FDG-PET on irradiated brain tumor: ten years' summary.

PURPOSE: To evaluate FDG-PET in post-radiotherapy differentiation of tumor recurrence/malignant degeneration and radiation reaction, and to assess the role of PET in terms of survival. MATERIAL AND METHODS: 117 consecutive patients with a total of 156 FDG-PET examinations with positive but non-diagnostic MRI and/or CT were included. Final diagnosis was based on histopathology or correlated with radiologic and clinical follow-up. Brain metastases from lung carcinomas were further studied separately. Survival time was analysed using the Kaplan-Meier method. RESULTS: There were 61 true-positive, 2 false-positive, 15 false-negative, and 51 true-negative PET examinations; 5 positive and 22 negative PET examinations were indeterminate. The positive predictive value of a PET examination was 96% in all and 100% in brain metastases from lung carcinoma. The negative predictive value based on the histopathologic results was 55.6%. Survival time was significantly longer in patients with negative PET. CONCLUSION: FDG-PET is a valuable tool in the detection of tumor recurrence, especially lung carcinoma metastasis. FDG uptake is a prognostic marker.

In vitro estrogenic activities of Chinese medicinal plants traditionally used for the management of menopausal symptoms.

The estrogenic activity of 70% EtOH extracts of 32 traditional Chinese medicinal plants, selected according to their reported efficacy for the treatment of menopausal symptoms, was assessed using a recombinant yeast system with both a human estrogen receptor expression plasmid and a reporter plasmid. Among them, 11 (34%) species proved to be active. Polygonum cuspidatum had the highest estrogenic relative potency (RP) (3.28 x 10(-3)), followed by Rheumpalmatum (3.85 x 10(-4)), Cassia obtusifolia (3.49 x 10(-4)), Polygonum multiflorum (2.87 x 10(-4)), Epimedium brevicornum (2.30 x 10(-4)), Psoralea corylifolia (1.90 x 10(-4)), Cynomorium songaricum (1.78 x 10(-4)), Belamcanda chinensis (1.26 x 10(-4)), Scutellaria baicalensis (8.77 x 10(-5)), Astragalus membranaceus (8.47 x 10(-5)) and Pueraria lobata (6.17 x 10(-5)). The EC(50) value of 17beta-estradiol used as the positive control was 0.205+/-0.025 ng/ml (RP=100). This study gave support to the reported efficacy of Chinese medicines used for hormone replacement therapy.

Rapid Identification of pathogenic rapidly growing mycobacteria by PCR-restriction endonuclease analysis.

INTRODUCTION: The accuracy and practicality of PCR-restriction endonuclease analysis (PRA) for rapid identification of pathogenic rapidly growing mycobacteria (RGM) isolates were evaluated. MATERIALS AND METHOD: PRA identification using an amplified 439-bp segment (amplicon) of the 65-kDa heat shock protein gene was compared to identification by conventional methods, for 39 clinically significant RGM isolates. RESULTS: The accuracy of PRA in the identification of RGM isolates was comparable to that of conventional methods. Moreover, PRA was able to identify RGM faster, within 2 to 3 working days compared to conventional methods which require 2 to 4 weeks to perform and complete different tests. CONCLUSION: PRA methodology could be easily incorporated into the clinical laboratory setting. This would be beneficial for the management of patients with infections due to pathogenic RGM.

Use of ligase chain reaction in early diagnosis of tuberculous meningitis.

INTRODUCTION: Nucleic acid-based amplification tests are currently licensed only for the detection of Mycobacterium tuberculosis in pulmonary specimens. There are insufficient data for extrapulmonary specimens. The aim of this study is to investigate the diagnostic value of these investigations in tuberculous meningitis. MATERIALS AND METHODS: We performed a prospective study using a commercial ligase chain reaction DNA amplification technique [Ligase chain result (LCx) M. tuberculosis; Abbott Laboratories, Abbott Park, IL, USA] on cerebrospinal fluid (CSF) to diagnose tuberculous meningitis and compared the results with standard microbiological data. Conflicting cases were resolved according to the final clinical diagnosis. A total of 54 CSF specimens from 54 patients were tested. RESULTS: Six (11.1%) specimens were culture-positive for M. tuberculosis; of these, only 1 (1.9%) was smear-positive. The sensitivity, specificity, positive predictive value and negative predictive value of the LCx assay, compared with culture results, were 66.7%, 100%, 100% and 96%, respectively. After resolution according to clinical data, the sensitivity, specificity, positive predictive value and negative predictive value were 33.3%, 100%, 100% and 75%, respectively. All controls had negative LCx results. There were no false positives. CONCLUSION: LCx assay is highly specific and complements conventional laboratory diagnostic methods (CSF smear and culture) in the diagnosis of tuberculous meningitis. In the appropriate clinical context, a positive result is strongly suggestive of tuberculosis and could enable antituberculous treatment to be started immediately.

The immune regulatory protein B7-H3 promotes osteoblast differentiation and bone mineralization.

B7-H3, a member of the B7 family of the Ig superfamily proteins, is expressed on the surface of the antigen-presenting cells and down-regulates T cell functions by engaging an unknown counterreceptor on T cells. Although B7-H3 is ubiquitously expressed, its potential nonimmune functions have not been addressed. We found that B7-H3 is highly expressed in developing bones during embryogenesis and that its expression increases as osteoblast precursor cells differentiate into mature osteoblasts. In vitro bone formation by osteoblastic cells was inhibited when B7-H3 function was interrupted by the soluble recombinant protein B7-H3-Fc. Analysis of calvarial cells derived from neonatal B7-H3 knockout (KO) mice revealed normal numbers of osteoblast precursor cells possessing a normal proliferative capacity. However, the B7-H3-deficient calvarial cells exhibited impaired osteogenic differentiation, resulting in decreased mineralized bone formation in vitro. These results suggest that B7-H3 is required for the later phase of osteoblast differentiation. Although B7-H3 KO mice had no gross skeletal abnormalities, they displayed a lower bone mineral density in cortical (but not trabecular) bones compared with WT controls. Consistent with the reduced bone mineral density, the femurs of B7-H3 KO mice were more susceptible to bone fracture compared with those of WT mice. Taken together, these results indicate that B7-H3 and its unknown counterreceptor play a positive regulatory role in bone formation. In addition, our findings identified B7-H3 as another molecule that has a dual role in the bone-immune interface.

Preliminary study on rapid identification of Mycobacterium tuberculosis complex isolates by the BD ProbeTec ET system.

The BD ProbeTec ET system for identification of Mycobacterium tuberculosis complex (MTBC) isolates from BACTEC 12B culture vials was evaluated in comparison with BACTEC NAP (p-nitro-alpha-acetylamino-beta-hydroxy-propiophenone) differentiation. Of 145 mycobacterial isolates tested, comprising 89 MTBC and 56 non-tuberculous mycobacteria (NTM), BD ProbeTec ET correctly identified 87 MTBC and 56 NTM but missed two MTBC. Three NTM were misidentified when NAP was incubated at 37 degrees C only. Overall sensitivity, specificity and positive and negative predictive values were respectively 97.8, 100, 100 and 96.6 % for the BD ProbeTec ET system and 100, 94.6, 96.7 and 94.6 % for NAP.

Direct identification of Mycobacterium haemophilum in skin lesions of immunocompromised patients by PCR-restriction endonuclease analysis.

PCR-restriction endonuclease analysis (PRA) was used for direct identification of Mycobacterium haemophilum in clinical specimens from immunocompromised patients. PRA correctly identified M. haemophilum in four smear-positive specimens. Direct identification by PRA takes 2 to 3 working days compared to the 3 to 5 weeks required for culture isolation and identification by conventional methods.

Chondrocyte interactions with porous titanium alloy and calcium polyphosphate substrates.

Chondrocytes maintain their phenotype and form cartilagenous tissue when cultured on calcium polyphosphate (CPP) or titanium alloy (Ti alloy), porous three-dimensional materials. To understand how these materials may influence chondrocyte phenotype and matrix synthesis, the early interactions of cultured cells with CPP and titanium alloy were examined. These were compared to chondrocytes grown in monolayer culture on tissue culture polystyrene, conditions in which cultured chondrocytes dedifferentiate and do not form cartilagenous tissue. Scanning electron microscopy of cells up to 72 h in culture showed that bovine chondrocytes on CPP, Ti alloy, and polystyrene were an admixture of round and spread cells. The spread cells on CPP and titanium alloy were not entirely flattened but maintained a polygonal shape. In contrast, spread chondrocytes in monolayer culture were flatter and significantly larger, a difference that was maintained even in the absence of serum. All cells cultured on CPP and Ti alloy exhibited subcortical ring-like distribution of actin filaments whereas the flattened cells on polystyrene showed actin filaments distributed throughout the cytoplasm. Cells on CPP and Ti alloy synthesized significantly less collagen and proteoglycans than cells cultured on polystyrene at 72 h of culture. In summary the cells on the porous three-dimensional materials differed from those on polystyrene in terms of cell morphology and size, actin cytoskeleton organization, and synthesis of selected matrix macromolecules. The data suggests that CPP and titanium alloy may mediate their effect by limiting cell spreading in part by favoring the maintenance of a ring-like actin distribution.

Early identification of Mycobacterium tuberculosis complex in BACTEC cultures by ligase chain reaction.

A total of 1431 acid-fast bacilli (AFB) in Bactec culture vials from 1427 patients was differentiated by the Bactec NAP method and tested by the LCx Mycobacterium tuberculosis ligase chain reaction system. In all, 1321 of 1325 M. tuberculosis complex (MTBC) isolates were correctly detected by the LCx assay. All the 106 non-tuberculous mycobacteria (NTM) isolates were negative by the LCx assay. No false MTBC-positive result was obtained from NTM isolates. However, the LCx assay failed to detect four MTBC isolates from one patient. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were 99.7, 100%, 100% and 96.4%, respectively. These data suggest that the LCx system can be used to identify MTBC in AFB-positive Bactec broth cultures when the growth index is > or = 100. The method gives a 100% PPV and allows a faster turnaround time for MTBC than the NAP test.

Nanoscale manipulation of pyrochlore: new nanocomposite ionic conductors.

The ionic conductivity of isometric pyrochlore, ideally A2B2O (7), is extremely sensitive to disordering of A- and B-site cations and oxygen anion vacancies. We report the first use of ion beam irradiation-induced disordering in Gd 2Ti 2O (7) to produce a strain-free, buried, disordered defect-fluorite layer approximately 12 nm thick within an ordered pyrochlore matrix. This approach provides a new means of creating nanoscale, mixed ionic-electronic conductors in pyrochlore ceramics, such as those required for solid-state electrochemical cells.

Crystal structure of thrombin-ecotin reveals conformational changes and extended interactions.

The protease inhibitor ecotin fails to inhibit thrombin despite its broad specificity against serine proteases. A point mutation (M84R) in ecotin results in a 1.5 nM affinity for thrombin, 10(4) times stronger than that of wild-type ecotin. The crystal structure of bovine thrombin is determined in complex with ecotin M84R mutant at 2.5 A resolution. Surface loops surrounding the active site cleft of thrombin have undergone significant structural changes to permit inhibitor binding. Particularly, the insertion loops at residues 60 and 148 in thrombin, which likely mediate the interactions with macromolecules, are displaced when the complex forms. Thrombin and ecotin M84R interact in two distinct surfaces. The loop at residue 99 and the C-terminus of thrombin contact ecotin through mixed polar and nonpolar interactions. The active site of thrombin is filled with eight consecutive amino acids of ecotin and demonstrates thrombin's preference for specific features that are compatible with the thrombin cleavage site: negatively charged-Pro-Val-X-Pro-Arg-hydrophobic-positively charged (P1 Arg is in bold letters). The preference for a Val at P4 is clearly defined. The insertion at residue 60 may further affect substrate binding by moving its adjacent loops that are part of the substrate recognition sites.

The role of ecotin dimerization in protease inhibition.

Ecotin is a homodimeric protein from Escherichia coli that inhibits many serine proteases of the chymotrypsin fold, often with little effect from the character or extent of enzyme substrate specificity. This pan-specificity of inhibition is believed to derive from formation of a heterotetrameric complex with target proteases involving three types of interface: the dimerization interface, a primary substrate-like interaction, and a smaller secondary interaction between the partner ecotin subunit and the protease. A monomeric ecotin variant (mEcotin) and a single-chain ecotin dimer (scEcotin) were constructed to study the effect of a network of protein interactions on binding affinity and the role of dimerization in broad inhibitor specificity. mEcotin was produced by inserting a beta-turn into the C-terminal arm, which normally exchanges with the other subunit. While the dimerization constant (K(dim)) of wild-type (WT) ecotin was found to be picomolar by subunit exchange experiments using FRET and by association kinetics, mEcotin was monomeric up to 1 mM as judged by gel filtration and analytical centrifugation. A crystal structure of uncomplexed mEcotin to 2.0 A resolution verifies the design, showing a monomeric protein in which the C-terminal arm folds back onto itself to form a beta-barrel structure nearly identical to its dimeric counterpart. The kinetic rate constants and equilibrium dissociation constants for monomeric and dimeric ecotin variants were determined with both trypsin and chymotrypsin. The effect of the secondary binding site on affinity was found to vary inversely with the strength of the interaction at the primary site. This compensatory effect yields a nonadditivity of up to 5 kcal/mol and can be explained in terms of the optimization of binding orientation. Such a mechanism of adaptability allows femtomolar affinities for two proteases with very different specificities.

Precursor of a novel scorpion venom peptide (BmKn1) with no disulfide bridge from Buthus martensii Karsch.

A full-length cDNA sequence encoding the precursor of a novel venom peptide (named BmKn1) with no disulfide bridge was first isolated from the venom gland cDNA library of Chinese scorpion Buthus martensii Karsch. The encoded precursor consisted of 70 amino acid residues including two parts: a signal peptide of 23 residues, and a putative mature venom peptide (BmKn1) of 47 residues. The sequence of BmKn1 showed no similarity to those of other scorpion venom peptides. BmKn1 may be the first member of a new venom peptide family from scorpion. Future research will be interesting to unravel further the pharmacological function of this novel scorpion venom peptide.

Recurrence of nephrotic syndrome after transplantation in CNF is due to autoantibodies to nephrin.

The novel gene NPHS1 is defective in the patients with congenital nephrotic syndrome of the Finnish type (CNF) leading to abnormal expression of the respective protein product nephrin in glomerular cells. CNF patients are treated with early nephrectomy and renal transplantation, but about 20% show recurrence of nephrotic syndrome (NS). We used indirect immunofluorescence microscopy and immunoblotting and an ELISA assay to search for circulating autoantibodies to nephrin, the protein defect in CNF patient kidneys. In serial serum samples gathered before and after recurrence of NS, we show an increased antibody titer to nephrin prior to the NS episode and a subsequent drop in antibody level after its successful treatment and reactivity of the high titer sera with glomeruli in indirect immunofluorescence microscopy as well. The results show that the transplantation treatment introduces a neoantigen inducing production of autoantibodies, which may be pathogenic for perturbation of the function of the glomerular filtration barrier.

Influence of fuel quality on vehicular NOx emissions.

The quality of gasoline and diesel fuel affects pollutant emissions from vehicles. By applying the COMPLEX model, developed by the US EPA and industry to relate fuel composition to vehicle emissions, this paper estimates the influence of improvements in gasoline quality to lower vehicular NOx emissions. A case study is performed for Guangzhou City that has NOx concentrations significantly above the national ambient air quality standards(NAAQS). The paper discusses the potential for reducing NOx in Guangzhou by improving the quality of gasoline.

Model calculating annual mean atmospheric dispersion factor for coastal site of nuclear power plant.

This paper describes an atmospheric dispersion field experiment performed on the coastal site of nuclear power plant in the east part of China during 1995 to 1996. The three-dimension joint frequency are obtained by hourly observation of wind and temperature on a 100 m high tower; the frequency of the "event day of land and sea breezes" are given by observation of surface wind and land and sea breezes; the diffusion parameters are got from measurements of turbulent and wind tunnel simulation test. A new model calculating the annual mean atmospheric dispersion factor for coastal site of nuclear power plant is developed and established. This model considers not only the effect from mixing release and mixed layer but also the effect from the internal boundary layer and variation of diffusion parameters due to the distance from coast. The comparison between results obtained by the new model and current model shows that the ratio of annual mean atmospheric dispersion factor gained by the new model and the current one is about 2.0.

[Determination of naoning pian by multi-wavelength linear regression method].

Assay of naoning pian was reported by multi-wavelength linear regression method in this paper. The program was edited by BASIC. The recoveries and RSD of pyramidon and caffeine were 98.03%-100.9%, 1.0% and 97.77%-99.39%, 0.61%, respectively. This method could be used for the determination of two components in naoning pian without separation. The method was simple, rapid, and results were satisfactory.
