Value of serum C-reactive protein measurement in the management of bone marrow transplant recipients. Part II: Late post-transplant period.

Seventeen bone marrow recipients transplanted for acute leukaemia (8), chronic leukaemia (1), severe aplastic anaemia (3), and various inborn errors of metabolism (5) had 22 episodes of documented infection in the late (greater than 3 months) post-transplant period. Serum C-reactive protein concentrations were considerably increased in patients with bacterial infections, but not in those with viral or fungal infections. Serum C-reactive protein values were normal in 20 patients transplanted for acute leukaemia (12), chronic leukaemia (1), severe aplastic anaemia (2), and various inborn errors of metabolism (5) who had active chronic graft versus host disease but no evidence of infection. These findings indicate that serum C-reactive protein concentrations are useful in the diagnosis and monitoring of bacterial infections even in the presence of chronic graft versus host disease.

A new generation of pulsatile infusion devices.

A selection of pulsatile infusion devices suitable for administering LHRH are described. The principles of operation and control options of the different pumps are explained, including the new compact direct-drive syringe pump. The relative merits of these infusion systems are discussed and it is concluded that size, ease of use and range of application are paramount.

Vitreous fluorophotometry in patients with hypertension.

Vitreous fluorophotometry performed on 22 patients with moderate to severe hypertension showed that patients with active retinopathy, as manifested by retinal hemorrhages, cotton-wool spots, retinal exudates and edema, or disc edema, generally had elevated values (20/28 eyes). Control of the BP resulted in a decrease in the values toward the normal range. Patients with only arteriolar sclerosis, even some with particularly high BP, generally had normal fluorophotometric values (14/16 eyes). The two exceptions showed minimal elevations. Vitreous fluorophotometry is a useful tool to quantitate the blood-retinal barrier in patients with moderate to severe hypertension and may be a sensitive indicator of treatment efficacy.

Efficient synthesis of enzymatically active calf chymosin in Saccharomyces cerevisiae.

We have constructed a high-efficiency expression vector to direct the synthesis of heterologous polypeptides in yeast. The vector is termed a sandwich expression vector as the heterologous gene is inserted between the 5' and 3' control regions of the efficiently expressed yeast PGK gene. We have used this vector to direct the expression of three derivatives of the calf chymosin cDNA gene; preprochymosin, prochymosin and chymosin. Prochymosin is synthesised to at least 5% of total yeast-cell protein and furthermore, it can be readily activated to produce an enzyme which has milk-clotting activity.

Experience with mebendazole in the treatment of inoperable hydatid disease in England.

Eleven heterogeneous patients with inoperable hydatid disease were treated with high doses of oral mebendazole, up to 200 mg/kg/day for 16 to 48 weeks. Toxicity was not encountered. Despite careful assessment it proved difficult critically to evaluate the efficacy of mebendazole. In four patients chemotherapy appeared to have been at least partially successful and success seems to have correlated with serum levels in excess of 100 ng/ml one to three hours after dosage. The problems of assessment are discussed and the need for controlled trials of mebendazole in hydatid disease are stressed.

Selective compounds derived from marine organisms: effects on cell division in fertilized sea urchin eggs.

An investigation of the pharmacological selectivity of the fertilized sea urchin egg assay was undertaken with the view that this echinoderm may serve as a useful model for detecting new compounds that inhibit cell division and also may yield substantive information on biochemical events that may be sensitive to drug action. One hundred and thirty purified marine natural products were tested as well as 14 known antineoplastic agents. Nine active marine cytotoxins were identified and, based on in vitro studies of microtubule assembly, five compounds inhibited tubulin polymerization. Of the 14 antineoplastic agents tested thus far, the transcriptional inhibitor daunomycin has proven active as well as the microtubule assembly inhibitors Colcemid, podophyllotoxin, vinblastine, and vincristine.

Testicular sonogram.

Precise localization, detection, and recognition of minor changes in testicular lesions are important because teratocarcinoma is notorious for manifesting as secondaries at the time the primary site is obvious to the clinician. In the past, questionable enlargement of the testis due to significant pathology required numerous radiographic invasive special procedures to provide a correct diagnosis. Due to the advent of the sophisticated digital ultrasound imager with high frequency quarter wave transducer, it is possible to detect minor changes in the tissue character of the testis, thus enabling the physician to tackle primary neoplasms prior to distant spread.In our case we were able to detect the abnormality in the testis, but unfortunately a large secondary abnormal mass was present. Even at that stage we were able to map out the extent of the lesion which was beneficial to the surgeon and the patient. Ultrasound studies were utilized in serial follow-up studies.
