Selective removal of pathogenetic macromolecules directly from whole blood by double filtration technique.

Double filtration plasmapheresis (DFPP) is a technique devised in an attempt to selectively remove pathogenetic macromolecules such as antibodies, immune complexes or viscosity-increasing substances directly from blood. In order to realize the differential filtration principle, two filters, one for separation of plasma and another for fractionation of plasma components, are installed in an extracorporeal circulation line. Selective separation of the plasma components is dependent on the filtration property of the two filters. Clinical experiences evidence the usefulness of this technique in regard to conservation of human blood products for supplementation.

[Experience in local hyperthermia with using Therma Tech 2000. First report].

Studies of the effects of hyperthermia on neoplastic tumors have been developed on the basis of biology. At the some time, machines for the clinical application of hyperthermia have been slowly improved. The Therma Tech 2,000, however, unlike most other pieces of hyperthermia equipment, generates RF energy at 13.56MHz. In the present study, rabbits bearing VX2 tumors were given hyperthermia treatment and measurements of their temperatures were taken.

[Experience in local hyperthermia with using ThermaTech 2000--second report].

In the use of hyperthermia treatment, it is apparent from the work of other investigators that only a proportion of large clinical neoplastic tumors of substantial size and deep location can be heated to adequate temperatures (over 42 degrees c). It is therefore of major significance that we have tested the concept that OK-432 can be used as an immunoheat potentiator. A ThermaTech 2000 was used as the radio-frequency generator at 13.56 MHz, including an ancillary computer in the thermometry unit. Minor response in advanced and metastatic tumors was observed in 3 of 3 evaluable patients using OK-432. With other anticancer drugs, no change was observed in 11 of 16 patients, and progressive disease in 5 of 16. However, OK-432 has severe side effects including chills & fever, therefore improved immunoheat potentiators are desirable.

[Combination of intra-arterial administration of anticancer agent and hemocarboperfusion in treatment of advanced cancer].

Nineteen patients with malignant tumors were treated with a combination of bolus administration of mitomycin C with direct hemoperfusion over charcoal, and 15 patients were valuable. Mitomycin C of 1 mg/kg was administered via a catheter placed into the regional artery. Seven out of 14 patients with genitourinary cancers: 4 out of 9 patients with urinary bladder, 2 of 4 patients with renal and one prostate responded to the therapy. There were 4 responders out of 8 in the one-shot administration group, on the other hand, 7 cases receiving continuous infusion of 60 to 120 minutes responded. In both the one-shot group and the continuous group nausea and vomiting were observed in 21% and 11% respectively. Three patients receiving continuous administration of mitomycin C had local ulcerations. Myelosuppressions such as leukocytopenia and thrombocytopenia were observed in 37% of the both groups.

[A compact whole-body hyperthermia system utilizing extracorporeal circulation for anticancer treatment].

A compact whole-body hyperthermia system utilizing extracorporeal circulation for anticancer treatment has been devised and its clinical applicability is discussed. Our system has the following advantages; 1. Our small heat-exchanger made of polyvinyl-chloride hollow fibers exhibited sufficient heat-exchanging capability. 2. Our system is so compact as not to require priming with blood components. 3. Temporary blood access, created by direct puncture of the femoral vessels, eliminates vascular surgery and cardiac overload to the patients.

Malignant tumours in dialysis patients: a nationwide survey.

A nationwide survey was made to find out patients with malignant tumours among the dialysis population. A total of 354 malignant tumours were detected during the time of survey. Consistent with the incidence in the Japanese general population, the most frequently detected malignant tumour was gastric cancer, accounting for about one third of the total. Overall the incidence of tumour death was 2.31 times higher in males and 4.3 times higher in females. However, the high incidence of gynaecological and urinary tract tumours would suggest that some of them might be causative factors for the development of renal failure.

Influence of uremic middle molecules on blood cells.

Uremic middle molecular weight substances are obtained from the ultrafiltrate of plasma of uremic patients. Seven subfractions are isolated by chromatography using Sephadex G-15 and DEAE 25 A columns. Each subfraction is tested for its inhibitory effect upon lymphocyte blastogenesis and influence upon osmotic fragility of red blood cells. Consequently, subfractions c and d exert notable effects on both lymphocytes and red blood cells. The main constituents of subfraction d are aspartic acid, glycine, glutamine, and cysteine. It is concluded that uremic middle molecular substances have a deleterious effect upon blood cells. Therefore, hemofiltration, which is advantageous for the removal of middle molecular weight substances, is considered to be a method of choice for uremic patients.
