[Studies on immunization strategy against hepatitis A in a highly prevalent area].

OBJECTIVE: To explore a strategy for immunization against hepatitis A (HA) in highly prevalent areas. METHODS: Maternal antibodies against hepatitis A virus (anti-HAV) were detected for newly born infants and infection rate of HAV was monitored for children at various stages of epidemic of HA. RESULTS: Proportions of positive maternal anti-HAV in newly born infants decreased from 97.4% to 5.0% during their first 13 months of life in a logistic curve pattern. HAV infection increased with age after one and a half years old during non-epidemic period, and kept in less than 10% before two years old. The age of the increase in HAV infection shifted to an earlier dates and the rate of infection speeded up more for children during an epidemic period. The "window" phase converting maternal anti-HAV to natural infection for infants in the highly prevalent areas was in 12 to 24 months after birth, because the age starting to increase HA infection lagged behind that of infection. CONCLUSION: Based on the prevalence of HA and economic situation, the appropriate time for children's HAV immunization is 12 to 24 months of age.

[Plasma levels of lipids, lipoproteins and apolipoproteins affected by endogenous testosterone].

Plasma testosterone(T), estrodiol(E), total cholesterol(TC), triglyceride(TG), high density lipoprotein-cholesterol(HDL-C), HDL2-C, HDL3-C, lipoprotein(a)[Lp(a)], apolipoprotein(Apo) AI and B100 were determined in 201 subjects, among them 102 patients with coronary heart disease(CHD) and 99 healthy subjects. It was observed that plasma T level was correlated negatively with TG(r = -0.47, P < 0.01) and Lp(a) (r = -0.163, P < 0.05), and positively with HDL3-C(r = 0.328, P < 0.01) and Lp(a)(r = -0.163, P < 0.05), and positively with HDL-3(r = 0.328, P < 0.01) and HDL3-C(r = 0.328, P < 0.01). Meanwhile, plasma levels of TG (1.47 +/- 1.16 mmol.L-1) and Lp(a)(25 +/- 17.8 g.L-1) were significantly higher in the subjects with low plasma T level (< 270 ng.ml-1) than those in the subjects with normal plasma T level (1.10 +/- 0.74 mmol.L-1 and 17 +/- 15.9 g.L-1, respectively), while plasma levels of HDL-C (1.31 +/- 0.30 mmol.L-1) and HDL3-C(0.88 +/- 0.22 mmol.L-1) were significantly lower in the former than in the latter (1.44 +/- 0.33 mmol.L-1 and 1.02 +/- 0.26 mmol.L-1, respectively). The results support that low plasma T level may be a risk factor for coronary heart disease, which may relate to the changes of plasma lipoproteins.

[Changes of blood lipids and apolipoproteins in patients with silent myocardial ischemia].

Serum lipid profiles and apolipoproteins were measured in 148 patients with silent myocardial ischemia (SMI) and 30 healthy control subjects comparable in age. The serum lipid profiles and apolipoproteins were abnormal in all types of SMI which were more significant in type II and III. The extent of ST segment depression in ECG were positively correlated with serum TG, TC, LDL, B100, B100/A1, Lp(a); and negatively correlated with A1, HDL1, HDL2, HDL-C/TC. Multiple-factor stepwise regression analysis revealed that the increased concentration of serum TG, LDL, and B100/A1 ratio and decrease of HDL2-C are independent risk factors in SMI.

[Clinical study on left ventricular diastolic heart failure in the aged patients].

Forty aged patients with LVDHF and 30 with left ventricular systolic heart failure (LVSHF), confirmed by radionuclide ventriculography were studied by clinical, echocardiography and treadmill test. Twenty healthy aged persons were served as control group. Compared with LVSHF, short duration and less severe manifestations of heart failure in LVDHF were observed. Compared with the control, left atrium was enlarged but not the left ventricular chamber, and LV wall was thickened in LVDHF, while enlargement of LV chamber was only found in LVSHF. However, LV systolic function was remained normal in LVDHF. The exercise tolerance was decreased in both LVDHF and LVSHF groups, but more marked in LVSHF.

[Changes of plasma testosterone level in male patients with coronary heart disease].

The plasma testosterone (T), estrodiol (E), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), HDL2-C, HDL3-C, apolipoprotein (apo) A1 and B100, and lipoprotein (a) [Lp(a)] were measured in 118 male patients with coronary heart disease (CHD) and 53 sex and age-matched healthy controls. The results showed that plasma T, HDL-C, HDL3-C levels and HDL-C/TC ratio were significantly lower and Lp(a) level was significantly higher in patients than in the controls. After adjusting body mass index, blood pressure and fasting blood glucose, the difference of plasma level between patients and controls persisted. Plasma T level correlated positively with the HDL-C, HDL3-C level and HDL-C/TC ratio and negatively with systolic blood pressure, apoB100 and Lp(a) level. Logistic regression was used to analyze the data, and this indicated that decreased T and HDL3-C levels, and increased Lp(a) level were the independent risk factors of CHD.

[Effect of apolipoprotein E gene plasma levels of lipids, lipoproteins, apolipoproteins and its relation with coronary heart disease].

Using a technique of polymerase chain reaction-restriction lengths polymorphism (PCR-RLP), the authors detected the genotypes of apolipoprotein EE(ApoE) in 102 normal control subjects and in 118 patients with coronary heart disease (CHD). Meanwhile, the effect of ApoE allele on plasma lipid, lipoprotein, apolipoprotein levels were analyzed, and ApoE allele frequencies of patients with CHD and of control subjects were compared. The results showed that the technique of modified PCR-RFLP was a useful measure to detect the genotypes of ApoE. ApoE gene was polymorphism, resulting in plasma lipid level variance among individuals. As compared to ApoE3 allele, ApoE4 allele had positive effect on plasma TC, LDL-C and ApoB100 levels; while ApoE2 allele had negative effect on plasma TC, LDL-C and ApoB100 levels. The frequency of E4 allele in patients with CHD was higher than that in control subjects. The results in the present study suggest that ApoE4 allele is probably the genetic marker of CHD.

[Relationship between ventricular arrhythmia and blood lipid in hypertensive patients with left ventricular hypertrophy].

The possible relationship among ventricular arrhythmia(VA), blood lipid(BL) and left ventricular hypertrophy(LVH) were investigated in 92 patients with essential hypertension(EH). The results showed that the incidence of VA and complex VA and ventricular tachycardia(VT) were significantly higher in EH with LVH than those in EH with non-LVH. The incidence of VA and complex VA and VT were highest in hypertensive patients with LVH accompanied with hyperlipemia(HL), and VA seemed closely related to left heart structure and BL. The results suggest that LVH and HL are the two additive risk factors of occurrence of VA in patients with EH.

[Relationship of lipoprotein(a) to fibrinolytic activities in healthy subjects].

Plasma tissue-type plasminogen activator(tPA) and plasminogen activator inhibitor(PAI-1) and lipoprotein(a) [Lp(a)] in 133 healthy subjects were determined. The results demonstrated that PAI-1 activity was higher in Lp(a) > or = 0.3 g.L-1 group than Lp(a) < 0.3 g.L-1 group (P < 0.05). Univariate statistical analyses showed Lp(a) concentration was positively associated with PAI-1 activity (P < 0.01), but not with age, sex, body mass index, blood lipids or tPA activity.

[Effects of serum bilirubin on lipoproteins].

The effects of serum bilirubin on blood lipids and lipoproteins in 237 subjects were investigated. The findings were that total cholesterol (TC) and low density lipoprotein-cholesterol (LDL-C) were negatively correlated with total bilirubin and direct bilirubin. The results indicate that bilirubin affects metabolism of lipoproteins and the low level of serum bilirubin is a new risk factor for coronary heart disease.

[Non-invasive detection of endothelial dependent dilatation dysfunction in patients with essential hypertension].

With high-resolution ultrasound, we measured the diameter of the brachial arteries at rest, during reactive hyperaemia (with increase flow causing endothelium-dependent dilatation), and after sublingual glyceryl trinitrate(GTN; causing endothelium-independent dilatation) in 61 subjects, consisting of 30 controls with normal blood pressure(NP) and 31 patients with established essential hypertension(EH). The results showed that in patients with EH, flow-mediated dilatation in arteries was much reduced for comparison with control group(P < 0.01). However, there was no significant difference in response to GTN between two groups(P > 0.05). It indicates that endothelium-dependent vasodilation is impaired in patients with essential hypertension.

[Detection of human papillomavirus in Chinese laryngeal carcinoma with digoxigenin labelled probe prepared by PCR].

The studies described here detect HPV DNA in 124 cases of fresh tissue samples with different lesions of larynx using a nonradioactive Digoxigenin Labelled Probe prepared by polymerase chain reaction (PCR). The labelled probe of HPV consensus primers was obtained, which can detect nine types of HPVs DNAs such as HPV 6, 11, 16, 18, 31, 33, 35, 42 and 58. The results showed that the rate of positive cases with HPV infection was 45.6% (31/68) in the group of laryngeal carcinoma, 20.0% (3/15) for the group of neck metastatic lymph node, 11.8% (2/17) for precarcinous lesions group and 6.3% (1/16) for the group of vocal cord polypus, respectively. Fifteen cases of normal laryngeal tissue adjacent to the carcinoma and 15 cases adjacent to the carcinoma were HPV DNA negative. The carcinogenic action of HPV in laryngeal carcinoma and the sensitivity and specificity of the probe prepared by PCR are discussed in this paper.

[Alteration and clinical significance of plasma levels of tumor necrosis factor in chronic heart failure].

The plasma levels of tumor necrosis factor (TNF) were measured by enzyme-linked immunoabsorbent assay in 109 patients who were grouped as "non-heart failure" (n = 36), "heart failure" (n = 36) and "cachexia" (n = 37). The results showed that there was no obvious difference in caloric intake among the three groups. Mean plasma levels of TNF were increased in patients with heart failure (0.51 +/- 0.2 ng.ml-1, P < 0.05) and even higher in patients with cachexia (6.19 +/- 2.76 ng.ml-1, P < 0.01) than in patients with non-heart failure (0.085 +/- 0.025 ng.ml-1). In the group with enhanced plasma levels of TNF (TNF > 100 pg.ml-1), the incidence of cachexia (61.0%) was obviously greater than that in the group with normal levels of TNF (17.6%). These findings indicate that plasma levels of TNF are elevated in patients with heart failure; high levels of TNF may play an important role in the pathogenesis of cardiac cachexia.

[Chemical constituents in the stem and leaf of Isodon oresbius].

Five triterpenoids were isolated from Isodon oresbius. Their structures were identified as ursolic acid, 2 alpha, 19 alpha-dihydroxy ursolic acid, 2 alpha, 3 alpha, 19 alpha-trihydroxy-12-ursen-28-oic acid, 2 alpha-hydroxy oleanolic acid and 2 alpha, 23-dihydroxy oleanolic acid by physico-chemical constants and spectral analysis.

Histochemical and immunohistochemical studies of distribution of acetylcholinesterase-positive fibers and peptidergic terminals in the nasal mucosa of rats.

OBJECTIVE: To further investigate the mechanism of nasal secretion closely related to the innervation patterns in nasal mucosa with emphasis on the acetylcholinesterase (AChE)-positive fibers and peptidergic terminals in nasal mucosa as well as trigeminal ganglion (TG) cells. METHODS: Histochemical demonstration of AChE-positive fibers, immunohistochemical study of the distribution patterns of multiple peptidergic terminals, double labelling of AChE and substance P (SP) and somatostatin (SOM) mRNA in situ hybridization were carried out in nasal mucosa and trigeminal ganglion (TG) in rats. RESULTS: AChE-positive terminals were mainly distributed in the mid to posterior one third of septal nasal mucosa, with greater staining density on the walls of small vessels and glands. There were fewer such terminals in turbinate mucosa. Tachykinins-ergic terminals, including substance P (SP)-, neurokinin A (NKA)-, neurokinin B (NKB)- and calcitonin gene-related peptide (CGRP)-ergic terminals, had an extensive localizations in nasal mucosa, involving the following areas: between epithelial cells, submucosa, the walls of small vessels, glands and venous sinusoids in both septal and turbinate nasal mucosa. Septal mucosa had the greater density. There were overlaps in the distribution of these peptidergic terminals. There were also vasoactive intestinal peptide (VIP)-, neuropeptide Y (NPY)- and galanin (GAL)-ergic terminals in nasal mucosa. But no neurotensin (NT)- and somatostatin (SOM)-ergic terminals were found. In situ hybridization revealed SOMmRNA expression in TG cells. AChE and nine neuropeptides existed in the cytoplasms of TG cells. Besides, AChE and SP could exist simultaneously in cytoplasms of TG cells. CONCLUSIONS: AChE-positive (corresponding to parasympathetic nerves) and peptidergic terminals have different distribution patterns in the nasal mucosa of rats, although an overlap does exist, indicative of their different physiological effects on the regulation of nasal secretion and other functions; AChE and multiple neuropeptides in the cytoplasm of TG cells might play a role in modulating the nasal secretion in response to stimuli in the nasal mucosa.

[The use of bi-pedical rotatory door muscle-skin flap reconstruction in extended partial laryngectomy for late (T3 and T4) glottic cancer].

The purpose is to radical treatment for late glottic cancer by surgery, to restore the essential function of the larynx, 62 patients of late (T3 and T4) glottic cancer were treated by extended vertical partial and subtotal laryngectomy. At the same time, an appropriate method of reconstruction of laryngeal function by bi-pedical rotatory door muscle-skin flap was presented. The decannulation rate was 87.1% and 85.0% cases enjoyed satisfactory voice. All cases resumed normal mouth-food-taking. The conclusion is that selective treated with extended partial laryngectomy is effective for T3 and T4 glottic cancer.

[Autonomic nervous activities in patients with obstructive sleep apnea before and during continuous positive airway pressure treatment].

OBJECTIVE: Heart rate power spectral analysis (HRPSA) can provide information of sympathetic and vagus activities. The aim of the study was to investigate nocturnal variations of autonomic nervous system (ANS) before and during continuous positive airway pressure (CPAP) treatment in patients with OSA. METHOD: 56 subjects with severe OSA were recruited to undergo 7-hour nocturnal polysomnography and ECG monitoring in sleep lab before and during CPAP treatment. 30 subjects without OSA were included as controls. Subjects were excluded if they were receiving medications known to influence ANS activities. ANS was evaluated by HRPSA. RESULT: In addition to repeated sleep apneas and oxygen desaturations, mean areas of LF (1 383 +/- 305) and HF (662 +/- 190) in OSA subjects were significantly higher than those in the controls (LF 125 +/- 64, P < 0.001; HF 163 +/- 78, P < 0.001). With effective CPAP treatment, LF (221 +/- 81) and HF (221 +/- 108) decreased remarkably (P < 0.001, separately) and reached the levels of controls (P > 0.05, separately). CONCLUSION: The results suggested that nocturnal ANS activities are abnormal in patients with OSA, especially the enhancement of sympathetic nervous activity. Fluctuation of ANS induced by apnea and oxygen desaturation may explain the association between OSA and cardiovascular diseases. CPAP treatment is effective in improving ANS activities by way of eliminating apnea and reversing hypoxemia.

[Relationship between Syndrome-Type in TCM and four trace urinary proteins in patients with bronchial asthma].

OBJECTIVE: To explore the possible relationship between Syndrome-Type in TCM and the 4 trace urinary proteins in patients with bronchial asthma. METHODS: The 4 kinds of trace urinary proteins in 217 patients with asthma were measured by radioimmunoassay, and that in 66 normal subjects were compared. RESULTS: The results showed that there were increments of urinary albumin, urinary IgG, urinary beta 2-microglobulin and urinary Tamm-Horsfall protein in patients with asthma; and the gradual increase tended to be accompanied by the continuous aggravation of patient's condition on TCM Syndrome-Type. Furthermore, these datas proved that the 4 trace urinary proteins might be the most sensitive indicators for detecting impaired renal functions than serum blood urea nitrogen and creatinine. CONCLUSIONS: There were the objective material foundations on TCM Syndrome-Type in asthmatics, which consisted of renal glomerular dysfunctions to filter out albumin and IgG, renal tubular dysfunction to reabsorb beta 2-microglobulin, and to synthesize and excrete Tamm-Horsfall protein.

[Treatment of simple obesity in children with photo-acupuncture].

OBJECTIVE: To study the treatment of simple obesity in children and observe its effect. METHODS: Two hundred and two patients were treated with self-produced photo-acupuncture apparatus (101 cases) or ear-pressing (101 cases), and their effects were observed and compared. RESULTS: Effects of both methods were satisfactory. The photo-acupuncture showed a better result than that of the ear-pressing. After treatment, the obesity indexes lowered significantly and levels of blood lipids, glucose, cortisol and triiodothyronine were all improved markedly. CONCLUSIONS: Photo-acupuncture is a safe, painless, nontraumatic and effective method for treatment of simple obesity, and it is easy to be accepted by children.
