A community survey of 271 Chinese migrants aged 15 years and older living in Auckland was conducted to assess self-rated adjustment and health. The majority of respondents came from Hong Kong and Taiwan. Despite significant changes in their lives, including the absence of family members, unemployment and underemployment, most did not report major adjustment problems or regret having come to New Zealand. Few considered their health to be poor. Forty-two per cent reported having consulted a doctor within the past 12 weeks. Factors significantly associated with having experienced major problems included being aged 26-35 years, rejection from locals and having low English proficiency. Factors associated with poor adjustment included expectations not having been met, regretting coming, low proficiency in English, recent arrival in New Zealand, unemployment, younger age and lower levels of education. Self-rated fair or poor health was found to be associated with Chinese-only reading knowledge, residency of more than 5 years and regretting having come to New Zealand.
This article describes the implementation of a computer assisted instruction clinical simulation, as an evaluation strategy for determining baccalaureate students' clinical decision-making skills. Students' reactions toward this strategy have been consistently positive. Similarly, faculty were satisfied with this method of assessment. Our experience has convinced us that computer simulation is an objective, efficacious, and pragmatic method for measuring students' clinical decision-making skills.
Nitric oxide has been shown to be an important neuronal signaling molecule that participates in both behavioral and synaptic plasticity. To better understand the potential mechanisms by which NO regulates synaptic function, the ability of NO to stimulate the modification of synaptic proteins by ADP ribosylation was examined. Two NO donors, sodium nitroprusside and 3-morpholinosydnonimine, stimulated the ADP ribosylation of proteins at apparent molecular masses of 42, 48, 51, 54, and 74 kD in hippocampal synaptosomes. This stimulation was likely owing to the production of NO by the donors; ADP ribosylation was not stimulated by non-NO decomposition products of sodium nitroprusside, and quenching of superoxide anion did not inhibit Sin-1-induced ADP ribosylation. Experiments using NAD+ that was radiolabeled on the nicotinamide moiety demonstrated that the modification of proteins of molecular masses of 30, 33, and 38 kD are not true ADP ribosylation, whereas labeling of the 42-, 48-, 51-, 54-, and 74-kD proteins likely represent ADP ribosylation. Some of the substrates were brain specific (54 and 74 kD), whereas others (42 and 51 kD) were present in multiple nonbrain tissues.
Total hip replacement is a common and expensive procedure and any attempt to shorten the length of hospital stay safely is desirable. This paper proposes a scoring system specifically designed for this patient population, to determine the optimal timing of safe discharge and post-acute care placement. The authors discuss the implications for the quality of patient care.
Mothers bottle feeding their infants need an adequate method of lactation suppression to prevent/lessen discomfort related to breast engorgement. This study compared the effectiveness of a non- pharmaceutical method (ice and tight bra) of suppressing lactation to a pharmaceutical therapy (bromocriptine mesylate). The sample consisted of 36 subjects who used the pharmaceutical therapy (drug group) and 17 subjects who used the non-pharmaceutical method (protocol group). The data showed that the drug group had significantly less engorgement, milk production, and discomfort on the fourth postpartum day, but the lactation process was prolonged until postpartum day nine or 16. The protocol group had more engorgement, milk leakage, and discomfort on postpartum day four, but the lactation process was completed before postpartum day 16.
The object of this study is to answer the question: Is acupuncture of any use in ophthalmology? Ophthalmology today can boast of 95% accuracy in diagnosis, but modern therapeutics has not caught up with diagnosis. Despite an inability to explain in modern scientific terms the healing power of acupuncture, documentation of over 500 cases treated shows that this modality can be successful in the treatment of eye diseases, especially in cases of retinitis pigmentosa, high myopia, cataracts, surgical aphakia, controlled glaucoma and re-attached ablation retinae. We notice in these cases that improvement occurs in the central acuity, and no improvement in the visual field, colorvision and nightblindness. Apparently acupuncture acts on the macula alone and not on the entire retina. Acupuncture is not intended to substitute for established medication or surgery in ophthalmology. It cannot restore a tissue or organ damaged beyond regeneration; "incurable" eye diseases per se are not cured by acupuncture. Only symptoms are relieved and dimness of vision is a symptom which can be alleviated in many cases with periodic treatment.

BACKGROUND/AIMS: Recombinant interferon (r-IFN) is an antiviral agent used to treat patients with chronic viral hepatitis patients. Unfortunately, the use is often limited due to its myelosuppressive properties. Currently, there are two forms of r-IFN commercially available: r-IFN alpha-2a and rIFN alpha-2b. Although both are thought to be equally effective, a comparative study of their myelosuppressive properties has not been undertaken. MATERIALS AND METHODS: In the present study, three groups of healthy adult volunteers (n = 6/group) were randomized to receive a two week course of either r-IFN alpha-2a or r-IFN alpha-2b (5 million units, subcutaneously, thrice weekly) or no treatment. All subjects were then followed for an additional two week post treatment; observation period. RESULTS: The results of the study revealed that both forms of r-IFN alpha caused a significant and similar decrease in white blood cell counts (maximum declines of 37.9 +/- 6.8% for alpha-2a and 39.5 +/- 6.0% for alpha-2b at day 4) and platelet counts (maximum declines of 19.5 +/- 8.6% for alpha-2a and 20.2 +/- 8.4% for alpha-2b at day 2) from baseline values. Following discontinuation of therapy, white blood cell and platelet counts returned to pretreatment levels. Hemoglobin levels remained unchanged throughout the study period. CONCLUSION: The results of this study indicate that r-IFN alpha causes a prompt and significant decrease in white blood cell and platelet counts. However, no differences exist between r-IFN alpha-2a and r-IFN alpha-2b in terms of their myelosuppressive properties in humans.

BACKGROUND/AIMS: This paper explores the reported association between Hashimoto's thyroiditis and hepatitis C virus (HCV) infections. MATERIALS AND METHODS: The results of serologic testing for anti-HCV antibody levels were documented in 28 patients (19 female, 9 male) with Hashimoto's thyroiditis and 23 age and gender matched controls with non-Hashimoto's thyroid disorders. RESULTS: The anti-HCV results were negative in both groups. CONCLUSIONS: These findings suggest that despite a reported high prevalence of Hashimoto's thyroiditis in patients with chronic HCV infections, the reverse is not true and hence routine anti-HCV screening cannot be advocated for patients with Hashimoto's thyroiditis.
