[The clinical analysis and review of literature of pulmonary carcinosarcoma and blastoma].

OBJECTIVE: To explore the clinical characteristics, diagnosis, treatment of pulmonary carcinosarcoma and blastoma. METHODS: Summarizing retrospectively 15 cases of pulmonary carcinosarcoma and 2 cases of pulmonary blastoma from January 1979 to November 1998, and reviewing literature to analyse their histologic, clinical feature, diagnosis and treatment. RESULTS: Pulmonary carcinosarcoma and blastoma both were found to be rare malignant tumors, and both epithelial and mesenchymal elements can be seen under the microscopy with different characteristic features. It was hard to be diagnosed before operation. The main symptoms of pulmonary carcinosarcoma were cough, expectoration, sometimes bloody phlegm. X-ray showed that peripheral type were most common, and 2-5 cm in size, well-distributed in density, some showed central or eccentric cavities whose wall was irregular. The margin of the focuses were clear, leafy, the distensive lymph nodes of hilum or mediastium could also be seen. Bronchoscopic examination revealed the bronchial cavity was obstructed by mass or became stricted. Local biopsy can found squamous, adenocarcinoma or large cell carcinoma, respectively. Malignant cell can also be found in sputum. So it was easy to be confused with lung cancer. 14 cases of the carcinosarcoma were treated surgically and diagnosed, 1 case was diagnosed by lymph node biopsy. The symptoms of pulmonary blastoma were fever and hemoptysis. X-ray or CT scan showed an isolated mass in the lung, with uneven density. Malignant cells in pleural effusion were found in one case and can not be diagnosed definetely but by pulmonary puncture. The another case was diagnosed by surgical operation. 15 cases of this group were followed up, in which 4 cases are still alive (including 1 case of pulmonary blastoma), the 1-year survival of carcinosarcoma is 30% (3/10), 2-year survival 20% (2/10), in which 1 case survived more than 6 years. Another pulmonary blastoma case only survived 3 months. CONCLUSIONS: These 2 kinds of malignant tumor are often easy to be misdiagnosed, and prognosis is very poor. Surgical resection is the main method of diagnosis and treatment.

[Systemic inflammatory response syndrome in critical patients - an analysis of 1,292 cases].

OBJECTIVE: To study the significance of the occurrence and the development of systemic inflammatory response syndrome (SIRS) in critical patients. METHODS: The clinical data of 1292 patients accepted by our hospital in ten months from October 1995 to July 1996 were analyzed. The patients met at least two of the criteria for SIRS such as fever, hypothermia, tachycardia, tachypnea or abnormal white blood cell count. RESULTS: 1292 (67.7%) of the 1909 patients investigated met two or more of the criteria for SIRS. In the 1292 cases, those who met two, three or four of the criteria were respectively 467 (36.1%), 526 (40.7%) and 299 (23.1%). 149 patients (11.5%) in the 1292 cases died of multiple organ dysfunction syndrome (MODS). In which 33 (7.1%), 57 (10.8%) and 59 (19.7%) respectively in patients with two, three and four of the criteria for SIRS. The proportion of patients suffering from acute respiratory distress syndrome, metabolic function dysfunction, disseminated intravascular coagulation and acute renal failure increased with the increase in the number of SIRS criteria that the patients met and with the increase in the proportion of patients progressing from SIRS to septic shock. The mortality rate of the patients also gradually increased while the patients with SIRS were developing sepsis, severe sepsis and septic shock, but there was no statistical significance (P > 0.05). CONCLUSION: It is suggested that bacterial infection, severe trauma and acute pancreatitis might cause SIRS and compensatory anti-inflammatory response syndrome might play an important role in the maintenance of internal environment of the body. A comprehensive understanding of SIRS might be of help to the management of critical diseases.

[Clinical effect of captopril in diabetic patients with complicated acute myocardial infarction].

OBJECTIVE: To observe the clinical character of the diabetic patients with complicated acute myocardial infarction and to evaluate the earlyterm effect of captopril on these patients. METHODS: In a multi-centre, randomized, double -blind and placebo-controlled clinical trial of ACEI for AMI in China. In a total of 14 666 AMI patients 1 376 had diabetes mellitus. The 1 376 diabetic patients with complicated AMI were randomized into captopril (669) and placebo (707). All the patients were treated with captopril (12.5mg three times daily) or placebo for four weeks. RESULTS: The results of the entire study showed that the four-week mortality of AMI showed significant difference between patients with and without diabetes (14.75% vs 8.89%, P < 0.0001). The four-week mortality of AMI in patients with diabetes showed no significant difference between those treated with captopril and placebo (14.95% vs. 14.57% P > 0.05). Rates of incidence of main complications within four weeks also showed no significant difference between the two groups. CONCLUSION: After AMI, for the patients with diabetes, treatment with captopril not show a beneficial effect on the early-term prognosis.

[Efficacy of TACE in treatment of intraportal tumor thrombi with double helical CT].

The present study observed alterations of the intraportal tumor thrombi before and after transcatheter hepatic arterial chemo-embolization(TACE) in 36 patients with massive hepatocellular carcinoma(HCC). Elscint double helical CT was used to scan liver in the portal phase before and after TACE. The results showed that the scan technique could be used to evaluate the distribution of intraportal tumor thrombi. After treating with TACE, retention of iodized oil was found in the intraportal tumor thrombi(14/36), and the intraportal tumor thrombi were reduced in 28 out of 36 cases. The survival rate was more than 1 year in 20 patients. These data indicate that the blood supply of the intraportal tumor thrombi comes from hepatic arterial system. TACE seems to be valuable for the treatment of intraportal tumor thrombi with massive HCC.

[Determination of columbianetin, columbianetin acetate and osthol in Angelica pubescens Maxim. f. biserrata Shan et Yuan by HPLC].

OBJECTIVE: To develop a method for the determination of columbianetin, columbianetin acetate and osthol in Angelica pubescens f. biserrata METHOD: HPLC method, ODS column (6.0 mm x 150 mm) with a mobile phase of methanol-acetonitrile-0.025 mol.L-1 phosphoric acid (32:20:48), and wave length detection at 320 nm. RESULT AND CONCLUSION: The method can be used for the quality control of raw medicinal materials.

[Determination of stilbene in radix Polygoni multiflori by HPLC and its stability study].

OBJECTIVE: To develop a HPLC method for determining stilbene in Radix Polygoni Multiflori and studying its stability. METHOD: HPLC conditions: An Econosphere C18 column, a mixture of methanol-water(30:70) as mobile phase, and UV detection at 310 nm. In addition, the stability of stilbene was studied at high temperature and in acidic solution. RESULT: The linear range of calibration curve was 2.24 x 10(-2)-1.12 x 10(-1) micrograms(n = 6) with a correlation coefficient of 0.99998, the average recovery with RSD being 99.96% +/- 1.54%. CONCLUSION: The method is sensitive, accurate and speedy. Stilbene in aqueous solution is stable, but high temperature(80 degrees C) tends to affect the stability. In acidic solution no stability can be found.

[Studies on chemical constituents of the roots of Gypsophila oldhamiana Miq].

OBJECTIVE: To separate compounds from the root of Gypsophila oldhamiana. METHOD: Chemical and chromatographic methods were used to separate the compounds. IR, MS, NMR were used to determine the structures of compounds. RESULT: Compound pentacosanoic acid, lacceroic acid, beta-sitosterol, alpha-spinasterol, daucosterol and sucrose were identified. CONCLUSION: All the above compounds were obtained from this genus for the first time.

[Determination of gallic acid in garden burnet by HPLC].

OBJECTIVE: To establish a HPLC method for the determination of gallic acid in garden burnet. METHOD: The sample was extracted with methanol under ultrasonic management for 25 min. Chromatographic conditions: Shimadzu LC-6A with SPD-6AV detector at 270 nm and auto injector SIL-6A, methanol-0.025 mol/L phosphoric acid(15:85) as the mobile phase. Shin-pack CLC-ODS column(6.0 mm x 15 mm). RESULT: Gallic acid in garden burnet was baseline separated and determined. The average content was 0.22%, recovery rate 97.4% and relative standard deviation 1.3%. CONCLUSION: Simple, rapid and accurate, the method provides new indexes for quality control of Garden Burnet.

[A comparative study of finger plethysmography and aortic pressure for monitoring the effect of external counterpulsation].

Invasive pressure detecting and monitoring of aortic root was conducted in 20 volunteers so as to study the effect of Enhanced External Counterpulsation (EECP) on aortic pressure and finger plethysmography. The results demonstrated that the changes of aortic diastolic pressure linearly related to the counterpusation's bladder pressure, and the aortic augmentation increased to the top of 168.56 mmHg with a D/S ratio of 2.75(amplitude ratio of diastolic waveform and systolic waveform, D/S ratio) following the bladder pressure of 0.45 Kg/cm2. With the increase of bladder pressure, the dp/dt of aortic augmentation elevated gradually, but a down-regulating trend was seen in systolic waveform. During EECP, the starting time of plethysmographic augmentation had a mean delay of 108.20 +/- 25.70 ms comparing with that of aortic augmentation. The duration between the starting time of aortic systolic waveform and that of finger plethysmogram was arround 96.69 +/- 38.70 ms. When the bladder pressure changed, no significant difference was observed among them. The D/S ratio of finger plethysmographic waveform changed in concord with that of aortic pressure with the promotion of bladder pressure. Therefore, a relationship between aortic D/S ratio (Y) and plethysmographic D/S ratio (x) was constructed by the linear regression analysis: Y = 0.376 + 0.457X. It can be concluded that EECP has significant effects on aortic D/S ratio, aortic peak augmentation and its dp/dt value. Finger plethysmography, to date, still plays important roles in monitoring and controlling EECP work safely and effectively.

[Cloning, sequencing and expression of Sinorhizobium meliloti strain 042B gene nodD].

S. meliloti strain 042B is a rhizobium strain which can form nodules both on alfalfa and on soybean. In this study, nodD gene of 042B was cloned into pBBR1MCS-5. By functional analysis in the system of R. leguminosarum bv. viciae LPR5045 (pSym-, pMP154), it was found that the NodD has responses both to luteolin and to genistein. This result showed that the 042B nodD gene is probably a specific nodulation determinant which determined its capability of nodulation on both alfalfa and soybean. The nodD fragment was then cloned into the expressional vector pThioHis A, B and C, and three recombinant plasmids pXDA, pXDB and pXDC were constructed. The plasmid pXDC was identified to be in the same open reading frame with the trxA gene of pThioHis C throuth sequencing analysis. Inducing IPTG and analyzing with SDS-PAGE, it was found that the fusion protein expressed from E. coli Top10(pXDC) with the molecular weight of TrXA and NodD together. The Western bolt result demonstrated that the expression result is the target gene nodD product.

[The effects of reactive oxidants on Bacillus thuringiensis parasporal crystals].

The effects of hydrogen peroxides(H2O2) and hydroxyl radicals(.OH) on the purified parasporal crystals from Bacillus thuringiensis var. Kurstaki HD-1 were studied by SDS-PAGE and bioassay. The results showed that to some extent H2O2 and .OH produced a direct and deleterious effect. Hydroxyl radicals from the system Fe(II)-EDTA-H2O2 indicated more serious damage than hydrogen peroxides. There was a 44.5% loss in activity against 12-h-old larvae of Heliothis armigera when parasporal crystals were treated with 0.3% H2O2 only. However, 85.2% activity lost when parasporal crystals were treated with 0.3% H2O2 in the presence of iron(II) and EDTA.

[Cloning of DNA fragment related to salt tolerance in Sinorhizobium meliloti 042B].

Total DNA partially digested by EcoR I was prepared for S. meliloti 042B, in which 15-25 kb DNA fragments were collected. Vector pLAFR I was purified and digested by EcoR I, and then the various DNA fragments of 042B were ligated with pLAFR I by T4DNA ligase. Gene library of S. meliloti 042B was constructed with pLAFR I using E. coli S17-1 as recipient. The number of bacterial recombinants obtained was about 8,000 and 95% of them contained foreign DNA fragments. Using NTG, 042B was mutated on FY plates and 12 sensitive strains were screened at 0.5 mol/L NaCl from 2,000 colonies. One of them was named GZ17 and selected as a recipient strain. By biparental mating the foreign DNA fragments were introduced from gene library of strain 042B into recipient strain GZ17 which is sensitive to 0.5 mol/L NaCl. Then the transconjugants were grown on FY plates containing tetracycline (20 micrograms/mL) and 0.5 mol/L NaCl. A 7 kb inserted DNA fragment related to salt tolerance was obtained. In subcloning experiment, a 4 kb DNA fragment related to salt tolerance was obtained.

[Genetic diversity among Chinese peanut rhizobia by rep-PCR analysis].

Repetitive sequences(repetitive extragenic palindromic[REP], and enterobacterial repetitive intergenic consensus [ERIC]) with the polymerase chain reaction(PCR) were used to fingerprint pure DNA extracted from 79 bradyrhizobial strains. These strains included 59 peanut rhizobia [Bradyrhizobium sp. (Arachis)] isolated from root nodules of 24 peanut (Arachis hypogaea L) cultivars from 23 sites in 11 provinces in China, 6 peanut rhizobial strains from other nations, and 14 reference strains of other bradyrhizobia. All the strains were clustered at the level of 11%, 2%, and 11, 24 clusters were obtained at the similarity of 50% with REP-PCR and ERIC-PCR fingerprints, respectively. The results showed that significant genomic diversity exists within the peanut rhizobia from China. Also they suggested that there are different distributions of REP and ERIC in genomic DNA of the peanut rhizobia. The combined results, REP plus ERIC data, were found between the both above. The genomic diversity seems not to correlate with their host and geographic origins. Our results supported this technique is a useful tool for genotypic characterization and identification of rhizobia as well as ecological studies.

[Multi-channel probing for emission spectra of metals stimulated by LD pumping laser].

Emission spectra of several metals stimulated at a tiny spot by an YAG laser pumped by a LD were obtained analyzed by means of a CCD optical multi-channel analyzer. The emission spectra collected and analyzed by multi-channel with high signal to noise ratio and high dynamic range were obtained, and the wavelengths accord well with the standard spectra of these metals. Application of this method was discussed.

Transgenic knockout mice exclusively expressing human hemoglobin S after transfer of a 240-kb betas-globin yeast artificial chromosome: A mouse model of sickle cell anemia.

Sickle cell anemia (SCA) and thalassemia are among the most common genetic diseases worldwide. Current approaches to the development of murine models of SCA involve the elimination of functional murine alpha- and beta-globin genes and substitution with human alpha and betas transgenes. Recently, two groups have produced mice that exclusively express human HbS. The transgenic lines used in these studies were produced by coinjection of human alpha-, gamma-, and beta-globin constructs. Thus, all of the transgenes are integrated at a single chromosomal site. Studies in transgenic mice have demonstrated that the normal gene order and spatial organization of the members of the human beta-globin gene family are required for appropriate developmental and stage-restricted expression of the genes. As the cis-acting sequences that participate in activation and silencing of the gamma- and beta-globin genes are not fully defined, murine models that preserve the normal structure of the locus are likely to have significant advantages for validating future therapies for SCA. To produce a model of SCA that recapitulates not only the phenotype, but also the genotype of patients with SCA, we have generated mice that exclusively express HbS after transfer of a 240-kb betas yeast artificial chromosome. These mice have hemolytic anemia, 10% irreversibly sickled cells in their peripheral blood, reticulocytosis, and other phenotypic features of SCA.
