An indirect fluorescent antibody technique for Haemophilus ducreyi.

An indirect fluorescent antibody technique which allows the detection of antibody against or the recognition of Haemophilus ducreyi has been developed. Heat inactivated antiserum was reacted uith prepared smears of Haimophilus species and selected organisms commonly associated with penile ulcerations. In serum absorbed with Haemophilus sp no fluorescence was observed with 156 heterologous organisms, but all six strains of bacteriologically confirmed H. ducreyi gave significant reactions with the absorbed anti-WD-68 serum.

Horny syphilid.

A thirty-seven year old man presented with volar lesions resembling widespread clavi. Careful physical examination and serologic tests established a diagnosis of secondary syphilis. The unusual lesions best fit the rare clinical entity of horny syphilid. The case emphasizes the importance of maintaining a high index of suspicion of syphilis and the value of the serologic tests in evaluating cutaneous eruptions.

Neisseria meningitidis in the anal canal of homosexual men.

Cultures were obtained from the oropharynx, urethra, and anal canal of 157 homosexual men. Neisseria gonorrhoeae was identified in 30 men (19.1%), Neisseria meningitidis in 32 men (20.4%), and Neisseria lactamicus and Branhamella catarrhalis in one man each. N. gonorrhoeae was isolated from the anal canal of three men (1.9%), and N. meninititidis was identified in the anal canal of seven men (4.5%). These findings suggest that confirmatory tests are indicated for homosexual males with rectal isolates presumptively identified as N. gonorrhoeae.

Nodular primary localized cutaneous amyloidosis.

An 83-year-old woman had nodular primary localized cutaneous amyloidosis. Several anetodermic, bullous-appearing lesions were observed. Biopsy showed massive amyloid deposition with destruction of elastic and collagen fibers, but no bullous formation. It was concluded that the amyloid deposition induced anetodermic changes creating the clinical appearance of bullae.

The effect of delaying incubation in a C02-enriched environment on gonococci.

Neisseria gonorrhoeae were isolated from 1,019 (39.8%) of 2,558 male patients examined in a venereal disease clinic. Three specimens per patient were plated on Thayer-Martin medium. One plate was placed in a C02 -enriched environment and incubated immediately. The other two were held at room temperature in a standard atmosphere for 1-4 hours before similar processing. A statistically significant loss of N. gonorrhoeae was found when placement in a candle jar and incubation was delayed 2 or more hours. Morever, the appearance of colonies was delayed and colonial size and number were significantly reduced in specimens held 2 hours or longer before incubation in a candle jar.

Combining cervical and rectal cultures for gonorrhea on a single modified Thayer-Martin plate.

Cultures of the cervix and rectum are recommended for the routine diagnosis of gonorrhea in women. Specimens are usually inoculated on separate plates of selective medium. The present study compares to the recovery rates of N. gonorrhoeae for cervical and rectal specimens plated separately to specimens combined on a single plate of modified Thayer-Martin medium (MTM). No statistically significant difference was detected between the two methods. It is concluded that the practice of combining cervical and rectal specimens on a single MTM plate provides the sensitivity of separately plated specimens at half the cost. Combining specimens could provide a significant cost benefit to gonorrhea control programs.

Culture of urinary sediment for the diagnosis of gonorrhoea in women.

1,001 consecutive women seen in a V.D. clinic were screened for gonorrhoea by cultures of urinary sediment, the endocervix, and anal canal. Recovery of Neisseria gonorrhoeae on culture of urinary sediment was 81-7 per cent. as productive as culture of the endocervix, and 79-2 per cent. compared to endocervical and anal canal culturing. Urogenital symptoms were present in 55-1 per cent. of women with negative cultures and 64-6 per cent. of those with gonorrhoea. A complaint of dysuria increased the likelihood of a positive urinary sediment or endocervical culture. Culture of urinary sediment is less sensitive than culture of the endocervix. However, the ease with which specimens are obtained and the relative simplicity of the technique could make this method an acceptable tool for screening large numbers of women for gonorrhoea.

How reliable is the morphological diagnosis of penile ulcerations?

Material from 100 consecutive men who presented with penile ulcerations was tested for a variety of microorganisms. Fifty-one patients had microorganisms identified that were considered primary pathogens. The laboratory data were compared with the clinical diagnoses and the diagnostic accuracy and index of suspicion calculated. The diagnostic accuracy for lesions due to Treponema pallidum was 77.8%, Herpesvirus hominis 62.9% and Haemophilis ducreyi 33.3%, whereas the indexes of suspicion were 88.2%, 67.7% and 300% respecitively. These data show that the clinical diagnosis of genital ulcers is oftern inaccurate and that definitive diagnosis requires laboratory tests.

The variability of syphilitic chancres.

Sixty-four patients who had darkfield microscopic evidence of primary syphilis underwent a standardized interview and physical examination. Two or more lesions were observed in 47% of patients, 7.8% of patients had non-indurated ulcers, and the borders of the ulcers of 7.8% were irregular and slightly undermined, suggesting lesions of chancroid. Only 27 patients (42.7%) had single, indurated chancres with regional lymphadenopathy. It is suggested that the "classic" chancre may be the "atypical" lesion.

Evaluation of hysterectomized women seen at a clinic for treatment of sexually transmitted diseases.

Thirty-one women who had had total hysterectomies, seen in a venereal disease clinic, were interviewed and examined. Thirteen (31%) of the 31 women had acquired one or more venereal infections after hysterectomy. The diseases, listed in order of decreasing frequency, were trichomoniasis (four cases), gonorrhea (three), early latent syphilis (three), vaginal candidiasis (three), secondary syphilis (two), late latent syphilis (two), and condyloma acuminatum (one). These results indicate that clinicians working in venereal disease clinics must maintain as high an index of suspicion of sexually transmissible diseases for women with hysterectomies as for all other women.

Pediculosis pubis in a clinic for treatment of sexually transmitted diseases.

The symptoms, clinical findings, epidemiology, and associated sexually transmitted diseases are described for 121 patients with pediculosis pubis who were seen in a clinic. Phthirus pubis was observed in the pubic hair of 118 patients, and in 38 men (37.3%) and 18 women (94.7%) the pubic region was the sole area of involvement. Pruritus was reported by 104 patients (85.9%), while excoriations were observed in 30 (24.8%) and maculae ceruleae in one. The median waits from detection of organisms to presentation at the clinic were two days for men and one day for women. This motivation for medical care was fortuitous, because 31.4% of the patients infested with P. pubis had other sexually transmitted diseases.

Genital bowenoid papulosis--squamous cell carcinoma in situ.

The case of a patient with bowenoid papulosis of the penis is described. Although his lesions resembled those of condylomata acuminata, a biopsy showed squamous cell carcinoma in situ. This case and those described in previously published reports confirm that squamous cell carcinoma in situ may exist in a papillomatous form on the genitalia of young adults. Venereologists must be familiar with this entity and must not dismiss all verrucous and papillomatous genital growths as condylomata acuminata.
