Clinical manifestations of human parvovirus B19 in adults.

Human parvovirus B19 has been associated with various clinical effects in a number of uncontrolled reports. To define the usual manifestations of B19 infection in adults and the factors that influence them we present a clinicoepidemiological study of an outbreak of B19 infection centered on a junior school. Four hundred fifty-three of 475 adults in this community were interviewed and blood was obtained for serological diagnosis. Fifty-four cases of recent infection were identified and were HLA typed. Fourteen of the cases were asymptomatic; 32 had an influenzalike illness; 23 a rash; and 26 an acute-onset polyarthropathy that was more common in women and lasted for up to 7 months. HLA-A, -B, and -C antigen frequencies were similar to a local control population and showed no association with symptoms except that HLA-DR1 was absent in those with persistent arthropathy.

Use of serum prolactin in rats as a determinant in detecting endophyte infected tall fescue seed.

The purpose of this study was to examine the effects of endophyte (Acremonium coenophialium) toxic tall fescue seed on serum prolactin, daily feed intake, and average daily weight gain in male rats. Thirty Sprague-Dawley male rats (218 g) were randomly assigned to treatments of (I) rat chow (50%) plus an endophyte-free tall fescue seed (50%), (II) same as I only restricted feed intake as III, (III) rat chow (50%) plus endophyte infected tall fescue seed (50%) and (IV) rat chow (50%) plus endophyte infected tall fescue seed (25%) plus endophyte free tall fescue seed (25%). Dry matter intake and average daily gain were lower (P less than .05) for rats fed treatment II and III than for those fed treatment I and IV, with treatment IV being lower (P less than .05) than treatment I. Body weights on days 4, 11 and 21 followed the same trend with the rats fed treatment I having higher weights at each weigh period than those on treatments II, III and IV; and those fed treatment IV having higher weights at each period than those on treatment II and III. Baseline concentrations of serum prolactin were similar among treatments. Serum prolactin concentrations were lower (P less than .05) in rats fed treatment III and IV than in those fed treatment II, and those fed treatment I were not different from all others on day 4. On day 11, rats fed treatment I, III and IV had lower (P less than .05) serum prolactin concentrations than those fed treatment II.(ABSTRACT TRUNCATED AT 250 WORDS)

Hydramnion and fetal renal anomalies.

Four instances of hydramnion associated with renal anomalies are reported: two patients had obstructive uropathies and two had a neoplastic type of renal dysplasia. In two infants water concentration tests revealed a defect in water-concentrating ability. It is postulated that the observed hydramnion resulted from fetal polyuria. In patients with unexplained hydramnion a search for renal anomalies is indicated.

Regional obstetric anesthesia and newborn behavior: effect over the first ten days of life.

This study examined the effects of carefully controlled amounts of analgesic premedications and anesthetics administered to mothers during delivery on the behavior of the newborn over the first ten days of life. The subjects were selected to minimize the synergistic effects of medication and other stress factors, such as abnormalities of pregnancy, labor, or delivery. The effects of these drugs on the behavior of these infants was small. The data provide a picture of the behavioral recovery of a group of minimally stressed newborns.

Early mother-infant reciprocity.

By three weeks of age, the human neonate demonstrates behaviours which are quite different with an object and with a human interactant. He also demonstrates an expectancy for interaction with his caregiver which has clearly defined limits, as demonstrated behaviourally. In microanalysis of videotape, we saw regularly a set of interactive behaviours which were demonstrable in optimal face-to-face interaction between infants and their mothers. All parts of the infant's body move in smooth circular patterns as he attends to her. His face-to-face attention to her is rhythmic with approach-withdrawal cycling of extremities. The attention phase and build-up to her cues are followed by turning away and a recovery phase in a rhythm of attention-non-attention which seems to define a cyclical homeostatic curve of attention, averaging several cycles per minute. When she violates his expectancy for rhythmic interaction by presenting a still, unresponsive face to him, he becomes visibly concerned, his movements become jerky, he averts his face, then attempts to draw her into interaction. When repeated attempts fail, he finally withdraws into an attitude of helplessness, face averted, body curled up and motionless. If she returns to her usual interactive responses, he comes alive after an initial puzzled period, and returns to his rhythmic cyclical behaviour which has previously characterized their ongoing face-to-face interaction. This attentional cycling may be diagnostic of optimal mother-infant interactions and seems not to be present in more disturbed interactions.

Recurrent short rib-polydactyly syndrome with unusual associations.

Short rib-polydactyly syndrome (SRPS) is manifested by short-limb dwarfism, short ribs with thoracic hypoplasia, and polydactyly. This inheritable syndrome has distinct imaging findings on prenatal sonography, and ancillary findings on both pre- and postnatal assessments may enable individual cases to be classified into 1 of 4 subtypes. In this report, we present a recurrent case of SRPS that was associated with a cystic hygroma and choroid plexus cysts. Although cystic hygromas are not uncommonly seen in skeletal dysplasias, the presence of concomitant cystic hygroma and choroid plexus cysts suggests a chromosomal abnormality such as trisomy 18.

A pilot study of women's attitudes towards breast-conserving surgery with primary radiation therapy for breast cancer.

Breast-conserving surgery ("lumpectomy") with primary radiation therapy is gaining acceptance as an alternative to mastectomy for breast cancer. Currently, little is known about the specific immediate and long-range biopsychosocial effects of breast-conserving therapy as compared to more traditional mastectomy procedures. Physicians' speculations about the women who chose breast-conserving treatment as opposed to mastectomy seem to have been influenced by the scientific debate concerning the efficacy of this procedure. This study is a preliminary investigation of the attitudes and experiences of women receiving breast-saving therapy for carcinoma of the breast.
