[The effect of cepharanthin on adjuvant chemotherapy induced bone marrow suppression in patients with breast cancer].

We studied the effect of Cepharanthin on bone marrow suppression induced by adjuvant chemotherapy in primary breast cancer patients in our hospital. The 36 patients with breast cancer were divided into two groups. Both groups were administered of MMC 20 mg intravenously on the day of radical operation. One group (23 patients) was administered of Cepharanthin 60 mg every day, but the other (13 patients) group was not administered. In the leukocyte count at 3 weeks after post operation, 82.6% (19 out of 23) of the patients who were administered cepharanthin kept more than 70% of pre-operation's leukocyte. On the other hand, only 46.1% (6 out of 13) of the patients who were not administered Cepharanthin kept more than 70% of pre-operation's leukocyte. The number of thrombocyte was not decreased in both groups. Side effects by Cepharanthin were not observed in this study.

[Continuous hepatic arterial infusion chemotherapy using implantable reservoir in liver metastases from colorectal cancer].

Continuous hepatic arterial infusion chemotherapy using implantable reservoir was performed for liver metastases from colorectal cancer, and the therapeutic effects, side effects, and complications were evaluated. 9 patients with unresectable liver metastases were as follows, 1. Group A; 3 patients, MMC 2 mg.one shot + 5-FU 250 mg/day.continuous infusion x 14 days, and then 5-FU tablets 150 mg/day.p.o. x 14 days, 2. Group B; 4 patients, MMC 2 mg.one shot + 5-FU 500 mg/day.continuous infusion x 7 days, and then 5-FU tablets 150 mg/day.p.o. x 14 days, 3. Group C; 2 patients, 5-FU 500 mg/day.continuous infusion x 14 days, and then free from agents for 14 days. In 2 of 3 group A patients, the catheters became dislocated and one died of perforation of duodenum. In group A and group B, no severe side effects were noted. But both of group C patients showed nausea, vomiting and diarrhea. In 8 of 9 patients (89%), serum CEA level fell below the preoperative level. In 4 of 7 patients who underwent CT scan, the size of the tumor regressed. Total infused dose of 5-FU was 8.17 +/- 7.56 g in group A, 16.9 +/- 2.88 g in group B, and 21.0 +/- 9.90 g in group C on average. In 2 patients of group B, therapy was repeated seven times.

Nucleotide sequence and expression of a novel human receptor-type tyrosine kinase gene (flt) closely related to the fms family.

A new human gene encoding a receptor-type tyrosine kinase was isolated by a weak cross-hybridization with v-ros oncogene. A cDNA of about 7.7 kb carried a 4.2 kb open reading frame, and the predicted amino acid sequence of 1338 residues contained extracellular, transmembrane and tyrosine kinase domains. Although its extracellular domain is approximately 220 amino acids longer than those of the products of the fms family, i.e. c-fms, c-kit and platelet-derived growth factor receptor genes, the overall structure including cysteine motifs in its extracellular domain and a long peptide insertion in its tyrosine kinase domain indicates that this new gene is closely related to the fms family. Consequently, the gene was designated as flt (fms-like tyrosine kinase) gene. The expression of the flt gene was strongly suppressed in most of the tumor cell lines examined so far, whereas this mRNA was expressed in a variety of normal tissues of adult rat.

[Molecular basis of mucopolysaccaridosis type VII (beta-glucuronidase deficiency)].

We have identified several mutations causing beta-glucuronidase (beta Gl) deficiency in three cases with mucopolysaccaridosis type VII (MPS VII). Enzyme assay of lysates of these lymphocytes or cultured fibroblasts showed little residual activity and that the beta Gl-specific mRNA levels were normal, as revealed by Northern blot analysis. Mutated cDNA clones including the entire coding sequencing were isolated from a library in case 1 and PCR (polymerase chain reaction) products in case 2 and 3 derived from cultured fibroblasts. Sequencing of the full-length mutated cDNA revealed C----T transitions, an event causing a single Ala619----Val change (cases 1 and 2) and Arg382----Cys and Pro649----Leu changes (case 3). The former change is detected by loss of the cleavage site for the enzyme Fnu 4 HI in the mutated cDNA. On the basis of the loss of Fnu 4 HI restriction site, the patients (cases 1 and 2) were shown to be a homozygote with this mutation and the parents and brother in case 1 were heterozygotes. The Ala619----Val and Arg382----Cys mutations disrupt a functional domain consisting of a region of sequence highly conserved among human, rat and bacterial beta Gl's, and they lower the enzyme activity, as tested by transfection of COS cells with expression vectors harboring the mutated cDNA. However the Pro649----Leu mutation does not lower the enzyme activity.

[Percutaneous drainage of renal and perinephric abscesses].

Two patients with renal or perinephric abscess were successfully managed by percutaneous drainage under ultrasonic guidance. We recommend percutaneous drainage as the first choice of treatment for a renal or perinephric abscess instead of traditional open surgery, especially in a high risk patient.

[The therapeutic effect of norfloxacin on chronic prostatitis].

The clinical efficacy of norfloxacin (NFLX) was evaluated on 40 patients. They had subjective symptoms suggestive of prostate inflammations and more than five white blood cells (WBC)/hpf in their prostatic secretions (EPS) or VB3. Of these, gram negative rods were isolated from the EPS in 3 patients and gram positive cocci were obtained in 26 patients. The overall clinical efficacy was determined at the second week. The effectiveness rate of the subjective symptoms was 82.5%. The effectiveness rate of the WBC in the EPS was 47.4%. The effectiveness rate of the bacteria in the EPS was 64.3%. The overall clinical effectiveness rate was 77.8%. A subjective side effect was observed only in one patient who had skin eruption like urticaria. Mild liver dysfunction of blood chemistry analysis was shown only in two patients but they had had long standing chronic hepatitis. We conclude that NFLX is an effective drug for the patients with chronic prostatitis.

[Increased levels of serum soluble interleukin 2 receptor (sIL2R) in patients with chronic liver diseases].

sIL2R in the sera of patients with viral liver diseases and primary biliary cirrhosis (PBC) were quantified with a solidphase enzyme immunoassay using two monoclonal antibodies against the receptor. As IL2 upregulated sIL2R release in vitro and in vivo, the serum levels of sIL2R might be a useful marker for T cell mediated immune responses. The sIL2R in patients with acute hepatitis, chronic hepatitis (CH) and PBC were significantly higher than those in control subjects. Serum sIL2R in patients with CH and PBC was capable of binding IL2 and did not affect IL2 depended immune responses of blastoid lymphocytes. These results suggest that IL2 production and sIL2R release increase significantly in patients with CH and PBC, and IL2 depended cytotoxic T cells may play a role of pathogenesis of CH and PBC.

[Cognitive function, cerebral blood flow and brain atrophy in olivo-ponto-cerebellar atrophy].

UNLABELLED: Eighteen cases of OPCA (9 males and 9 females, age 39-70, mean 57.8 +/- 8.1 years). Mean duration of illness was 65.7 +/- 36 months. Age-matched controls for Brain-CT were 34 cases (13 males and 21 females) (age 39-70, mean 56.8 +/- 9.4 years), in which CT was examined because of functional disorders, and 50 healthy volunteers (25 males and 25 females) (age 36-72, mean 56.6 +/- 7.9 years) for regional cerebral blood flow (rCBF) and mental function. Brain Atrophy Index (BAI) and Ventricular Atrophy Index (VAI) were calculated by digitizer (PCPAD) on CT films in the slice of lateral ventricle level, basal ganglia level and cerebello-pontine level. There were 10 patients of OPCA who examined CT prior to this study. Interval of the two examinations was 21-76 months, mean 53 months. Regional cerebral blood flow (rCBF) were measured by 133 Xe inhalation methods. Intelligence were evaluated with Okabe's Intelligence Scale (Shortened and modified Wechsler's Memory Scale for Japanese Aged) and Kohs' Block Design Test. Depressive state was evaluated with Zung's Self-rating Depression Scale (SDS). RESULTS: 1) Cerebello-pontine atrophy was significantly prominent but no cerebral atrophy was observed in OPCA. 2) The rCBF was mildly but significantly reduced in OPCA. 3) Performance and verbal intelligence were significantly low in OPCA. 4) OPCA patients showed significant depressive state. 5) Grade of ADL did not show significant participation in the rCBF, intelligence and depressive state. 6) There was no significant correlation between duration of illness and rCBF or mental functions. 7) Cerebello-pontine atrophy significantly progressed in OPCA patients followed-up.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical studies of ceftibuten in the field of urology].

We studied clinical effects and safety of ceftibuten (7432-S, CETB), a new oral cephem antibiotic, against chronic complicated urinary tract infections in 9 cases and bacterial prostatitis in 10 cases. CETB was administered at a daily dose of 400-600 mg divided into twice or 3 times for a duration of 1-4 weeks. The overall clinical effect was 83.3% according to the criteria of UTI Committees (excellent: 4 cases, moderate: 1, poor: 1) in evaluated 6 cases, and the efficacy rate was 77.8% in the bacterial prostatitis cases according to physicians' evaluation (good: 7 cases, fair: 2, unknown: 1). There was 1 case of nausea, vomiting and lightly diarrhea on the third day after treatment but those tendencies all disappeared after stopping administration. So, we concluded that CETB was a useful agent for chronic complicated UTI and bacterial prostatitis with a daily dose level of 400-600 mg except in severe cases.

[A case of renal schwannoma].

A case of renal schwannoma is presented. A 51-year-old female was admitted to our hospital because of a left renal mass on April, 16, 1987. A drip infusion pyelogram showed encasement of the lower and medial calyx. An abdominal CT-scan showed a 2.5 cm mass with irregular density. Selective left renal arteriography showed the hypovascular tumor of the left kidney. Transperitoneal radical nephrectomy was performed on May, 1, 1987. Histological diagnosis was renal schwannoma arising from renal pelvis. The patient was well without recurrence 20 months after operation. Our present case is the 12th case of renal schwannoma reported in the English and Japanese literature.

Sequence analysis of operator mutants of the phase-1 flagellin-encoding gene, fliC, in Salmonella typhimurium.

In phase-2 cells of diphasic Salmonella strains, expression of the phase-1 flagellin-encoding gene, fliC, is repressed by the repressor encoded by the fljA gene. Nine operator-constitutive (Oc) mutants of fliC were isolated from S. typhimurium by selecting those which could express fliC in the presence of the repressor. Among them, eight mutants could express fliC both in the presence and the absence of the repressor, whereas the ninth one could express only in the presence of the repressor. Nucleotide sequence analysis revealed that the Oc mutations of the former type were all located between bp 7 and 20 upstream from the coding region of fliC, which suggests that this region may correspond to the operator for fliC. The latter mutant was found to have a tandem duplication of 28 bp which contains a part of the operator sequence, and seems to require the repressor to activate fliC expression.

Modification by catechol and resorcinol of upper digestive tract carcinogenesis in rats treated with methyl-N-amylnitrosamine.

Modifying effects of the environmental contaminant catechol, and its isomers resorcinol and hydroquinone, on methyl-N-amylnitrosamine (MNAN)-induced carcinogenesis were studied in male F344 rats. Groups of 15 rats were given three i.p. injections of 25 mg/kg of body weight of MNAN within the initial 2-wk period, and commencing 1 wk thereafter they were administered 0.8% catechol, 0.8% resorcinol, or 0.8% hydroquinone in powdered basal diet or were given basal diet alone for 49 wk. Additional groups of 10 to 15 rats were similarly treated without prior carcinogen exposure. Histological examination after sacrifice at wk 52 revealed that the incidences of tongue papillomas and esophageal squamous cell carcinomas in the groups given MNAN followed by catechol (57.1% and 64.3%) or resorcinol (50% and 58.8%) were significantly higher than those in the carcinogen only controls (9.1, and 0%, respectively). Hydroquinone also enhanced the development of esophageal squamous cell carcinomas but was less active than catechol or resorcinol. The incidence of alveolar hyperplasia in the lungs of the group given MNAN followed by catechol (0%) was, in contrast, significantly reduced as compared to the control value (54.5%). Hydroquinone and resorcinol showed a similar but non-significant tendency. These results indicated that the environmental contaminant, catechol and its isomers, may play a role in the development of human upper gastrointestinal cancer, in addition to exerting modifying effects in other organs.

Promotion by dihydroxybenzene derivatives of N-methyl-N'-nitro-N-nitrosoguanidine-induced F344 rat forestomach and glandular stomach carcinogenesis.

Modifying effects of resorcinol, hydroquinone, p-tert-butylcatechol (PTBC), p-methylcatechol (PMC), and o-methylcatechol on N-methyl-N'-nitro-N-nitrosoguanidine (MNNG)-induced forestomach and glandular stomach carcinogenesis were investigated in F344 rats. Groups of 15 to 16 male 6-wk-old animals were given a single intragastric administration of 150 mg/kg of body weight of MNNG and starting 1 wk later were administered powdered diet containing 0.8% resorcinol, 0.8% hydroquinone, 1.5% PTBC, 1.5% o-methylcatechol, 1.5% PMC, or basal diet alone for 51 wk. Additional groups of 10 to 15 rats each were treated with the phenolic compounds or received basal diet without prior carcinogen exposure. Histological examination after sacrifice at Wk 52 revealed that squamous cell carcinoma development in the forestomachs of rats treated with MNNG followed by PTBC (75%, P less than 0.001) or MNNG followed by PMC (100%, P less than 0.001) was significantly greater than in animals receiving MNNG alone (20%). Treatment with PMC alone also resulted in a 40% yield of papilloma. In the glandular stomach, incidences of adenomatous hyperplasias in rats treated with MNNG followed by PTBC (31.3%, P less than 0.05) or PMC (100%, P less than 0.001) and the incidence of adenocarcinomas in rats treated with MNNG followed by PMC (100%, P less than 0.001) were significantly higher than in controls. In addition, PMC alone induced a 100% yield of adenomatous hyperplasias and 6.7% of adenocarcinomas. Thus, the results demonstrated that PTBC and PMC treatment significantly enhances forestomach and glandular stomach carcinogenesis and that PMC itself may possess weak carcinogenic potential in these organs. The ortho-position appears to be important for this dihydroxybenzene activity.
