Effect of the stromelysin-1 promoter on efficacy of pravastatin in coronary atherosclerosis and restenosis.

It has proved difficult to identify high-risk patients for atherosclerosis and to determine how they might respond to medication. Recently, a common promoter variant of the human stromelysin-1 gene has been reported, which has been shown to affect the transcription. We investigated whether this polymorphism had any impact on the risk of events, especially restenosis and progression of coronary artery disease and whether the effect was modulated by treatment with pravastatin. The stromelysin-1 genotype was determined for 496 men with coronary artery disease and cholesterol levels between 4.0 and 8.0 mmol/L, participating in the Regression Growth Evaluation Statin Study (REGRESS) study, a clinical trial assessing the effect of the lipid-lowering drug pravastatin on the progression of atherosclerosis. Patients in the placebo group with 5A6A or 6A6A genotypes had more clinical events than patients with the 5A5A genotype (26% and 12%, respectively, p = 0.03). In the pravastatin group, the risk of clinical events in patients with 5A6A or 6A6A genotypes was lower, compared with placebo, whereas it was unchanged in those with a 5A5A genotype (p value for interaction: 0.038). Also, the incidence of repeat angioplasty in the placebo group was greater in patients with the 6A6A or 5A6A genotypes, compared with 5A homozygotes (38% and 40%, respectively, vs 11%, p = 0.09). Again, treatment substantially reduced the incidence in heterozygotes and 6A homozygotes (0% and 15%, respectively), whereas it was unchanged in 5A homozygotes (28%, p for interaction: 0.002). These effects were independent of the effects of pravastatin on the lipid levels. Thus, this study suggests that the stomelysin-1 promoter polymorphism confers a genotype-specific response to medication in determining clinical event-free survival and the risk for symptom-driven repeat angioplasty. This variant may therefore act as a predictor, not only of disease progression, but also of response to therapy and risk of restenosis.

Genetic variation at the matrix metalloproteinase-9 locus on chromosome 20q12.2-13.1.

Allelic association methods are better suited than linkage analysis for mapping of susceptibility genes that confer modest increases in risk in complex diseases. In both family- and population-based association studies, it is very useful to have prior knowledge of all sequence variants and the degree of linkage disequilibrium in a candidate gene region. In this study, we scanned sequence variants in a 2.2-kb promoter sequence and all 13 exons (totalling 3.3 kb) of the matrix metalloproteinase-9 gene, which is associated with coronary heart disease and a candidate for other diseases involving connective tissue remodelling, such as cancer metastasis. The sequences had a total of ten variable sites, four in the promoter, five in the coding region (three of which alter the amino acid encoded) and one in the 3' untranslated sequence. Sequence inspection suggests that some of the variants will have a functional impact on either level of expression or enzymatic activity. Tight linkage disequilibrium was detected between variants across the entire length of the gene (approximately 9 kb), and frequencies of different haplotypes were determined. The data provide an essential tool for studies of the possible contribution of genetic variation at the matrix metalloproteinase-9 locus to genetically determined susceptibility to a number of important diseases. The results also provide experimental data on the extent of linkage disequilibrium in the general population, which is yet to be resolved.

[Expression of MAGE-1 gene in hepatocellular carcinoma].

OBJECTIVE: To explore the possibility of tumor associated antigen encoded by MAGE-1 gene used as a target for immunotherapy in HCC patients. METHOD: The expression of MAGE-1 gene in 39 HCCs and 3 HCC cell lines and 5 non-HCC patients liver tissues was examined by RT-PCR method. RESULTS: The expression rate of MAGE-1 mRNA was significantly higher in HCC patients than in paratumor tissues (P < 0.01, 58.9% vs 10.3%). All the 3 HCC cell lines expressed MAGE-1 gene positively. All 5 non-HCC patients' liver tissues did not express MAGE-1 gene. CONCLUSION: The high frequency of expression of MAGE-1 gene in HCCs suggests the possibility of MAGE-1 used as a target for immunotherapy in HCC patients.

Normal electrocardiogram with total occlusion of the left anterior descending coronary artery.

Total occlusion of the left anterior descending coronary artery is usually characterized by ST-segment elevation in the anterior leads of the surface electrocardiogram. We report a case of a patient who had a persistently normal electrocardiogram throughout his hospitalization despite the angiographic findings of total occlusion of the left anterior descending coronary artery and no collateral vessels. Percutaneous transluminal coronary angioplasty with stent placement was performed successfully.

Surgical treatment of hepatocellular carcinoma and related basic research with special reference to recurrence and metastasis.

OBJECTIVE: To summarize the progress of surgical treatment of hepatocellular carcinoma (HCC) and related basic research at the Liver Cancer Institute of Shanghai Medical University in the recent years, with special reference to recurrence and metastasis. METHODS: Published and unpublished update clinical and experimental data in the above-mentioned areas are summarized. RESULTS: Surgical resection has played an important role in improving prognosis of HCC, the 5-year survival were 63.4% for small HCC resection (n = 806), 39.6% for large HCC resection (n = 1061), 64.7% for cytoreduction (using hepatic artery cannulation and ligation) and sequential resection of initially unresectable HCC (n = 93), 56.0% for cytoreduction using transcatheter arterial chemoembolization (TACE) and followed by resection (n = 65), and 22.4% for hepatic resection with removal of tumor thrombi in portal vein (n = 103). Unfortunately, the 5-year recurrent rate after curative resection of HCC was up to 61.5%, which was mainly a result of intrahepatic "metastasis" and multicentric origin of HCC. Clinically, re-resection of subclinical recurrence yielded 56% of 5-year survival (n = 202); prevention of recurrence by transcatheter arterial chemoembolization (TACE) + Interferon, or LAK/IL-2 therapy have decreased 3-year recurrent rate from 33% to 11%-18%. In experimental aspect, metastatic human HCC model in nude mice (LCI-D20) and HCC cell line with metastatic potential (MHCC97) have been established; studies on HCC invasiveness in the molecular level revealed similar results that reported in other solid cancers, and small HCC showed slightly better biological characteristics as compared with large HCC; microvessel density (MVD) that reflecting angiogenesis adversely correlated with 5-year survival of small HCC; experimental interventions using antisense H-ras, bispecific antibody, BB94, as well as anti-angiogenic agents (TNP470, suramin, CAI, heparin, antisense VEGF, etc.) have been demonstrated to inhibit tumor growth and lung metastasis in nude mice model. CONCLUSIONS: Recurrence and metastasis are the major obstacle to further improve prognosis of HCC, studies should be conducted both in clinical and experimental aspects, "HCC invasiveness" will be the major target to be studied, particularly in the molecular level, and anti-angiogenesis will be one of the important approach.

[The diagnostic value of 13C-urea breath test in Helicobacter pylori colonization density and the severity of gastritis].

OBJECTIVE: To evaluate the diagnostic value of (13)C-urea breath test (UBT) in Helicobacter pylori (Hp) colonization density and the severity of gastritis. METHODS: The diagnostic accuracy of UBT in the diagnosis of Hp infection with reference to histology (HE and Warthin-Starry stain) was analyzed and the UBT results (DOB values) among the groups of gastritis with different degrees of severity were compared. RESULTS: By using UBT, 96 of 103 patients with Hp -positive histology were detected (sensitivity, 93.2%), the specificity was 89.1%. DOB value in the Hp colonization dense group was significantly higher than in the other groups (low and moderately dense) (P < 0.05). A significant but weak, correlation was found between the DOB values and the colonization density either with (rs = 0.599 0, P < 0.000 1) or without (r(s) = 0.245 7, P = 0.015 8) inclusion of histologically negative patients. In Hp positive patients, DOB values neither in the groups of active gastritis (0 - 3 grade) nor in the groups of chronic gastritis (1 - 3 grade) had no significant difference. There was no correlation between DOB values and severity of inflammatory infiltration. CONCLUSION: (13)C-UBT has high sensitivity and specificity in the detection of Hp infection. There is significant but weak, correlation between DOB values and the colonization density of Hp. DOB values could not reflect the density of neutrophil and lymphocyte infiltration.

[Effects of methyl tertiary butyl ether on liver tissue gene expression in rats].

OBJECTIVE: Methyl tertiary butyl ether (MTBE) is a new gasoline additive, with certain carcinogenecity in animal experiments. To study its possible mechanism of carcinogenesis in animals, expression of protooncogene c-myc and functional gene glutathione S-transferase-P (GST-P) in the liver tissues of rats chronically exposed to MTBE was detected. METHODS: Forty male SD rats with body weights of 180 - 200 grams were randomly divided into four groups and gavaged with MTBE dissolved in soybean oil with doses of 1 000, 600, 200 and 0 mg/kg, respectively, once daily and five days a week for 13 weeks. Liver tissues of the animals were frozen quickly in liquid nitrogen and their total RNA extracted with one-step method. The probes of c-myc and GST-P were labeled with digoxin by random primer method, and dots hybridization with RNA was used. RESULTS: Levels of expression of c-myc gene in the liver tissues of rats increased significantly, but not for those of GST-P. CONCLUSION: MTBE can induce the higher expression of c-myc gene, which suggests it can promote cell proliferation-one of possible mechanisms of carcinogenesis in animals.

[Studies on tannin and hydrolysate in three species of Chinese Caesalpinia plants].

OBJECTIVE: To get Chinese Caesalpinia plants that can replace the imported Tara to produce tannin and gallic acid. METHOD: Extract tannin with water, condense and hydrolyze the extracts with alkali, the hydrolysates were identified. Furthermore, an enlarged scale experiment was conducted. RESULT: Tannin contents of the plants were 32.2%, 46.6% and 58.5% respectively, the converted ratios of raw material to hydrolysate were 154:1, 11.8:1, 3.09-2.79:1 respectively. CONCLUSION: We got the Tara-replacing plants in Chinese Caesalpinia plants for the first time.

[Species and distribution of Chinese medicinal plant Caesalpinia].

OBJECTIVE: To gain a clear idea on the resources of medicinal plant Caesalpinia in China. METHOD: Conducting field investigation and consulting related specimens and data. RESULT: The distribution, growing environment and medicinal parts of 14 species of Caesalpinia have been clarified, and a key for their identification is given. CONCLUSION: A scientific basis for further study of the medicinal plant Caesalpinia in China has been provided.

[Investigation on the fruit output and ecological environment of Caesalpinia in China].

OBJECTIVE: To estimate the fruit output of Caesalpinia in China and understand its ecological environment. METHOD: On-the-spot investigation in the producing area. RESULT: The output of 8 species of Caesalpinia was estimated and their ecological environment was described. CONCLUSION: The natural resources of Chinese Caesalpinia, especially C. decapetala and C. minax, are rich, and thus worth exploring and developing.

[The biological behavior of jugular glomus tumor and the perioperative management].

OBJECTIVE: Based on our experiences, the biological behavior and perioperative management of the jugular glomus tumor are introduced and discussed in order to prevent the possible mortality and morbidity. METHODS: Five cases of jugular glomus tumor treated in our institute were reviewed. Laboratory examination, modern imaging techniques such as CT, MRI, 131I-MIBG, DSA, as well as embolization of the blood vessels and/or antihypertensive drugs were selectively used in the preoperative period. RESULTS: Surgical removal of the tumors was successful in this group. CONCLUSION: Perioperative management plays an important role in the prevention of surgical complications.

[Study on the genotoxicity of methyl tertiary-butyl ether].

Methyl tertiary-butyl ether(MTBE), a new gasoline additive, is added to gasoline as an octane enhancer and used to reduce automotive emissions. Using Ames test, unscheduled DNA synthesis (UDS) assay and cell micronucleus test, the authors evaluated the genotoxicity of domestic produced MTBE. MTBE did not show any mutagenic potential in Ames assay(TA98, TA100 strains) with or without rat liver-derived metabolic activation system (S-9). Rat primary hepatocyte UDS assay showed that MTBE could damage DNA. In NIH3T3 cell micronucleus test, MTBE showed negative results. It could be concluded that MTBE has some genotoxicity at DNA level.

Developmental changes in the glycosylation of glycoprotein hormone free alpha subunit during pregnancy.

Glycoprotein hormone alpha subunit, in its free form (free alpha), is a major placental product. Its glycosylation was found to change dramatically during the advancement of pregnancy. In this study, we have analyzed these glycosylation changes in five normal pregnancies. Binding to Lens culinaris lectin increased dramatically in all subjects between weeks 14 and 17 from the last menstrual period, indicating more core fucosylation as well as possible changes in branching of glycans. Studies using Datura stramonium agglutinin confirmed that the type of triantennary branching changed in this period of pregnancy. The precise structural nature of these changes was determined by high-pH anion-exchange chromatography and electrospray ionization mass spectrometry. Amounts of core fucosylation and of triantennary glycans increased substantially from early to late second trimester, and a shift was observed from 1-->4/1-->3- toward predominantly 1-->6/1-->6-branched triantennary structures. The glycosylation changes occurred in all five individuals at the same time period in gestation, suggesting developmental regulation of N-acetylglucosaminyltransferases IV and V and alpha6-fucosyltransferase during normal pregnancy. These enzymatic activities also appear to be affected in malignant transformation of the trophoblast. Our findings have important implications for the proposed use of specific forms of glycosylation as markers for cancer, as the relative amounts of these glycans in normal pregnancy will be determined by gestational age.

Effects of two new lubricants on the mutagenicity of scooter exhaust particulate matter.

Scooter exhaust particulate matter, which is known to be mutagenic, has caused heavy air pollution in Shanghai. There are about 400,000 scooters in the city, and the number will not significantly decrease for several years. In order to manage the pollution caused by scooter emissions, many new catalytic instruments and additives were developed. In this paper, the effect of two new lubricants on the emission of scooter exhaust particulate matter was evaluated using gas chromatography/mass spectrometer (GC/MS), in vitro Salmonella/microsomal assay and the in vivo mouse micronucleus assay. The purpose was to learn if these new lubricants could have the ability to reduce the exhausts of particulate matter, and decrease the mutagenicity of exhaust particles. The domestic Jinniao (TN36L) vehicle was chosen as the test vehicle. The results showed that the two new lubricants reduced the emission of scooter exhaust particulate matter, including the quantity of particulate matter and the organic substances adsorbed onto the particles. Compared with the control oil, the two new lubricants reduced the emission of particulate matter by 33% and 36%. When using the two new lubricants, only 20 and 23 kinds of compounds were identified separately in the organic extracts of the exhaust particulate matter by GC/MS, while 45 kinds of compounds were detected in the organic extracts of the exhaust particulate matter when the control oil was used. Mutagenicities of the extracts of scooter exhaust particulate matter were significantly reduced when the two new lubricants were used, including the number of histidine-independent colonies over the background in TA98 with and without S9 mix in the Salmonella test (F2 sample); and the frequencies of bone marrow micronucleated polychromatic erythrocytes in the mouse micronucleus assay (both F1 and F2 samples). These results seem to indicate that these new lubricants can effectively reduce the pollution of scooter exhaust particulate.

Organization of quinto-frontal structures in hatchling ring doves (Streptopelia risoria).

Transganglionic and retrograde tracing procedures were applied to peripheral and central trigeminal structures in hatchling ring doves. The organization of the trigeminal ganglion, its somatotopic projections upon the principal sensory nucleus (PrV), and the projections of PrV upon the telencephalon are similar in adult and hatchling Columbiformes. The results suggest that development of feeding patterns in these species involves experiential differentiation of trigeminal sensorimotor circuits present at hatching.

The effect of sheng bai solution on irradiated mice bone marrow cell division index and DNA content.

Before and after general irradiation with 60Co-gamma, mice were orally given Sheng Bai Solution (SBS) for one week. SBS alleviated the irradiation-induced reduction of bone marrow cell chromosome division index. The irradiation-induced decrease of marrow DNA amount, thymic and splenic fractions, and total leukocyte number were restored to some extent. SBS also helped to ameliorate general condition of patients.

A limited renal injury may cause a permanent form of neurogenic hypertension.

Previously, we have shown that an acute injury to the kidney produced by an intrarenal injection of phenol causes an immediate increase in blood pressure and in norepinephrine (NE) secretion from the posterior hypothalamus. The studies suggest that in this model afferent impulses from the kidney to central integrative structures in the brain may be responsible for the increase in blood pressure. To further evaluate whether a renal injury caused by the intrarenal injection of phenol leads to a permanent elevation of blood pressure and whether this is mediated by increased sympathetic nervous system activity, we examined the chronic effects (4 weeks) of an intrarenal injection of 50 microL of 10% phenol on blood pressure and NE secretion from the posterior hypothalamus. Systolic blood pressure increased from 128 +/- 2.1 to 176 +/- 1.5 mm Hg (P < .01) 4 weeks after receiving the intrarenal injection of phenol, but it did not change in rats that received the vehicle (128 +/- 2.4 and 135 +/- 1.7 mm Hg) and in rats that were subjected to renal denervation (127 +/- 3.4 and 124 +/- 1.0 mm Hg). The secretion of NE from the posterior hypothalamic nuclei was greater (P < .01) in rats that received phenol (253 +/- 9.6 pg/mL) than in controls (158 +/- 8.6 pg/mL) and denervated rats (170 +/- 2.1 pg/mL). These studies have shown that a limited injury to one kidney may cause a permanent elevation of blood pressure and this is associated with increased sympathetic nervous system activity.
