Microvascular decompression for trigeminal neuralgia.

Of 42 patients undergoing a microvascular decompression for trigeminal neuralgia with a follow-up of 4 to 7 years, 32 (76.19%) had no postoperative recurrence of neuralgic pain, 4 (9.52%) had a minor recurrence and 6 (14.29%) had a major recurrence. It was considered that the neuralgic recurrence might be due to some possibly missed pathogenic vessel during operation or a new compression of the trigeminal nerve root occurring after operation.

[Relation between the anterior ethmoid nerve and perennial rhinitis].

The distribution of the autonomic nerves in nasal mucosa was studied by anatomical and histochemical techniques. Cholinergic nerve fibers had been found in the anterior ethmoid nerve. It has been established that the cholinergic nerve fibers in the nasal mucosa mainly came from vidian nerve and anterior ethmoid nerve, the latter distributed in the sensitive region with high density serous glands. The cause of failure of vidian neurectomy in the treatment of perennial rhinitis has been discussed and it is suggested that vidian neurectomy and anterior ethmoid neurectomy should be performed concomitantly in treating perennial rhinitis.

[Hemodynamic effects of the Bezold-Jarisch reflex in rabbit].

The Bezold-Jarisch reflex induced by intracoronary injection of nicotine (ICIN) of 5 micrograms/kg was studied in 40 anesthetized rabbits. Heart rate (HR), arterial blood pressure (ABP), left ventricular pressure (IVP), left ventricular dp/dt (LV dp/dt) and cardiac output (CO) were monitored and total peripheral resistance (TPR) was calculated. HR, ABP, LVP and LV dp/dt were decreased following ICIN. These inhibitory responses induced by ICIN were enhanced by sino-aortic denervation but abolished by bilateral vagotomy. Decreases in CO and TPR were also observed in response to ICIN. With the heart paced artificially to prevent bradycardia, the decreases in CO, TPR and ABP were still significant. Bilateral stellate ganglion section plus atropinization which abolished the decreases in HR and CO, did not significantly change the hypotensive response to ICIN. These data suggest that the hypotension evoked by Bezold-Jarisch reflex in rabbit is largely due to the decreases in TPR and partially caused by the decrease in CO.

[Change of sympathetic efferent discharges in different regions during the Bezold-Jarisch reflex in rabbit].

In 46 anesthetized rabbits, the sympathetic efferent discharges in different vascular regions were recorded during the Bezold-Jarisch reflex induced by intracoronary injection of nicotine. The discharges of renal nerve, cardiac nerve, splenic nerve, ramus of stellate ganglion to cervical plexus and ramus of superior cervical ganglion to external carotid artery decreased significantly following injection of nicotine. The decrease in efferent discharge is more prominent in renal, cardiac and splenic nerves than in the other two nerves. These results demonstrated that the reduction in discharge of sympathetic efferent fibers of different regions and hence decrease in total peripheral resistance might play a significant role in initiating hypotension during the Bezold-Jarisch reflex.

Role of acetylcholine, corticoids and opioids in the rostral ventrolateral medulla in stress-induced hypertensive rats.

Hypertension was induced in Sprague-Dawley and Wistar rats by irregular foot shocks combined with a buzzing noise for 2 h twice a day for 1-2 weeks. The plasma catecholamine, corticosterone, angiotensin II, glucose and lipids were found to increase in parallel. The acetylcholine (ACh) and choline acetyltransferase in rostral ventrolateral medulla (rVLM) increased markedly). Microinjection of ACh or cholinergic agonists into rVLM induced a pressor effect, and microinjection of M receptor blockers had a depressor effect. Electrophysiological studies showed that the stress-induced hypertension was closely related to the activation of a cholinergic system in rVLM. Microinjection of corticoids into rVLM had led to a pressor response which could be blocked by Ru38486, spironolactone, cholinergic blockers or verapamil. Microinjection of morphine and mu- or delta-receptor agonists into rVLM caused bradycardia and a reduction of arterial pressure that could be blocked by naloxone.

[Incidence of hydatidiform mole at the Paulista Medical School].

Between January 1980 and December 1989, at Escola Paulista de Medicina, a referral centre, 75 patients with hydatiform mole were assisted. The frequency of the hydatiform mole was calculated in 65 patients, whose treatment was performed in our Hospital, out of 13,986 pregnancies, which had occurred in this period of study. Thus, the incidence was 1:215 pregnancies, similar to that found in Asian and African countries.
