Changes in muscle strength properties caused by harvesting of autogenous semitendinosus tendon for reconstruction of contralateral anterior cruciate ligament.

A prospective study was conducted of how the muscle strength of the donor knee is affected by harvesting of the autogenous semitendinosus tendon (St) for use as a substitute graft material in cruciate ligament reconstruction. There were 25 patients from whom only the St was harvested from the contralateral (i.e., healthy/donor) knee. Using a Biodex System II (Biodex, New York, NY), the strength of the donor knee was measured during both extension and flexion, both before and 12 months after the tendon harvesting procedure. A comparative study was made of the preharvest and postharvest values for the peak torque and peak torque angle in the isokinetic contraction. There were no statistically significant differences between the preharvest and postharvest peak torque values of the donor knee. However, the peak torque angle decreased significantly after the tendon harvest; the range of the mean decrease was from 11.7 degrees to 15.0 degrees. This indicates that there was a change to a small flexion angle (P < .05). After the tendon harvest, regardless of the applied angular velocity, more than 80% of the cases showed a change of torque curve shape in which there was no peak in the latter half, and the position of the peak was shifted to the left. In conclusion, the results of this study indicate that harvesting of the autogenous St does not affect the peak torque, but the peak torque angle during flexion of the donor knee is reduced.

Evaluation of platelet adhesion and activation on materials for an implantable centrifugal blood pump.

A totally implantable centrifugal artificial heart has been developed in which a pivot bearing supported centrifugal pump is used as a blood pump. The following have been adopted as blood contacting materials in our pump: titanium alloy (Ti-6A1-4V) for the housing and impeller, alumina ceramic (Al2O3) for the male pivots, and ultrahigh molecular weight polyethylene (PE) for the female pivots. Greater antithrombogenicity is required for an implantable blood pump. To examine the thrombogenicity of these materials, we evaluated in vitro platelet adhesion and activation, which may play key roles in thrombogenesis on foreign surfaces. Ti-6A1-4V, Al2O3, and PE were compared with polycarbonate (PC), silicone carbide (SiC), and pure titanium (pTi). Platelet adhesion was assessed using monoclonal antibody (CD61) directed against glycoprotein IIIa. Platelet activation was evaluated by measuring P-selectin (GMP-140) released from irreversibly activated platelets. Each material with a surface area of 16.6 cm2 was incubated with 2.5 ml of plasma or 2.5 ml of heparinized fresh whole blood for 3 h at 37 degrees C. The optical density (OD) at a wavelength of 450 nm for CD61 was 0.93+/-0.35 in PC, 0.34+/-0.13 in PE, 0.27+/-0.13 in pTi, 0.26+/-0.01 in Al2O3, 0.21+/-0.04 in SiC, and 0.12+/-0.12 in Ti-6A1-4V. The GMP-140 levels of the tested materials were not significantly different from the control value (45.9+/-7.2 ng/ml). These results indicate that Al2O3, PE, and Ti-6A1-4V, which are incorporated into our implantable centrifugal pump, have satisfactory antithrombogenic properties in terms of platelet adhesion. However, platelet activation by any material was not observed under the static condition in this study.

Provirus load in patients with human T-cell leukemia virus type 1 uveitis correlates with precedent Graves' disease and disease activities.

We previously demonstrated the increased provirus load in the peripheral blood of patients with human T-cell leukemia virus type 1 (HTLV-1) uveitis (HU). To delineate the relevance of the increased provirus load to clinical and immunologic parameters, we studied the correlation between them. Seventy-nine HU patients (24 male and 55 female) were included in the study, with their informed consent. Plasma samples and genomic DNA of the peripheral blood mononuclear cells were isolated and the provirus load was estimated by semi-quantitative polymerase chain reaction of the gag region sequence. Serum levels of anti-HTLV-1 antibodies and soluble IL-2R were determined by electrochemiluminescence immuno assay and by ELISA, respectively. Disease activities were assessed and graded 0 to 4 according to the evaluation system. Recurrence of the disease during the follow-up period was diagnosed ophthalmologically. The provirus load was significantly higher in the HU patients after Graves' disease (GD) than in those without GD (P<0.05). It correlated with disease activities assessed in terms of vitreous inflammation and interval to recurrence (both P<0.05). In the HU patients without GD, it correlated with the serum levels of soluble IL-2 receptor (P<0.01), and nearly with those of HTLV-1 antibody (P=0.063). These correlations were not found in the HU patients after GD under methimazole treatment. The results suggested a direct involvement of HTLV-1-infected cells in the pathogenesis of uveitis, and raise the possibility that hyperthyroidism may contribute to the clonal expansion of HTLV-1-infected cells.

Current therapeutic apheresis technologies for inflammatory bowel disease.

A large number of physicians have indicated that patients with inflammatory bowel diseases (IBD) such as ulcerative colitis (UC) and Crohn's disease (CD) respond to current apheresis technology treatment. However, the mechanism of the apheresis procedure is undefined for patients with IBD. IBD appears to be caused by a complex of interactions from the genes, environment, and the immune system; therefore, the immune system plays a crucial role in the inflammatory responses. In this process, lots of interactions occur simultaneously, and they cross relate with each other. This review paper briefly discusses the etiology and pathogenesis of IBD and attempts to elucidate the mechanism that occurs after apheresis treatment.

Double filtration plasmapheresis maintains normal adhesion molecule levels.

Levels of plasma soluble vascular cell adhesion molecule-1 (sVCAM-1), soluble intercellular adhesion molecule-1 (sICAM-1), and von Willebrand factor (vWF) increase in patients with peripheral vascular or ischemic heart disease. These factors are related to the progression of atherosclerosis. Furthermore, these substances and thrombomodulin (TM) are indicators for assessing the degree of damage to the endothelium. To evaluate the effect of double filtration plasmapheresis (DFPP) on these molecules, the plasma levels of vWF, sICAM-1, sVCAM-1, and TM were measured in 4 familial hypercholesterolemia (FH) patients who underwent treatment with DFPP at 2 week intervals for more than 3 years. The levels of sVCAM-1 and sICAM-1 in hypercholesterolemia patients with ischemic heart disease as a control was 773 +/- 109 and 334 +/- 82 ng/ml. These values were higher than the normal value. In the FH patients who underwent DFPP treatment, the average sICAM-1 levels were 221 +/- 47 and 197 +/- 36 ng/ml before and after, respectively. The average sVCAM-1 levels were 601 +/- 87 and 486 +/- 60 ng/ml. There were no significant differences between the pre- and post-DFPP values. The activities of plasma vWF before and after DFPP treatment were 158 +/- 23 and 45 +/- 9%. The levels of plasma TM before and after treatment were 3.0 +/- 0.3 and 3.4 +/- 0.5 FU/ml. From these results, it is suggested that DFPP treatment does not damage the endothelium and may prevent the progression of atherosclerosis by removing the substances that induce the production of sICAM-1 and sVCAM-1 due to long-term treatment.

[Management for osteoporosis in a society composed largely of elderly people--from a practical orthopaedic point of view].

Aging of Japanese population in recent years is advancing at unexpected speed. The incidence of osteoporosis and fracture increase rapidly with aging of the population. Since 1970, at opening of our department, I have expected that a society would be composed largely of elderly people. To prevent elderly people from being beg-ridden and demented, I have made efforts to prevention and treatment of osteoporosis. Purpose of this paper is to show concerning the screening of osteoporosis in this area of Hakodate city and southern part of Hokkaido, and to report our treatment for osteoporosis. There are many ways of measuring bone mineral density (BMD), and dual energy X-ray absorptiometry (DEXA) is the simplest and the most accurate. Concerning of diagnosis of osteoporosis, we investigated the change of the BMD after hip surgery. On the other hand, we also reported the effectiveness of Vitamin-K for osteoporosis. No studies have ever been tried to clarify about this treatment. We are making efforts to enlighten the people on prevention, early diagnosis and treatment of osteoporosis.

Arthroscopic studies of variants of the anterior horn of the medical meniscus.

The objective of this study was to arthroscopically analyse the morphology and dynamics of variants of the anterior horn of the medial meniscus of the knee (VAMM) and to then consider the pathological significance of these variants. VAMM was defined as knees in which the anterior horn of the medial meniscus is not attached to the tibia. Between April 1992 and March 1995, arthroscopy was performed on 953 knees of 903 patients. At the time of this examination, observation and probing were performed to determine the condition of the synovium, the synovial plica, the cartilage in all compartments, the meniscus, the cruciate ligaments, and the popliteal tendon. In particular, detailed examination was made of the anterior horn of the medial meniscus with regard to the point of insertion to the tibia and the degree of movement in knee flexion/ extension. Cases of VAMM diagnosed on the basis of the arthroscopic findings were classified into the following four categories: the ACL (anterior cruciate ligament) type, where the anterior horn of the medial meniscus was attached to the ACL; the transverse ligament type, where the anterior horn of the medial meniscus was attached to the transverse ligament; the coronary ligament type, where the anterior horn of the medial meniscus was attached to the coronary ligament; and the infrapatellar fold type, where the anterior horn of the medial meniscus was attached to the infrapatellar synovial fold. These patients were then analyzed with regard to the arthroscopic findings and the intra-articular lesions other than VAMM. In 98 (10.9%) of the total patients, 103 knees were classified as VAMM. Classification of those 103 knees using the above criteria showed 39 ACL type knees, 51 transverse ligament type knees, 11 coronary ligament type knees, and 2 infrapatellar fold type knees. The arthroscopic findings indicated that the anterior horn of the medial meniscus was not attached directly to the tibia in any of these knees. Probing and flexion/extension of the knee revealed hypermobility at the anterior horn of the medial meniscus. In this study, anterior knee pain syndrome was diagnosed in 12 (11.7%) of the 103 VAMM knees. In addition, there was no clear history of trauma in 20 of 23 knees found to have an isolated medial meniscus tear. In these cases, even detailed arthroscopic observation proved the causes of the symptoms or injury. On the basis of these findings, we surmised that the anterior portion shows hypermobility at the time of flexion/extension of the knee, regardless of the type of VAMM. In this study, we discussed the possibility that the existence of VAMM may become the cause of pain or injury to the meniscus.

Metabolism of tetramethrin isomers in rat: IV. Tissues responsible for formation of reduced and hydrated metabolites.

1. To identify the sites of formation of the reduced metabolites, 3-hydroxy-cyclohexane-1,2-dicarboximide (3-OH-HPI-1 and -2), 1,2-cyclohexanedicarboxylic acid (TCDA) and 1-hydroxy-1,2-cyclohexanedicarboxylic acid (1-OH-HPA), in rat treated with 14C-labelled (1RS, trans)-tetramethrin, [3,4,5,6-tetrahydrophthalimidomethyl (1RS, trans)-chrysanthemate], bile-duct cannulated animals were orally or intravenously administered 14C-labelled 3,4,5,6-tetrahydrophthalimide (TPI) or 3,4,5,6-tetrahydrophthalic acid (THPA), precursors of these metabolites, and bile, urine and faeces were collected for analysis. 2. 3-OH-HPI-1 and 3-OH-HPI-2, which are cis-form reduced metabolites, and 1-OH-HPA were detected in bile and urine samples of the bile-cannulated rat treated intravenously and orally with 14C-labelled TPI, indicating their formation in tissues or blood. TCDA, a trans-form reduced metabolite, was not detected in bile, urine or faeces of the bile-cannulated rat treated intravenously with 14C-THPA, but was found in the faeces after oral application, indicating formation in the gastrointestinal tract. 3. To clarify whether 1-OH-HPA is produced from THPA via TCDA (hydroxylation via reduction) or by direct addition of H2O to its double bond (hydration), rats were orally administered 14C-labelled TCDA, and metabolites in urine and faeces were analysed. The observed lack of 1-OH-HPA indicated formation by direct addition of H2O to the double-bond of THPA. 4. To specify which tissues form reduced and hydrated metabolites, in vitro metabolism studies were carried out. Reduction to the cis-form was found to take place in blood cells, reduction to the trans-form took place in the gastrointestinal tract contents, and hydration took place in the liver and the intestinal tract contents.

Mechanical white blood cell damage in rotary blood pumps.

Mechanical trauma of white blood cells (WBC) due to the operation of a rotary blood pump was examined, using a simple method of trypan blue dye exclusion test for a cell viability measurement. The degree of WBC trauma was investigated using a roller pump (RP) and 3 commercially available centrifugal pumps (Bio-Medicus [BP], Capiox [CP], Nikkiso [NK]), and compared with the red blood cell (RBC) trauma. Each pump was operated 3 times at a flow rate of 5 L/min against the total pressure head of 350 mm Hg for 6 h in a mock circuit with 400 ml of fresh bovine blood. Blood was sampled at 2 h intervals measuring plasma free hemoglobin concentration and the percentage of damaged WBC in the trypan blue dye exclusion test. Each pump demonstrated a linear increase in the degree of WBC trauma, and there were differences among the tested pumps (RP > BP > CP > NK). These findings were similar to those of the free hemoglobin measurements. To compare the degree of RBC and WBC trauma, the probability (gamma, omega) of RBC and WBC to be damaged was calculated, respectively. gamma = delta DRBC/delta N, omega = delta DWBC/delta N where DRBC and DWBC are the ratios of the damaged RBC and WBC, respectively, and N is the passing number defined as Qt/V (Q, flow rate; t, time; V, circulating volume). The data of this study demonstrated that the omega value was approximately 20 times or more greater than the gamma equally in all the tested pumps. This suggests that a WBC is more vulnerable to mechanical damage by a rotary blood pump than a RBC.

Effects of ultrathin silicone coating of porous membrane on gas transfer and hemolytic performance.

To assess the effect of an ultrathin (0.2 microm) silicone-coated microporous membrane oxygenator on gas transfer and hemolytic performance, a silicone-coated capillary membrane oxygenator (Mera HP Excelung-prime, HPO-20H-C, Senko Medical Instrument Mfg. Co., Ltd., Tokyo, Japan) was compared with a noncoated polypropylene microporous membrane oxygenator of the same model and manufacturer using an in vitro test circuit. The 2 oxygenators showed little difference in the oxygen (O2) transfer rate over a wide range of blood flow rates (1 L/min to 8 L/min). The carbon dioxide (CO2) transfer rate was almost the same in both devices at low blood flow rates, but the silicone-coated oxygenator showed a decrease of more than 20% in the CO2 transfer rate at higher blood flow rates. This loss in performance could be partly attenuated by increasing the gas/blood flow ratio from 0.5 or 1.0 to 2.0. In the hemolysis study, the silicone-coated membrane oxygenator showed a smaller increase in plasma free hemoglobin than the noncoated oxygenator. The pressure drop across both oxygenators was the same. These results suggest that the ultrathin silicone-coated porous membrane oxygenator may be a useful tool for long-term extracorporeal lung support while maintaining a sufficient gas transfer rate and causing less blood component damage.

Membrane apheresis technology: historical perspective and new trends toward bioincompatible systems.

During the past 25 years, membrane apheresis technology has been well developed through the use of biocompatible devices and immunomodulation. Now, however, we must move into a new era reconsidering the concepts of apheresis technology and considering the urgent need to develop a bioincompatible apheresis system. In the past, our aim in this field was to develop the best blood compatible system possible. With these systems, best efforts were made to reduce procedurally induced immunomodulation effects. However, it is these authors' opinion that procedurally induced immunomodulation effects should be augmented rather than reduced by incorporating such a bioincompatible apheresis system. Augmented immunoactivation and immunosuppression introduced by such systems should add therapeutic effects to the apheresis procedures. Therefore, we anticipate that the current marginally effective diseases may benefit from this strategic change in apheresis procedures.

Strategy of leukocyte filtration for immunomodulation: development of stainless steel leukocyte filter.

Commercially available leukocyte filters are frequently used to prepare leukocyte depleted blood products for prevention of transfusion reactions. Recently, immunomodulation by using leukocyte filtration was evaluated. At this time, a new leukocyte filter was fabricated with a 4 microm diameter stainless steel fiber. The goal of this study was to assess the efficiency of the stainless steel filter for leukocyte and platelet removal by comparison with the polyester filter that is commercially available. The production of humoral factors, including interleukin-1 beta, tumor necrosis factor-alpha, and interleukin-1 receptor antagonist (IL-1 Ra), was also evaluated. The results show that the stainless steel filter has more than 2 times greater efficiency in leukocyte removal than the polyester filter. Furthermore, the cytokine studies indicate good biocompatibility of the filter, and the stainless steel filter has a possibility of inhibiting inflammatory cytokines by inducing interleukin-1 receptor.

Antibacterial activity of two alkylamines integrated an indane scaffold: mimicry of a complementary unit on magainin 2.

Based on the antibacterial activity of 9-phenylnonylamine (pC9a) against Escherichia coli (ATCC29522) and Staphylococcus aureus (ATCC25923), we have further tested the inhibitory ability of the growth of the bacteria by (+/-)1-(4-aminobutyl)-6-benzylindane (PM2) and (+/-)1-benzyl-6-(4-aminobutyl) indane (PM3), that is, two kinds of 1,6-disubstituted indanes. In an in vitro assay, they showed almost the same antibacterial activities against the bacteria as pC9a, as well as that of magainin 2 analogs (i.e., the peptides MSI-78 and 87-ISM), except in the case of 87-ISM against S. aureus. At the MIC (minimum inhibitory concentration) values, however, their killing rate of E. coli is actually quicker than pC9a. This indicates that an indane scaffold, used as a template to mimic a part of the alpha-helical structure of magainin 2, can accelerate the killing rate. At present, however, it is unknown whether either the hydrophobicity or the alpha-helical structure, or both, of the indane scaffold is involved in accelerating the rate. Moreover, these two indanes also showed stronger antibacterial activity against two strains of Helicobacter pylori (ATCC43526, ATCC43579) than either pC9a or magainin 2 related peptides.

Characterization of the I-E(d)--restricted peptide recognized by an anti-idiotypic CD4+ T cell line.

We have previously reported a CD3+ CD4+ CD8- T cell line, J-2R which specifically recognized J558 individual idiotope (IdI) of anti-alpha (1-->3) dextran antibodies in an I-E(d) restricted manner. The J-2R proliferated in response to J558 IdI-derived peptides; however, the ability of the peptides to evoke the proliferation of J-2R was different. In the present study, we investigated the interaction between J558 IdI-derived peptides and I-E(d) molecules in competition experiments using a M104E IdI-derived peptide, M88-105. The M88-105 inhibited the proliferation of J-2R induced by J558 IdI-derived peptides. Furthermore, the proliferation induced by the peptides J92-109 and J96-105 was inhibited by the M88-105 at much lower inhibitor/antigenic peptide ratios, compared to the proliferation induced by the J88-105. Thus, shift of the framework to C-terminus and deletion of N-terminus amino acid residues from the 18-mer peptide J88-105 made the peptides more susceptible to the inhibition by the M88-105. Sequencing of the J-2R T cell receptor (TcR) revealed that J-2R used TcR, V alpha 1, J alpha 44; V beta 15, D beta 1, J beta 1.5. These results suggest that the peptides, J88-105, J92-109 and J96-105, directly bind to I-E(d) molecules, and that the capacity of J558 IdI-derived peptides to activate J-2R depends on the affinity to the I-E(d) molecules.

Study of the forces of stabilizing complexes between chlorophylls and heterocyclic amine mutagens.

Chlorophyllin (CHL), a water-soluble derivative of chlorophyll, forms molecular complexes with heterocyclic amine mutagens in vitro. In a previous study [Dashwood and Guo (1993): Environ Mol Mutagen, 22:164-171], we observed an inverse correlation between the binding constants of several mutagen-CHL complexes and the antimutagenic potency of CHL in the Salmonella assay. The present investigation utilized molecular mechanics methods of energy minimization and spectrophotometric titration to examine structural features of chlorophylls, chlorins, and porphyrins that might be important for complex formation with heterocyclic amines. The exocyclic amine group of the mutagen aligned consistently with acid groups in CHL, suggesting that H-bond or electrostatic interactions facilitate complex formation. Replacement of the exocyclic amine with a nitro group abrogated this specific orientation and raised the minimized energies of the complexes. No relationship was found between complex strength and the specific positions of amine or methyl groups on the mutagen. However, the presence of methyl groups increased the minimized energies and lowered the binding constants of the complexes, perhaps due to partial disruption of pi-pi interaction by steric effects. All of the compounds examined, including chlorophyll a, required the presence of pi-pi interactions to form stable complexes with the heterocyclic amines. In general, the present results were in agreement with the inhibitory potency of each compound in the Salmonella assay, and they provide further support for the hypothesis that chlorophylls in the diet might act as interceptor molecules of food-borne carcinogens and mutagens.

Histologic investigation of hollow implants retrieved for psychological reasons.

OBJECTIVE: The purpose of this study was to radiologically, histologically, and histometrically evaluate bone in the hollow portion of three implants retrieved for psychological reasons. STUDY DESIGN: Three hollow implants retrieved from two patients were studied. We investigated the radiologic and histologic changes of these implants with the use of radiographs, light microscopy, image processing, and fluorescent microscopy. RESULTS: There were no radiologic and histologic degeneration around the implants. Histometric analysis of the hollow indicated that the average percentage of bone contact rate was 33.5% in case 1, 74.5% in case 2, and 18.4% in case 3; the average percentage of bone filling was 25.1%, 33.9%, and 6.6%, respectively. There was a great variation among the three cases in bone to implant contact and bone filling. CONCLUSION: The hollow portion in case 1 that penetrated into the maxillary sinus was encapsulated with fibrous tissue. The amount of bone tissue in the hollow portion seems to depend on the initial bone quality of the recipient sites.

Bone SPET of symptomatic lumbar spondylolysis.

The aim of this study was to clarify the clinical role of bone single photon emission tomography (SPET) of the lumbar spine in young persons with persistent lumbar pain which might be due to spondylolysis. Thirty-one bone SPET studies were performed on 25 patients (19 males, 6 females) aged 7-26 years (average 15.6 years) who had suffered lumbar pain associated with physical activity, and who were suspected of having spondylolysis. Planar and SPET images of the lumbar spine were obtained 2-3 h following the injection of 99Tc(m)-methylene diphosphonate using a single-head rotating gamma camera. The findings on the bone scintigram were compared with those on the radiograph. Bone scintigraphy at presentation was positive in only 7 of 15 sites of the pars interarticularis defects demonstrated on plain radiographs. On the other hand, seven sites of the pars interarticularis which were normal on the radiograph were positive on bone SPET. As clinical symptoms improved after immobilization using a lumbar corset, bone SPET tended to revert towards normal. The planar image was abnormal in only 8 (42%) of the 19 abnormal sites on the SPET image. A bone SPET study is indicated in patients who are negative on radiological tests and who are still suspected of having spondylolysis. If SPET is positive in these patients, the increased bone uptake is most likely suggestive of a state of 'stress reaction', and may be a good indicator for patient management. If negative, further radiological examinations will be required for proper assessment of the origin of lumbar pain.
