[Role of echography in the evaluation of renal masses. Apropos of 65 cases].

65 renal tumours were investigated by echography. The group was made up of 24 simple cysts, 18 hydatid cysts and 23 carcinomas of the kidney. Analysis of these cases led to the following conclusions: 1) Echography should be carried out whenever a tumour syndrome is found by urography. 2) Solid tumours are malignant. They rarely pose any diagnostic problem. For fluid-containing tumours, a parasitic aetiology may be assumed in the presence of characteristic features and whenever the clinical context is suggestive. A simple cyst with a non-echogenic and homogeneous structure is easy to diagnose. The only diagnostic problem which remains is that of tumours with a heterogeneous echostructure. Arteriography is then of great value. 3) In solid tumours, echography must include study of the renal vein, inferior vena cava and liver. 4) The indications of arteriography are hence limited to heterogeneous tumours and carcinomas of the kidney where is particularly useful is assessing venous spread. 5) Taking all tumours together, the reliability rate of echography is 76%. 6) By virtue of its harmless nature and low cost, it has become an examination of choice in the diagnosis of renal tumours.

[Malignant adrenocortical tumor. A propos of a case].

The authors report a case of malignant cortico-adrenaloma in a young woman of 27. This malignant tumour represents 10% of the causes of Cushing's syndrome. It is noted for the rapid onset of the syndrome and a marked hyperandrogenism. The diagnosis depends on an I.V.P. with retropneumoperitoneum completed by arteriography and ultrasound or by more recent investigations: the isotope scan and the C.T. scan. Its prognosis is very poor and the treatment consists of an extended adrenalectomy.

Adrenal Cortex Neoplasms↗

[Hypertension secondary to a "renin tumor". One case (author's transl)].

Eleven cases of hypertension secondary to a renal tumour secreting renin have been described in the literature between 1967 and 1978. The authors here report another case which presented with severe hypertension and a cerebrovascular accident in a 30-year-old woman. Intravenous urography and renal arteriography revealed a tumour of the upper pole of the left kidney. Estimation of renin levels in the renal veins demonstrated hypersecretion on the side of the tumour. Tumourectomy was followed by normalisation of blood pressure levels. Histological examination confirmed a diagnosis of a tumour of the juxtaglomerular apparatus.

[Hereditary degenerative spinocerebellar diseases in Tunisia with manometric studies in bladder disorders (author's transl)].

During a survey conducted in Tunisia in 1978, 204 cases of hereditary degenerative spinocerebellar diseases were discovered among members of 117 families. The cases included 109 patients with Friedreich's ataxia, 28 with Piere Marie's heredo-ataxia, 20 with Strumpell-Lorrain's disease, and 47 with intermediary forms. The latter group included incomplete forms of Friedreich's and Pierre Marie's diseases. The onset or progression of the disease was linked to a febrile episode in 25 p. cent of the cases. Emphasis is placed on the presence of bladder sphincter disorders in approximately one third of the patients with Friedreich's or Pierre Marie's diseases. Manometric studies in 17 cases demonstrated the presence of normal bladders in 4 cases, hypertonicity of the bladder in 5 patients, and hypesthesic retention-type bladders in 5 other cases. In 3 patients the disorder was difficult to classify. These results show that sphincter disorders should not constitute a criterium for exclusion of the diagnosis of spinocerebellar degeneration.

[Hydatid cyst of the kidney (author's transl)].

The authors report a series of 78 cases of hydatid cyst of the kidney collected during the past 17 years. There study material is based upon 72 IVU, 19 retrograde urograms, 22 renal arteriograms, 58 laboratory examinations, 12 renal echographies and 76 surgical operations. The usual presenting symptoms consist of lumbar or lumbo-abdominal swellings, lumbar pain, renal colic, hydaturia and haematuria, whilst bearing in mind that cysts of the upper pole of the right kidney may present with digestive problems as their first symptoms. Almost half of these cysts were calcified. The appearances by IVU were of a renal tumour. In endemic areas, the presence of calcifications is fairly typical of hydatid cyst. In the absence of calcifications, the IVU appearance is that of a renal cyst or carcinoma. The diagnosis is based upon the following: hydaturia, the existence of a second hepatic or pulmonary lesion and positive serological reactions. Renal echotomography has radically transformed the diagnostic approach since although recent, this investigation is reliable and shows identical appearances in the kidney to those found in hydatid cysts of the liver. The use of echography has made it possible to limit arteriography to heterogeneous masses with a solid component. Improved knowledge of echotomography should make it possible to avoid the topographical errors in which a renal hydatid cyst is thought to be in the liver. In the opinion of the authors, treatment should be surgical, whatever the procedure including sterilisation of the cyst by the injection of hypertonic saline and removal of the hydatid, i.e. the cyst ("kystectomy"). After these basic steps the surgeon may decide between simple resection of the protruding part of the cyst (partial perikystectomy, 25 cases) or ablation of all the adventitia (total perikystectomy, 26 cases). When damage to the renal parenchyma is too extensive, nephrectomy may be necessary (19 cases).

[Phaeochromocytoma with psychiatric disturbances. One case (author's transl)].

A case of phaeochromocytoma which presented with hypertension and severe psychiatric disturbances including three attempts at suicide. These psychiatric problems regressed completely following removal of the tumour. Although inconstant, the combination of hypertension and psychiatric problems should always lead to studies for the possible presence of a phaeochromocytoma. The mechanism of these disturbances is not clearly understood but either a direct action of catecholamines on cerebral tissue or changes secondary to blood pressure variations are suggested.

Adrenal Gland Neoplasms↗

Retrovesical rare tumor: the schistosomial spermatocyst.

A case of large retrovesical tumor in a 50 year-old man is reported here. The diagnosis of schistosomial spermatocyst was made by histopathology. The authors underline the difficulty of the topographic diagnosis in large retrovesical masses.
