[The psychopathology of tumors of the 3rd cerebral ventricle].

36 patients with tumors of diencephalic region of different histologic nature involving cavity of the third ventricle of the brain were observed. 27.7% of the patients had disorders of mental and physical development from the very first years of life. In presurgical period there was a wide spectrum of paroxysmal and permanent mental disorders from asthenia to rude disturbances of memory and consciousness which regressed or disappeared after the operation. The correlation was observed between such disorders, histologic structure and location of the tumor; individual characteristics of the patient (age, dextrality, sinistrality); scope and character of the operation (approach to the tumor, surgical interventions on the structures of the third ventricle).

[Clinical picture of colloid cysts of the III ventricle].

Clinical analysis of 74 cases of colloid cysts of the III-d ventricle was performed. Dynamics of clinical symptom complex was studied both in preoperative and early postoperative period as well as in remote periods after surgery. Most of the patients were treated in N.N. Burdenko Institute of Neurosurgery during 1980-1995. Peculiarities of the disease development before clinical manifestation, variations of complicated postoperative period, prognostically significant symptoms, possible mechanisms of clinical syndromes development were considered.

[Clinico-psychopathologic and electroencephalographic correlations in patients with cranio-cerebral trauma during laser therapy].

A total of 96 patients with severe craniocerebral trauma were treated and divided into 3 groups: 1, with prevailing left brain hemisphere lesions manifested by right-sided pyramidal symptoms (N = 32); 2, prevailing right brain hemispheric lesions and left hemisyndrome (N = 35); 3, lacking in distinct focal interhemispheric differences in trauma localization with prevalent general cerebral signs. Control group consisted of 45 patients conventionally drug-treated without laser. Laser therapy was performed as a course of 12 to 15 sessions according to regimens settled for each group of patients. Positive result was an accelerated regression of both neurological and psychopathological manifestations of severe trauma. The treatment proved most efficient (with considerable improvement) in 85% of the Group 2 patients. Considerable improvement was noted in 23% of Group 1 patients; improvement was evident in 58% of the Group 3 patients. As a whole, laser treatment proved 25% more effective as related to the control group.

[Functional asymmetry of the brain in patients with epilepsy].

Individual functional brain asymmetry profiles were different in epileptic patients and healthy individuals. The patients displayed reduced from 39.6% to 17.8% incidence of left profile and increased incidence of combined profile (from 60.4% to 81.5%). The relationship of functional asymmetry profiles to sex, lateralization of epileptic focus and clinical course of the disease was described, the regularities of the profiles' changes in the course of the disease analyzed.

[Korsakoff's syndrome in the clinical picture of craniocerebral injury].

The localization and the nature of brain lesions are among the most debatable issues in Korsakoff's syndrome (KS). In 28 patients with head trauma the neurological investigation and CT scans helped to distinguish between the following stages of KS progress and reversal: (1) whole brain response with consciousness deterioration and psychomotor excitement; (2) predominantly focal involvement of the right hemisphere; (3) regression of KS clinical manifestations.

[Recovery of consciousness after prolonged coma in patients with severe craniocerebral injuries].

The authors specify and describe 7 stages of regaining consciousness by patients who had been in coma from 11 to 180 days, including opening of the eyes; fixation of the eyes on an object; recognition of close relatives; understanding of directly addressed speech; patient's own speech; amnestic confusion; consciousness with all formal signs of being clear. The condition of the patients at these stages is compared with EEG findings suggestive of the functional disintegration of the brain as a system in comatose patients, the vegetative status and the restoration of connections between different departments of the brain corresponding to the recovery of long absent consciousness of patients. It is concluded that at the current stage of medical sciences the maximum duration of coma compatible with the subsequent full recovery of mental activity of the patient under 30 years does not exceed 11 days.

[Clinical features of focal brain lesions in the left-handed and ambidextrous].

On the basis of a study of 31 patients who demonstrated deviations from dextrality (Simistrals and ambidextrals) the authors describe some traits of the nervous and mental changes in focal brain lesions. There are insignificant correlations between the character of nervous and mental changes and the side of a brain lesion. The study demonstrated a wide variability of the clinical symptomatology, a peculiarity of each neurological and psychopathological phenomenon, distinguishing them from similar changes in dextrals. The studied contingent revealed prevalence of a disturbed sensory cognition in the clinical picture; the presence of special phenomena which most likely are not seen in dextrals and which are also related to a pathology of sensory cognition. It is being assumed that these clinical traits may testify to an insufficiency of speech lateralization in sinistrals and that an insufficient speech lateralization is accompanied by other than in dextrals organization of sensory processes.

[Consciousness disorders in focal lesions of the right and left cerebral hemispheres].

In the light of modern concepts of functional asymmetry in the human brain, the authors analyse paroxysmal conditions of changed consciousness which occurred in 160 patients with lesions of the hemispheres due to brain tumors (75 patients) and in epilepsy (85 patients). It is shown that certain forms of changed consciousness are characteristic of disorders of the right and left hemispheres in right-handedness. This clinical fact is taken as a basis for the assumption that in the formation of consciousness both hemispheres in different ways play a certain role and they provide different "formating" kinds of consciousness: the right hemisphere--a direct perception or a sensory perception of the environment and itself, the left--process of abstract perception based on speech, symbols, semantics.
