Medical treatments and cares for geriatric syndrome: new strategies learned from frail elderly.

In Japan, there are 21 million older people above 65 years, and about 8% of them are frail elderly. Geriatrics is to study the frail elderly as to why they become frail elderly, and to treat patients properly or the remaining 92% older people not to become frail elderly. In order to promote health of the older people, geriatricians have to take deep insights for cares as well as medical treatments. With such a will, we find the way to prevent diseases in the older people. In this review, we describe medical treatments and cares for promoting successful aging.

Correction of the maxillary occlusal plane relieves persistent headache and shoulder stiffness.

It has been known for many years that deformations of the occlusal plane of the teeth cause indefinite symptoms such as headaches or stiffness of the shoulders. However, how the occlusal plane of the teeth should be corrected remains uncertain. The purpose of this study was to examine whether a correction of the deviation of the maxillary occlusal plane (MOP) from the center of dens of axis vertebrae (DAV) improves symptoms in patients having intractable headache or shoulder-stiffness. Forty patients who complained of dental abnormalities and persistent headache or shoulder-stiffness that had not responded to conventional medical treatment and 17 healthy controls were recruited. All subjects received a lateral cephalometric x-ray examination to measure a distance from the MOP and the center of DAV. In the healthy subjects, both the upper and the lower shift of the MOP from the center of DAV were minimal (the upper shift was 1 +/- 2 [mean +/- S.D.] mm and the lower shift was 4 +/- 4 mm). By contrast, the patients had a significantly greater deviation of the MOP from the center of DAV. Dental adjustment treatment was performed in fourteen patients who had a substantial deviation of the MOP from the center of DAV. Those patients were asked about their symptoms which were scored using a point system and were compared before and after treatment. An adjustment procedure of the MOP passing through the DAV significantly relieved clinical symptoms in these patients (before 42.5 +/- 34.4 vs after 7.0 +/- 8.2, p < 0.01). Correction of the MOP passing through the near center of DAV might be effective in relieving clinical symptoms associated with dental deformities.

Suppressive effects of red wine polyphenols on voltage-gated ion channels in dorsal root ganglionic neuronal cells.

Polyphenols are ubiquitous plant metabolites with multiple pharmacological properties. Using whole-cell patch-clamp current recording techniques, we studied the effects of polypnenols extracted from red wine (purity > 90% from Cabernet Sauvignon grape wine) on the activities of voltage-operated Na+-, K+-, and Ca2+-channel currents in mouse dorsal root ganglionic neuronal cells. The polyphenols suppressed all of the channel activities with half-effective concentrations of about 2.5, 4.0, and 0.8-1.5 micro g/ml, respectively. In contrast, they showed no noticeable effects on the ion channels in other types of cells, including large conductance K+-channels in mouse lacrimal acinar cells. Thus, the polyphenols suppress firings of the action potential in the neuronal cells and could show a sedative effect on the excitation. We expect that red wine can be used as a remedy for excessive sensory stimuli.

Cognitive decline in patients with long-term domiciliary oxygen therapy.

Cognitive and psycho-physiological condition in patients with long-term domiciliary oxygen therapy (DOT) remains uncertain. A cross sectional analysis was performed to investigate the age-related changes in cognitive and psycho-physiologic functions in patients with chronic respiratory failure receiving long-term DOT. Two expert practitioners visited the patient's home and examined them for analysis of cognitive function, emotional status, physical activity and degree of dyspneic sensation. One hundred and thirty-five patients completed the study. Control data from a cohort of 718 community dwellers were also included in this study. Male patients had significantly higher rates of chronic obstructive pulmonary disease (71% vs 47%, p = 0.001), lower values of forced expiratory volume in one second (FEV1.0) % (49.7 +/- 10.3 [standard deviation, S.D.] vs 66.0 +/- 7.5% predicted, p = 0.002) and higher Borg score, an indicator of dyspneic sensation, during daily exercise (3.2 +/- 0.8 [S.D.] vs 1.4 +/- 0.6, p = 0.01) compared with female patients. Linear regression analysis based on mean Mini-Mental State Examination scores, an indicator of cognitive function, showed that age-related cognitive decline was more pronounced in female patients than in female controls (-0.524/year, R2 = 0.426 vs -0.120/year, R2 = 0.027, p < 0.0001), while there was no significant difference between male patients and male controls (-0.156/year, R2 = 0.054, vs -0.077/year, R2 = 0.016, p = 0.231). These results demonstrate that age-related cognitive decline is more exaggerated in female patients receiving long-term DOT which should be taken into consideration in caring for patients with chronic respiratory failure.

Delayed expression of large conductance K+ channels reshaping agonist-induced currents in mouse pancreatic acinar cells.

Epithelial secretory cells display cell-specific mechanisms of fluid secretion and express large conductance voltage- and Ca2+-activated K+ (Maxi-K) channels that generate the membrane negativity for effective Cl- exit to the lumen. Rat and mouse pancreatic acinar cells had been thought to be peculiar in this sense because of the previously reported lack of Maxi-K channels. However, this view is not entirely correct as evidenced in the present paper. Searching for their presence in pancreatic acinar cells in mice from 5 to 84 weeks of age with patch-clamp current measurements, we demonstrated that the expression of Maxi-K channels is regulated in an age-associated manner after birth. The expression started at approximately 12 postnatal weeks and increased steadily up to 84 weeks. In support of this, RT-PCR could not detect mSlo mRNA, the Maxi-K gene, at either 7 or 8 weeks but could at 58 and 64 postnatal weeks. These results suggest that a key steering element for fluid secretion, the Maxi-K channel, is progressively re-organized in rodent pancreas. A pancreatic secretagogue, acetylcholine, evoked Maxi-K channel current overlapping to various degrees on the previously known current response. This suggests that the rise in internal Ca2+ activates Maxi-K channels which reshape the mode of secretagogue-evoked current response and contribute to Cl- driving in fluid secretion in an age-associated fashion.

Clinical significance of heparanase activity in primary resected non-small cell lung cancer.

Clinicopathological significances of heparanase activity in non-small cell lung cancer (NSCLC) were investigated by analyzing 76 resected specimens of NSCLC. Heparanase activities in NSCLC were significantly higher than non-cancerous lung tissues (P < 0.0001). The heparanase activities of NSCLC were significantly higher in larger diameter tumors (P = 0.0141) or with metastasis to ipsilateral mediastinal lymph nodes (P = 0.0004). The activities of heparanase in primary tumors were increased significantly according to the pathological stage of the progression of the disease (P =0.0009). Among the clinicopathological parameters, histological cell type and evidence of ipsilateral lymph node metastasis showed a significant association with elevated heparanase activities, whereas age, degree of differentiation and tumor diameter did not. Kaplan-Meier curves for overall and disease-free survival demonstrated a significant difference between patients with elevated and non-elevated heparanase activity by log-rank test (P = 0.0145 and 0.0002, respectively). Multivariate analysis showed heparanase activity was an independent factor to influence disease-free survival in our study. These results suggest that heparanase activity could be used as a prognostic indicator for postoperative patients with NSCLC and heparanase might be a promising molecular target for treatment of NSCLC.

Increased plasma 8-isoprostane levels in hypertensive subjects: the Tsurugaya Project.

To examine the relationship between 8-isoprostane and blood pressure, we measured plasma 8-isoprostane concentration and home blood pressure levels in an elderly Japanese population. Our study population comprised 569 subjects aged 70 years and over who were not receiving antihypertensive medication. On the basis of their blood pressure values, the participants were classified into three groups: normotensive (home blood pressure <135/85 mmHg), hypertensive (home blood pressure 135/85-160/90 mmHg), and severely hypertensive (home blood pressure > or =160/90 mmHg). The mean plasma 8-isoprostane level in the severely hypertensive group (21.1+/-5.2 pg/ml) was significantly higher than that in the normotensive (20.2+/-4.9 pg/ml) or hypertensive (19.7+/-5.1 pg/ml) group, and this result was unchanged when we adjusted for possible confounding factors such as age, sex, use of vitamin A, C or E supplements, smoking status, drinking status, body mass index, use of non-steroidal anti-inflammatory drugs, history of diabetes, hypercholesterolemia, home heart rate and serum creatinine level. Thus, the level of plasma 8-isoprostane appears to be elevated in older subjects with severe hypertension.
