In vivo guidance of regenerating nerve by laminin-coated filaments.

Laminin is reportedly an extremely potent neurite-promoting agent in vitro. We investigated the effect of laminin on regeneration of axons in vivo, using a cord of polyester filaments coated with laminin as a guide, replacing a 10-mm segment of the rat sciatic nerve. After 4 weeks the cross section of the nerve guide at its midportion was examined under an electron microscope. Many regenerated axons were seen in the laminin-coated group, whereas no axon was observed in the controls. We conclude that laminin guides the regeneration of axons in vivo.

Lowered levels of bicarbonate in seminal plasma cause the poor sperm motility in human infertile patients.

Both the adenylate cyclase activity and the motility of human sperm were stimulated by bicarbonate with the same concentration dependency. The correlation between bicarbonate levels in semen and the motility of sperm from the patients with male infertility was investigated. Bicarbonate in semen was found to originate mainly from the seminal vesicles, and a significant positive correlation was observed between bicarbonate levels and volume of semen. The motility of infertile sperm was also found to correlate positively to the seminal levels of bicarbonate. These results suggest that the lowered levels of bicarbonate in semen are at least in part responsible for the poor sperm motility in infertile patients, as a result of the failure in the activation of sperm adenylate cyclase.

A comparative study of seminal trace elements in fertile and infertile men.

Fourteen kinds of trace elements were analyzed in the semen of 22 fertile men and 69 infertile men by means of inductively coupled plasma optical emission spectroscopy. Ca had the highest concentration, and the second and third highest concentrations were Zn and Mg, respectively. The concentrations of Ca, Cr, Mg, Pb, Sr, and Zn were almost the same between fertile and infertile men. Ni had a lower concentration in infertile than in fertile men. Many trace elements, such as Al, Cd, Cu, Mg, Mn, Mo, Sn, and Zn, had significantly higher concentrations in infertile men with normozoospermia than in fertile men. There was a significantly positive correlation between Zn and Mg levels in fertile and infertile men.

[Congenital subclavian steal syndrome: a case of right aortic arch with atretic aberrant left subclavian artery].

Surgical experience of a rare case of congenital subclavian steal syndrome is reported. A 53 year old man was admitted to our hospital complaining of black out attack and headache. Non invasive and angiographic studies revealed the subclavian steal associated with right aortic arch and atretic aberrant left subclavian artery. Through the supraclavicular incision, the direct anastomosis of the left common carotid and subclavian artery was performed. Postoperative angiograms showed a perfect reconstruction of the left subclavian artery and the disappearance of the subclavian steal and about on third amount of the complaints are still remained. Concerning a review of the literatures, several problems are discussed.

Therapeutic effect of a new antibacterial substance ofloxacin (DL8280) on pulmonary tuberculosis.

A new antibacterial substance, ofloxacin (DL8280), was used in the treatment of pulmonary tuberculosis. The agent was given to 19 patients with chronic cavitary lung tuberculosis as single doses of 300 mg daily for 6 to 8 months. A decrease in the amount of tubercle bacilli in the sputum was observed in almost all patients, and negative conversion occurred in 5. Resistance of tubercle bacilli to the agent appeared in patients who did not show negative conversion. No side effects were observed during the administration period. The agent was considered useful in lung tuberculosis therapy.

Aortic atresia with Ebstein's and Uhl's anomaly in corrected transposition of the great arteries: clinicopathologic findings.

The common form of aortic atresia is associated with atrioventricular and ventriculoarterial concordant connections, but it has been very rarely found in "discordant connection" in the literature. In a 6-day-old male infant, clinical and postmortem pathologic features of this rare lesion, associated with Ebstein's and Uhl's anomaly in "congenitally corrected transposition" in situs solitus, are described. A possible pathogenesis is suggested for the concomitant presence of aortic atresia and poor-functioning, morphologic right ventricle.

[A case of gas gangrene in a vesico-vaginal fistula].

A case of gas gangrene is presented. The patient was a 78-year-old woman who was admitted with the diagnosis of vesico-vaginal fistula. Cystography revealed a vesico-intestinal fistula and leakage of contrast medium into the prevesical space, in addition to the vesico-vaginal fistula. Right ureterostomy was performed. Two weeks postoperatively, she complained of severe pain and swelling of her right thigh. The swelling grew rapidly and general condition became worse. A diagnosis of gas gangrene was made 8 days after her first complaint by demonstrating subcutaneous and intramuscular gas formation in X-ray. She died the next day. By the bacteriological examination, this case was non-clostridial gas gangrene.

[Evaluation of ceftizoxime in the treatment of respiratory tract infections].

Ceftizoxime (CZX) was used for 33 patients with respiratory tract infections; 22 patients with pneumonia, 3 patients with pulmonary abscess, 4 patients with diffuse panbronchiolitis and 4 patients with acute exacerbation of bronchiectasia. Clinical effects of CZX were evaluated in 33 patients; excellent in 16 and good in 14 patients. The efficacy rate was 91%. Bacteriological effects of CZX were prominent in 7 patients infected with S. pneumoniae, H. influenzae, K. pneumoniae and Citrobacter, but not in a patient infected with P. aeruginosa. The elimination rate was 92% (12/13). As the side effects, exanthema in 1 patient and gastrointestinal symptoms (nausea and vomiting) in 1 patient were observed. However, they improved without any treatment by cessation of CZX use. Abnormal laboratory findings were observed in 2 patients with elevated GOT and/or GPT. They normalized after cessation of drug. The usefulness of CZX was 82% (27/33). Therefore, CZX is a very useful drug and its effects are promising in clinical management of respiratory tract infections.

[Antiemetic combination of metoclopramide and methylprednisolone for cisplatin-induced vomiting].

An antiemetic combination of metoclopramide and methylprednisolone was administered to 16 lung cancer patients receiving cisplatin (80 cmg/m2) alone or in combination with other drugs. Metoclopramide was administered four times intravenously at a dose of 1.5 mg/kg. Methylprednisolone was administered three times intravenously at a dose of 125 mg. Sixteen patients received a total of 34 chemotherapy courses. No vomiting occurred in 70% of 34 chemotherapy courses and mild emesis (one or two vomiting episodes) occurred in 18% of chemotherapy courses. Side effects were minimal and included mild sleepiness (nine patients), diarrhea (three patients), and hiccups (three patients). It is concluded that a combination of metoclopramide and methylprednisolone is very effective in preventing cisplatin-induced vomiting.

Mechanism of interference between influenza A/WSN and B/Kanagawa viruses.

Simultaneous infection of MDCK cells with influenza viruses A/WSN and B/Kanagawa resulted in mutual interference with virus protein synthesis and in significant suppression of A/WSN growth. When infection by one virus preceded the other by 1 or 2 h, growth of the superinfecting virus was selectively inhibited at the level of transcription. Interference by the pre-infecting virus was strongly dependent on the expression of the viral genome but not on haemagglutinin activity. When the replication of both virus types was restricted to primary transcription by cycloheximide, the only translation products following removal of the drug were those of the preinfecting virus. This result was not affected by blocking secondary transcription by actinomycin D. These findings suggest that intertypic interference occurs at the level of primary transcription. This concept was supported further by the observation that a ts mutant of A/WSN (ts-65) with a defect in primary transcription interfered only with superinfection by B/Kanagawa at the permissive temperature.
