[The beneficial effects of short-term administration of a LHRH agonist (buserelin) in in vitro fertilization].

We have assessed the beneficial effects of administering a short course of an agonist of LHRH (Buserelin) in vitro fertilisation. 274 cycles were treated with CC-HMG, 159 were treated with HMG alone and 253 by Buserelin-HMG. These were compared retrospectively. The use of a short course of Buserelin lowered significantly the rate of failed treated cycles (7% instead of 19% when it was used with CC-HMG and 27% when used with HMG). This made it possible to administer hCG in all cases in the programme (100% as against 69% and 89% respectively for CC-HMG and HMG without Buserelin), and increase significantly the number of oocytes that could be recovered and the number of embryos available for transfer. The overall number of pregnancies per cycle treated is significantly raised when Buserelin is used with HMG (20.4% as against 13.5% for CC-HMG and HMG). The number of pregnancies that progressed, on the other hand, was similar when the cycle was stimulated with HMG alone (10.6%) or with Buserelin-HMG (10.6%).

Distribution of intact and Fab2(1) fragments of anti-human chorionic gonadotrophin antibodies in nude mice bearing human choriocarcinoma xenografts.

A direct comparative study of the distribution of intact and Fab2(1) fragments of mouse monoclonal anti-human chorionic gonadotrophin antibody, W14A, in nude mice bearing a human choriocarcinoma xenograft, indicates that the tumour:blood ratio is generally improved with fragments. Some parameters which may determine the distribution are outlined.

Borrelia-induced meningoradiculitis--two different forms of the disease.

The pattern of CSF cytological findings in the acute stage and after 10 days of antibiotic and steroid treatment was examined in 14 patients with meningoradiculitis due to an arthropode-transmitted infection with Borrelia duttoni (burgdorferi). At the acute onset of neurological symptoms, 12 patients revealed lymphocytic pleocytosis (cell count 50-500 X 10(6) cells/l) with numerous plasma cells (5-12% of the total cell count) and neutrophilic granulocytes (0.2-3.4%). 10-14 days after the onset of penicillin infusion and steroid treatment, cell count was reduced by at least 50% in 8 patients. Plasma cells then varied between 2 and 8%, whereas the neutrophilic granulocytes had disappeared. Two patients with slowly progressive neurological symptoms had a normal cell count, and inflammatory changes in CSF cytology were not observed.

Diagnosis and treatment of meningeal carcinomatosis in ten patients with breast cancer.

Clinical and radiological findings of 10 breast cancer patients with meningeal carcinomatosis were recorded. Special attention was drawn to CSF findings. Initial CSF cytology revealed malignant cells in all cases with a background of inflammatory cells in 7. Oligoclonal bands were found in the CSF of 3 out of 5 cases examined. Repeated CSF control under therapy showed elevation of lactate to be a better diagnostic parameter than lowered glucose. Six patients responded well to CNS radiation therapy and intrathecal chemotherapy. Median duration of survival was 6.4 months.

A microprocessor-controlled assay for the estimation of human placental lactogen.

A radioimmunoassay for human placental lactogen (HPL) is described using the KEMTEK 3000, which is a modular radioimmunoassay apparatus controlled by a microprocessor. Operation of the KEMTEK 3000 is largely automatic and it requires minimal intervention from the operator. It is capable of 300 reactions per hour so that a large number of estimations can readily be performed. HPL was assayed by a double antibody method on serum samples from pregnant women and patients with trophoblastic tumours.

Sensitive radioimmunoassays using partially purified gamma globulins coupled to enzacryl (acrylamide polymer) solid support.

Enzacryl polythiolactone is a cross-linked acrylamide polymer with an active thiolactone group, capable of coupling to lysine, serine and tyrosine residues. Gamma globulins from antisera specific to human chorionic gonadotropin, carcinoembryonic antigen, alpha-foetoprotein or casein were isolated using Protein A Sepharose and were subsequently coupled to enzacryl. The resulting coupled antibodies were found to provide greater sensitivity and convenience in radioimmunoassay studies than conventional double antibody precipitation methods using the same antisera.

Measurement of plasma renin activity as a valid estimation of plasma angiotensin status.

Measurement of plasma renin activity is widely used as an indirect assessment of plasma angiotensin II concentration. There has been some controversy over the validity of this assay as an estimate of circulating angiotensin II levels because, during the in vitro generation of angiotensin I by renin, over a period of time, substrate concentration may diminish to such an extent that it becomes rate-limiting, giving an artificially low reflection of angiotensin II levels. In this paper the initial angiotensin I concentration, that is the concentration before in vitro angiotensin I generation, has been compared with the corresponding plasma renin activity for 2752 individual plasma samples. A linear relationship was found between the initial angiotensin I concentration and the plasma renin activity below 60 ng ml-1 h-1. This indicates that, under the conditions of this assay, substrate does not appear to become rate-limiting except at exceedingly high levels of plasma renin activity. These results appear to provide further validation for the use of plasma renin activity measurement as a reflection of the concentration of circulating angiotensin II levels.

Advances in capillary electrochromatography.

This paper presents practical aspects of capillary electrochromatography (CEC). Preparation of capillary columns, including terminating frits and immobilization of the packed bed, is described in detail. Longevity and reproducibility of CEC columns is demonstrated. Instrumental aspects, sample introduction, sensitivity of detection, and gradient elution are discussed in detail. A number of examples that illustrate the versatility of CEC, in particular, for providing an isocratic separation alternative to conventional gradient separation in HPLC, are given.
