Identification of the metabolites of roxithromycin in humans.

The semisynthetic antibiotic roxithromycin (RXM) exists in an (E)-configuration. Metabolites of RXM in the bile of four cholecystectomy patients with T-tube drainage and in the urine and plasma of four healthy volunteers after single oral doses of 150 mg of RXM were investigated. A total of 15 metabolites were found in bile, urine, and plasma by HPLC with ion trap mass spectrometric and electrochemical detection. These metabolites were identified as descladinose derivative of RXM (M1), erythromycin-oxime (M2), N-, O-, and N,O-di-demethylated derivatives of RXM (M3, M4, and M6), and N-mono- and N-di-demethylated derivatives of erythromycin-oxime (M5 and M7), as well as the (Z)-isomers (M8-M15) of RXM and metabolites M1 to M7, respectively. Structures of six major metabolites (M1-M4, M8, and M10) were established by chromatographic and mass spectrometric determination and comparison with synthesized standards. The stability of RXM and the six synthesized substances was investigated to exclude artifact products. These results, together with previous findings, suggest that biotransformation pathways elucidated for RXM include: 1) isomerization of RXM derivatives, from E-isomer to Z-isomer; 2) O-demethylation; 3) N-demethylation; 4) hydrolysis of the cladinose moiety; and 5) dealkylation of the oxime ether side chain. Secondary metabolism via these pathways was also evidenced. The O-demethylation and isomerization of RXM derivatives represent two novel biotransformation pathways recovered for RXM.

Medical abortion in China.

This paper reviews the literature on early medical abortion in China. Mifepristone was approved for termination of pregnancy up to 49 days' gestation in China in 1986. The commonly accepted regimens are 150 mg mifepristone administered in several small doses or a single 200 mg dose followed by 0.6 mg oral misoprostol. Complete abortion rates are 90% to 97%. Ultrasound is strongly recommended to rule out ectopic pregnancy. Patients remain in the clinic for four to six hours after misoprostol. If the fetal sac is not expelled during the four-hour observation, the second dose of 0.4 mg misoprostol is given in many of the clinics. Rates of emergency curettage ranged from less than 1% to 4%, and rates of blood transfusion were less than 1%. The major complication of concern is undiagnosed ectopic pregnancy. The Chinese experience has shown that medical abortion is well accepted as an alternative to surgical intervention when provided under medical supervision and administrative monitoring.

[Theories of pulse wave in arteries].

This paper introduces the importance of studying pulse wave propagation and neviews the development process of the theories of pulse wave propagation. The main research achievements of nonlinear theories of pulse wave propagation are described, and the developing directions of studying pulse wave propagation are briefly stated.

Treatment of incipient diabetic nephropathy by clearing away the stomach-heat, purging the heart fire, strengthening the spleen and tonifying the kidney.

It is the view of the authors that accumulation of heat and dampness in the heart and stomach and deficiency of both spleen and kidney with blood stasis would be the main predisposing factors in pathogenesis of incipient diabetic nephropathy. A controlled clinical study of 32 patients treated with modified recipe of Wang Kentang's Bai Fu Ling Wan, a proved formula of clearing away the stomach-heat, purging the heart fire, strengthening the stomach and tonifying the kidney, and simultaneously resolving blood stasis and turbid qi was performed. The total effective rate was 93.8%, superior to that in the control group treated with Capoton (59.9%). A marked improvement in symptoms, control of blood glucose level and reduction of urinary protein obtained in the treatment group suggests that this treatment is effective in retarding the development of the disease.

[Retrovirus vector expresses luciferase and TPO gene under the control of doxycycline in NIH3T3 cells].

OBJECTIVE: To use the retrovirus vector Tet-On regulation system to control the expression of TPO and luciferase gene by doxycycline (Dox). METHODS: Packing the recombinant Tet-On retrovirus, and infecting it to NIH3T3 cells to establish recombined Tet-On retrovirus cell lines, RevTet-On3T3. To construct the pRevTRETPO, the new cell line was transferred to PT67 cell to package the recombinant retrovirus which contained both the TRE and TPO. This virus was RevTet-On3T3 used to establish the two inserting recombinant retrovirus cell strain RevTet-On3T3TPO. Doxycycline was used to control the expression of TPO in RevTet-On3T3. RESULTS: When Dox (5 mg/L) was added to the RevTet-On3T3 cells, a higher activity of luciferase was seen (mean, 5.9 x 10(4) RLU.S) compared with the mean activity (2.6 x 10(3) RLU.S) in the absence of Dox. When Dox was added to the RevTet-On3T3TPO cells, cell populations had high TPO expression (mean, 12.8 ng/ml) in 5 mg/L of Dox (On state), and a low TPO expression (mean, 0.56 ng/ml) in the absence of Dox (Off state). CONCLUSION: The RevTet-On gene expression system could modulate the expression of multiple genes by tetracycline and its derivatives, providing a useful tool for gene therapy.

[Reconstructing the limb end defects with a free mini-flap, a report of 18 cases].

OBJECTIVE: Some operative procedures were introduced for repairing hand or foot defects using the free mini-flap from the finger or toe. METHODS: Eighteen cases of finger tip or foot wound were treated with the free mini-flap and vessel anastomosis. The size of the flap was 2 cm x 1 cm to 6 cm x 4 cm and the length of the pedicle was 2 to 4 cm. RESULTS: All the eighteen free mini-flaps survived and the results were satisfactory. CONCLUSION: The method of the mini-flap is a good option for covering of thumb, finger or foot defects when a local or an island flap is not available.

Cardiovascular actions of Radix Stephaniae Tetrandrae: a comparison with its main component, tetrandrine.

A comparison of the cardiovascular actions of the extract of Radix Stephaniae Tetrandrae (RST), the root of a Chinese hero Stephania tetrandra S Moore, in rats with those of tetrandrine (Tet), the best known active component of RST was reviewed. The RST extract inhibits Ca2+ influx into the myocyte and reduces protein release during reperfusion with a Ca2+ containing solution following perfusion with a Ca2+ free solution (Ca2+ paradox), and arrhythmia during reperfusion in the isolated perfused heart. It also reduces the infarct size induced by ischemia/reperfusion in vitro and in vivo. In addition, the RST extract suppresses elevation of arterial blood pressure in DOCA-salt hypertensive rats. It does not further reduce the heart rate and coronary flow significantly during myocardial ischemia. The effects are similar to those of Tet. When compared with the same doses of Tet alone, the RST extract, of which 9% is Tet, produces equally potent effects on infarction, arrhythmias, coronary flow and heart rate, and has a greater inhibitory effect on protein release during Ca2+ paradox. The combination at 1:1 ratio of Tet and fangchinoline (Fan), another main component, which constitutes 6% of the RST extract and has no significant effects on the heart, produces comparable effects on protein release during Ca2+ paradox as Tet alone. The observations suggest that the efficacy of the RST extract cannot be accounted for by Tet alone. Some of the effects may be due to an interaction between the components of the extract. The RST extract also produces similar effects as verapamil, a prototype Ca2+ channel antagonist widely used in the treatment of ischemic heart diseases and hypertension, except that verapamil, at 1 mumol/L, a concentration that produces similar cardiac effects as the RST extract, further reduces heart rate significantly during ischemia. So the RST extract may be a therapeutically better agent in the treatment of ischemic heart diseases and hypertension than Ca2+ channel antagonists because of the absence of the inhibitory effect on heart rate during myocardial ischemia.

Asymmetry of brain functional activation: fMRI study under language and music stimulation.

OBJECTIVE: To determine the asymmetry of the human brain functional activation. METHODS: With the help of GE Signa Horizon MRI system, 14 cases of right-handed volunteers were examined and the blood oxygenation level dependent method was used. The T1-weighted images were obtained with spin echo pulse sequence and the functional imaging (T2*-weighted) was performed using a single shot echo planar imaging pulse sequence. Data analysis was done with Sun Sparc Workstation and by the method of student t test or correlation analysis. RESULTS: Most of activation areas were in the left hemisphere under language stimulation, while they were in the right side under music stimulation. Besides, a few brain areas in the contralateral cerebral cortex were also activated under both stimulations. CONCLUSION: The present study supported the hypothesis of the asymmetry of brain functional activation and many brain areas of the cerebral cortex as well as both hemispheres worked in coordination. In addition, it also proved that fMRI is a feasible method in the study of human brain in vivo.

Relationship between the renin-angiotensin system genes and diabetic nephropathy in the Chinese.

OBJECTIVE: To clarify whether genetic variants of the renin-angiotensin system (RAS) contribute to the development of diabetic nephropathy (DN) in the Chinese. METHODS: Totally 173 Chinese subjects of Han nationality from Shanghai were classified into! control, DN (-) and DN (+) groups. The latter was subdivided according to diabetic duration at the onset of DN and the stage of DN. Genotyping of five polymorphic sites in four key genes of the RAS: the AGT-T174M, AGT-M235T and AGTR1 genotypes were determined by PCR/restriction enzyme digestion. The insertion/deletion (I/D) and [ACAC] n-STR microsatellite polymorphic markers were used for ACE and REN genotyping, respectively. Statistical analysis showed comparisons of gene frequencies between any two groups were made with Fisher's exact test or Chi-square test. Logistic regression analysis was performed to identify predictors of DN. RESULTS: The frequencies of ACE-DD genotype and ACE-D allele were much higher in DN(+) group than in DN (-) group (0.25 vs 0.05, 0.47 vs 0.29, respectively), so were the frequencies of TT genotype and T allele in AGT-M235T (0.73 vs 0.54, 0.85 vs 0.68, respectively). DN (+) DUR < 5 years group had greatly increased frequencies of AGT-M235T allele and ACE-DD genotype in comparison with DN(-) group (0.92 vs 0.68 and 0.28 vs 0.05, respectively). Logistic regression analysis further identified these two genes as contributing factors to DN. Although AGTR1 and AGT-T174M genotyping analysis revealed differences in frequency distribution between DN (+) and DN (-) or control groups, logistic regression analysis failed to implicate them in the development of DN. CONCLUSIONS: Our study revealed RAS genes, ACE and AGT-M235T but not AGT-T174M, AGTR1 or REN genotypes, as contributing factors for DN in type 2 diabetes mellitus in Chinese.

[Effect of longshoudan on serum tumor necrosis factor and circulating endothelial cell levels in acute cerebral infarction patients].

OBJECTIVE: To investigate the effect of Longshoudan (LSD) on the serum tumor necrosis factor (TNF) and circulating endothelial cell (CEC) levels in acute cerebral infarction (ACI) patients, and the mechanism of LSD in treating ACI. METHODS: The levels of serum TNF-alpha and CEC in 40 ACI patients before and after LSD oral administration were measured by radioimmunoassay, and compared with 20 routinely treated ACI patients, and 20 healthy subjects as control group. RESULTS: (1) The levels of serum TNF-alpha and CEC in ACI patients were significantly higher than those in the control group, there was significant correlation between the increased extent and infarction size, the larger the size, the higher the levels of serum TNF-alpha and CEC; (2) In ACI patients, the serum TNF-alpha level was positively correlated with CEC level (r = 0.68, P < 0.01); (3) The serum TNF-alpha and CEC level of all ACI patients after 4 weeks were reduced, and LSD treated group was significantly lower than that of routinely treated group (P < 0.01). CONCLUSION: LSD could protect cerebrovascular endothelial cells and ischemic neurons through lowering the increased levels of serum TNF-alpha and CEC in ACI patients, and alleviate the damage of cerebrovascular endothelial cells.

[An investigation on risk factors of cerebral arteriosclerosis].

OBJECTIVE: To investigate the risk factors of cerebral arteriosclerosis (CAS) in the special population--the old cadre in organizations affiliated directly under Fujian Province. METHODS: The investigation was carried out according to the case control method of epidemiology and conditional logistic regression model was used. RESULTS: Multivariate analysis showed that CAS was closely related with the clinostatic systolic blood pressure (CSBP, OR: 2.13), triglyceride (TG, OR: 2.08), diabetes mellitus (OR: 1.31), glycosylated hemoglobin (OR: 1.16), recall of portrait characteristic (OR: 0.89), Kidney-Deficiency Syndrome (OR: 1.54) and Yin-Deficiency with Yang-Excess Syndrome (OR: 1.59). And there was a quantity-effect relationship between CSBP, TG and CAS. CONCLUSION: CSBP is the most important risk factor of CAS in the special population of the old cadre, and the TCM Syndromes of Kidney-Deficiency and Yin-Deficiency with Yang-Excess are also independent risk factors of CAS.

[Clinical study on hemorrheologic changes for metastatic state of gastric cancer and its relationship with syndrome-type in traditional Chinese medicine].

OBJECTIVE: To explore the hemorrheologic changes of gastric cancer in various stages, and the relationship between different phases and Syndrome-types. METHODS: Sixty-three gastric cancer patients and 30 healthy controls matching in sex and age were studied. According to the phases of tumor and the differentiation standards of deficiency and excess syndrome in TCM, the patients were divided into 6 groups: Non-metastasis (NM), lymphnode metastasis (LM), distant metastasis (DM) groups, and Deficiency (D), Excess (E), mixed deficiency and excess (DE) groups. The hemorrheology criteria including whole blood viscosity (BV), plasma viscosity (PV), hematocrit (HCT), fibrinogen (Fib), platelet count (PC), platelet aggregation rate (PAR) were measured. RESULTS: The value of BV, PV, Fib, PC and PAR in gastric cancer groups were higher significantly (P < 0.05, P < 0.01) than those in the healthy control group, but BV, PV in the LM and DM groups were higher than those in the healthy control group significantly (P < 0.05), while difference between NM group and the healthy control group was insignificant (P > 0.05). Hypercoagulability state presented in all the three Syndrome Type groups of gastric cancer patients, but the difference in some criteria between D and E group were significant (P < 0.05, P < 0.01); and DE groups was greatly higher than D and E groups in high-shear BV, Fib and PC (P < 0.05, P < 0.01). CONCLUSION: The value of BV, PV Fib, PC and PAR could be regarded as good referential parameters in reflecting the condition of evil excess.

[The relationship between laminin-receptor and nm23 protein expression and its correlation with interstitial microvascular density and tumor metastasis in breast carcinoma].

OBJECTIVE: To study the relationship between 67,000 laminin, laminin-receptor (LN-R) and nm23 protein expression, interstitial microvascular density (MVD) and tumor metastasis in breast carcinomas. METHODS: The expression of laminin (LN), LN-R, FVIIIRAg and nm23 protein were detected in 73 cases of breast carcinoma with immunohistochemical technique and analyzed. RESULTS: A significant difference in LN expression was found in breast carcinoma according to their pathological grade. A positive relation exists between LN-R expression and lymph node metastasis. A positive relationship exists between the degree of LN-R and MVD expression. A significant difference in lymph node metastasis cases exists between nm23 protein positive expression group and negative expression group. When the expression of nm23 protein was inhibited, the degree of LN-R, MVD expression increased and positively related with lymph node metastasis. CONCLUSION: LN-R expression enhancement, microvascular density increase and nm23 expression inhibition may be the reliable markers for predicting tumor metastasis and prognosis.

[Ultrastructural and immunohistochemical characteristics of pericytes during neovascularization in breast carcinoma].

OBJECTIVE: To study the morphology of pericyte, the relationship between pericytes and neovascularization of breast carcinoma. METHODS: Ultrastructural, immunohistochemical and quantitative morphology analysis techniques were used to study 89 cases of human breast carcinoma. 4 human granulation tissues cases were used as control. RESULTS: The positive cells for FVIIIRAg were endothelial cells situated inside the vascular wall with abundant cytoplasm containing caveolae and Weibel-palade bodies. The positive cells for alpha-SMA were pericytes with cytoplasm containing abundant myofilament, and connections with endothelial cells. The pericyte-endothelial junctions can be found. Quantitative morphological analysis showed that the number of pericytes in the low-microvascular density areas are much higher than that in the high- microvascular density areas (P < 0.0005). CONCLUSION: Ultrastructural observation and immunohistochemical staining of alpha-SMA are useful in identifying pericytes. The results of this study imply that pericytes may possibly inhibit angiogenesis.

[Effect of natrahagin, a Chinese cobra venom proteinase on hemorrheology in rat].

The effects of natrahagin, a Chinese cobra venom proteinase on hemorrheology and plasma fibrinogen level in rat was investigated. The results showed that intravenous injection of natrahagin (0.025-0.1 mg/kg) dose-dependently decreased the whole blood viscosity and the plasma viscosity (P < 0.05 or P < 0.01) in rat, but caused no significant effect on hematocrit (P > 0.05). Natrahagin also decreased the plasma fibrinogen level (P < 0.05 or P < 0.01) of rat in a dose-dependent manner at these doses.

[Photoluminescent properties of organic film in flat optical microcavity].

The microcavity is sandwiched between a quarterwavelength distributed Bragg reflector(DBR) and a metal Ag reflective mirror. A single layer of a Tris(8-quinolinolato) aluminum (Alq) film was used as the light-emitting layer. The photoluminescent properties of the optical microcavity and that of the Alq film were studied at the same excitation condition. Compared with the Alq film, the significantly narrowed spectral emission linewidth from 90 nm to 10 nm was observed, the PL emission intensity of the microcavity at the resonant mode is enhanced by the order of 1. The spectral narrowing and intensity enhancement of the microcavity is attributed to the microcavity effect.
