Enterobius vermicularis causing symptoms of appendicitis in Nepal.

This study set out to determine the prevalence of Enterobius vermicularis in surgically removed appendices and to assess the possible relation of the parasite to acute appendicitis. All 624 surgically removed appendices received in the Department of Pathology, BPKIHS, Dharan, Nepal during 2(1/2) years (August 1999-January 2002) were examined. E. vermicularis was identified in nine (1.62%) appendices from the patients with a clinical diagnosis of appendicitis. The parasite was most frequently seen in histologically normal appendices (6/71) and was rarely associated with histological change of acute appendicitis (3/539). No cases of E. vermicularis infestation occurred in appendices showing chronic inflammation or removed during the course of other surgical procedures. E. vermicularis was found more frequently in uninflamed and histologically normal appendices (8.45%) than those which were inflamed with histopathologic changes of acute appendicitis (0.56%). It may be a cause of symptoms resembling acute appendicitis although the mechanism for this does not involve mucosal invasion by the parasite.

Pyomyoma in a postmenopausal woman: a case report.

Pyomyoma (suppurative leiomyoma of the uterus) is a rare condition associated with a high fatality rate. Only surgical intervention is lifesaving. It usually develops in association with either recent pregnancy or in postmenopausal patients who have underlying vascular disease. Since 1945 only 15 cases of pyomyoma have been reported in the literature. We present a rare case of pyomyoma in a 64-year-old woman with a brief review of the literature.

Nucleolar organizer regions as a prognostic indicator in epithelial cancers of the ovary.

Recent studies have shown that quantification of silver-stained nucleolar organizer region associated proteins (AgNORs) is correlated with ploidy and proliferative activity in several neoplasms. The prognostic value of this marker, however, has not been well defined in epithelial ovarian carcinomas (EOCs). We studied 84 cases of EOCs in an attempt to assess the potential prognostic significance of AgNORs and to compare the results with other prognostic factors in EOCs. Clinical, operative and treatment details, and follow-up data were recorded for each case. In univariate analysis, a highly significant correlation of AgNOR count with clinical stage (p < 0.01), histologic grade (p < 0.05), response to chemotherapy (p < 0.05), and biologic behavior (p < 0.01) was found. High AgNOR counts were observed in late stage (III and IV) tumors, grade 3 carcinomas, tumors in patients with partial response to chemotherapy, progressive disease, recurrence, or death from tumor. In multivariate analysis, AgNOR count at cut off 3.5 was found to be the third prognostic variable after response to therapy and stage. These observations suggest that AgNOR count holds promise for prediction of tumor aggressiveness in EOCs. However, its role as an independent prognostic variable of patient survival remains questionable.

Germ cell tumors of the ovary: a clinicopathologic study of 121 cases from Nepal.

BACKGROUND: Although many reports have been published about germ cell tumors of the ovary in developed countries, there has been no such documentation from Nepal. The retrospective study presented here reports the clinicopathologic profile of germ cell tumors of the ovary studied at B. P. Koirala Institute of Health Sciences, Dharan, Nepal. METHODS: A retrospective analysis of 121 histopathologically proven cases of germ cell tumor of the ovary operated on at either our institute or somewhere else (but processed in our institute) from November 1995 to April 2001 (5.5 years) was done. Clinical data, histopathologic findings and complications were recorded. RESULTS: The prevalence of germ cell tumors was 43.36% (121/279) of all ovarian neoplasms. Patient age varied from 8 to 65 years (median 31 years). Tumor occurrence was most frequent in patients aged 21-40 years. Only eight of 121 cases (6.61%) were malignant; the rest (93.39%) were mature teratomas. Of great interest was the unexpectedly high number of cases (47.93%) found in patients who were hill natives such as Rai and Gurung. Pain and abdominal fullness were common symptoms noted in 85.95% and 79.31% of patients, respectively. Seventeen (14%) asymptomatic cases were found either on routine physical examination (12 cases) or during pregnancy (five cases). The left ovary was involved in 39.7% cases and the right in 35.5%. Bilateral involvement was seen in 24.8% of cases. Torsion was noted in 20.66% and was the most common complication. Of all the germ cell tumors 93.39% were cystic and only 6.61% were solid on gross appearance. There were three cases of monodermal benign teratoma, four cases of immature teratoma and one case of malignant transformation. CONCLUSION: Mature teratoma is the most common germ cell tumor and accounts for 40.50% of all ovarian neoplasms. The high prevalence of germ cell tumors of the ovary found among patients who were hill natives needs to be explored further.

Diffuse hyperplastic oncocytosis of parotid gland: a case report.

Oncocytic tumors of salivary glands are rare. Diffuse oncocytosis is the rarest lesion among 3 principal groups of oncocytic lesions as classified by WHO. Only seven cases have been previously reported in the literature. We report a case of diffuse hyperplastic oncocytosis of the parotid gland in a 63-year-old male with a brief review of literature. To the best of our knowledge this is the first case from Indian subcontinent.

Paget's disease of the nipple diagnosed on cytology: a case report.

Paget's disease of the breast nipple is an eczematous skin change of the nipple-areola complex often associated with an underlying in-situ or invasive carcinoma. The usefulness of cytology in the diagnosis of Paget's disease has been suggested only in few studies despite the fact that it offers definitive advantages over surgical excision biopsy. Easy practicability, quick, cost-effective and non-invasive stands opposite a limited sensitivity as disadvantage. We report a case of Paget's disease of nipple diagnosed by nipple touch smears.

Pigmented trichoblastoma: report of a rare case.

Trichoblastoma is a rare benign skin appendage tumour with follicular differentiation. Pigmented variant of trichoblastoma is an extremely uncommon tumour and only three cases have been described in the literature. We report a rare case of pigmented trichoblastoma in a 54-year-old female.

Primary malignant tumours of eye and adnexa in Eastern Nepal.

BACKGROUND: The purpose of the present paper was to report the spectrum of primary malignant tumours of eye and adnexa at BP Koirala Institute of Health Sciences, Nepal, from 1995 to 2000. METHODS: A retrospective study of medical records with histopathological confirmation of malignant tumours of the eye and adnexa was done for the years 1995-2000. A total of 116 consecutive medical records from the Department of Pathology at BP Koirala Institute of Health Sciences were retrieved. All those patients with primary ophthalmic malignancies were included and non-malignant cases were excluded. RESULTS: There were 80 patients of which 39 (48.8%) were male and 41 (51.2%) were female. Four patients had bilateral involvement. The most common malignancy was retinoblastoma (45.2%), followed by basal cell carcinoma (22.6%). CONCLUSION: Retinoblastoma is the most common eye cancer. The incidence of melanomas is correspondingly lower than that reported in the West. The present pilot study, the first of its kind, will lay the foundation for the monitoring of the future pattern of ophthalmic malignancies in Nepal and provide a basis for comparison elsewhere.

Choroidal melanoma with secondary glaucoma.

The case of a 40-year-old female patient with choroidal melanoma with secondary glaucoma presenting as a painful blind right eye is reported. Liver metastasis was detected by ultrasonography. The choroidal tumor measured 2 x 2.1 x 1.5 cm; histopathology showed that it was of the spindle cell (spindle A) variety. Such tumors are rare in non-white races and secondary glaucoma is an uncommon presentation.

Primary squamous cell carcinoma of the endometrium: a case report.

Squamous cell carcinoma of the endometrium is extremely rare female genital malignancy. Only 64 cases have been reported in the literature. A case of primary squamous cell carcinoma of endometrium in a 55-year old-woman is reported. Examination revealed pyometra and cervical stenosis. A total abdominal hysterectomy with bilateral salpingo-oophorectomy was performed. Histopathology confirmed the diagnosis.

Ulcerative post-kala azar mucosal leishmaniasis masquerading as a carcinoma--a case report.

Post kala-azar mucosal leishmaniasis (PKML) is relatively rare in the Indian subcontinent. We describe an ulcerative variant of PKML with nasal involvement in a Nepalese adult male. He had ulcerated plaque over the upper lip with extensive involvement of nasal columella and septum. He came from endemic area for kala-azar and had a previous history of kala-azar. There were plenty of LD bodies in microsections and FNA of submandibular lymph node.

Visceral leishmaniasis in two cases of leukemia.

Two cases of visceral leishmaniasis (VL), one in a 51-year-old man with accelerated-phase chronic myeloid leukemia and another in a 35-year-old woman with acute myeloblastic leukemia, are reported. Incidental finding of Leishman-Donovan (LD) bodies in patients with leukemia highlights VL as a potent opportunistic infection in immunosuppressed patients.

Oculosporidiosis in eastern Nepal: a report of five cases.

Rhinosporidiosis is endemic in India, Sri Lanka, and parts of East Africa and South America; sporadic cases are reported in other countries. We report on five patients from eastern Nepal with oculosporidiosis. The conjunctiva was involved in two patients and the lacrimal sac was involved in three patients; treatment was by simple total excision of the conjunctivalgrowth (patients with conjunctival involvement) and by dacryocystectomy (patients with lacrimal sac involvement). This is the second case report from Nepal.

Mucinous carcinoma of rectosigmoid in a 15-year-old child: a case report.

A 15-year-old girl with Duke's B mucinous carcinoma of the rectosigmoid was treated with surgical resection and adjuvant chemotherapy. The patient is alive and has been disease-free for 15 months. Colorectal carcinoma is extremely rare in children and adolescents. Adenocarcinoma of colon is a virulent disease in children and has a poor prognosis. This is because of the poor histological characteristics and difficulty in diagnosis.

Inflammatory fibroid polyp of the jejunum presenting as intussusception.

Inflammatory fibroid polyp (IFP) of jejunnum is a rare nonneoplastic lesion of gastrointestinal tract. We report a case of a 45-year-old man presenting with small bowel obstruction due to jejuno-jejunal intussusception of an inflammatory fibroid polyp. To the best of our knowledges, this is the eighth reported case with such a presentation in English medical literature. Segmental resection of the jejunum was performed and histopathology confirmed the diagnosis.
