Identification of very-long-chain acyl-CoA dehydrogenase deficiency in three patients previously diagnosed with long-chain acyl-CoA dehydrogenase deficiency.

Long-chain acyl-CoA dehydrogenase (LCAD) deficiency is a disorder of fatty acid beta-oxidation. Its diagnosis has been made based on the reduced activity of palmitoyl-CoA dehydrogenation, i.e., in fibroblasts. We previously showed that in immunoblot analysis, an LCAD band of normal size and intensity was detected in fibroblasts from all LCAD-deficient patients tested. In the present study, we amplified via polymerase chain reaction and sequenced LCAD cDNA from three of these LCAD-deficient cell lines, and found perfectly normal LCAD sequences in two of them, indicating that at least these patients were not deficient in LCAD. The third patient was homozygous for an A to C substitution at 997, although it is unknown whether or not 997-C is a normal polymorphism. Although the LCAD sequence data were puzzling, a new enzyme, very-long-chain acyl-CoA dehydrogenase (VLCAD), was recently identified. Because VLCAD also has high activity with palmitoyl-CoA as substrate, it was possible that defective VLCAD may cause reduced palmitoyl-CoA dehydrogenating activity. We performed immunoblot analysis of VLCAD in six "LCAD-deficient" patients; VLCAD was negative in three of them, two of whom had a normal LCAD cDNA sequence. These results indicated that a considerable number of the patients who had previously been diagnosed as having LCAD deficiency in fact have VLCAD deficiency.

Absolute stereostructures of trifoliones A, B, C, and D, new biologically active diterpenes from the tuber of Sagittaria trifolia L.

Four new biologically active diterpenes, trifoliones A, B, C, and D, were isolated from the tuber of Sagittaria trifolia L. together with three new glycosides, sagittariosides a and b and arabinothalictoside. Their stereostructures were determined on the basis of chemical and physicochemical evidence which included the application of a modified Mosher's method and an exciton chirality method. Trifoliones A, B, C, and D exhibited inhibitory effects on the histamine release from rat mast cells induced by compound 48/80 or calcium ionophore A-23187.

Effects of MKS-492 on antigen-induced bronchoconstriction and allergic reaction in guinea pigs and rats.

Effects of R[+]-8-([1-[3,4-dimethoxyphenyl]-2-hydroxyethyl]amino) -3,7-dihydro-7-[2-methoxyethyl]-1,3-dimethyl-1H-purine-2,6-dione (MKS-492), a reported type III isozyme inhibitor of cyclic nucleotide phosphodiesterase, on antigen- or platelet activating factor (PAF)-induced bronchoconstriction and allergic reactions in guinea pigs and rats were investigated. 1) MKS-492 inhibited antigen-induced bronchoconstriction in guinea pigs. Aminophylline also inhibited the reaction. 2) MKS-492 inhibited PAF-induced bronchoconstriction and inhibited the increase in airway responsiveness to histamine in guinea pigs, although aminophylline failed to affect these reactions. 3) MKS-492 relaxed guinea pig tracheal muscle in vitro more potently than aminophylline. 4) MKS-492 inhibited leukotriene B4 (LTB4)-induced airway eosinophilia in guinea pigs. 5) MKS-492 inhibited passive cutaneous anaphylaxis and mediator-induced skin reactions in rats more potently than aminophylline. Both drugs inhibited antigen- and phospholipase A2-induced histamine release from guinea pig lung tissue. 6) MKS-492 inhibited PAF-induced O2- generation from guinea pig alveolar macrophages. These results indicate that MKS-492 is a more potent inhibitor of allergic bronchoconstriction and PAF- or LTB4-induced inflammatory reactions in guinea pigs and the allergic cutaneous reactions in rats when compared to aminophylline.

[Feelings of well-being and depression in relation to social activity in normal elderly people].

In order to clarify the influence of social activities on the feeling of well-being or depression in the elderly, we studied these relationships in 2 groups with different activities. Group I consisted of 26 subjects (mean age, 77.2 years) living in a retirement house and exposed to relatively few social stimuli. Group II consisted of 47 subjects (mean age, 75.6 years) who were living in their own homes and were confirmed to be socially active. The Morale Scale and the Zung Self-Rating Depression Scale (SDS) were used to evaluate feelings of well-being or depression. The morale scale in group I was significantly lower than in group II. In particular, there was a significant difference in the factor related to aging. The SDS score of group I was significantly higher than that of group II. The depressive state incidence was significantly higher in group I than in group II in all subjects. There was a highly significant correlation between the morale scale score and the SDS score. These results indicate that group I is less satisfied and more depressed than group II. We conclude that social environmental factors are extremely important for the quality of life of elderly people.

[The effect of long-term alkali therapy for recurrent calcium stone patients with distal renal tubular acidosis].

Alkali therapy is widely accepted as a treatment for recurrent calcium stone patients with distal renal tubular acidosis (dRTA). Nine patients, five with complete and four with incomplete type of dRTA, were treated with alkali agents for more than three years; an average period of 10.8 years. In five patients, new stone formation and stone growth were completely prevented by this treatment. Among the four failed cases, three did not take enough alkali agents (in spite of our medication), and the other had recurrent urinary tract infection resulting in infectious stone formation. In conclusion, the long-term efficacy of alkali therapy for prevention of recurrent stone formation with dRTA is confirmed when the patient takes enough alkali agents and the urinary tract infection is well controlled.

[A case of type II cryoglobulinemia involving glomerulopathy associated with hepatitis C antibody].

We reported a case of type II cryoglobulinemia involving glomerulopathy associated with HCV-induced liver cirrhosis. The patient was a 57-year-old woman. Her past history included chronic hepatitis at 51 years and rheumatoid arthritis at 53 years of age. At 46 years, an erythematous lesion appeared on her legs, which was diagnosed as allergic vasculitis by skin biopsy. At 50 years, proteinuria, hematuria and hypertension were recognized. The next year, the first renal biopsy was performed and showed membranoproliferative glomerulonephritis (MPGN). Recently, the edema of her legs has progressed, and the laboratory data showed proteinuria, hematuria, hypocomplementemia, rheumatoid factor positivity, and increase of monoclonal IgG kappa chain. The second renal biopsy revealed an endocapillary proliferative glomerulonephritis-like lesion with marked infiltration of monocytes and macrophages. The subendothelial deposit showed a fine fibril-like pattern. She was treated with steroids and double filtration plasmapheresis (DFPP) therapy, but the treatment was not very effective. She died of liver cirrhosis, which was probably induced by hepatitis C virus (HCV), and sepsis. Generally, the patients of type II cryoglobulinemia often showed HCV antibody positivity, pointing to HCV as an etiological factor. In this case, renal biopsy was performed twice in the same patient, and the histologic findings suggest the clinicopathological course of cryoglobulinemia.

[Lymphocyte subsets and interleukin-2 receptor in breast cancer patients].

The lymphocyte subsets and IL-2R (p55/Tac) in breast cancer patients were analyzed using monoclonal antibody and flow cytometry until 6 post-operative month. Results were as follows: 1. Lymphocyte fraction of CD3, 4, 8 was maintained from pre-operative to post-operative 6 month. 2. IL-2R value was lower in recurrence group than in breast benign disease group and it was lower. In pre-operative IL-2R in Stage III group than in Stage I and, Stage II. It was lower in DNA aneuploidy group than in DNA diploidy group. 3. At the post-operative 6 month, IL-2R was higher in Stage III group, ly-factor positive group, v-factor positive group and DNA aneuploidy group than in recurrence group. When breast cancer recurred after resection, IL-2R value returned to initial value. In conclusion, IL-2R may be possibly one of the factors of recurrence of breast cancer.

[Relationship between the Ki-67 nuclear antigen content and cell-cycle perturbation on WiDr cells treated with 5-FU].

The profile of Ki-67 nuclear antigen content according to the change in the cell-cycle of WiDr cells affected by 5-fluorouracil (5-FU) was studied by flow cytometry. The cases who received 10 micrograms/ml for 1 hr and showed reproliferation in the growth curve presented significant accumulation at G2M phases. However, significant increase of Ki-67 antigen content was not observed in G2M phases. Meanwhile, the cases who continuously received 10 micrograms/ml for 72 hr and showed the suppression of cellular proliferation without reproliferation presented gradual accumulation into S phase and Ki-67 antigen content was found to be significantly increased. This increase occurred in G1 phase, and the cells with Ki-67 antigen content of the largest increase in G1 phase shifted to S phase. It indicates a possibility that Ki-67 nuclear antigen content is destined to be controlled specifically in G1 phase of the cell-cycle. 5-FU seems to damage this regulation system and, as a result, increase Ki-67 nuclear antigen.

Photopic electroretinogram implicit time in retinitis pigmentosa.

While the implicit time of cone responses in the electroretinogram (ERG) has been suggested to have some clinical implications in retinitis pigmentosa (RP), its evaluation is not always easy in cases of typical RP because of the small amplitudes of the responses. An intense flash stimulus would be expected to improve the recordability of photopic ERG, which represents the cone responses. However, the implicit time in normal eyes has been shown to be delayed with increased stimulus intensity. Therefore, we evaluated photopic ERGs to a white flash stimulus of 3 different intensities as well as to a red flash in 14 cases of typical RP. The recordability of the response to white flash improved with the increased intensity of the stimulus. Most RP cases showed prolonged implicit time compared with that in normal eyes to each flash stimulus. Some patients who showed exceptionally normal or mildly delayed implicit time were considered to have good prognosis since they had noticed night blindness at an older age and most of their rod response in ERG was recordable.

[A case of sleep apnea syndrome with significant alteration of apnea index by sleep position].

An obese 37-year-old man (130% ideal body weight) was admitted to our hospital with the complaint of excessive daytime sleepiness. During all-night polysomnography, he showed predominantly obstructive sleep apnea and an apnea index (AI) of 57.5, and was diagnosed as having obstructive sleep apnea syndrome (OSAS). However, AI values calculated separately for the time in the supine position (AI-S) and in the lateral decubitus position (AI-L) were 82.4 and 5.9, respectively. Moreover, duration of apnea, lowest SaO2, and the quality of sleep were improved when sleeping in the lateral compared to the supine position. After weight reduction of 7 kg, AI in total decreased to 33.2, although AI-S was 77.3 and AI-L was 3.8. The improvement of AI in total to be due to a relative increase during sleep in the lateral position. These results suggest that in some OSAS patients, the sleep position should be taken into account in assessing the severity of the disease or in evaluating the effects of therapy. This study also suggests the efficacy of sleep position adjustment in the treatment of OSAS.

Effect of nicotine on cognitive disturbance in animals with acetylcholine deficient brain.

The effect of nicotine on learning disturbance was tested in rats fed with a choline-deficient diet (Ch free) or in senescence-accelerated mice (SAM). Concentration of acetylcholine in the brain was significantly lower in rats fed with Ch free and SAM. Passive avoidance learning shows that rats fed with Ch free and SAM showed significantly impaired learning, compared to controls. Nicotine (0.04 mg/kg) administered intraperitonealy significantly improved learning in animals with learning disturbances, as well as in controls. Vitamin B12 facilitated acetylcholine synthesis or release in the brain and improved the cognitive disturbance. We therefore suggest that acetylcholine contributes to learning and that nicotine may improve learning in an acetylcholine deprived rats.

[Molecular biology of alpha-adrenergic receptor and essential hypertension].

The primary physiological agents for adrenergic receptors are only two, epinephrine and norepinephrine, which have been used to differentiate alpha- and beta-adrenoceptors. The pharmacological properties can distinguish subtype alpha 1-, alpha 2-, beta 1- and beta 2-receptors. Recently additional subtypes have been characterized by radioligand binding techniques and molecular biological techniques. Molecular mechanisms of regulation as well as regulatory site of gene expression of alpha-adrenergic receptors has been extensively studied. In addition, the association analysis of alpha 2-adrenergic receptor gene RFLP in essential hypertension has been performed. Eventhough we could not find any association of an alpha 2 C10-Bsu36I RFLP with essential hypertension in Japan, the finding showed significant ethnic RFLP difference at the gene locus for alpha 2 C10. The association of alpha 2 adrenergic receptor RFLP with blood pressure has been also studied using F2 generations of SHR and WKY and Dahl salt sensitive rats and resistant rats. Further studies are now in progress to clarify the role of these alpha-adrenergic receptors in cardiovascular diseases.

[Mediastinal diffuse large-cell lymphoma with sclerosis diagnosed by needle biopsy].

A 28-year-old woman was admitted to our hospital with cough. CT scan indicated a huge mass in the anterior mediastinum. A transcutaneous needle biopsy showed tumor cell infiltration and compartmentalizing fibrosis. The tumor cells were positive for CD20 and CD45, and JH rearrangement was evident. The patient was diagnosed as mediastinal diffuse large-cell lymphoma with sclerosis. The tumor responded to combination chemotherapy. It should be emphasized that mediastinal diffuse large-cell lymphoma can be diagnosed using a small specimen obtained by transcutaneous needle biopsy.
