[Experimental study on delayed reinnervation of laryngeal adductor and abductor].

To investigate time of delayed reinnervated laryngeal muscle, 15 dogs were divided into two groups. The right recurrent laryngeal nerves of 10 dogs in experimental group were cut, and repaired at 4, 6, 8, 10 and 12 months intervals by transposition of the phrenic nerve to the recurrent laryngeal nerve after cutting and suturing the adductor branch to the main branch of ansa cervicalis. The right recurrent laryngeal nerves of 5 dogs in control group were cut, but did not repair. Laryngoscope, electromyography, contractile tension of laryngeal muscle and histologic studies were performed at six months postoperatively. The results showed that fair recovery of adduction and abduction was noted within ten months interval, and the effect of adduction was better than that of abduction. The effect decreased gradually with the denervated time increased. The conclusion demonstrated that delayed reinnervation of laryngeal muscle should be performed within ten months.

[NiTi shape-memory alloy cramp used in repair of skull defect].

Fixation of silicon-polyester fiber network on skull defect was usually difficult to handle and the fixation was unstable. In order to solve these problems, NiTi shape-memory alloy cramp was adopted and 101 patients with skull defects were selected for this clinical trial. Among them, there were 79 males and 22 females, and the age ranged from 12-55 years old. The area of skull defect ranged from 3 cm x 4 cm to 10 cm x 16 cm. All of these patients received repairing of the skull defects by silicon-polyester fiber network which was fixed by NiTi memory alloy cramps. After operation, there was no complication. One hundred patients were followed up for 1-8 years, in which 97 cases returned to their normal work, and only 2 cases had a transient dysfunction of frontal muscle for 2 months. In addition, There were no loosening of the cramps, displacement of plastic network and malignant degeneration. The NiTi shape-memory alloy cramps had the following advantages: 1. Simple operative procedure; 2. Rigid fixation; 3. Mild postoperative tissue reaction; 4. Few postoperative complication; 5. Favorable effect of skull repair; 6. No interference with CT and MRI image; 7. No carcinogenicity.

A comparative observation on treatment of heart failure due to diastolic insufficiency of the left ventricle--a report of 43 cases.

43 cases of heart failure due to diastolic insufficiency of the left ventricle were treated with TCM differential treatment as the treatment group, the total effective rate being 86.0%. Statistically significant difference (P < 0.05) in the therapeutic effects was found between the treatment group and control (25 cases) group. Improvement of the left ventricular diastole in the treatment group was more obvious than that of the control group (P < 0.01), while contractility of the left ventricle was also improved to a certain extent.

[Effect of recombinant human FLT3 ligand (rhFL) on in vitro expansion of human cord blood CD34+ cells].

OBJECTIVE: To determine the effects of recombinant human FLT3 ligand (rhFL) on in vitro expansion of human cord blood CD34+ cells. METHODS: Human cord blood CD34+ cells were enriched by using a magnetic cell isolation system. CD34+ cells were incubated in liquid culture in the presence of different cytokines. The total number of all cells and the number of CD34+ cells and hematopoietic progenitor cells (CFU-GM and BFU-E) were counted at various time intervals. RESULTS: Human cord blood CD34+ cells could be enriched to the purity of more than 80%. The number of CD34+ cells increased 3.4 folds in the presence of rhFL + IL3 + IL6 + GMCSF + EPO in seven days, while in the control group without rhFL, it increased 1.5 folds. The difference of the number of more matured hematopoietic progenitor cells in the CD34+ cell population between the control group and the rhFL group was not significant. CONCLUSION: Only the primitive hematopoietic progenitor cells are expanded by rhFL according to the results obtained in the exclusion method.

[Total body irradiation etoposide followed by autologous hematopoietic stem cell transplantion for non-Hodgkin's lymphoma].

OBJECTIVE: To evaluate the therapeutic effectiveness and safety of total body irradiation(TBI) and Etoposide(Vp-16) as a preparative regimen for autologous hematopoietic stem cell transplantation(AHSCT) with non-Hodgkin's lymphoma(NHL). METHODS: Twenty-four patients with intermediate and high grade NHL underwent AHSCT. They achieved complete remission (CR) or partial remission (PR) after induction chemoradiotherapy. Twenty-three patients had first CR or PR, and one third CR. Ten patients underwent autologous bone marrow transplantation (ABMT) and 14 autologous peripheral blood stem cell transplantation (APBSCT). The preparative regimen was TBI 800(700-850) cGy/Vp-16 757(323-1140) mg/m2. RESULTS: At a median follow-up of 22.5(2-92) months, the one-year disease free survival(DFS) was 86.7% (13/15), 3-year, 5-year and 7-year DFS were 80.0% (12/15) in patients with CR before AHSCT. The DFS was 66.7% (4/6) in patients with PR before AHSCT. The patients who had relapsed before AHSCT(3 cases) did not reach DFS. The hematopoietic function recovery was rapid in APBSCT than ABMT. CONCLUSION: The clinical results of AHSCT for intermediate and high grade NHL who achieved CR or PR after induction therapy are satisfactory. The TBI/Etoposide is an effective and safe preparative regimen for AHSCT in NHL patients.

[Change of cricoarytenoid joint after recurrent laryngeal nerve damaged].

The arytenoid mobilities and histological changes of cricoarytenoid (CA) joints were observed in 1, 2, 4, 6, 8, 10, 12 months after the injury of recurrent laryngeal nerve (RLN). Eighteen adult dogs were randomly divided into three groups: 1. Unilateral RLN partial damaged (4/18), 2. Unilateral RLN complete damaged (7/18), 3. Bilateral RLN damaged (7/18). The results demonstrated that arytenoid mobilities return to normal and no fibroses of CA joints were found in 6 months after RLN injury in group one. The arytenoid fixations were found in 6 and 4 months after RLN injury in group two and three, respectively. The fibroses of CA joints were more severe in group three than group two after RLN injury.

[Characteristics of radiofrequency current catheter ablation in the treatment of elderly patients with supraventricular tachycardia].

To study the effects and characteristics of radiofrequency current catheter ablation (RFCA) in treatment of elderly patients with supraventricular tachycardia (PSVT), fifty-three elderly patients and fifty non-elderly patients with PSVT were included in this study. RFCA were performed in both groups. The group of elderly patients included 26 patients with atrioventricular nodal reentrant tachycardia and 27 patients with atrioventricular reentrant tachycardia due to 29 atrioventricular accessory pathways (Aps). Twenty-one patients were accompanied with hypertension and coronary heart diseases and 5 sick sinus syndrome cases in the elderly group. All patients in both groups were treated successfully with RFCA. The procedure time of ablation of slow pathway in elderly group was shorter than that of the non-elderly group (P < 0.01). A mild symptom of arterial thrombosis was found in 2 cases of the elderly group after treatment and was cured with aspirin. These results suggest that PFCA is very effective and safe in the treatment of elderly patients with PSVT, especially for patients accompanied with sick sinus syndrome.

[Mobilization of autologous peripheral blood stem cells by cyclophosphamide and recombinant human granulocyte colony-stimulating factor(rhG-CSF)].

OBJECTIVE: To observe the effect of cyclophosphamide (CTX) and recombinant human granulocyte colony-stimulating factor(rhG-CSF, Filgrastim) on autologous peripheral blood stem cells (APBSC) mobilization. METHODS: CTX (3.7 +/- 0.2) g/m2 was intravenously injected the first day. rhG-CSF (4.5 +/- 0.6) micrograms.kg-1.d-1 was injected subcutaneously from the day of white blood cell (WBC) nadir to the day before the end of APBSC harvest. APBSC harvest was started when WBC > 2.5 x 10(9)/L and finished when accumulated mononuclear cells (MNC) of APBSC > 5 x 10(8)/kg. CFU-GM, BFU-E culture and CD34+ cells detection of the APBSC was performed. RESULTS: Twenty cases underwent the APBSC mobilization. The nadir of WBC was (1.1 +/- 0.5) x 10(9)/L at day (9 +/- 1). rhG-CSF was injected from day (10 +/- 1) and continued for (6 +/- 1) days. APBSC harvest began on day (13 +/- 1) and continued for (4 +/- 1) days. Accumulated MNC harvest was (8.4 +/- 1.9) x 10(8)/kg, CFU-GM (18.7 +/- 10.3) x 10(4)/kg, BFU-E (18.5 +/- 8.7) x 10(4)/kg, and CD34+ cells (20.9 +/- 5.7) x 10(6)/kg. No severe toxicity was observed. Hematopoietic reconstitution was very well in 18 patients received the APBSC transplantation. CONCLUSION: CTX combined with rhG-CSF was a safe and highly effective method for APBSC mobilization.

[Study on contractile properties of the posterior cricoarytenoid muscle after delayed reinnervation].

To study date on the contractile properties of posterior cricoarytenoid muscle after delayed reinnervation of different reinnervated methods. Twenty four dogs were reinnervated at 0,4,5,6,10 and 12 month interval following recurrent laryngeal nerve via the phrenic nerve anastomosed to the recurrent laryngeal nerve after cutting the adductor branch and ansa cervicalis-sternothyroid muscle pedicle implanted into the posterior cricoarytenoid muscle. After 6 months, a series of contractions were recorded from each side in twenty living dogs. The results showed that contractile force of reinnervated muscle decreased gradually with the time of denervation, but contractile force of muscle was no significantly difference between reinnervated side of nerve anastomosed group in 4 months after denervated and normal side, and it was significantly difference between nerve anastomosed group and nerve-muscle pedicle implanted group at some time of delayed reinnervation. The contractile time of reinnervated side of two operated groups was similar to that of normal side. The conclusion demonstrated that the contractile properties can indicate exactly reinnervated degree of muscle, and the earlier reinnervation was performed, the better curative effect was.

[The relationship between chromatographic retention value and grey model parameters].

The relationship between development parameter a, grey action variable u in the grey model GM(1,1) and parameters A,B which express the relation of k' and phi, that is, 1nk' = A + B phi have been studied. The functions of these two kinds of parameters which are a = -B lambda and u = B lambda eA[(1 - e-B lambda)(-1) - 1] are given. Therefore, it can predict the position of peaks and the possibility of separating the components in different mixtures. It verifies the experimental data, and gives out a proper method to amend equations. The results show that the function is fit for not only the research of chromatographic parameters of explosives but also the prediction of A,B values of other compounds.

[Change of cell proliferation after the denervation laryngeal adducent muscles in dogs].

OBJECTIVE: To investigate the changes of cell proliferation state after denervation of laryngeal adducent muscles in dog. METHODS: The muscles were obtained from 8 dogs with recurrent lanyrgeal nerves injured. The cross sectional areas of muscle cells were measured by VIDS Image Analysis System in HE stained sections. A quantitative collagen/muscle ratio was measured by TJTY-300 image analysis system in Van Gieson stained sections. The percentages of S phase cell nucleus were calculated by image cytometry/DNA quantitative analysis with CAS 200 System. RESULTS: It showed that the cross sectional area of muscle cells decreased gradually after denervation (out-standing in 6 months after denervation) and collagen/muscle ratio increased significantly with time. The percentage of S phase nucleus was high at 5 months and then decreased. CONCLUSION: The proliferation of thyroarytenoid muscle and lateral cricoarytenoid muscle cell of dog remained at high level in 5 months after denervation and then slowed down and atrophy of muscles occurred 6 months after denervation. This suggests that nerve repair should be performed early. Examination of cell proliferation might indicate if the denervation of cells irreversible. It is helpful in choosing proper surgery and estimating the effect of nerve repair.

[Experimental studies on antitumor activity of new recombinant human tumour necrosis factor in nude mice bearing nasopharyngeal carcinoma].

OBJECTIVE: Nude mice model bearing nasopharyngeal carcinoma were established using nasopharyngeal squamous carcinoma cell line (CNE-2). METHODS: New recombinant human tumor necrosis factor (NrhTNF-alpha) and carboplatin were administered locally or systemically, single or in combination. The microstructural and ultra-micro-structural changes of carcinoma after administration of NrhTNF-alpha were observed under light and electron microscopes. RESULTS AND CONCLUSION: 1. After treatment, the tumor showed hemorrhage, necrosis, regression, the survival time was prolonged. 2. Locally administered NrhTNF-alpha had more significant antitumor effect than intravenously administered of NrhTNF-alpha. 3. There is a notable synergistic antitumor effect by the combination use of NrhTNF-alpha and carboplatin. 4. Microscopy showed that cell changes mostly took place in plasma especially in mitochondria and endoplosmic reticula. The mitochondria were highly bloated. Besides, some nuclei were condensed and fragmented, resulted in cell death. These studies provide experimental basis for biological treatment of nasopharyngeal carcinoma.

[Treatment of the unilateral paralysis of vocal fold--comparison between laryngeal framework surgery and reinnervation].

OBJECTIVE: To compare the effects of laryngeal framework surgery and reinnervation in the treatment of unilateral paralysis vocal fold. METHODS: Twenty-three cases of unilateral vocal fold paralysis were operated with laryngeal framework surgery and reinnervation and the voice assessment, acoustic analysis, laryngoscopy, electromyography were done pre- and postoperatively. RESULTS: The short and long term curative effects of the laryngeal framework were good, but with regard to long term result, the reinnervation that can recover the thickness, elasticity and tension of vocal fold was better, and the earlier was the operation done, the better would be the result. CONCLUSION: These two operations were both effective in the treatment of unilateral paralysis of vocal fold, but it should be selected according to the requirement of patient, the pattern of damage, the degree of paralysis and movement of cricoarythenoid joint.

[Clinical studies on the dental and occlusal conditions of molars with vertical cracking of the root].

OBJECTIVE: To study the contributing factors inducing vertical cracking of the root of molars. METHODS: The dental, periodontal, and occlusal conditions of molars with vertical root cracking were studied in 34 cases (39 teeth). RESULTS: The age range of patients was 40-60 years. The crackings occurred mostly at mesial or mesio-buccal roots, their periodontal conditions were much more worse than those roots without cracking and manifested by deep pocket. The abrasion of the occlusal surfaces was more severe than those molars with no cracking, their contact areas were unstable and deviated buccallly. The direction of force during chewing was therefore horizontal, viz., mesio-distally and bucco-lingually. CONCLUSION: The vertical root cracking of molars is associated with traumatical occlusal contact due to severe abrasion which increases the loading during chewing.

[Computer aided photocclusion analysis of traumatic occlusion].

OBJECTIVE: This study was designed to analyse traumatic occlusion quantitatively. METHODS: A photocclusion method using computer image processing technique is described, the processing software provide the occlusal contact area, contact point numbers, contact force, maximum stress, and average stress distribution of all teeth by tables and figures. RESULTS: (1) The traumatic contact area was larger and contact force was greater than that of the normal occlusal contact. (2) There was an irregularity force distribution from molars to incisal area. (3) The symmetrical distribution of contact forces and contact areas were destroyed. CONCLUSION: Computer aided-photocclusion method can be used to diagnose traumatic occlusion.

[Detection of small alveolar bone lesions by digital subtraction radiography].

OBJECTIVE: To employ digital subtraction radiography (DSR) system developed by our school and co-ordinative unit, for diagnosis of small artificial alveolar bone lesions, in order to evaluate the system. METHODS: Seventy-two 0.2-0.9 mm3 lesions were produced in premolar and molar areas of the dry human mandibles. The reference and subsequent radiographs were taken before and after lesions being made. The small lesions were detected by conventional radiographs and by DSR method respectively. RESULTS: As for 0.2-0.5 mm3 lesions, the detections by both DSR and conventional methods were less than 50%. As for 0.7 mm3 lesions, the detection by DSR and conventional methods were 57.5% and 38.7%, respectively (P < 0.01). As for 0.9 mm3 lesions, the accuracy of the detection by DSR and conventional methods was 89.3% and 57.3% (P < 0.01). CONCLUSION: The detectability of small alveolar bone lesions by DSR system was obviously superior to the conventional method.
