesion of the chest wall tumor invading the inferior one third of sternum, right third to sixth ribs, a part of the right middle lobe, the pericardium and a part of the diaphragma and small nodules on the parietal pleura were resected, and a diagnosis of chondrosarcoma of the right fifth or sixth rib and pleural dissemination of the tumor was established. The defect of pericardium was repaired with artificial dura mater and the defect of bony chest wall was repaired with a double layer of Marlex mesh. Although the resection was palliative because of pleural dissemination, she is now working as a housewife without difficulty of breathing.

Nine cases of malignant mesothelioma of pure epithelial and biphasic types (five pleural, three peritoneal, and one pericardial mesotheliomas), seven cases of benign adenomatoid tumor of the uterus, and 21 cases of peripheral pulmonary adenocarcinoma of non-mucus-producing type were examined immunohistochemically for expression of keratin, vimentin, carcinoembryonic antigen (CEA), surfactant apoprotein, Lewis blood group antigens, and Tn antigen. The majority (78%) of the malignant mesotheliomas expressed keratin, but CEA and surfactant apoprotein were not detected in any mesotheliomas. On the other hand, pulmonary adenocarcinomas expressed not only keratin (100%), but also CEA (62%) and surfactant apoprotein (62%). The expression of Lewisa blood group antigen and Tn antigen was detected in 76% and 62% of the pulmonary adenocarcinomas, respectively, but only one mesothelioma was stained for Lewisa antigen. This study reveals that the majority of malignant mesotheliomas can be distinguished from pulmonary adenocarcinomas by immunohistochemcial staining for CEA, surfactant apoprotein, Lewisa antigen, and Tn antigen. Immunohistochemically, adenomatoid tumors behaved similarly to malignant mesotheliomas.
The prognosis of coronary artery aneurysms complicating Kawasaki disease was assessed by serial coronary angiography in 33 patients. They underwent initial angiography with four months, and one or two follow-up angiographies between 11 months to 6.3 years after the onset of the illness. These patients had 69 coronary aneurysms at the initial study. The maximal diameter of the aneurysms was expressed as a percent of the normal value. Of 44 aneurysms with a diameter less than 300% of normal, 43 were found to have regressed on follow-up. Of 13 aneurysms with a diameter 300 to 400% of normal, four were proved to have regressed, eight persisted and one stenosed. Of 12 aneurysms with a diameter 400% of normal or more, one was found to have regressed, four persisted, two stenosed and five obstructed. Thus, the prognosis of aneurysms less than 300% of normal in diameter is excellent, whereas that of aneurysms 400% of normal or more in diameter is serious.
Obligate moderately thermophilic bacilli and obligate moderately thermoacidophilic bacilli contained spermine as the major polyamine in addition to putrescine and spermidine. The identity of spermine was confirmed by thin-layer chromatography and high-performance liquid chromatography before and after treatment with putrescine oxidase. Using these methods, thermospermine and spermine can be separated; thermospermine was not present in these organisms. On the other hand, various facultative thermophiles and mesophilic strains of the genus Bacillus, including alkalophiles and halophiles, lack spermine and other tetraamines. No spermine was detected in several strains of mesophilic or facultative slightly thermophilic lactic acid bacteria, Lactobacillus and Streptococcus.
